PURPOSE: To prospectively explore the incidence and risk factors of moderate to severe pain after primary and secondary orbital implantation following evisceration or enucleation surgery.
METHODS: One hundred eighteen patients under general anesthesia for orbital implantation were enrolled in this study. In 91 patients, primary orbital implantation followed evisceration, and in 27 patients, the implantation was secondary after previous evisceration or enucleation surgery. Medical interventions for all participants were followed by standardized surgical, anesthetic, and analgesic protocols. Po

Various surgical techniques are available for the extirpation of an eye, depending on the underlying disease. Typical indications encompass tumors, inflammatory and infectious processes, glaucoma, trauma as well as congenital malformations. Surgical procedures include evisceration (resection of the intraocular space with preservation of the sclera), enucleation (extirpation of the entire eyeball including sclera and cornea) and exenteration (resection of the complete orbital soft tissue). The early detection of postoperatively manifesting complications, such as implant extrusion, conjunctiviti

IntroductionEven with recent improvements, the optimal timing of orbital implant placement after evisceration surgery - particularly in infected cases - remains unclear. This study aimed to compare the long-term outcomes of primary silicone orbital implants after evisceration for infectious and non-infectious causes, focusing mainly on implant-related complications such as exposure or extrusion.MethodsA total of 32 eyes that underwent evisceration were analyzed. Patients were divided into two groups: infectious causes (Group A, n = 10) and non-infectious causes (Group B, n = 22). Postoperative

PURPOSE: To describe an anterior scleral capping technique using human donor sclera or preserved pericardium during ocular evisceration to enable the placement of larger orbital implants.
METHODS: Retrospective interventional case series. Six consecutive patients underwent evisceration for blind, painful eyes without infection or suspected intraocular malignancy. Following standard evisceration, the largest possible spherical implant was inserted. A human donor scleral button (5 cases) or preserved pericardium (1 case) was sutured to the anterior scleral surface as a cap over the implant. Mai

PURPOSE OF REVIEW: To examine the proposed mechanisms of vision-threatening injuries occurring secondary to orbital and facial trauma: traumatic optic neuropathy (TON), retrobulbar haemorrhage (RBH) and penetrating eye injury. To evaluate the evidence supporting different management options for traumatic vision-threatening injury.
RECENT FINDINGS: Despite considerable debate over the roles of surgical decompression and systemic steroid therapy for TON, these interventions have not been proved to be more effective than conservative management and there is limited evidence that the use of stero

AIMS: To report the surgical outcome of tectonic graft using glycerol-preserved donor corneas to treat perforated keratitis.
METHODS: The medical records were reviewed of all patients treated for perforated keratitis using glycerol-preserved corneas at a single institution between 1 July 2004 and 31 June 2010. The clinical features, precipitating factors, adjuvant therapies, and therapeutic outcomes were analyzed. Success was defined as re-epithelialization of the ocular surface without evisceration.
RESULTS: Fourteen eyes from 14 patients (6 male and 8 female) were included. Age ranged from

Camel bites are uncommon. They are more common during the rutting season where male camels become more aggressive. Herein, we report a unique case of a 25-year-old man who was repeatedly bitten to his face and neck by an aggressive camel that resulted in left eye evisceration, parotid duct, and facial nerve injury. To our knowledge, Globe rupture caused by a camel bite has not been previously reported.

PURPOSE: Orbital implants are used as fillers following enucleation or evisceration surgeries to replace the lost volume for better cosmesis and motility of the artificial eye. Over the last decade porous hydroxyapatite (HAp) implants derived from the naturally occurring corals, are increasingly used. Recently synthetic HAp-based implants have been introduced. After fibrovasculrisation they have the added advantage of being directly integrated with the artificial shell, thereby increasing the motility to a great extent. The current study, evaluated the efficacy of two different models of synth

We studied the occurrence of ulcerative keratitis in five eyes of four patients who were examined at the University of Maryland Hospital ophthalmology clinic over a 12-month period. All were young women who were intravenous drug abusers, with no known predisposing factors for ulcerative keratitis. Two patients had acquired immunodeficiency syndrome (AIDS), one was human immunodeficiency virus (HIV)-positive, and the fourth refused HIV testing. One had a corneoscleral limbus to corneoscleral limbus keratitis; three had inferiorly located corneal ulcers (bilateral in one patient with AIDS). Corn

Panophtalmitis is an intra-ocular and intra-orbital rare infection, but its consequences are often horrendous, with loss of both function and anatomy of the eye. The surgical approach and the antibiotherapy should be given as soon as possible, both by intra-vitreal and intra-venous injections. We report the case of a 49-year-old man who developed a Bacillus cereus panophtalmitis. A delay in the diagnosis lead to the loss of vision and the evisceration of the sick eye. Bacillus cereus panopthalmitis should be suspected in all cases of post-traumatic endophtalmitis with a soiled metallic foreign

The paper presents a case of spontaneous perforation of the cornea happened on an eye with neglected relapsed herpetic keratitis. It is discussed the role of corticotherapy, local toxicity of used antiviral drugs and neurotrophic troulles in the territory of the ophthalmic nerve, as being factors which could have hurried up the evolution towards the perforation. The relapses specific to infections with herpes simplex are favoured by the cantonment of the virus, in latent state, in the trigeminal ganglion but also possibly in the ocular tissues (including the cornea). The use of corticosteroids

Five patients from two families had similar features including keratoglobus, blue scleras, hyperextensibility of the hand, wrist, and ankle joints, sensorineural conduction hearing alterations, and mottling of the teeth. Keratoglobus had been observed in all patients at, or shortly after, birth. Corneal perforations developed in seven of the ten eyes after minimal trauma. Repair of these perforations was complicated by the extremely thin corneas and six eyes had to be either enucleated or eviscerated. Histopathological examination of two of the enucleated eyes showed the corneal stromas of bot

Eighteen cases of culture-proven Bacillus endophthalmitis were reviewed. The majority (13/18) were secondary to penetrating ocular trauma. Others were secondary to corneal ulcer (1), postoperative (1), and endogenous (3). Bacillus cereus was the most commonly identified species (12/18). Antibiotic sensitivities demonstrated uniform sensitivity to vancomycin and the aminoglycosides. Visual prognosis was generally poor; only two patients maintained 20/200 or better in the affected eye. Fourteen eyes were enucleated or eviscerated. The remaining two eyes had light perception vision at the last fo

This paper describes the clinicopathologic findings in 38 eyes that were surgically enucleated or eviscerated after cataract extraction with implantation of an intraocular lens (IOL). Endophthalmitis and/or complications of wound healing accounted for loss of the eye in 29 cases. Eight of the 38 eyes had complications of IOL insertion that necessitated multiple surgical procedures. These procedures were the removal of an IOL, the secondary insertion of an IOL, and penetrating keratoplasty. Two cases of endophthalmitis were the result of contamination of the solution used to sterilize the lens.

A 59-year-old male contact lens wearer was admitted to the intensive care unit for COVID-19 pneumonia. As contact lenses were not removed during admission, the patient developed bilateral corneal abscesses, leading to evisceration of the left eye. Complete corneal melting was observed in the right eye, which was preserved with a therapeutic scleral lens filled with moxifloxacin until donor tissue was available for a sclerokeratoplasty combined with amniotic membrane transplantation. In the second procedure consisting of phacoemulsification, capsular bag implantation of the intraocular lens was

We describe a case of right eye tubercular (TB) panuveitis with corneal involvement where repeated intraocular sampling was unsuccessful. Finally, evisceration and histopathology of ocular tissue confirmed the diagnosis of ocular tuberculosis. A chest X-ray showed signs of pulmonary TB. The patient was lost to follow-up but reported 2 months later with the right eyelid margin nodules with surface ulceration. Both eyelid and pulmonary lesions were resolved with anti-TB therapy.
