Potentials and limitations of peripheral nerve injury models in rodents with particular reference to the femoral nerve.

Restoration of function after peripheral nerve repair in humans is unsatisfactory. Various causes of poor recovery have been proposed. Still, we do not understand which of these potential factors are indeed detrimental and do not know how to manipulate them experimentally in a clinically feasible way. Future success largely depends on methodological improvement in rodent models. An example of recent progress is the introduction of new functional and anatomical outcome measures in the facial nerve injury paradigm which led to novel insights into facial nerve regeneration and a new therapeutic c

Sural nerve grafting for long defects of the femoral nerve after resection of a retroperitoneal tumour.

Most injuries to the femoral nerve are iatrogenic in origin and occur during resection of large retroperitoneal tumours. When the defect is considerable a nerve graft is mandatory to avoid tension across the suture line. We describe two cases of iatrogenic femoral nerve injury which recovered well after reconstruction with long sural nerve grafts. The probable reasons for success were that we performed the grafting soon after the injury, the patients were not too old, the nerve repairs were reinforced with fibrin glue and electrical stimulation of the quadriceps was administered to prevent mus

Nerve conduction studies of the lateral femoral cutaneous nerve. Implications in the diagnosis of meralgia paresthetica.

The diagnosis of meralgia paresthetica, while based on clinical suspicion, can be confirmed via electrophysiological testing. Previous testing of the sensory conduction of the lateral femoral cutaneous nerve placed the stimulating electrode one centimeter medial to the anterior superior iliac spine (ASIS). This position is both uncomfortable for the patient and difficult for the operator to perform. The following article suggests an alternate position for the stimulating electrode, four centimeters distal to the ASIS. Twenty subjects served as their own controls and both the old technique and

Facial nerve reconstruction using vascularized nerve grafts based on the lateral femoral circumflex vascular system.

AIM: When unilateral facial nerve resection is performed for tumor resection, immediate reconstruction is desirable to achieve more natural recovery of facial expressions. We performed facial nerve reconstruction using vascularized nerve grafts based on the lateral femoral circumflex vascular system. Herein, we describe the details, advantages, and challenges associated with this technique. PATIENTS AND METHODS: We evaluated 9 patients who underwent reconstruction using a vascularized nerve graft harvested from the lateral femoral circumflex vascular system. All patients had at least 1 year o

Ultrasound-guided approach for axillary brachial plexus, femoral nerve, and sciatic nerve blocks in dogs.

OBJECTIVE: To describe an ultrasound-guided technique and the anatomical basis for three clinically useful nerve blocks in dogs. STUDY DESIGN: Prospective experimental trial. ANIMALS: Four hound-cross dogs aged 2 +/- 0 years (mean +/- SD) weighing 30 +/- 5 kg and four Beagles aged 2 +/- 0 years and weighing 8.5 +/- 0.5 kg. METHODS: Axillary brachial plexus, femoral, and sciatic combined ultrasound/electrolocation-guided nerve blocks were performed sequentially and bilaterally using a lidocaine solution mixed with methylene blue. Sciatic nerve blocks were not performed in the hounds. After t

Ultrasound-guided femoral nerve block combined with lateral femoral cutaneous nerve block in a patient with congenital insensitivity to pain and anhidrosis: a case report.

Congenital insensitivity to pain with anhidrosis (CIPA), also known as hereditary sensory and autonomic neuropathies (HSAN I-V), is an exceptionally rare autosomal recessive disorder. The pathogenesis of CIPA remains not fully elucidated. Clinical manifestations primarily include the absence of pain perception, painless injuries to the extremities, oral mutilation, anhidrosis accompanied by hyperthermia, and delayed intellectual development. Given the autonomic dysfunction in CIPA patients, previous reports on anesthetic management for this rare disorder have predominantly depended on general

Complete transection of the sciatic nerve following closed femoral fracture.

Complete transection of the sciatic nerve following femoral fracture is extremely rare. In the setting of closed injury it has only been reported in two other cases. Here we present a teenage motorcyclist who sustained a closed left, mid-femoral fracture following a road traffic collision with complete transection of the sciatic nerve. Despite being a closed injury, the obvious limb deformity of the patient and extreme pain prompted immediate nerve block during the primary survey making formal neurological assessment difficult. This case highlights the possibility of complete major nerve trans

Iliacus haematoma causing femoral nerve palsy: an unusual trampolining injury.

We report the case of a 15-year-old boy who presented to accident and emergency following a trampolining injury. Initially, the patient was discharged, diagnosed with a soft tissue injury, but he re-presented 48 h later with worsening low back pain and neurological symptoms in the left leg. Subsequent MRI revealed a left iliacus haematoma causing a femoral nerve palsy. The patient was managed conservatively and by 6 months post injury all symptoms had resolved. This is the first reported case of an iliacus haematoma causing a femoral nerve palsy, after a trampolining injury. We believe this ca

Complete femoral nerve resection with soft tissue sarcoma: functional outcomes.

BACKGROUND: The functional consequences of resecting the femoral nerve in conjunction with soft tissue sarcoma management are not well described. In comparison, sciatic nerve involvement by sarcoma was once considered an indication for amputation, but sciatic resection is now commonly performed as part of a limb-salvage approach. We compared functional outcomes following resection of either the femoral or sciatic nerve in patients with soft tissue sarcoma. We also compared both groups with patients with large thigh sarcomas without nerve involvement. METHODS: The prospectively collected datab

Femoral, saphenous nerve palsy after tourniquet use: a case report.

Persistent motor and sensory abnormalities after surgery may affect the rehabilitation process. Patients with continued weakness may be perceived as lacking motivation by health care providers. However, there may be an underlying pathophysiologic abnormality preventing patients from progressing through their rehabilitation programs. We report a case of a 20-year-old man who underwent surgical repair of multiple knee structures with the use of a pneumatic tourniquet. Several weeks after surgery, electromyographic evaluation was done because he was having difficulty in his rehabilitation because

[Femoral nerve paralysis after vaginal hysterectomy and its forensic importance].

Paresis of the femoral nerve following vaginal hysterectomy is a rare event, only 21 cases have been reported in the literature. This lesion is caused by pressure of the femoral nerve through the ligamentum inguinale during extreme flection and abduction of the thigh when vaginal surgery is done in lithotomy position. To avoid this femoral nerve lesion during vaginal hysterectomy the inclination angle between thigh and body should not be smaller than 45 degrees and the abduction angle should not exceed 45 degrees in relation to a vertical midline. The correct positioning of the patients has to

[Damage to the lateral cutaneous femoral nerve after transfemoral angiography].

We report on two patients with lesions of the lateral cutaneous nerve of the thigh after transfemoral angiography. Symptoms were hypesthesia, dysesthesia and hypalgesia of the right lateral thigh. One patient had no hematoma, the other a hematoma of the medial thigh which did not enlarge thigh circumference. Both patients had had a tight pressure bandage applied for 24 hours. There were no signs of a femoral nerve lesion. Both patients were severely distressed by paresthesia. The numbness subsided after three weeks in one patient, but pain persisted. The other patient had symptoms for over six

Bilateral iliacus haematoma with femoral nerve palsy complicating anticoagulant therapy.

A patient is reported who sustained bilaterial iliacus haematoma with femoral nerve palsy during treatment with constant intravenous infustion of heparin for deep venous thrombosis. She was promptly treated with operative decompression and recovered completely from the palsy. Daily examinations of the blood revealed that the plasma heparin concentration, activated partial thromboplastin time, APTT, and thrombin time all were above the therapeutic range at the time when the bleeding started, and before the initial symptoms occurred. Early operative decompression is considered to be the ideal tr

Right-sided meralgia paresthetica from lateral femoral cutaneous nerve neuroma.

Meralgia Paresthetica (MP) causes outer thigh discomfort, numbness, and tingling. Compression or injury to the Lateral Femoral Cutaneous Nerve (LFCN), which gives sensory innervation to outer aspect of the thigh. It frequently gets impinged beneath the inguinal ligament resulting in in sensory impairment in the distribution of LFCN. Compression of the LFCN is a frequent cause of MP, whereas LFCN neuromas is a rare cause. This case report describes a unique case of Meralgia Paresthetica produced by a LFCN neuroma and enhances our knowledge of Meralgia Paresthetica.

Posterior femoral cutaneous nerve impingement post marathon: A case report.

INTRODUCTION: Posterior femoral cutaneous nerve (PFCN) neuropathy is an uncommon, potentially under-recognized cause of low back, buttock, perineal, and posterior and lateral thigh pain and numbness. Most PFCN injuries are caused by intramuscular injections; however, this is the first reported case of a PFCN injury of hamstring pathology due to tendonitis caused by long-distance running. CASE REPORT: A middle-aged man presented with right lower buttock pain, which started 2 weeks after running a marathon. The pain was located medial and superior to the ischia tuberosity and extended distally
