Ethambutol (EMB) is a first-line anti-tuberculosis drug that is also considered in treatment regimens for infections caused by non-tuberculous mycobacteria (NTM). EMB targets the arabinosyl transferases EmbCAB, which are important for the synthesis of cell wall constituents. To further explore and narrow down the structural variability of EMB, we synthesized three series of new EMB analogs. OBJECTIVE: To investigate the incidence of ethambutol (EMB) induced optic neuropathy prescribed at a relatively low dose of ≤ 15 mg/kg/day for the treatment of tuberculosis (TB) or Mycobacterium avium complex (MAC) lung disease.
DESIGN: Patients diagnosed with TB or MAC lung disease received multidrug regimens including EMB at a single institution from August 2003 to July 2009. Visual monitoring was performed at baseline and at regular follow-up. The incidence of EMB-induced visual disturbances was evaluated.

Visual evoked responses (VERs) were recorded in 47 children, aged 3-13 years with tuberculosis, treated with ethambutol (20 mg/kg/day) as a part of the antitubercular regimen. VERs were evoked by monocular whole field stimulation, the stimulus being provided by a black and white checker-board pattern reversed every 560 msec and recorded before the commencement, 2, 4, 6, 9 and 12 months of therapy and between 3 to 6 months after stopping the drug. Six patients with severe ocular side effects caused by therapeutical doses of the tuberculostatic drug ethambutol were investigated during the course of recovery with psychophysical and electrophysiological methods. A 67 year old woman presented with miliary tuberculosis. She was treated with streptomycin, isoniazid, rifampicin, ethambutol and pyrazinamide. Report on 6,200 control examinations of patients treated with Ethambutol. Subjective troubles of vision do not influence continuation of therapy. In 8 patients (0,5%) symptoms of neuritis nervioptici accompanied with visus deterioration required to stop therapy (case reports on these 8 patients). Ophthalmologic control examinations are recommended before beginning therapy, during continuous therapy in hospital every eight week, and in case of ambulatory intermitting therapy every third to fourth month.

Pyrazinamide (PZA) combined with either ethambutol (EMB) or a fluoroquinolone for 6-12 months is one of the treatments recommended for latent tuberculosis infection (LTBI) in contacts exposed to multidrug-resistant tuberculosis (MDR-TB). The results were analysed, of the results obtained by the associated treatment with rifampicin and etambutol 2/7 for at least 6 months in 53 patients with silico-tuberculosis and positive bacteriologic tests. Drug reaction with eosinophilia and systemic symptoms (DRESS) is a T cell mediated, drug-specific immune response associated with end-organ damage. We present the case of a 65 year old woman presenting with a confluent rash, fever, and abdominal pain. Two months prior, she noted a finger lesion which progressed to cellulitis, requiring two debridements with culture growing Mycobacterium marinum.
