Ljungan virus: a commentary on its association with fetal and infant morbidity and mortality in animals and humans.

Epidemiologic and experimental data support the notion that Ljungan virus (LV), endemic in some rodent populations in Sweden, Denmark, and the United States, can cause morbidity and mortality in animals and humans. LV infection can cause type I diabetes mellitus, myocarditis, and encephalitis in bank voles and experimental mice, and lemmings. Mouse dams infected with LV experience high rates of stillbirth that may persist across generations, and their fetuses may develop cranial, brain, and limb malformations. In humans, epidemiologic and serologic data suggest that LV infection correlates wit

Open fetal spinal cord surgery in rats with low mortality achieved by prevention of oligohydramnios.

Myelotomies were performed with an open microsurgical technique in rat fetuses aged between E16 and E18. In 87 consecutively treated fetuses the net surgical mortality was as low as 7%. The cause of death due to fetal surgery is shown to be amniotic fluid loss. The high survival rate is attributed to the prevention of oligohydramnios. The technique is described in detail and the results are discussed.

Contemporary Outcomes and Factors Associated With Mortality After a Fetal or Postnatal Diagnosis of Common Arterial Trunk.

BACKGROUND: Common arterial trunk (CAT) is a rare anomaly with a spectrum of pathology. We sought to identify current trends and factors associated with postnatal outcomes. METHODS: This was a single-centre review including 153 live births with planned surgery. Patients were analyzed as 2 cohorts based on era of CAT diagnosis (1990 to 1999 vs 2000 to 2014) and complexity of disease (simple vs complex). "Complex" required the association with significant aortic arch obstruction, truncal valve (TV) stenosis/regurgitation, and/or branch pulmonary artery (PA) hypoplasia, respectively. RESULTS: S

Fetal growth restriction and the risk of perinatal mortality-case studies from the multicentre PORTO study.

BACKGROUND: Intrauterine growth restriction (IUGR) is the single largest contributing factor to perinatal mortality in non-anomalous fetuses. Advances in antenatal and neonatal critical care have resulted in a reduction in neonatal deaths over the past decades, while stillbirth rates have remained unchanged. Antenatal detection rates of fetal growth failure are low, and these pregnancies carry a high risk of perinatal death. METHODS: The Prospective Observational Trial to Optimize Paediatric Health in IUGR (PORTO) Study recruited 1,200 ultrasound-dated singleton IUGR pregnancies, defined as E

Significance of motor vehicle crashes and pelvic injury on fetal mortality: a five-year institutional review.

BACKGROUND: Motor vehicle crashes (MVC) are the leading cause of maternal injury during pregnancy. It is estimated that 1,300 to 13,000 fetal deaths/yr result from maternal MVC. METHODS: Pregnant women with injuries were identified by ICD-9 codes containing pregnancy and injury from hospital medical records and Trauma/ED registries. Records were reviewed for demographic data, fetal gestational age, mechanism of injury, injuries sustained, Glasgow Coma Score, loss of consciousness (LOC), Injury Severity Score (ISS), and maternal and fetal outcome. Fisher's exact test, t test, and logistic regr

Fetal and neonatal mortality of small-for-gestational age infants. A 15-year study of 381 cases.

Fetal and neonatal mortality of small-for-gestational age (SGA) infants in 1968-1982 were studied in the region of the University Central Hospital of Turku, Finland. During the study period, there were 254 fetal and 127 neonatal deaths in SGA infants. The fetal mortality rate of SGA infants declined from 49.9/1000 to 14.0/1000. The neonatal mortality rate of SGA infants declined from 23.8/1000 to 8.3/1000. The severely SGA infants with a birth weight below the 2.5th percentile had three times higher neonatal mortality rates than SGA infants with a birth weight between the 2.5th and the 10th pe

The Implications of Socioeconomic Status by ZIP Code on Maternal-Fetal Morbidity and Mortality in San Antonio, Texas.

Introduction Over the past 20 years, the number of pregnancy-related fatalities in the United States has been on the rise. Increases in maternal and fetal mortality have been attributed to low socioeconomic status (SES). This raises the question of whether all geographical locations are proportionally affected by this upward trend in pregnancy-related fatalities. San Antonio is one of the largest cities in the United States and is known for its economic segregation. This study aims to compare the maternal and fetal health outcomes of mothers from diverse socioeconomic backgrounds in San Antoni

An Analysis of the Trend of Fetal Mortality Rates among Working and Jobless Households in Japan, 1995-2019.

This study aimed to identify differences in the trends of artificial and spontaneous fetal mortality rates between working and jobless households depending on ages, periods, and birth cohorts in Japan. Vital Statistics data from 1995 to 2019 and age groups in 5-year increments from 15 to 19 years through 45 to 49 years were used. Bayesian age-period-cohort analysis was used to evaluate changes in each of the outcomes. As a result, the difference in maternal age-standardized rate of both the artificial and spontaneous fetal mortality rates between the two types of households decreased in the pe

The role of Doppler indices in predicting intra ventricular hemorrhage and perinatal mortality in fetal growth restriction.

OBJECTIVES: The aim of this study is to determine whether Doppler indices predict intra-ventricular hemorrhage and perinatal mortality in fetal growth restricted pregnancies (FGR). MATERIAL AND METHODS: In this cohort study, 43 FGR fetuses underwent multi-vessel Doppler ultrasounds weekly or twice weekly after admission. Blood gases of the umbilical cord were analyzed immediately after delivery. Ultrasonography of the neonatal brain was performed after birth. Intra ventricular hemorrhage (IVH) and perinatal mortality were studied as outcomes. RESULTS: The median gestational age at the diagno

Association of electronic fetal monitoring during labor with cesarean section rate and with neonatal morbidity and mortality.

Data from the 1980 National Natality Survey by the National Center for Health Statistics were used to assess the relation of electronic fetal monitoring (EFM) during labor with cesarean section rates and neonatal morbidity and mortality. In univariate analyses, EFM was associated with higher cesarean section rates, lower five-minute Apgar scores, and a higher rate of respiratory distress. Logistic regression analysis controlling for other risk factors for poor neonatal outcome indicated that the association of EFM with higher cesarean section rates persisted (odds ratio 1.45, 95% CI 1.16, 1.81

Dose-response relations of palatal slit, cleft palate, and fetal mortality in mice treated with a glucocorticoid.

C57BL/6 (C57BL) and SWV mice were treated subcutaneously with triamcinolone acetonide in a single dose of 1.0-7.0 mg/kg on day 12 of pregnancy, and the palate of their fetuses was examined at term. In C57BL mice palatal slit occurred spontaneously and its frequency increased with increasing doses of triamcinolone. However, this defect was not seen in SWV fetuses, even when dams were treated with the doses that induced cleft palate. The frequency of cleft palate increased in both C57BL and SWV as the dose of triamcinolone increased. Fetal mortality increased in SWV, but not in C57BL, with incre

The 1980 National Natality Survey and National Fetal Mortality Survey--methods used and PHS agency participation.

Seven Public Health Service agencies collaborated with the National Center for Health Statistics in designing, funding, and analyzing the 1980 National Natality Survey (NNS) and 1980 National Fetal Mortality Survey (NFMS). The 1980 NNS-NFMS were nationally representative surveys based on samples of 9,941 live birth vital records and 6,386 fetal death vital records, which were weighted up to reflect U.S. estimates of 3,612,258 live births and 19,202 fetal deaths at 28 weeks' gestation or more. Four types of potential respondents who were associated with the sampled deliveries (married mothers,

Chronic anti-phencyclidine monoclonal antibody therapy decreases phencyclidine-induced in utero fetal mortality in pregnant rats.

Illicit drug use during pregnancy is a serious social and public health problem inflicting an array of deleterious effects on both mother and offspring. We investigated the hypothesis that a murine anti-phencyclidine (PCP) monoclonal antibody (mAb6B5; K(D)=1.3 nM) can safely protect mother and fetus from PCP-induced adverse health effects in pregnant rats. Pregnant Sprague-Dawley rats (n=4-5) were intravenously administered bolus injections of PCP (1mg/kg) on multiple days during pregnancy. They were also chronically treated with anti-PCP mAb6B5 at 45 mg/kg as a PCP antagonist. This dose provi

Prognostic value of fetal left ventricular myocardial performance index for mortality or ECMO in congenital diaphragmatic hernia.

BACKGROUND: We aimed to determine whether fetal left ventricular myocardial performance index (LV MPI) at 30-34 weeks' gestation predicts mortality or need for extracorporeal membrane oxygenation (ECMO) in congenital diaphragmatic hernia (CDH), and whether it adds predictive value beyond the observed-to-expected lung-to-head ratio (O/E LHR). METHODS: Retrospective single-center cohort (2017-2024) of prenatally diagnosed CDH with LV MPI measured at 30-34 weeks. The primary outcome was death before NICU discharge or ECMO. Multivariable logistic regression evaluated LV MPI adjusted for O/E LHR a

Impaired Fetal Environment and Gestational Age: What Is Driving Mortality in Neonates With Critical Congenital Heart Disease?

Background Infants with critical congenital heart disease (CCHD) are more likely to be small for gestational age (SGA) or born to mothers with maternal placental syndrome. The objective of this study was to investigate the relationship between maternal placental syndrome, SGA, and gestational age (GA) on 1-year mortality in infants with CCHD. Methods and Results In a population-based administrative database of all live-born infants in California (2007-2012) we identified all infants with CCHD without chromosomal anomalies. Our primary predictor was an impaired fetal environment (IFE), defined
