Distribution of Boophilus species ticks in Swaziland.

Recent outbreaks of bovine babesiosis caused by Babesia bovis in Swaziland had indicated the presence of the vector tick Boophilus microplus in the country although it had never before been directly identified. Engorged female Boophilus ticks were collected from cattle at diptanks in the course of a tick resistance survey and used to map the distribution of the two different species of Boophilus. B. decoloratus was found to be widespread throughout the country. B. microplus was identified for the first time in Swaziland and was found to have a patchy distribution. The implications of these fin

Epidemiology and aetiology of urethritis in Swaziland.

The annual incidence of urethritis can be estimated to be at lest 3750 per 100,000 population in Swaziland. In a study of 109 males with symptomatic urethritis 80% had gonorrhoea, 6% non-gonococcal urethritis (ngu) and 14% were classified as having no 'objective' urethritis (less than 5 polymorphonuclear leucocytes per highpower field in the urethral smear). The relative frequency of gonorrhoea was 80 to 95% and of non-gonococcal urethritis 5 to 20 according to which criteria are used for patient selection and/or diagnosis of ngu. Chlamydia trachomatis was cultured in 3.4% of the cases with ur

Contraceptive use among adolescent girls and young women ages 15-24 in seven high HIV prevalence countries.

BACKGROUND: Adolescent Girls and Young Women (AGYW; ages 15-24) continue to use contraceptives at lower rates than older women in sub-Saharan Africa. We describe contraceptive use among AGYW in seven Southern African countries (Botswana, Eswatini, Lesotho, Malawi, Mozambique, Zambia and Zimbabwe). METHODS: Cross-sectional, nationally representative household-based data from seven Population-based HIV Impact Assessment surveys (conducted between November 2019 and February 2022) were analyzed using survey weights to create descriptive results and pooled odds of modern contraceptive use. RESULT

Venom diversity in Naja mossambica: Insights from proteomic and immunochemical analyses reveal intraspecific differences.

BACKGROUND: Intraspecific variations in snake venom composition have been extensively documented, contributing to the diverse clinical effects observed in envenomed patients. Understanding these variations is essential for developing effective snakebite management strategies and targeted antivenom therapies. We aimed to comprehensively investigate venoms from three distinct populations of N. mossambica from Eswatini, Limpopo, and KwaZulu-Natal regions in Africa in terms of their protein composition and reactivity with three commercial antivenoms (SAIMR polyvalent, EchiTAb+ICP, and Antivipmyn A

An assessment of financial catastrophe and impoverishment from out-of-pocket health care payments in Swaziland.

BACKGROUND: As the drive towards universal coverage is gaining momentum globally, the need for assessing levels of financial health protection in countries, particularity the developing world, has increasingly become important. In Swaziland, the level of financial health protection is not clearly understood. OBJECTIVE: To assess financial catastrophe and impoverishment from out-of-pocket payments for health services in Swaziland. METHODS: The nationally representative Swaziland Household Income and Expenditure Survey (2009/2010) dataset is used for the analyses. Data are collected by the Cen

Antibodies against Duvenhage virus in insectivorous bats in Swaziland.

Several rabies-related lyssaviruses have been associated with bat species in southern Africa, the rarest of these being Duvenhage virus (DUVV), for which only five isolations have been made over five decades. Three of these were from human fatalities, and it is not known which bat species acts as reservoir. In studying a population of Nycteris thebaica in the kingdom of Swaziland, a landlocked country bordering Mozambique and South Africa, we found evidence of the circulation of a lyssavirus. Virus-neutralization assays indicated DUVV-neutralizing antibodies in 30% of the sera collected from t

Sterilization of gowns: making the most of a scarce commodity during the COVID-19 pandemic.

Baylor Clinic in Mbabane, Eswatini, convened a crisis meeting to tackle critical shortages of long-sleeved disposable gowns that resulted from COVID-19 pandemic constraints on available personal protective equipment (PPE). A strategy deemed safe, affordable and sustainable was adopted to autoclave and re-use gowns based on a risk-stratified approach. Key objectives were to ensure essential infection control and prevention (ICP) for medical doctors, nurses, and laboratory teams. Administrative, environmental and personal protective measures for ICP were enhanced through regular staff training.

Pyomyositis in Swaziland.

Pyomyositis was studied in 25 patients over a period of 1 year. The patients lived in a subtropical region, latitude about 27 degrees S, but the disease described is the same as that which occurs in the tropics and which goes by the name of "tropical myositis". The aetiology proposed for tropical myositis is regarded as not applicable to the same disease described here, of which the cause remains uncertain. This aspect of the problem is discussed with reference to the possible role played by a deficiency of ascorbic acid in the diet. Incision and drainage is the only method of treatment.

Schistosomiasis in Swaziland--a comparative study of three irrigated estates.

The prevalence of Schistosomiasis and the distribution of the snail intermediate hosts on three adjoining irrigated estates in the lowveld of Swaziland was investigated. The prevalence rates of Schistosoma haematobium and S. mansoni among estate employees was lower than anticipated and there was a marked difference between the prevalence rates of S. mansoni between estates. The reasons for this and the influence of water supplies and agricultural practice on the prevalence of schistosomiasis are discussed.

HPV cancer burden by anatomical site, country, and region in 2022.

HPV vaccination can prevent more than 90% of HPV infection-related cancers. Understanding the epidemiological burden of HPV-related cancers can better promote HPV vaccination and HPV-based screening programs. We used three databases-Global Burden of Disease 2021 (GBD2021), Global Cancer Observatory 2022 (GLOBOCAN2022), and Cancer Incidence in Five Continents Plus (CI5Plus)-to evaluate the global disease burden of cervical cancer, penile cancer, vulvar cancer, vaginal cancer, laryngeal cancer, oropharyngeal cancer, oral cancer, anal cancer, and the relative contribution of HPV infection. A descriptive summary of the WHO availability assessments of medical abortion medicines in eight African countries.

BACKGROUND: The use of medical abortion using either a combination of mifepristone and misoprostol, or misoprostol alone has contributed to increased safety and decreased mortality and morbidity. The availability of quality medical abortion medicines is an essential component in the provision of quality abortion care. Understanding the factors that influence the availability of medical abortion medicines is important to help in-country policymakers, program planners, and providers improve availability and use of medical abortion. METHODS: Using a national assessment protocol and an availabili

First report of cryptosporidiosis in paediatric patients in Swaziland.

Diarrhoeal disease is a significant cause of morbidity and mortality in many African countries, especially among the immunocompromised and the under five population. Over the last decade, the protozoan parasite Cryptosporidum spp. has been shown to be a significant cause of gastro-enetritis in the immunocompetent and immunocompromised in both the developed and developing world. This study reports the first finding of Cryptosporidum spp. oocysts in 2 of 48 (4.2%) faecal samples obtained from Swazi paediatric outpatients (two year old male and three year old female), presenting with diarrhoea at

A missed opportunity: birth registration coverage is lagging behind Bacillus Calmette-Guérin (BCG) immunization coverage and maternal health services utilization in low- and lower middle-income countries.

BACKGROUND: Civil registration and vital statistics (CRVS) systems lay the foundation for good governance by increasing the effectiveness and delivery of public services, providing vital statistics for the planning and monitoring of national development, and protecting fundamental human rights. Birth registration provides legal rights and facilitates access to essential public services such as health care and education. However, more than 110 low- and middle-income countries (LMICs) have deficient CRVS systems, and national birth registration rates continue to fall behind childhood immunizatio

Generational inversions: 'working' for social reproduction amid HIV in Swaziland.

How do people envision social reproduction when regular modes of generational succession and continuity are disrupted in the context of HIV/AIDS? How and where can scholars identify local ideas for restoring intergenerational practices of obligation and dependency that produce mutuality rather than conflict across age groups? Expanding from studies of HIV/AIDS and religion in Africa, this article pushes for an analytic engagement with ritual as a space and mode of action to both situate local concerns about and practices for restoring dynamics of social reproduction. It describes how the endur

Recognition of pneumonia by primary health care workers in Swaziland with a simple clinical algorithm.

In developing countries primary health care workers are being trained to manage and treat acute respiratory infections with a protocol developed by the WHO. We tested the ability of nurses and nursing assistants in Swaziland to recognise the signs and symptoms of pneumonia; with the results of a paediatrician's examination as "gold standard", sensitivities and specificities were calculated. Danger signs of stridor and abnormal sleepiness were poorly recognised (sensitivity 0-50%) by the health care workers, as was audible wheeze. Severe undernutrition, tachypnoea, and chest wall indrawing were

Influence of antenatal screening on perinatal mortality caused by syphilis in Swaziland.

In a survey of 283 deliveries in Swaziland, active syphilis (positive results in the Treponema pallidum haemagglutination assay (TPHA) and the rapid plasma reagin (RPR) test) was found in 37 (13.1%) and possibly active infection (positive TPHA but negative RPR test results) in a further 87 (30.7%). The perinatal mortality of untreated mothers with active disease was 21.9% (7/32). The RPR test carried out antenatally by nurses had a sensitivity of 36% (13/36) and predictive accuracy of 48% (13/27). Awareness of this incidence of syphilis led to improved antenatal clinic measures and the prophyl

Inference of malaria reproduction numbers in three elimination settings by combining temporal data and distance metrics.

Individual-level geographic information about malaria cases, such as the GPS coordinates of residence or health facility, is often collected as part of surveillance in near-elimination settings, but could be more effectively utilised to infer transmission dynamics, in conjunction with additional information such as symptom onset time and genetic distance. However, in the absence of data about the flow of parasites between populations, the spatial scale of malaria transmission is often not clear. As a result, it is important to understand the impact of varying assumptions about the spatial scal
