Novel techniques to improve split-thickness skin graft viability during vulvo-vaginal reconstruction.

BACKGROUND: Split-thickness skin grafts are often utilized for vulvo-vaginal reconstruction. Unfortunately, infection and sloughing may occur in up to 22% of patients with standard techniques especially at irradiated recipient sites. CASES: We report seven cases of vulvo-vaginal reconstruction using split-thickness skin grafts. In this series, we used fibrin tissue adhesives with and without vacuum-assisted closure devices to augment graft adherence and viability. We briefly describe the clinical history, surgical techniques, and outcomes of the cohort. CONCLUSION: Fibrin tissue adhesives an

Salvage of the ruptured spleen in an infant with very low birth weight.

Hemorrhage from the traumatized spleen needs surgical intervention if the patient's cardiovascular status is unstable and not stabilizing with medical support. It is well recognized that salvage of the injured spleen is preferable to splenectomy because of risks of overwhelming postsplenectomy infection (OPSI). The authors report the case of a 746-g newborn with splenic hemorrhage that required surgical intervention to control bleeding. A 2-component fibrin tissue adhesive and a cellulose fabric were used.

On-site fabrication of Bi-layered adhesive mesenchymal stromal cell-dressings for the treatment of heart failure.

Mesenchymal stromal/stem cell (MSC)-based therapy is a promising approach for the treatment of heart failure. However, current MSC-delivery methods result in poor donor cell engraftment, limiting the therapeutic efficacy. To address this issue, we introduce here a novel technique, epicardial placement of bi-layered, adhesive dressings incorporating MSCs (MSC-dressing), which can be easily fabricated from a fibrin sealant film and MSC suspension at the site of treatment. The inner layer of the MSC dressing, an MSC-fibrin complex, promptly and firmly adheres to the heart surface without sutures

Early effects of fibrin sealant on colonic anastomosis in rats: an experimental and case-control study.

BACKGROUND: Leakage from colonic anastomoses leads to mortality and morbidity. Fibrin adhesives can be used to increase the strength of the anastomosis. In this study, we evaluated the early effects of fibrin sealant and hyaluronic acid-carboxymethylcellulose on colonic anastomosis in rats. METHODS: Anastomoses were made in the descending colon of 38 female Wistar-Albino rats, in three groups: control group (n = 12), group 1 treated with hyaluronic acid-carboxymethylcellulose (n = 16), and group 2 treated with fibrin sealant (n = 10). After 72 hours, adhesion scores, bursting pressure, ruptur

Cell barrier function of resident peritoneal macrophages in post-operative adhesions.

Post-operative adhesions are a leading cause of abdominal surgery-associated morbidity. Exposed fibrin clots on the damaged peritoneum, in which the mesothelial barrier is disrupted, readily adhere to surrounding tissues, resulting in adhesion formation. Here we show that resident F4/80HighCD206- peritoneal macrophages promptly accumulate on the lesion and form a 'macrophage barrier' to shield fibrin clots in place of the lost mesothelium in mice. Depletion of this macrophage subset or blockage of CD11b impairs the macrophage barrier and exacerbates adhesions. The macrophage barrier is usually

Tensile strength of adhesives in peripheral nerve anastomoses: an in vitro biomechanical evaluation of four different neurorrhaphies.

BACKGROUND: The fundamental prerequisite for prognostically favorable postoperative results of peripheral nerve repair is stable neurorrhaphy without interruption and gap formation. METHODS: This study evaluates 60 neurorrhaphies on femoral chicken nerves in terms of the procedure and the biomechanical properties. Sutured neurorrhaphies (n = 15) served as control and three sutureless adhesive-based nerve repair techniques: Fibrin glue (n = 15), Histoacryl glue (n = 15), and the novel polyurethane adhesive VIVO (n = 15). Tensile and elongation tests of neurorrhaphies were performed on a tensil

Development of an In Situ Printing System With Human Platelet Lysate-Based Bio-Adhesive to Treat Corneal Perforations.

PURPOSE: Corneal perforation is a clinical emergency that can result in blindness. Currently corneal perforations are treated either by cyanoacrylate glue which is toxic to corneal cells, or by using commercial fibrin glue for small perforations. Both methods use manual delivery which lead to uncontrolled application of the glues to the corneal surface. Therefore, there is a need to develop a safe and effective alternative to artificial adhesives. METHODS: Previously, our group developed a transparent human platelet lysate (hPL)-based biomaterial that accelerated corneal epithelial cells heal

Fibrin sealant for treatment of cerebrospinal fluid leaks.

OBJECTIVE: Persistent cerebrospinal fluid leaks in the human population are rarely found in otherwise healthy individuals, but occur in patients with comorbid illnesses. These leaks are frequently resistant to dural suturing or closure of the defect site with connective tissue, cartilage, or plastic materials. In this study, fibrin sealant (ViGuard Fibrin Sealant was used to adhere muscle grafts to surgically created dural defects to close cerebrospinal fluid leaks in chinchillas. Histologic evaluation of the defect sites were conducted to assess healing and tissue response in the test and con

[Application of fibrin adhesive systems in the maxillary sinus as an alternative to tamponage].

During the past two years 22 patients with a chronic maxillary sinusitis have been operated in our clinic using a special catheter set for the application of fibrin glue instead of a gauze strip tamponade as a hemostypt. With 20 of these patients the results of this method were evaluated in a clinical investigation. All investigated sinuses were clinically, roentgenologically and sonographically asymptomatic, as were also sinuscopic controls with a part of these patients.

The Regenerative Capacity of Tissue-Engineered Amniotic Membranes.

Scaffolds can be introduced as a source of tissue in reconstructive surgery and can help to improve wound healing. Amniotic membranes (AMs) as scaffolds for tissue engineering have emerged as promising biomaterials for surgical reconstruction due to their regenerative capacity, biocompatibility, gradual degradability, and availability. They also promote fetal-like scarless healing and provide a bioactive matrix that stimulates cell adhesion, migration, and proliferation. The aim of this study was to create a tissue-engineered AM-based implant for the repair of vesicovaginal fistula (VVF), a de

Fibrin(ogen) Is Constitutively Expressed by Differentiated Intestinal Epithelial Cells and Mediates Wound Healing.

Fibrinogen is a large molecule synthesized in the liver and released in the blood. Circulating levels of fibrinogen are upregulated after bleeding or clotting events and support wound healing. In the context of an injury, thrombin activation drives conversion of fibrinogen to fibrin. Fibrin deposition contains tissue damage, stops blood loss, and prevents microbial infection. In most circumstances, fibrin needs to be removed to allow the resolution of inflammation and tissue repair, whereas failure of this may lead to the development of various disorders. However, the contribution of fibrinoge

The use of tissue sealant in parotidectomy: a systematic review and meta-analysis.

OBJECTIVE: Drains are used post-parotidectomy to reduce seroma and haematoma formation. Tissue-derived thrombin sealant can enable a drainless procedure, allowing for an earlier discharge, less discomfort and a more cost-efficient method. This study aimed to assess whether tissue sealant improves wound-related outcomes in parotidectomy. METHOD: A systematic literature review was performed using a standardised published methodology and custom database search strategy. A fixed-effect meta-analysis of the combined complications was conducted. RESULTS: Thirteen studies were identified relating t

Medical Thoracoscopy-assisted Thoracographic Fibrin Glue Sealing Method for Intractable Pneumothorax: A Case-based Proposal.

We herein report a case of refractory pneumothorax in a patient with interstitial lung disease that was successfully treated using medical thoracoscopy-assisted thoracographic fibrin glue sealing (MTS-TGF). This novel hybrid technique enables accurate leak localization and safe delivery of fibrin glue under direct visualization and fluoroscopic guidance. The procedure achieved substantial leak reduction followed by complete resolution without surgical intervention. MTS-TGF may enhance the precision and safety of conventional TGF, particularly in anatomically complex or high-risk patients. This

Fibrin sealant for management of complicated obstetric lacerations.

BACKGROUND: Fibrin sealant commonly is used topically for hemostasis in cardiovascular surgery. Complicated vulvar and vaginal bleeding after vaginal delivery can be difficult to manage using traditional techniques. CASE: A 21-year-old primipara, after a spontaneous vaginal delivery of a twin gestation, was found to have expanding right labial swelling and ecchymosis extending from the superior part of the labia majora to the ischial fossa that was approximately 10 cm wide. Surgical exploration was pursued, but poor tissue quality limited the effectiveness of traditional hemostatic techniques

New techniques in the treatment of common perianal diseases: stapled hemorrhoidopexy, botulinum toxin, and fibrin sealant.

There have been several recent advances in the treatment of common perianal diseases. Stapled hemorrhoidopexy is a procedure of hemorrhoidal fixation, combining the benefits of rubber band ligation into an operative technique. The treatment of anal fissure has typically relied upon internal sphincterotomy; however, it carries a risk of incontinence. The injection of botulinum toxin represents a new form of sphincter relaxation, without division of any sphincter muscle; morbidity is minimal and results are promising. For the treatment of fistula in a fistulotomy remains the gold standard, howev

[Duodenocephalopancreatectomy with pylorus sparing and occlusion of the main bile duct with fibrin glue].

The leakage of pancreaticojejunostomy represents the main technical post-operative complication after pancreaticoduodenectomy. Because pancreatic fistula can be the source of morbidity and mortality and it is related to the integrity of the pancreatic-enteric anastomosis, a number of technical modification have been suggested, including pancreaticojejunostomy, pancreaticogastrostomy and occlusion of the main pancreatic duct with fibrin glue. The incidence of leakage is particularly high in case of narrow ducts and tender pancreatic glands; in such conditions some experiences in literature sugg

A novel treatment for patients with hereditary haemorrhagic telangiectasia.

Hereditary haemorrhagic telangiectasia (HHT) is an autosomal dominant disorder characterized by dermal, mucosal, and visceral telangiectases as well as pulmonary and cerebral arteriovenous malformations. Recurrent epistaxis occurs in the majority of patients, and by the very nature of the thin walled vessels involved it is often refractory to conventional forms of treatment. We present the case of an 82-year-old lady with intractable epistaxis secondary to HHT, that was successfully controlled by the application of fibrin glue.

Evaluation of fibrin sealants for central nervous system sealing in the mongrel dog durotomy model.

BACKGROUND: Watertight repair of the dura is imperative after neurosurgical procedures involving the brain or spinal cord because inadequately treated leakage of cerebrospinal fluid (CSF) from punctured dura can have serious consequences such as meningitis, arachnoiditis, or epidural abscess. OBJECTIVE: To assess the efficacy of Evicel Fibrin Sealant (Human) to prevent CSF leakage using a 2.0-cm durotomy mongrel dog repair model and to compare the tissue response with Tisseel (a fibrin sealant) and Duraseal (a synthetic polyethylene glycol [PEG] hydrogel sealant). METHODS: The canine durotom
