Breast Cancer Incidence and Factors Affecting Delayed Presentation in a Rural Fiji Population: A Qualitative Study.

BACKGROUND: Breast cancer disproportionately affects low- and middle-income countries with increased mortality rates compared to high income countries. For the Pacific island country of Fiji, numerous factors have been hypothesized for the high mortality rate. Delay in initial presentation is thought to be a major contributor. This delay is poorly understood. METHODS: A retrospective mixed-method study of patients who were diagnosed with breast cancer at Labasa Hospital in Fiji from 2018 to 2022 was conducted. Incidence and mortality rates were obtained from hospital records. Further demograp

Videography of pathways for enteric pathogen exposure among children in urban informal settlements in Fiji and Indonesia.

BACKGROUND: Children in urban informal settlements are vulnerable to enteric pathogen exposures due to inadequate availability of clean water, sanitation, and wastewater treatment. These exposures can contribute to diarrhea, malabsorption, and poor growth. Understanding how children interact with their environments – particularly through mouthing behaviors – can help identify high-risk environmental sources, exposure pathways, and opportunities for intervention. The objective of the study is to characterize mouthing behaviors among young children and assess the environmental contexts in which

Eyelash Epilation in the Absence of Trichiasis: Results of a Population-Based Prevalence Survey in the Western Division of Fiji.

BACKGROUND: The WHO definition of trachomatous trichiasis (TT) is "at least one eyelash touching the globe, or evidence of recent epilation of in-turned eyelashes", reflecting the fact that epilation is used as a self-management tool for TT. In Fiji's Western Division, a high TT prevalence (8.7% in those aged ≥15 years) was reported in a 2012 survey, yet a 2013 survey found no TT and Fijian ophthalmologists rarely see TT cases. Local anecdote suggests that eyelash epilation is a common behaviour, even in the absence of trichiasis. Epilators may have been identified as TT cases in previous surv

Genomic epidemiology of the Mycobacterium tuberculosis complex in Fiji reveals community transmission and human Mycobacterium bovis infection.

INTRODUCTION: Tuberculosis (TB) remains a major public health concern in Fiji, but genomic data on the circulating Mycobacterium tuberculosis complex (MTBC) are limited. METHODS: We analysed 65 MTBC isolates collected during 2019-2022 using whole-genome sequencing to characterize lineage distribution, drug resistance, and transmission patterns. RESULTS: Of the isolates, 62 (95.4%) were Mycobacterium tuberculosis and 3 (4.6%) were Mycobacterium bovis. Lineage 4 was the most common (29/62, 46.8%). Genomic analysis identified 12 transmission clusters, with epidemiological links involving househ

Segmentation and quantification of testicular histology images using machine learning bioimage analysis tools; Ilastik and Fiji software.

Histomorphological and histochemical techniques are widely used in infertility studies to assess testicular damage, determine the mechanisms involved, investigate potential interventions strategies, monitor treatment response and prognosis. Testis, a primary male reproductive organ is a compartmentalized organ made up of several seminiferous tubules and supporting tissue. Hence, focal damage is common, and accordingly, making accurate and insightful deductions require careful analysis of almost the entire testis section. However, manual analysis of testis histology sections to extract quantifi

Clinical decision rules for diagnosis of Streptococcus pyogenes sore throat in Fiji: a prospective diagnostic accuracy study.

BACKGROUND: Acute rheumatic fever is an immune-mediated condition triggered by Streptococcus pyogenes sore throat and possibly skin infection, with a substantial burden in resource-limited settings. Clinical decision rules (CDRs) are commonly used to guide antibiotic treatment of sore throat based on signs and symptoms, but their diagnostic accuracy varies by study and setting. This work aimed to assess the accuracy of multiple CDRs in Fiji to diagnose S. pyogenes sore throat. METHODS: We conducted a prospective diagnostic accuracy study at two primary healthcare centres in Suva, Fiji, enroll

Acceptability of a nationwide scabies mass drug administration (MDA) program in Fiji: a qualitative interview-based study.

BACKGROUND: Fiji has among the highest global reported prevalence of scabies. Mass drug administration (MDA) has been identified as a potentially effective strategy to control scabies, but acceptability of MDA from the perspectives of people receiving and delivering scabies MDA programs remains underexplored in Fiji and globally. METHODS: A qualitative study was conducted after completion of the national MDA campaign. Participants included 44 community members and 12 key informants across the Central and Western Divisions of Fiji. Semi-structured face-to-face and virtual interviews were condu

Fiji-Assisted Automatic Quantitative Volumetric Analysis of Choroidal Neovascularization in a Laser-Induced Choroidal Neovascularization Mouse Model.

PURPOSE: The laser-induced choroidal neovascularization (CNV) mouse model is the most frequently used animal model of CNV. To test new therapeutic agents that suppress CNV, CNV measurement in an accurate, precise, and efficient manner is important. We present the utility of Fiji-assisted automatic volumetric quantification of CNV in comparison with two-dimensional CNV analyses. METHODS: Laser-induced CNV was induced in C57BL/6J mice according to the established protocol. After CNV induction, mice were treated with intravitreal injection of either phosphate-buffered saline solution (PBS) or Af

Amoebic Liver Abscesses in Fiji: Epidemiology, Clinical Presentation and Comparison of Percutaneous Aspiration and Percutaneous Catheter Drainage.

BACKGROUND: Amoebic liver abscess (ALA) is a common clinical problem in tropical countries related to poor sanitation. The epidemiology and clinical presentation of ALA in Fiji has not been previously described. It is unclear whether percutaneous aspiration (PA) or percutaneous catheter drainage (PCD) has better outcomes. PURPOSE: The aims were to describe the epidemiology and clinical presentation of ALA in Fiji and to compare the outcomes of PA and PCD for treatment of ALA. METHODS: A retrospective case note review of patients treated with either PA or PCD between 2010 and 2015 was perform

Human Leptospirosis Infection in Fiji: An Eco-epidemiological Approach to Identifying Risk Factors and Environmental Drivers for Transmission.

Leptospirosis is an important zoonotic disease in the Pacific Islands. In Fiji, two successive cyclones and severe flooding in 2012 resulted in outbreaks with 576 reported cases and 7% case-fatality. We conducted a cross-sectional seroprevalence study and used an eco-epidemiological approach to characterize risk factors and drivers for human leptospirosis infection in Fiji, and aimed to provide an evidence base for improving the effectiveness of public health mitigation and intervention strategies. Antibodies indicative of previous or recent infection were found in 19.4% of 2152 participants (

Costs of primary healthcare presentations and hospital admissions for scabies and related skin infections in Fiji, 2018-2019.

Scabies and related bacterial skin and soft tissue infections (SSTIs) are highly prevalent in many tropical, low- and middle-income settings. These skin conditions contribute to higher healthcare costs and burdens on healthcare systems. The Big Skin Health Intervention Fiji Trial ("Big SHIFT") carried out surveillance for scabies and SSTIs from July 2018 to June 2019 in the Northern Division of Fiji, an area with high prevalence of scabies, prior to a division-wide ivermectin-based mass drug administration (MDA) campaign. Using data from Big SHIFT, we sought to estimate the annual direct healt

First evidence of concurrent enzootic and endemic transmission of Ross River virus in the absence of marsupial reservoirs in Fiji.

BACKGROUND: Ross River virus (RRV) is a zoonotic alphavirus transmitted by several mosquito species. Until recently, endemic transmission was only considered possible in the presence of marsupial reservoirs. METHODS: RRV seroprevalence was investigated in placental mammals (including horses, cows, goats, pigs, dogs, rats, and mice) in Fiji, where there are no marsupials. A total of 302 vertebrate serum samples were collected from 86 households from 10 communities in Western Fiji. RESULTS: Neutralizing antibodies against RRV were detected in 28% to 100% of sera depending on the species, and n
