Occipital nerve block is effective in craniofacial neuralgias but not in idiopathic persistent facial pain.

Occipital nerve block (ONB) has been used in several primary headache syndromes with good results. Information on its effects in facial pain is sparse. In this chart review, the efficacy of ONB using lidocaine and dexamethasone was evaluated in 20 patients with craniofacial pain syndromes comprising 8 patients with trigeminal neuralgia, 6 with trigeminal neuropathic pain, 5 with persistent idiopathic facial pain and 1 with occipital neuralgia. Response was defined as an at least 50% reduction of original pain. Mean response rate was 55% with greatest efficacy in trigeminal (75%) and occipital

[Microcompression of Gasser's ganglion. A treatment of essential facial neuralgia. Apropos of 70 cases].

Percutaneous micro-compression of the trigeminal ganglion for trigeminal neuralgia, using the technique of Mullan and Lichtor (1983), with some modifications, was performed during the last five years, in our institution in 70 patients. 97.5% of the patients were initially relieved of their pain. There were 14 recurrences (20.5%). 9 of these patients underwent a second micro-compression with 8 excellent results. The follow up examination 6 to 60 months (average: 16.5 months) showed that 88.5% of the patients were free of pain (54 times after one micro-compression and 8 times after two). Sequell

Trigeminal neuralgia versus atypical facial pain. A review of the literature and case report.

Trigeminal neuralgia and atypical facial pain are common conditions of facial pain. Although these two pain conditions are classically well separated in textbooks, a straightforward diagnosis may not always be possible because of the overlapping clinical signs and symptoms. In this article, a comparison and differentiation between the clinical and diagnostic features of these two pain conditions are presented. The general characteristics, etiologic characteristics, pathophysiology, differential diagnostic criteria, and therapeutic options of trigeminal neuralgia and atypical facial pain are de

[Treatment of facial neuralgias: retrogasserian thermocoagulation under iterative discontinuous narcosis with propanidid].

Coagulation of the Gasserian ganglian is currently recommended in the treatment of trigeminal neuralgia due to the slight risks that it presents. It is accompanied by brief but intense periods of pain which necessitate the need of an Anaesthetist. As well as we have been able to verify in the course of sixty-two of these procedures, a discontinuous narcosis obtained by reiterated injections of propanidid presents a satisfying solution. It permits the neurological examinations, which need absolute cooperation of the patient, before each new coagulation to be accomplished in good conditions.

LOW-DOSE NALTREXONE REVERSES FACIAL MECHANICAL ALLODYNIA IN A RAT MODEL OF TRIGEMINAL NEURALGIA.

Trigeminal neuralgia (TN) is a type of neuropathic pain characterized by intense pain; although anticonvulsants are used as an option to relieve pain, adverse side effects can decrease patient adherence. In this context, a low dose of naltrexone is effective in relieving pain in other pain conditions. Thus, the objective of the present study was to evaluate the analgesic effect of low-dose naltrexone on facial mechanical allodynia in a rat model of TN, as well as its effect(s) on biomarkers in the central nervous system (tumor necrosis factor-alpha, brain-derived neurotrophic factor [BDNF], in

[A case of delayed facial palsy following gamma knife radiosurgery for intractable trigeminal neuralgia].

A 77-year-old woman with hypertension and senile depression had suffered from medically unresponsive trigeminal (left ophthalmic) neuralgia despite microvascular decompression surgery for twice. The patient underwent stereotactic gamma knife radiosurgery (77 Gy) for the neuralgia, resulting in pain relief. However, approximately 20 months after the radiosurgery, she developed left facial palsy with hydrodipsia, left xerophthalmia, and left facial hypesthesia. Oral prednisolone was administered, and these symptoms disappeared in several months. This is the first report of facial palsy following

[Partial vertical nucleotomy. A neurosurgical intervention in some forms of therapy-resistant facial neuralgia].

The partial vertical nucleotomy is presented as a new neurosurgical method for the treatment of therapy-resistant pain in the spreading area of the N. trigeminus. Strict indications are laid down which are mainly restricted to therapy-resistant pain in the above mentioned spreading area in the presence of diffusely growing malignant tumours, after traumas and infections and predominantly in forms of idiopathic trigeminal neuralgia that has been treated without any results over a period of many years. In the experimental part of the article; material and method as well as results of anterograde

Chronic Facial Pain: Trigeminal Neuralgia, Persistent Idiopathic Facial Pain, and Myofascial Pain Syndrome-An Evidence-Based Narrative Review and Etiological Hypothesis.

Trigeminal neuralgia (TN), the most common form of severe facial pain, may be confused with an ill-defined persistent idiopathic facial pain (PIFP). Facial pain is reviewed and a detailed discussion of TN and PIFP is presented. A possible cause for PIFP is proposed. (1) Methods: Databases were searched for articles related to facial pain, TN, and PIFP. Relevant articles were selected, and all systematic reviews and meta-analyses were included. (2) Discussion: The lifetime prevalence for TN is approximately 0.3% and for PIFP approximately 0.03%. TN is 15-20 times more common in persons with mul

Dandy's vein decompression for trigeminal neuralgia: A rare venous conflict causing intractable facial pain.

BACKGROUND: Trigeminal neuralgia (TN) is predominantly caused by arterial compression of the trigeminal nerve; however, venous conflicts, particularly involving Dandy's vein and the superior petrosal sinus (SPS), represent an uncommon but significant etiology. CASE DESCRIPTION: We report a case of a 40-year-old female suffering from debilitating right-sided facial pain due to a rare "scissor effect" produced by the superior petrosal sinus and Dandy's vein (vein of the cerebellomesencephalic fissure) compressing the trigeminal nerve. Surgical decompression was performed by interposing autologo

CT-Guided Stellate Ganglion Pulsed Radiofrequency Stimulation for Facial and Upper Limb Postherpetic Neuralgia.

Objective: Postherpetic neuralgia (PHN) is the most common complication of herpes zoster, manifesting as a persistent, spontaneous, knife-like pain or paroxysmal burning that seriously affects a patient's quality of life. An effective treatment of PHN is lacking. This retrospective study examined the efficacy and safety of stellate ganglion (SG) pulsed radiofrequency (PRF) on facial and upper limb PHN. Methods: Eighty-four patients with PHN on the face or upper limbs were enrolled for the study. Patients were randomly divided into two surgical groups according to the order of enrollment; one g

Trigeminal neuralgia and atypical facial pain: use of the McGill Pain Questionnaire for discrimination and diagnosis.

Although trigeminal neuralgia and atypical facial pain can usually be distinguished on the basis of several well-established criteria, differential diagnosis is difficult in a small number of patients. The purpose of this study was to determine whether the McGill Pain Questionnaire (MPQ) is capable of discriminating between the two syndromes and might therefore serve as a diagnostic aid. The MPQ was administered to 74 patients in Pittsburgh and the data were analyzed statistically in Montreal. In an analysis of the data of 53 patients, 91% of the patients were correctly classified on the basis

Facial lymphatic malformation: the root cause of trigeminal neuralgia is not its root. Illustrative case.

BACKGROUND: Trigeminal neuralgia (TN) is commonly caused by a vascular compression of the trigeminal nerve (classical TN). Due to the high frequency of this compression at the nerve's root entry zone, a misunderstanding has arisen in the literature that this location is the only site that can cause TN. The present case demonstrates that TN can be caused by compression anywhere along the nerve-even in the face. OBSERVATIONS: A 15-year-old female presented with medically refractory left TN. MRI revealed no neurovascular conflict at the root entry zone, but ultrasound revealed a lymphatic malfor

Trigeminal root demyelination is sufficient to induce trigeminal neuralgia-like facial pain in adult rodents.

BACKGROUND: Current etiology-centered management guidelines have not brought about satisfactory outcomes for trigeminal neuralgia patients. Therefore, there is an urgent call for pathology-targeted diagnosis and treatment modalities for this intractable disease. Trigeminal root demyelination has been frequently shown or suggested in trigeminal neuralgia patients of diverse etiologies and then naturally regarded as the common pathological cause of this disease. However, this causal relationship has never been validated so far with success in either preclinical or clinical studies. One of the ke

Daily olive oil intake is feasible to reduce trigeminal neuralgia facial pain: A pilot study.

Extra virgin olive oil (EVOO) is thought to contribute to neuroprotection and, thus, may influence pain symptoms experienced by adults with demyelination-related trigeminal neuralgia (TN). This study aimed to determine the feasibility of daily intake of EVOO and its potential to alleviate facial pain of TN. Adults, self-reporting as female and affected by TN, were enrolled in a 16-week nonblinded, parallel study. After a 4-week baseline, participants were randomized to 60 mL/day EVOO or control (usual diet and no supplemental EVOO) for 12 weeks. Participants completed a daily questionnaire on

Peripheral nerve field stimulation for trigeminal neuralgia, trigeminal neuropathic pain, and persistent idiopathic facial pain.

OBJECTIVE: Peripheral nerve field stimulation (PNFS) is a promising modality for treatment of intractable facial pain. However, evidence is sparse. We are therefore presenting our experience with this technique in a small patient cohort. METHODS: Records of 10 patients (five men, five women) with intractable facial pain who underwent implantation of one or several subcutaneous electrodes for trigeminal nerve field stimulation were retrospectively analyzed. Patients' data, including pain location, etiology, duration, previous treatments, long-term effects and complications, were evaluated. RE
