Eleven patients with mycosis fungoides (MF) were treated with recombinant alpha-interferon (rIFN-alpha 2a2) in combination with etretinate (seven patients) or alone. Fifteen women with lichen sclerosus (LS) of the vulva were treated for 3 months with 0.6 mg/kg of etretinate. Treatment with etretinate is known to be effective for patients with psoriasis. However, it has been reported that the administration of etretinate often generates side effects which disturb lipoprotein metabolism. Medical records of 30 dogs with histologically confirmed sebaceous adenitis that were treated with isotretinoin or etretinate were reviewed. Akitas and Standard Poodles were overrepresented, compared with the general hospital population. Thirteen dogs had concurrent pyoderma. The retinoids were administered for a minimum of 2 months. Papillon-Lefèvre syndrome (PLS) is a rare autosomal recessive genodermatosis characterized by the association of palmoplantar keratoderma with severe periodontopathy resulting in premature loss of both deciduous and permanent dentition. We treated a patient with PLS with etretinate and followed him for 10 years. Treatment was started at age 9 years, soon after the eruption of secondary dentition. Etretinate (RO 10-9359) is a synthetic retinoid used instead of Vitamin A--toxic in large dose--during treatment of some skin diseases. In this work biological properties of Etretinate were studied by administration of this drug (2 mg/kg/day) in rats in a vitamin A-deficient state. Body growth, some seric parameters and some tissue enzymatic activities were studied. Results of this restitution experiment show that Etretinate corrects all parameters affected by vitamin A-deficiency testicular growth excepted. So Etretinate exhibits properties similar to that of retinoic acid.

Administration of etretinate in a 29-year-old female patient suffering from severe pustular psoriasis caused a dramatic increase in liver enzymes. Liver biopsy revealed changes characteristic for drug-induced hepatitis. After normalization of liver parameters following withdrawal of etretinate, isotretinoin was administered during a severe pustular relapse. Forty-six patients with Lichen sclerosus (Ls) of the vulva were treated with peroral etretinate for 3 months. The pretreatment serum levels of vitamins A and E, and selenium were measured. Etretinate therapy proved to be effective in the treatment of Ls; however, it did not affect the vitamin A status of the Ls patients. Vitamin E and selenium levels did not differ from the control values, nor did they have any prognostic value.

The recurrence preventing effect of Etretinate on 174 superficial bladder tumors was examined by a randomized study using the envelope method. After transurethral resection of the bladder tumor, the tumor-free patients were divided into two groups, one administered one 10 mg capsule of Etretinate once a day, and the other group untreated (control group). Acrodermatitis continua of Hallopeau is considered by many to be a variant of pustular psoriasis because of similar histologic features. Its clinical picture is unique and helps to distinguish acrodermatitis from pustular psoriasis. We present a patient with acrodermatitis continua of Hallopeau resistant to conventional therapy. Etretinate (Ro 10-9359) is a new aromatic retinoic acid derivative for the treatment of severe psoriasis and other dyskeratoses. A randomized double-blind study was performed in forty patients with severe psoriasis. Etretinate, in contrast to placebo, clearly reduced the psoriatic area and severity index score, and this difference was statistically significant from week 2. Many treatments are used for epidermotropic cutaneous T cell lymphomas (CTCL) such as mycosis fungoides (MF) and Sézary syndrome (SS). All pretend to be effective, but none is really curative.
