A family practice residency cervical screening project: perceived screening barriers.

BACKGROUND AND OBJECTIVES: A community-based family practice residency program initiated a cervical screening project to provide free Pap smears to women who had not had one in 3 years. The research identified reasons why patients had not been screened within the past 3 years. METHODS: A total of 214 consecutive participants in the free Pap smear clinic completed questionnaires. The questionnaire asked about reasons why screening had not occurred over the last 3 years. RESULTS: Altogether, 65.4% of the study group reported cost-related factors as barriers to participation; 37.9% cited either

["Silent" areas of the brain--a challenge in family practice].

Based on two medical histories found among complaints dealt with by the Norwegian Board of Health the author points out the main symptoms of frontal or temporal lobe failure. Following a history of three weeks one of the patients died as a result of an overlooked cerebral abscess. During the history three different doctors had made wrong diagnostic suggestions, mainly because of the coincidence of an influenza epidemic in the neighbourhood. The other patient died from a cerebral tumour. In his history epilepsy was the main symptom for many months. This diagnosis was overlooked because the doct

Adult immunization in a network of family practice residency programs.

A substantial proportion of morbidity and mortality associated with vaccine-preventable diseases occurs among adults. Teaching residents about disease prevention is mandated in the curriculum guidelines for family practice programs. A cooperative study among the Kansas City family practice residency programs was begun to look at immunization behaviors in these teaching programs. A retrospective audit of medical records and a prospective survey of residents and faculty were performed. From the medical records of 400 patients seen for health maintenance examinations, the frequency of tetanus-dip

Diphenhydramine in insomniac family practice patients: a double-blind study.

One hundred eleven mildly to moderately insomniac patients participated in a double-blind, placebo-controlled study to establish the efficacy of diphenhydramine as an over-the-counter (OTC) sleep aid. A two-week crossover design was employed in which all patients received both diphenhydramine and placebo for one week each. The daily diphenhydramine dose was 50 mg at bedtime. Results obtained indicate that diphenhydramine improved various sleep parameters, including sleep latency, to a significantly higher degree than did placebo. In addition, patients on diphenhydramine reported feeling more r

Causes of abnormal vaginal bleeding in a Family Practice Center.

Abnormal vaginal bleeding had an incidence of 20.0 per 1,000 woman-years in patients of the Kanawha Valley Family Practice Center. Contraception-related bleeding was the most common cause, accounting for 28 percent of the cases. Anovulatory and dysfunctional uterine bleeding were the next most common causes, accounting for 15 percent and 13 percent, respectively. All three causes were uncommon in women aged 41 to 55 years, and absent in women older than 55 years. Perimenopausal bleeding, the dominant cause of abnormal vaginal bleeding in the 41-to-55-year group, was also absent in women older

Complications associated with IUD use in a family practice setting.

This study compares the complication rate of intrauterine contraceptive devices (IUD) with other contraceptive measures in a residency practice. The study population included 220 randomly selected women who had IUDs inserted by residents over a five-year period. One hundred similarly selected women started on birth control pills (BCP) were used as a control group. Of the IUD patients, 8.6 percent developed pelvic inflammatory disease vs 2 percent of the BCP patients. The incidence of gonorrhea was not significantly different between the two groups: 8.2 percent for the IUD groups vs 7 percent f

Establishing a practice based primary care research network. The University Family Practice Network in South Australia.

BACKGROUND: Practice based research networks enable the study of primary care problems in a primary care setting. To be successful, our experience indicates that a network will need to incorporate a number of key components. In South Australia, a University Family Practice Network has been established to undertake primary health care research and teaching and to contribute to the general practice workforce. The University Family Practice Network is composed of five practices and is managed jointly by the Department of General Practice, Adelaide University and the South Australian Centre for Ru

Comparing women's views on family and sexual problems in family and gynecological practices.

This study compares the role of community gynecologists and family physicians, seen from the patients' perspective. A sample of 126 female patients in family practices and 212 patients in gynecological practices were surveyed about their preferences and expectations of whether their doctor should address personal, family and sexual problems. The effects of covariates (such as the type of doctor, patient age, family status, child desire) on patient expectations were simultaneously assessed by multiple logistic regression. Between 40% and 70% of the patients contacted confirmed the family-orient

Family practice maternity care in America: ruminations on reproducing an endangered species--family physicians who deliver babies.

BACKGROUND: The majority of family physicians do not deliver babies. One reason might be the family physician's intrinsic comfort with person- or patient-centered care compared with the common obstetric approach of disease or physician-centered maternity care. Another reason might be the uncritical intrusion of technology into maternity care. In addition, family physicians often are made to feel unwelcome in many maternity care systems. METHODS: The medical literature from 1984 to 1994 was searched for the topics of obstetrics, maternity care, family-centered birthing, and family practice edu

"Family-as-Root" and "Family-Centeredness" in hospice care: a concept analysis and implications for nursing practice.

BACKGROUND: Families are widely recognised as central actors in hospice and palliative care, and “family-centered care” has become a dominant practice model. However, the ontological and axiological status of the family in many cultural contexts—particularly East Asian Confucian traditions—extends far beyond the functional roles usually described by family-centeredness. This study conceptualizes and clarifies “Family-as-Root” - a culturally grounded, ontological-axiological orientation to family that is widely recognizable in Chinese contexts but remains under-theorized in international hospic

Death notification in clinical practice: protocols, clinician impact and family outcomes.

Death notification (DN) is a critical clinical task that initiates and influences the bereavement trajectory and carries profound implications for clinician well-being. Evidence links DN quality to cognitive-emotional processing of loss, attachment reorganisation and mobilisation of social support. A diverse ecosystem of DN protocols offers stepwise guidance, but outcome data, cultural adaptation and notifier support remain limited. Drawing on DN protocol methodology, DN and prolonged grief disorder mechanisms and contemporary grief theory, this State of the Science article argues that DN shou

Adherence to opioid prescribing guidelines at an academic family medicine practice.

INTRODUCTION: Opioid prescribing practices and guidelines are outlined by the United States Centers for Disease Control and Prevention (CDC), but limited data are currently available regarding their use in clinical practice. A primary care residency clinical site at an academic medical center attempted to improve compliance with the CDC best practices in 2022. As a quality improvement initiative, a policy was created, and education was provided to clinicians and patients. The clinical impact of these interventions was analyzed. METHODS: A retrospective chart review was performed for patients

Engaging suffering: towards a mindful re-visioning of family therapy practice.

The myth of problem-free living is easily identifiable in Western culture through its childhood fairy tales and modern love stories. Even if inadvertently, the field of mental health perpetuates this myth with the very concept of "mental health," which implies a state without suffering. Using mindfulness, Buddhists assume an alternative stance towards suffering: rather than eliminate it, they compassionately engage it. This article explores the potentials of mindfulness as an alternative approach to suffering within the context of marriage and family therapy. Family therapists can integrate mi

Experience with routine vaginal pH testing in a family practice setting.

BACKGROUND: Despite recommendations by Centers for Disease Control and the American College of Obstetricians and Gynecologists, pH testing is infrequently performed during the evaluation of vaginitis. Consequently, little information exists on its use in a primary care setting. OBJECTIVE: The aim of this study was to describe our experience with routine pH testing, particularly the relationship between symptoms, pH and wet-mount microscopy. METHOD: A retrospective chart review was performed on 203 consecutive cases evaluated for vaginitis by wet-mount microscopy. RESULTS: Of the 203 cases,

The role of bipolarity in depression in the family practice setting.

The literature suggests that bipolar spectrum disorders are more prevalent than previously thought but still poorly recognized. In the primary care setting, this poor recognition is largely the result of an insensitive, cross-sectional approach and clinicians' lack of familiarity with the phenomenology of bipolar II. Failure to recognize the role of bipolarity in depressive illness is more often a cause of the poor outcome of this illness in this setting than under dosing with antidepressants. Hypomania is easily missed in clinical evaluations and, as currently defined by DSM-IV, may not repre
