Femoral head and ipsilateral femoral neck fractures are serious and complicated injuries, which usually yield unsatisfactory results using conventional hip-preserving surgery. The key point of the management and prognosis mainly lies in femoral neck fractures. An apparent and consecutive relationship exists between femoral neck fractures and femoral head fracture-hip dislocation in such injuries. It is believed that disastrous triad of femoral head (DTFH) could summarize these specific injuries, and reflect the injury mechanism and prognostic characteristics. Based on our clinical observation

Recent studies indicating a relationship between duration of bisphosphonate therapy and risk of atypical femoral fractures, together with epidemiological data demonstrating divergent trends in hip versus subtrochanteric fracture incidence, support the contention that there is an association between bisphosphonate therapy and atypical femoral fractures. However, although there are biologically plausible mechanisms by which bisphosphonates may impair bone strength, a direct causal association has yet to be established. The vast majority of patients treated with bisphosphonates do not develop aty

BACKGROUND: An ipsilateral fracture of the femoral neck is seen in association with 1% to 9% of femoral shaft fractures, and 20% to 50% of these injuries are missed initially. Recognition of an associated femoral neck fracture prior to stabilization of the femoral shaft fracture is imperative to avoid or minimize complications of displacement and osteonecrosis.
METHODS: A protocol to look for a femoral neck fracture in all patients with a femoral shaft fracture was instituted at a single level-I trauma center. This protocol consisted of a dedicated anteroposterior internal rotation plain radi

BACKGROUND: Periprosthetic femoral fractures (PFFs) are serious complications in hip arthroplasty for femoral neck fractures. The rates of intraoperative (iPFFs) and postoperative PFFs (pPFFs) are higher in cementless stem cases than in cemented cases. This study aimed to investigate the effects of cerclage cabling on PFF prevention in cementless arthroplasty for femoral neck fractures.
METHODS: This retrospective study included 329 consecutive patients who underwent hip arthroplasty using a cementless stem for femoral neck fractures. A total of 159 and 170 patients were in the non-cabling an

A series of 110 patients with 114 fractures of the femur were reviewed an average of 3.9 years after injury. Demonstrable knee ligament laxity was present in 31 (27 per cent) of these patients, while 13 (11 per cent) complained of instability. Thirty-three patients with 34 ipsilateral femoral and tibial shaft fractures were examined an average of 3.7 years after injury. Demonstrable knee ligament laxity was present in 18 (53 per cent) of these patients, while 6 (18 per cent) complained of instability. Most of the patients with instability had a rupture of the anterior cruciate ligament with or

The following distinctions can be made between fractures of the femoral neck in elderly persons and similar fractures in younger patients: They are much more common in the elderly. They are more frequent in females than in males. There is a much higher incidence in the white than in the black population. Fractures of the opposite hip are more common in the elderly. Associated medical conditions in the elderly may make the patient more susceptible to falls and may increase the degree of osteopenia. There is a high rate of mortality in elderly patients within 1 year of the fracture. Minor trauma

Femoral head perfusion in adults was studied by superselective i.a. DSA in 35 normal subjects, in 9 patients with medial femoral neck fracture preoperatively, and in 36 patients with femoral head necrosis before bone grafting, and as therapeutic control after grafting. All patients with femoral neck fracture showed an interruption of the proximal nutrient branches. In 93% of the cases with femoral head necrosis, these branches were rarefied or the medial circumflex femoral artery was interrupted. Postoperative DSA after pedicled pelvic bone graft revealed a regularly perfused graft in 82%. Sup

OBJECTIVE: To assess ballistic femoral shaft fracture outcomes in comparison with closed and open femoral shaft fractures sustained by blunt mechanisms. We hypothesized that ballistic femoral shaft fractures would have similar outcomes to blunt open fractures.
DESIGN: Retrospective cohort study.
SETTING: Academic Level I trauma center.
PARTICIPANTS: Patients 16 years and older presenting with ballistic (140), blunt closed (317), or blunt open (71) femoral shaft fractures.
MAIN OUTCOMES: Unplanned return to operating room, fracture-related infection, soft tissue reconstruction, nonunion, im

INTRODUCTION: Hardware removal in healed trochanteric fractures (TF) in the absence of infection or significant mechanical complications is rarely indicated. However, in patients with persistent pain, prominent material and discomfort in the activities of daily living, the implant is eventually removed. Publications of ipsilateral femoral neck fracture after removal of implants from healed trochanteric fractures (FNFARIHTF) just because of pain or discomfort are rare. The purpose of this systematic review of the literature is to report on the eventual risk factors, the mechanisms, the clinical

We present the case of a 39-year-old sportswoman who was involved in a motorcycle accident in 2012. She sustained a complex proximal femoral fracture that involved the neck and trochanteric region. We decided to preserve the natural bone stock and repair the fractures, adopting skeletal traction in the first stage and then implanting an intramedullary nail. Twenty months later the fracture was healed clinically and radiologically and the patient was able to walk and run without pain.

Fractures of the femoral neck in childhood and adolescence are a rare accident injury. Despite their rarity they are significant for the individual child because of the probability of complications, reported in up to 60% of the cases in the literature. They are caused on the one hand by direct damage to the growth joint, which according to recent studies is regarded as a single, continuous, transverse growth plate which includes the trochanteric joint; and on the other by damage resulting from a circulation disorder. Most authors advocate emergency surgical treatment to relieve the hemarthrosi

PRESENTATION: We report a case of a 60-year-old male with bilateral lower limb amputations who sustained a fracture to his right residual femur after weight bearing on his prosthetic leg during therapy. This was attributed to his raised body mass index that fell in the morbidly obese category.
DIAGNOSIS: Patient was referred for orthopaedic management and reviewed by the multidisciplinary prosthetic rehabilitation team following the fracture recovery.
TREATMENT: He was deemed unsuitable for further prosthetic usage and ultimately referred to the national weight management service and the bar

Hemiarthroplasty is a common treatment for femoral neck fractures in the elderly population. The main complications are periprosthetic dislocation and infection, which potentially impact morbidity and quality of life and may contribute to mortality. This procedure can be technically demanding, and adequate closure of the capsule and soft tissue cannot be emphasized enough. One advantage of a bipolar prosthesis is that it can be easily converted to a total hip arthroplasty without replacing the femoral component and with approximately the same complication rates as a revision total hip arthropl

The femoral neck fractures in children are very rare. It is difficult to elaborate a standard of treatment of this disorder in children, because of a little amount of publications on this subject. The common complications which occur are physis premature closure, coxa vara, avascular necrosis of the head and neck of femur and ocasionally nonunion. The paper analyses 18 cases of fracture in children at the age of 4 to 16 years old (average 8 years old) who were treated at the clinic since 1990 to 2003. Cohort of patients included 7 boys and 11 girls. Fractures were classified according to Delbe

Femoral diaphyseal fractures usually result after trauma of high magnitude and because of this, can be life-threatening injuries or may result in considerable physical disability if not treated with care and caution. Nonoperative treatment of these fractures continues to be popular among the patient population in the Indian subcontinent, which in majority of cases, leads to healing in malalignment, shortening of the limb, chondromalacia patellae, and loss of knee motion. Although the majority of these fractures are being treated by operative methods today, success of the treatment depends larg

Fifty-four femoral neck fractures in young adults (20-50 years old; Group I) were compared with 397 fractures in the elderly population (older than 60 years of age; Group III). Patients in Group I were more frequently men and had often sustained vertical shear fractures in high-energy injuries. Treatment consisted mostly of open reduction and internal fixation (88%). There were no infections, pulmonary embolisms, or deaths. The rate of avascular necrosis was 10%; the rate of delayed union or nonunion, 17%; and the revision rate, 20%. Reoperation did not preclude an excellent final outcome. Pat
