From the lumbar spine to foot, the joints of the lower extremity are all intimately connected. Their movements are synchronized in a complex biomechanical dance. Pain in one joint tends to affect the joint above and joint below. Understanding the influence of adjacent-joint disease on the hip can help us better counsel patients undergoing hip surgery. Low-back pathology has been shown to negatively influence outcomes after hip arthroscopy. Ipsilateral knee pain appears to do the same. Patients with femoroacetabular impingement syndrome with ipsilateral knee pain should be counseled that their

Research to investigate causes for continued symptoms in patients who undergo hip arthroscopic treatment is an opportunity to improve outcomes. A disconcertingly large number of patients (47%-66%) show a joint effusion/synovitis on magnetic resonance imaging at 6 to 12 months after hip arthroscopy for femoroacetabular impingement syndrome, and a recent study shows that this is associated with inferior 2-year clinical outcomes compared to patients without effusions. Perhaps the effusions are associated with comorbid structural abnormalities such as cartilage degeneration, capsule defect, labral

Several mechanisms either support or decrease stability of the hip joint. Primary stability of the hip comes from bony coverage of the femoral head, influenced by acetabular version and femoral antetorsion. In addition, soft tissue structures, such as the acetabular labrum, the ligamentum teres, and the hip capsule play a significant role in maintaining joint stability. Untreated hip instability may lead to pathological force transmission between the acetabular socket and femoral head, and subluxation resulting in osteoarthritis. Historically, pelvic and/or femoral osteotomies have been perfor

INTRODUCTION: The radiological study of the sagittal alignment of the spine in static and dynamic positions has allowed a better understanding of the clinical results of total hip prostheses. According to the Roussouly classification, the sacral slope in a standing position characterizes the patient with a stiff spine (less than 35̊) or a flexible spine (greater than 35̊). The objectives of this study were to compare, in a population of patients operated on for femoroacetabular impingement (FAI), firstly, the kinematics of the lumbar-pelvic-femoral complex and secondly, the clinical presentati

INTRODUCTION: With the recent interest in hip-spine relationship, the link between femoroacetabular impingement (FAI) and the lumbar-pelvic-femoral complex (LPFC) appears decisive. The objectives of this study were (1) to compare the pelvic version, as well as (2) all the parameters of the LPFC between two populations of healthy subjects, comparing those with radiological signs of FAI to those without and finally, (3) to evaluate the LPFC parameters according to the type of FAI.
HYPOTHESIS: Asymptomatic subjects with radiological signs of FAI had superior pelvic anteversion.

BACKGROUND: Acetabular morphology is an important determinant of hip biomechanics. To identify features of acetabular morphology that may be associated with the development of hip symptoms while accounting for spinopelvic characteristics, one needs to determine acetabular characteristics in a group of individuals older than 45 years without symptoms or signs of osteoarthritis. Previous studies have used patients with unknown physical status to define morphological thresholds to guide management.
QUESTIONS/PURPOSES: (1) To determine acetabular morphological characteristics in males and females

BACKGROUND: Femoroacetabular impingement syndrome (FAIS) is one of the most common prearthritic hip deformities. Since FAIS is a mechanical pathology, surgical correction of the underlying deformity is the sole causal treatment. If surgery is indicated, a surgical technique that results in complete deformity correction with least morbidity should be selected.
ARTHROSCOPY: Due to advancements in techniques and instruments, most pathologies in FAIS can nowadays be addressed arthroscopically. Hip arthroscopy can be successfully performed if the locations of the pathologies are anterior and later

Patients with femoroacetabular impingement syndrome (FAIS) often have extra-articular disorders, such as external snapping hip (ESH). We recommend that obvious ESH be addressed by endoscopic transversal iliotibial band (ITB) release during hip arthroscopy for FAIS because the residual serious snapping caused by ESH negatively affects the outcome of hip arthroscopy. However, for mild ESH without indications for severe trochanteric bursitis on magnetic resonance imaging, we still propose that physical therapy, extracorporeal shock wave therapy, or local injection be performed for pain relief. Intra-articular injuries are common after dislocation and fracture of the hip joint and can be addressed using hip arthroscopy. The most common indications for this procedure are loose bodies, labral tears and chondral defects. In addition, preexisting femoroacetabular impingement can be addressed at the time of surgery. Arthroscopically guided fracture reduction and fixation has been described. We present two case reports of intra-articular lesions after traumatic hip dislocation. The first is a case of a man with an anterior labral tear and loose bodies after closed hip reduction. The second

Altered hip loading and biomechanics in individuals with femoracetabular impingement syndrome (FAIS) may affect the joint's habitual mechanical environment, potentially increasing the risk of osteoarthritis . Examining differences in contributions of muscle and external loads (i.e., gravitational and intersegmental-inertial forces) to hip contact forces, compared with controls, may aid our understanding of FAIS pathomechanics and assist with the development of more effective treatments. Whole-body motion and electromyograms of 14 lower limb muscles were acquired from 41 participants with FAIS

BACKGROUND: Femoroacetabular impingement (FAI) is a common cause of hip pain in adolescents and may lead to osteoarthritis. While conservative treatments are the first-line option, arthroscopy offers a minimally invasive approach to repairing labral tears and correcting bone deformities. This study analyzes the outcomes of patients under 18 years old who underwent hip arthroscopy for FAI in a multicenter retrospective cohort with a minimum 5-year follow-up.
METHODS: This was a retrospective multicenter study. Patients under 18 with a confirmed FAI diagnosis and a minimum follow-up of 5 years

INTRODUCTION: Arthroscopic and mini-open surgery are both valid options for managing symptomatic femoroacetabular impingement (FAI). The current systematic review compared the clinical outcomes of arthroscopic surgery versus mini-open surgery for femoroacetabular impingement. The primary outcomes assessed included patient-reported outcome measures (PROMs) and the incidence of complications.
MATERIAL AND METHODS: This systematic review followed the PRISMA statement. The Web of Science, PubMed, and Embase were accessed in June 2025 without additional filters or temporal constraints. All clinica

The hip joint is a complex structure in both form and function, which presents challenges for accurate diagnosis and treatment of pathology. Assessing acetabular version on plain radiographs can be particularly challenging as this requires extrapolation of 3-dimensional anatomy from 2-dimensional radiographic markers. The limitations inherent with plain radiographs of the hip have contributed to the increased use of advanced imaging including computed tomography scan and magnetic resonance imaging in the hip preservation world. Modern computed tomography scans provide incredibly detailed evalu

Hip arthroscopy is increasingly used in the management of femoroacetabular impingement syndrome (FAIS), yet the definition of treatment failure remains unsettled. In response to the recent study by Mygind-Klavsen et al., we argue that conversion to total hip arthroplasty (THA), although clinically important, is a late and highly selective endpoint that does not capture the broader spectrum of unsuccessful hip preservation. Patients who remain symptomatic, fail to achieve clinically meaningful improvement, or undergo revision without THA are not represented by arthroplasty conversion alone. We
