Mallet finger injuries: the signs, symptoms, diagnosis and management.

Patients commonly present to UK emergency departments with injuries to the tips of their fingers. Mallet finger is one of the most common injuries, resulting from an injury to the extensor tendon over the dorsal surface of the distal phalanges of the hand. Timely recognition, diagnosis and management are required to prevent complications. This article provides an overview of the pathophysiology, signs, symptoms, diagnosis and management of mallet finger injuries.

[Nail fold formation using a thenar flap in complete syndactylia].

In complete syndactyly fingers are fused along their whole length and the nail is continuous over a narrow osseous bridge between the nail processes of the distal phalanges. When the fingers are separated there is no adjacent skin to form a lateral nail fold and, therefore, the finger end will become distorted. By using a thenar flap to the dorsal aspect of the syndactylic finger tips material is transferred to create normal lateral nail folds when the fingers are separated some months later.

Brachydactyly type B: case report and further evidence for clinical heterogeneity.

We present a male child with digital and radiographic findings consistent with brachydactyly type B. His left hand had 2-4 syndactyly, shortened first and fifth digits and single palmar and fifth finger flexion creases. The nail on the second finger was hypoplastic. The right hand had a small thumb and curving of the second finger towards the midline. The left foot had a short left hallux with an absent nail, 2-4 syndactyly and an absent toe. His right foot had a short, dorsiflexed hallux with a hypoplastic nail, 2-3 syndactyly and an overlapping toe. Radiographs showed asymmetrical hypoplasia

Duplicated distal phalanx of thumb or hallux in trisomy 13: A recurrent feature in a series of 42 fetuses.

Trisomy 13 or Patau syndrome (PS) is a well-known aneuploidy characterized by a polymalformative syndrome. We described a large series of fetuses with PS and compared them with cases described in the literature, most of which were live-born. In all, 42 fetuses, aged from 14 to 41 gestational weeks (GW), were examined. The main defects observed were similar to those described in live-born patients: congenital heart defects (76%), holoprosencephaly spectrum anomalies including arhinencephaly and hypotelorism (74%), urinary tract anomalies (71%), ear anomalies (69%), postaxial polydactyly (67%),

Idiopathic Fenestrated Complex Syndactyly in a Unique Crisscross Fashion.

This case presents surgical treatment of a unique form of syndactyly: an isolated fenestrated, complex, crisscross syndactyly of the right middle and ring fingers. A 2-year-old boy presented with the ring finger lying dorsal and the middle finger lying volar, with the middle phalanges syndactylized. A surgical release was performed with a subsequent z-plasty, 2 years later, for scar elongation. At the age of 4, he has essentially full function of his hand with minimal limitations. This case demonstrates that 2 digits that were syndactylized in a coronal plane (ring finger dorsal and middle fin

Cemented K-wire fixation for the treatment of shaft fractures of middle phalanges.

BACKGROUND: The objective of this report is to introduce an external-fixation technique using the combination of K-wires and cement. METHODS: From February 2009 to January 2015, 51 patients with shaft fractures of middle phalanges were treated with cemented K-wire fixation. The mean age of patients at surgery was 41 years. The mean time interval from injury to operation was 6±5.78days. Injured digits included index (n=18), long (n=15), ring (n=7), and little (n=11) fingers. Types of fractures were transversal (n=32), short oblique or spiral (n=5), and comminuted (n=14) fractures. Active range

Expanding clinical spectrum of non-autoimmune hyperthyroidism due to an activating germline mutation, p.M453T, in the thyrotropin receptor gene.

OBJECTIVE: To describe clinical and genetic features of a Thai family with non-autoimmune hyperthyroidism (NAH) caused by an activating germline mutation in the thyrotropin receptor (TSHR) gene. PATIENTS: Three affected individuals from the same family (a father and his two children) were studied. Clinical and imaging findings were reviewed and compared. GENETIC ANALYSIS: Genomic DNA was extracted from peripheral blood leukocytes and mutation analysis of the entire coding sequence of the TSHR gene was performed in both children and their parents by direct DNA sequencing. RESULTS: A heterozyg

The concentric circles method to define and assess anterior subluxation in bony mallet finger.

We have described and validated the concentric circles method for assessing anterior subluxation in the bony mallet finger. We retrospectively analysed 114 radiographs from cases of bony mallet finger assessed by four hand surgeons and one radiologist using three approaches: subjective visual inspection, the concentric circles method and the malalignment method. The concentric circles method defines subluxation as a disruption of the normal concentric annulus between the middle phalangeal head and distal phalangeal articular surface on true lateral radiographs. It showed improved accuracy (94%

Rotations of three-joint fingers: a radiological study.

The aim of the current study was to test a protocol of quantification of phalangeal three-dimensional (3D) rotations during flexion of three-joint digits. Three-dimensional-specific software was developed to analyze CT reconstruction images. A protocol was carried out with six fresh-frozen upper limbs from human cadavers free from any visible pathology (three females, three males). CT millimetric slices were done for reconstruction of hand bone units. Orthonormal coordinate systems of inertia were calculated for each unit. Three-dimensional phalangeal rotations were estimated between two stati

Secondary reconstruction by temporoparietal free fascial flap for ring avulsion injury.

We report the secondary reconstruction of an amputated finger following a ring avulsion injury using a temporoparietal fascial flap (TPFF). There have been reports of preserving degloved fingers using a cross-finger flap and an abdominal flap, but it is difficult to obtain good results with these procedures. The TPFF is an ideal, thin flap that gives excellent results, preserving movement of the phalangeal joint.

Osteotomy for clinodactyly.

Clinodactyly, finger deviation in the coronal plane, affects the little finger most commonly. Patients typically tolerate clinodactyly without functional limitation, but with increasing deformity, they may have difficulties with specific tasks such as playing musical instruments. In those who do not tolerate the deformity, surgical options include physiolysis for young patients and osteotomy for older patients. An opening wedge osteotomy increases finger length while correcting the coronal deformity. Surgical results of the opening wedge osteotomy are satisfying and complications are rare, wit

Vascular anatomy of the finger dorsum and a new idea for coverage of the finger pulp defect that restores sensation.

The cutaneous vascular anatomy of the finger dorsum was studied by dissection under loupe magnification of 71 fingers from 19 preserved cadaver hands. All specimens were injected with red latex or epoxy resin through a cannula in the brachial artery to identify the small vessels. The finger dorsum was supplied by the terminal branches from the dorsal metacarpal artery around the metacarpophalangeal joint. Other areas were nourished by the dorsal cutaneous branch from the proper palmar digital artery. There were two or three dorsal branches in the proximal phalangeal region and two in the middl

Case report of a novel nonsyndromic unilateral syndactyly of the hand.

Syndactyly is a cutaneous and/or bony digital malformation with possible webbing of adjacent fingers or toes and uni- or bilateral occurrence. We report an 84-year old woman with a novel non-syndromic congenital malformation of her left hand. Clinical examination showed that she only had four digits. Radiograph of the hand revealed synostosis of the second and third proximal phalanx, resulting in a triangular shaped bone with relatively normal articulations at both ends. The phalangeal base of the fused finger tapers distally and is broader than the middle phalangeal bases of the ring and litt

Extra phalangeal crease - A trait in forensic identification.

In the past, many biological, anthropological and forensic studies have been conducted on variations in finger and palm ridge patterns, however, finger crease patterns have not received much attention in the literature. The photocase shows an obvious extra phalangeal crease in the little finger as an extremely rarely reported characteristic. Similar unique characteristics need to be reported for their rarity and significance in forensic investigations.
