Comparison Among Conventional Multivariable Analysis, Proxy Exposure Analysis, and Instrumental Variable Analysis: Effectiveness of Two-or-More-Dose Vaccination for Measles and Rubella in University Students.

A catch-up campaign for measles and rubella (MR) vaccinations was conducted among third-year high school (MR-4) students between 2008 and 2012 as part of governmental policy. We examined three analytical methodologies to assess the vaccination policies. We measured the antibody titers of the first-year students involved and not involved in the catch-up campaign and analyzed the levels of antibody titers after two-or-more-dose vaccination using conventional regression analysis, the individual's birth year using proxy exposure analysis, and the birth fiscal year using two-stage least square (2SL

Health policy, collaboration and investment in the post-COVID era: a review from UHC and health security integration perspective.

BACKGROUND: The COVID-19 pandemic exposed weaknesses in health systems globally, highlighting chronic underinvestment, fragmentation, and a lack of preparedness with catastrophic impact on public health, economies and societies. Lessons learned reconfirmed the necessity to build health systems resilience, integrating efforts to achieve Universal Health Coverage (UHC) and health security in tandem. RESULTS: This study identified 63 global and regional policies, collaborations, and investments (collectively termed “initiatives”) that came out post-COVID-19 and reviewed them in reference to their

Transforming the Process of Recognition of Medical Specialist in the Ministry of Health: A Shift from Specialist Gazettement to Supervised Work Experience.

The supervised practice of newly credentialed medical practitioners within the public sector following completion of specialist training, referred to previously in Malaysia as specialist gazettement or post-qualification working experience (PQWE), is a unique regulatory mechanism aimed at safeguarding the quality and safety of specialist care. This probationary phase enables senior consultants or specialists to assess the competence and readiness of the practitioners for independent clinical practice. In response to evolving demands on healthcare delivery and workforce governance, the Ministry

Moving from evidence to practice: Models of care for the prevention and management of musculoskeletal conditions.

With musculoskeletal conditions now identified as the second highest cause of the morbidity-related global burden of disease, models of care for the prevention and management of disability related to musculoskeletal conditions are an imperative. Musculoskeletal models of care aim to describe how to operationalise evidence-based guidelines for musculoskeletal conditions and thus support implementation by clinical teams and their health systems. This review of models of care for musculoskeletal pain conditions, osteoarthritis, rheumatoid arthritis, osteoporosis and musculoskeletal injuries and t

The One Big Beautiful Bill Act and the future of prostate cancer care.

The One Big Beautiful Bill Act (OBBBA), signed into law in July 2025, introduced sweeping changes to Medicaid financing. Although framed as measures intended to foster independence and fiscal sustainability, these policies risk destabilizing continuity of care for cancer patients. Prostate cancer patients represent a sizeable population that may be impacted by the OBBBA, as prostate cancer is the most commonly diagnosed cancer in men in the United States. Patients with favorable-risk prostate cancer who are managed through active surveillance may in particular be at risk of poor oncologic outc

Understanding cost variation in medical and health sciences education: An institutional perspective.

Strengthening the health workforce is a central policy challenge in fragile and low-income settings, yet governments often lack basic information on what it costs to train health professionals. This study provides the first institutional level evidence on the structure and magnitude of medical education costs in Afghanistan using detailed administrative and financial records from Kabul Medical University. A retrospective costing approach combined with a step-down cost accounting framework is used to allocate all direct and shared expenditures across faculties and across years of study. The res

European models of long-term care in the home and community.

In the 1980s, faced with a rapidly increasing elderly population and soaring costs of health and long-term care services, many European governments began to reexamine fiscal policies that often encouraged institutionalization of frail and dependent elders. A number of these countries have now turned to new models of home and community-based care. This report describes home care policies that serve the needs of frail elders in Sweden, Denmark, the Netherlands, and Great Britain, with special attention to experimental projects that have tested varying approaches for providing high quality, low-c

Analysing government health expenditure in Africa: Trends, determinants and policy options.

BACKGROUND: In April 2001, African Union (AU) Heads of State adopted the Abuja Declaration, pledging to allocate at least 15% of government expenditure to health. More than two decades later, many African health systems remain underfunded, with external aid increasingly unsustainable as official development assistance declines. AIM: This article examines progress towards the Abuja target, the role of political economy in shaping government health expenditure (GHE), and policy options for sustainable domestic financing. SETTING: The authors examined the Global Health Expenditure across 53 AU

Avoidable Hospitalizations in the Military Health System, Fiscal Years 2018-2019.

INTRODUCTION: Ongoing health reforms in the Military Health System (MHS) are expected to shift locations of ambulatory care for up to 1.9 million beneficiaries. We sought to model the impact of this policy by determining potentially avoidable hospitalizations in the MHS based on different primary care settings. MATERIALS AND METHODS: We used the MHS Data Repository to conduct a retrospective cross-sectional study of TRICARE Prime and Prime Plus beneficiaries aged 18 to 64 years during fiscal years 2018-2019. Crude and adjusted risk ratios for each Agency for Healthcare Research and Quality pr

Identifying food environment policies and research needs in Latin America and the Caribbean: insights from the Community of Practice in Nutrition and Health in Latin America and the Caribbean (COLANSA).

BACKGROUND: Nutrition-related health issues remain a significant public health challenge in Latin America and the Caribbean (LAC), where rapid transformations in food systems have contributed to increasing obesity and diet-related noncommunicable diseases. Government policies are key to shaping healthier food environments, but their implementation and evaluation vary widely across countries. This study aimed to identify national-level public policies that promote healthy food environments in the LAC region and to outline priority areas for research to strengthen their implementation, from the

Pandemic waves and the time after Covid-19 - Consequences for the transport sector.

This paper discusses the dual role of the transport sector in the Covid-19 pandemic: spreading the virus around the world and being most negatively impacted by the pandemic. This paper describes and analyzes the following: (a) actions taken by the governments and international community in order to control the spreading and to alleviate negative economic impacts including massive fiscal and monetary stimulus funding; (b) detailed discussions on the impacts of the pandemic on air transport, rail and bus transport, and urban transit, and major countries' responses to reduce the negative effects;

Uptake of the Heart Failure Management Incentive Billing Code by Family Physicians in Ontario, Canada: A Retrospective Cohort Study.

In 2008, the Ontario Ministry of Health and Long-Term Care (MOHLTC) implemented an incentive payment, Q050A billing code, to family physicians for provision of comprehensive guideline-based care for patients with heart failure in the community. Our objective was to report on the uptake of this program from fiscal years 2008-2014. We determined the numbers of claims billed per year and the proportion of eligible patients with congestive heart failure (CHF) for whom a physician billed. The code was billed by 10.4% of all family physicians in 2008-2009, which increased to 15.1% in 2014-2015. The

Gender and Programming.

The current study examines U.S. prison programming availability and participation by gender on a national level. The authors build upon previous literature by using national-level data, something that has been done in very limited cases previously. The main concern of this study is gender and its effects on programming availability and participation. The U.S. corrections field has undergone major changes in regard to population trends, fiscal constraints, policies, and research over the last few decades without a large-scale examination of the effects of these changes on programming across the

Health Policy for Obesity and Metabolic Syndrome in Asian Health Systems: Three Design Principles, Three Binding Constraints, and the Governance That Decides between Them.

BACKGROUND: Obesity drives metabolic syndrome and cardiometabolic disease in Asia, where complications develop at lower levels of adiposity than in Western populations. We reviewed Asian obesity policies to identify design features that support their durability and how access to incretin-based medications fits within these policies. METHODS: This narrative review used a structured search of PubMed/MEDLINE and Embase (January 2000 to March 2025) supplemented by grey literature from public health agencies. Of 1,884 records, 612 underwent full-text review and 236 met inclusion criteria. Two revi

Promoting Healthier Purchases: Ultraprocessed Food Taxes and Minimally Processed Foods Subsidies for the Low Income.

INTRODUCTION: Fiscal policies can shift relative food prices to encourage the purchase and consumption of minimally processed foods while discouraging the purchase and consumption of unhealthy ultraprocessed foods, high in calories and nutrients of concern (sodium, sugar, and saturated fats), especially for low-income households. METHODS: The 2017-2018 packaged food purchase data among U.S. households were used to derive household income- and composition-specific demand elasticities across 22 food and beverage categories. Policy simulations, conducted in 2022-2023, assessed the impact of nati

Addressing barriers to the prevention, diagnosis and treatment of hepatitis B and C in the face of persisting fiscal constraints in Europe: report from a high level conference.

In the WHO-EURO region, around 28 million people are currently living with chronic viral hepatitis, and 120,000 people die every year because of it. Lack of awareness and understanding combined with the social stigma and discrimination exacerbate barriers related to access to prevention, diagnosis and treatment services for those most in need. In addition, the persisting economic crisis has impacted on public health spending, thus posing challenges on the sustainable investment in promotion, primary and secondary prevention, diagnosis and treatment of viral hepatitis across European countries.

Pension reforms, economic security, and mental health: The need for a human rights-based approach.

Pension systems play a crucial role in providing economic security and supporting well-being in later life. However, as governments implement reforms to ensure financial sustainability-such as raising the retirement age, reducing benefits, and shifting to defined-contribution schemes-these measures often overlook their psychological and social consequences. Pension insecurity has been linked to heightened stress, anxiety, and depression, as well as increased social isolation, particularly among vulnerable populations, including those in physically demanding jobs, low-income workers, and indivi

Testing the Environmental Kuznets Curve Hypotheses in Chinese Provinces: A Nexus between Regional Government Expenditures and Environmental Quality.

With rapid economic growth, the Chinese government expenditures at various levels have increased adequately. At the same time, the environmental quality in China has deteriorated significantly. In this study, provincial-level data for 31 Chinese provinces during 2007-2017 are used to investigate the impacts of government expenditure on the emissions of three specific measures of environmental degradation. The main objective of this study is to examine the influence of government expenditures, economic growth per capita, environment protection expenditure, and added second-sector value on envir

Health economic design for cost, cost-effectiveness and simulation analyses in the HEALing Communities Study.

BACKGROUND: The HEALing Communities Study (HCS) is designed to implement and evaluate the Communities That HEAL (CTH) intervention, a conceptually driven framework to assist communities in selecting and adopting evidence-based practices to reduce opioid overdose deaths. The goal of the HCS is to produce generalizable information for policy makers and community stakeholders seeking to implement CTH or a similar community intervention. To support this objective, one aim of the HCS is a health economics study (HES), the results of which will inform decisions around fiscal feasibility and sustaina
