Antimicrobial resistance among neonatal pathogens in developing countries.

INTRODUCTION: Knowledge of antimicrobial resistance and trends in resistance patterns among major pathogens causing infections in young infants (up to 90 days of life) is an important component of developing community-based management strategies. Hospital-based data suggest alarming rates of resistance to ampicillin and gentamicin, the first-line antimicrobial agents recommended by WHO for treatment of serious infections in young infants. METHODS: We searched the literature published since 1990 for studies from developing countries reporting resistance among serious community-acquired infecti

Antibiotic use in neonatal sepsis.

Neonatal sepsis is a life-threatening emergency and any delay in treatment may cause death. Initial signs of neonatal sepsis are slight and nonspecific. Therefore, in suspected sepsis, two or three days empirical antibiotic therapy should begin immediately after cultures have been obtained without awaiting the results. Antibiotics should be reevaluated when the results of the cultures and susceptibility tests are available. If the cultures are negative and the clinical findings are well, antibiotics should be stopped. Because of the nonspecific nature of neonatal sepsis, especially in small pr

Emergence of CMY-2-Producing Escherichia coli in Korean Layer Parent Stock.

The use of antimicrobials in food animals is the major determinant for the propagation of resistant bacteria in the animal reservoir. Especially, parent stock (PS) produces thousands of eggs for commercial laying hens and can transfer a variety of viral and bacterial agents to chicks. The objective of this study was to investigate the presence and distribution of third-generation cephalosporin-resistant and extended-spectrum β-lactamase (ESBL)- and plasmid-mediated AmpC (pAmpC)-producing Escherichia coli in the PS level of the layer operation system in Korea. A total of 591 E. coli isolates we

Pediatric acute dacryocystitis due to Eikenella corrodens: A case report.

Eikenella corrodens is a facultatively anaerobic gram-negative rod bacterium in the oropharynx and respiratory tract. It is a member of HACEK (Haemophilus spp., Aggregatibacter spp., Cardiobacterium hominis, E. corrodens, and Kingella kingae) group commonly associated with endocarditis and craniofacial infections. It is usually susceptible to penicillin, second and third-generation cephalosporins, and carbapenem, but has variable susceptibility to first-generation cephalosporin. We herein provide a description of the first case of pediatric acute dacryocystitis caused by E. corrodens. The pati

The management of bacterial meningitis in children.

Bacterial meningitis is still a major cause of death and disability in children worldwide. With the advent of conjugate vaccines against the three major pathogens, the burden of disease is increasingly concentrated in developing countries that cannot afford the vaccines. Antibiotic resistance is an increasing problem; in developed countries, high-level resistance to beta-lactams among Streptococcus pneumoniae necessitates the addition of vancomycin to third-generation cephalosporins. In many developing countries, the problems are more fundamental. Increasing resistance of S. pneumoniae to peni

Cefotetan-induced immune hemolytic anemia.

Immune hemolytic anemia due to a drug-adsorption mechanism has been described primarily in patients receiving penicillins and first-generation cephalosporins. We describe a patient who developed anemia while receiving intravenous cefotetan. Cefotetan-dependent antibodies were detected in the patient's serum and in an eluate prepared from his red blood cells. The eluate also reacted weakly with red blood cells in the absence of cefotetan, suggesting the concomitant formation of warm-reactive autoantibodies. These observations, in conjunction with clinical and laboratory evidence of extravascula

No way back for quinolones in the treatment of gonorrhoea.

OBJECTIVES: To assess the changing prevalence of quinolone resistant Neisseria gonorrhoeae (QRNG) over 3 years following implementation of cephalosporin treatment (for gonorrhoea). METHODS: Case review of all episodes of gonorrhoea diagnosed in the genitourinary medicine clinic in Nottingham during the first 6 months of each year between 2002 and 2005. RESULTS: The prevalence of QRNG peaked at 52.5% in January 2002. Three years after third generation cephalosporins were implemented for the treatment of gonorrhoea, QRNG persists in the local population with a prevalence of 9.1% over the first

Assessing the evidence for antibiotic management of laboratory-confirmed Streptococcus A skin infections to prevent acute rheumatic fever and rheumatic heart disease: a systematic review.

OBJECTIVES: Acute rheumatic fever (ARF) and rheumatic heart disease (RHD) are potential sequelae of untreated group A streptococcal (Strep A) infections. Guidelines focus on treating Strep A pharyngitis but seldom on skin infections. This systematic review explored whether directed antibiotic therapy for superficial Strep A skin infections prevents ARF/RHD. METHODS: We searched PubMed, Scopus, Cochrane Library, and clinical trial registries for published and ongoing trials measuring the eradication of Strep A and clinical resolution of polymicrobial infections with antibiotics through Decembe

Defining Safe and Effective Cefazolin Dosing Regimens for MSSA Infections in the CNS: Leveraging Sparse Real-World Data and PBPK Modeling.

BACKGROUND AND OBJECTIVE: Infections in the central nervous system (CNS) are serious and carry a significant risk of morbidity and mortality. Though commonly used as prophylaxis for neurosurgical interventions, cefazolin as a treatment for CNS infections due to methicillin-susceptible Staphylococcus aureus (MSSA) has been debated owing to the perceived inability to achieve adequate concentrations at the site of infection. The objective of the current study was to evaluate the dose-exposure-response relationship of cefazolin in the CNS. METHODS: To leverage sparse data of cefazolin in the cere

In Vitro Antibacterial Efficacy of Cetirizine and N-Acetylcysteine Alone and in Combination with Cefalexin on Canine Methicillin-Sensitive and -Resistant Staphylococcus pseudintermedius.

Staphylococcus (S.) pseudintermedius, as a commensal of the skin and mucosa, leads to a variety of diseases in dogs, most commonly skin and ear infections. The development of methicillin-resistant S. pseudintermedius (MRSP) is an emerging risk for animals and humans. The aim of this study was to test cetirizine and N-acetylcysteine as synergistic substances with cephalexin for treating S. pseudintermedius infections. Each of the five methicillin-sensitive S. pseudintermedius (MSSP) isolates and five MRSP isolates, and one control strain were tested. The minimal inhibitory concentration (MIC) o

Differential risk signals of cutaneous adverse drug reactions among β-lactam antibiotics: ceftriaxone shows increased signals for SJS/TEN in clinical and real-world cohorts.

BACKGROUND: Cutaneous adverse drug reactions (CADRs) are triggered by numerous antibiotic drugs. Most drug reactions are in the spectrum of a maculopapular eruption (MPE) with a self-limiting disease course. In contrast, severe cutaneous adverse reactions (SCARs), including Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN), are rare but may follow a life-threatening course. In this study, we investigated the key drug-related triggers of CADRs in a retrospective single-center cohort and, based on these findings, evaluated the differential risk of the implicated agents using large-sc

Extended-spectrum beta-lactamase producing enterobacteriaceae in a tertiary care hospital in Trinidad and Tobago.

Extended-spectrum beta-lactamase (ESBL) mediated resistance to third generation cephalosporins, amongst the family Enterobacteriaceae, is emerging worldwide. This is the Caribbean's first survey on ESBL production, and was conducted during two six-month periods in 1998 and 2001, in a tertiary health institution in Trinidad and Tobago. Consecutive ampicillin resistant isolates of the family Enterobacteriaceae from in-patients were screened for resistance to third generation cephalosporins, and for ESBL production. The proportion of isolates found to be ESBL producers was similar in both samples

[Development and current status of Pseudomonas aeruginosa sensitivity to antibiotics].

The response of Pseudomonas aeruginosa to antibacterial drugs is remarkably stable and is characterized by multiresistance. This organism is uniformly resistant to ampicillins, first and second generation cephalosporins and kanamycin and most often resistant to streptomycin, tetracyclines, chloramphenicol, nalidixic acid, sulphonamides, co-trimoxazole and nitrofurans. Very few of the conventional antibiotics are active against Pseudomonas spp.: polymyxin is virtually always active in vitro but gives disappointing therapeutic results; little change has been observed over years in the incidence

Soluble mediators and the interaction of drugs in IBD.

PURPOSE: The emergence of antibiotic resistance in pathogenic Enterobacteriaceae is a public health problem in tropical countries such as Burkina Faso. Antibiotic resistance could be identified using a variety of approaches. This study aimed to estimate the prevalence of pathogenic enterobacteria strains from three sources, as well as their antibiotic resistance profile to biotope and climatic season. MATERIAL AND METHODS: The methodological approach consisted of identifying Enterobacteriaceae from human (urine, stool), animal (eggs, milk, fish), and environmental (soil, lettuce) samples, fol

Changing etiology of community-acquired bacterial meningitis in adults: a nationwide multicenter study in Korea.

Epidemiologic data on the etiologic organisms is important for appropriate empirical antibiotic treatment of bacterial meningitis. We identified the etiologies of community-acquired bacterial meningitis in Korean adults and the associated epidemiological factors. A retrospective, multicenter nationwide study was carried out. Patients 18 years of age or older with community-acquired bacterial meningitis with a confirmed pathogen were enrolled. Demographic, clinical, and microbiological data were collected. One hundred and ninety-five cases were collected. Streptococcus pneumoniae was the most c

[Antimicrobial therapy in dermatology].

The extensive and sometimes indiscriminate use of antibiotics sometimes without strict indications has led to increases in both bacterial resistance and sensitization of patients. Systemic antibiotics in skin infections are indicated when a severe local infection occurs which spreads into the surrounding tissue or when there are signs of systemic infection. There are special indications in patients with peripheral arterial occlusive disease, diabetes or immunosuppression. Topical use of antibiotics should be abandoned and replaced by antiseptics. The beta-lactam antibiotics are the antibiotics

Comparison of Cefazolin and Ceftriaxone Enterobacterales Susceptibilities for Inpatient Treatment of Urinary Tract Infections and Risk of Hospital-onset Clostridioides difficile Infection.

PURPOSE: Urinary tract infection (UTI) is the second most common indication for antibiotic therapy among inpatients in the United States. Ceftriaxone, a third-generation cephalosporin, is habitually chosen to treat inpatient UTIs due to familiarity, cost, and perceived safety. However, third-generation cephalosporins increase the risk of health care facility-onset Clostridioides difficile infection (HOCDI) more than any other antibiotic group, while no statistical risk exists for first-generation cephalosporins. Recent evidence comparing Enterobacterales susceptibility for first- and third-gen

Acute exacerbations of chronic bronchitis. Preventing treatment failures and early reinfection.

Although antimicrobial agents from a number of classes have been used with a good degree of clinical success for acute bacterial exacerbations of chronic bronchitis, the incidence of resistance by beta-lactamase-producing strains to certain penicillins and cephalosporins continues to increase and represents a growing clinical problem. There also have been reports of significant resistance by Streptococcus pneumoniae to tetracyclines and of treatment failures caused by this organism among patients receiving fluoroquinolones.
