Predictors of the development of febrile neutropenia in chemotherapy for advanced germ cell tumors.

OBJECTIVES: To identify the predictive factors for the development of febrile neutropenia (FN) in the course of chemotherapy for patients with germ cell tumors. METHODS: From January 2005 to December 2018, 80 patients were treated with induction chemotherapy for advanced germ cell tumors at Kanagawa Cancer Center Hospital, Japan. Of these, we retrospectively analyzed 267 cycles of chemotherapy. The incidence of FN was used as the objective variable. As predictive factors, we analyzed age, international germ cell consensus classification (IGCCC), laboratory data at the start of chemotherapy in

Association of Vancomycin Trough Concentration and Clearance With Febrile Neutropenia in Pediatric Patients.

BACKGROUND: Febrile neutropenia promotes renal drug excretion. Adult and pediatric patients with febrile neutropenia exhibit a lower vancomycin concentration/dose (relative to bodyweight) ratio than those with other infections. In pediatric patients, renal function relative to bodyweight varies depending on age, and vancomycin clearance is age dependent. This study aimed to analyze the effects of febrile neutropenia on the pharmacokinetics of vancomycin in age-stratified pediatric patients. METHODS: This retrospective, single-center, observational cohort study analyzed 112 hospitalized pediat

Cost-effectiveness of home-based care of febrile neutropenia in children with cancer.

INTRODUCTION: Home-based treatment of febrile neutropenia (FN) in children with cancer with oral or intravenous antibiotics is safe and effective. There are limited data on the economic impact of this model of care. We evaluated the cost-effectiveness of implementing an FN programme, incorporating home-based intravenous antibiotics for carefully selected patients, in a tertiary paediatric hospital. METHODS: A decision analytic model was constructed to compare costs and outcomes of the home-based FN programme, with usual in-hospital treatment with intravenous antibiotics. The programme include

The role of procalcitonin and presepsin in the septic febrile neutropenia in acute leukemia patients.

BACKGROUND: The source of bacterial infection in neutropenic acute leukemia patients is detected in about 20-30% of cases. Bacterial cultures may require a long incubation period and risk false-positive and false- negative results. Therefore, biomarkers distinguishing septic febrile neutropenia from other etiologies in acute leukemia patients play the important role in patient assessment and treatment planning. This study aims to determine the role of procalcitonin (PCT) and presepsin (PSPN) in infectious complication in comparison to C-reactive protein (CRP) on the first and third day at the

Evaluation of a risk-guided strategy for empirical carbapenem use in febrile neutropenia.

Febrile neutropenia (FN) is associated with substantial morbidity and necessitates empirical broad-spectrum antimicrobial treatment. In this prospective cohort study, a risk-guided management strategy for FN using empirical piperacillin/tazobactam (TZP) or a carbapenem was evaluated. The analysis involved 723 FN episodes in hospitalised adult patients, including those with severe sepsis or prior infection/colonisation with extended-spectrum β-lactamase (ESBL)-producing Enterobacteriaceae. Propensity score matching analysis was used to adjust for baseline differences between treatment groups an

Febrile neutropenia in a metastatic melanoma patient treated with ipilimumab - case report.

BACKGROUND: Ipilimumab is a fully human monoclonal antibody (mAb) targeting cytotoxic T-lymphocyte antigen-4 (CTLA-4). Ipilimumab is currently approved in the U.S. and Europe for the treatment of metastatic melanoma in the first- and second-line treatment. Treatment with ipilimumab is linked to immune-related adverse events (irAEs) occurring in the majority of patients. These specific AEs include dermatitis, gastrointestinal disorders (diarrhea, colitis), hepatitis, hypophysitis, hypothyroidism, neuropathy, and iritis/inflammation of the ciliary body. CASE REPORT: We report a case of febrile

Metagenomic profiling of blood-associated microbial DNA signatures in leukemia-associated febrile neutropenia.

Febrile neutropenia (FN) is a life-threatening complication of chemotherapy, but the low microbial biomass of blood makes shotgun metagenomic profiles highly sensitive to technical background. We reanalyzed 47 publicly available patient sequencing runs representing 43 unique patient-timepoint samples from 19 SRA-labeled patients, together with 23 no-template-control (NTC) runs spanning 21 sequencing batches. To distinguish reference-catalogue content from progressively stronger evidence of patient-associated signal, we applied batch-matched NTC correction together with nested abundance thresho

Cost-Effectiveness of Early Empirical Antibiotic Cessation in Pediatric High-Risk Febrile Neutropenia.

PURPOSE: Early discontinuation of empirical antibiotic therapy (EAT) is non-inferior to continuation of EAT until absolute neutrophil count (ANC) ≥500 cells/mm3 for children with high-risk febrile neutropenia (FN). In this prospective trial-based economic evaluation, we aimed to estimate the healthcare resource utilization and cost-effectiveness of early discontinuation of EAT versus continuation of EAT in children with high-risk FN in India. PATIENTS AND METHODS: Clinically stable children aged 2-18 years with acute leukemia, high-risk FN, and afebrile status for ≥72 h were randomized to sto

Early discharge for low-risk pediatric febrile neutropenia: safe only in selected centers.

Marathe et al. report encouraging results from a step-down pathway for low-risk fever and neutropenia in pediatric and adolescent-young adult oncology patients, with reduced hospitalization and no sepsis or infection-related mortality. Their work is an important contribution to risk-adapted supportive care. This Article Commentary argues, however, that early discharge is not a universally portable intervention. Its safety depends not only on patient biology and initial clinical stability, but also on the surrounding health-care infrastructure: rapid microbiology, timely antibiotics, 24-h oncol

Timing and clinical burden of febrile neutropenia and treatment discontinuation during neoadjuvant chemotherapy.

BACKGROUND: Febrile neutropenia (FN) remains a major complication of neoadjuvant chemotherapy (NAC) and is a frequent cause of treatment disruption. However, FN is a heterogeneous clinical entity, and the impact of its timing and clinical severity on treatment continuity has not been fully elucidated. This study aimed to characterize the timing and clinical burden of FN and to clarify their associations with treatment discontinuation during NAC. METHODS: We retrospectively analyzed 137 consecutive patients with HER2-positive breast cancer who received NAC. FN timing was assessed by chemothera

Comparison of vancomycin and teicoplanin for febrile neutropenia following hematopoietic stem cell transplantation.

Febrile neutropenia is a common complication of hematopoietic stem cell transplantation. Although anti-MRSA agents such as vancomycin and teicoplanin are widely used for treatment, their clinical efficacy and safety during the early post-transplant period remain unclear. In this study, we retrospectively compared the shortterm mortality and safety of vancomycin and teicoplanin in patients with febrile neutropenia during the early post-transplantation period. This study analyzed patients who received vancomycin or teicoplanin injections for febrile neutropenia within 30 days of hematopoietic st
