Role of the cardio-pulmonary exercise test and six-minute walking test in the evaluation of exercise performance in patients with late-onset Pompe disease.

In patients with late-onset Pompe disease, we explored the role of the Cardiopulmonary Exercise Test (CPET) and the Six-Minute Walking Test (6MWT) in the assessment of exercise capacity and in the evaluation of the effects of enzyme replacement therapy (ERT). Eight patients affected by late-onset Pompe disease, followed up at the Centre for Neuromuscular Diseases and treated with ERT, underwent a baseline evaluation with a spirometry, a CPET and a 6MWT. Four of them were restudied after 36 months of treatment. Three patients showed a reduction in exercise capacity as evaluated by peak oxygen u

Invasive cardiopulmonary exercise testing: Physiologic assessment of unexplained dyspnea and exercise intolerance.

Unexplained dyspnea and exertional intolerance are common, burdensome clinical problems that may persist despite routine resting cardiopulmonary testing. Invasive cardiopulmonary exercise testing (iCPET) integrates breath-by-breath gas exchange with invasive hemodynamic and blood gas assessment during incremental exercise, enabling evaluation of physiologic abnormalities that may be unapparent at rest. This review summarizes practical considerations for iCPET performance (including upright cycle ergometry, catheter-based pressure measurements, direct Fick cardiac output, and symptom assessment

Validity of Alfred Step Test Exercise Protocol (A-STEP) as a Surrogate VO2Max Cardiopulmonary Exercise Test (CPET) to Cycle Ergometry in Adults With Cystic Fibrosis.

BACKGROUND: The Alfred Step Test Exercise Protocol (A-STEP) and feasibility study were previously published. The aim here was to determine the validity of the A-STEP compared to cycle ergometry (CPET) in adults with CF. METHODS: The A-STEP and CPET were carried out in random order 2 weeks apart. A wearable, portable metabolic system was used to measure breath-by-breath and minute-by-minute sampling of O2, CO2, heart rate, and VO2. The main outcome measures were VO2max and HRmax. RESULTS: Seven stable-state adults (3 male) on CFTR modulator therapy with a mean (SD) and range of age 38.2 (13.4

Determinants of Reclassification of Exercise Performance Across Cardiopulmonary Exercise Testing Prediction Equations.

RATIONALE: Volume of oxygen uptake at peak exercise (V̇O₂peak) is a key indicator of cardiorespiratory fitness used to assess surgical risk and clinical trial outcomes. However, multiple prediction equations exist for its interpretation, often yielding inconsistent classifications of exercise performance. OBJECTIVE: To determine how different prediction equations affect exercise performance classification and identify factors contributing to its variability. METHODS: We performed a post-hoc analysis of cardiopulmonary exercise test (CPET) data collected from 332 never-smoking flight attendan

Exercise limitations in amyloid cardiomyopathy assessed by cardiopulmonary exercise testing-A multicentre study.

AIMS: Amyloid cardiomyopathy is caused by the deposition of light chain (AL) or transthyretin amyloid (ATTR) fibrils, that leads to a restrictive cardiomyopathy, often resulting in heart failure (HF) with preserved or reduced ejection fraction. This study aimed to determine whether cardiac output reduction or ventilation inefficiency plays a predominant role in limiting exercise in patients with amyloid cardiomyopathy. METHODS: We conducted a multicentre prospective study in patients with AL or ATTR cardiomyopathy who underwent cardiopulmonary exercise testing across four centres. Patients we

Exercise in Hypertrophic Cardiomyopathy: Recent Conceptual Changes and Recommendations for Pre-Exercise Tests.

Traditionally, individuals with hypertrophic cardiomyopathy (HCM) have been advised not to participate in more than low-intensity exercises. HCM was originally described in the context of sudden death, and early literature from the registry showed that HCM was the most common cause of sudden cardiac death in young athletes. Therefore, there has long been a concern that exercise could trigger ventricular arrhythmia and sudden cardiac death. Although a few patients with HCM may progress along deteriorating disease pathways, many have no clinically significant symptoms or adverse events, no need

Graded Exercise Testing Versus Simulated Competition Exercise in Trained Older Males.

PURPOSE: Exercise-based rehabilitation is a standard therapy for patients with heart disease. Despite examples of patients who have extended normal rehabilitation exercise into competitive participation, there are no clear-cut guidelines for patients whether they should participate in competitive-level exercise. This study investigated the occurrence of complications, physiologic responses, and exercise patterns during simulated competitive exercise in active, older nonathletes (most with a history of cardiovascular disease) and compared these with responses during maximal incremental exercise

The Role of Cardiopulmonary Exercise Testing in Hypertrophic Cardiomyopathy.

This review emphasizes the importance of cardiopulmonary exercise testing (CPET) in patients diagnosed with hypertrophic cardiomyopathy (HCM). In contrast to standard exercise testing and stress echoes, which are limited due to the ECG changes and wall motion abnormalities that characterize this condition, CPET allows for the assessment of the complex pathophysiology and severity of the disease, its mechanisms of functional limitation, and its risk stratification. It is useful tool to evaluate the risk for sudden cardiac death and select patients for cardiac resynchronization therapy (CRT), ca

Exercise Testing in the Risk Assessment of Pulmonary Hypertension.

TOPIC IMPORTANCE: Right ventricular dysfunction in pulmonary hypertension (PH) contributes to reduced exercise capacity, morbidity, and mortality. Exercise can unmask right ventricular dysfunction not apparent at rest, with negative implications for prognosis. REVIEW FINDINGS: Among patients with pulmonary vascular disease, right ventricular afterload may increase during exercise out of proportion to increases observed among healthy individuals. Right ventricular contractility must increase to match the demands of increased afterload to maintain ventricular-arterial coupling (the relationship

Practical guide to cardiopulmonary exercise testing in adults.

Unexplained exertional dyspnoea or fatigue can arise from a number of underlying disorders and shows only a weak correlation with resting functional or imaging tests. Noninvasive cardiopulmonary exercise testing (CPET) offers a unique, but still under-utilised and unrecognised, opportunity to study cardiopulmonary and metabolic changes simultaneously. CPET can distinguish between a normal and an abnormal exercise response and usually identifies which of multiple pathophysiological conditions alone or in combination is the leading cause of exercise intolerance. Therefore, it improves diagnostic

[Cardiopulmonary exercise testing in the management of systemic sclerosis].

Cardiopulmonary complications are the leading cause of mortality in patients with systemic sclerosis (SSc) requiring an early identification. A global and comprehensive approach is needed due to the complexity of the overlapping aetiologies of dyspnoea in SSc. Through its integrative approach of ventilatory, metabolic, cardiovascular, skeletal muscular and gas exchange findings, cardiopulmonary exercise testing (CPET) has been known to identify and sort competing mechanisms of exercise limitation in scleroderma patients presenting with dyspnoea. CPET may be used to screen for pulmonary arteria

Cardiopulmonary exercise testing in the assessment of pulmonary hypertension.

The assessment of patients with suspected or confirmed pulmonary arterial hypertension (PAH) and secondary pulmonary hypertension (PH) continues to evolve and, in recent years, evidence demonstrating that cardiopulmonary exercise testing (CPX) provides valuable information has grown at an impressive rate. The key premise supporting the use of CPX is that certain variables obtained provide insight into the degree of ventilation/perfusion mismatching secondary to altered pulmonary hemodynamics. In this article, we discuss the pathophysiology of PAH and secondary PH and its impact on cardiac func

Exercise testing as a diagnostic entity in mitochondrial myopathies.

Exercise intolerance is one of the most common symptoms in patients with mitochondrial myopathies (MM). At the whole body level, this is characterized by a reduction in maximal oxygen consumption (VO2max) with an excessive carbon dioxide production (VCO2), increased rating of perceived exertion and a hyperdynamic circulatory response at a given exercise intensity. Fewer patients with MM display overt muscle atrophy and weakness even in the absence of a peripheral neuropathy. At the level of the skeletal muscle, the abnormal exercise response in MM patients is characterized by an increase in; d
