New needle holder facilitates percutaneous fluoroscopy-guided sacroiliac puncture.

PURPOSE: To evaluate the success of a new guidance device for sacroiliac joint puncture. MATERIAL AND METHODS: The device was first tested on a human anatomical model and thereafter on two groups of patients. In the first group (n=8), the traditional hand-guided method was used, and in the second group (n=10) the needle-holder device. The parameters evaluated in each group were "the number of attempts", defined as the number of skin punctures, and "fluoroscopy-injection time", defined as the time from the beginning of the fluoroscopy to the end of the correct insertion of the needle into the

Thermogangliolysis of the Gasserian ganglion under computed tomography fluoroscopy.

Gasserian ganglion block is an established treatment for trigeminal neuralgia. A landmark approach assisted by X-ray fluoroscopy is the most common method; however, visualization of the foramen ovale is difficult in some cases. Here we report two cases in which a novel technique using modern computed tomography (CT) fluoroscopy was employed. A 63-year-old woman suffering pain in the maxillary nerve area was treated by thermogangliolysis under CT fluoroscopy. The patient was positioned on a CT stage with the head in an overhanging position. The CT gantry was set at an oblique angle to obtain a

Palliative esophageal stent placement using endoscopic guidance without fluoroscopy.

AIMS: Fluoroscopy is not available in every endoscopic unit. This situation leads to delays in treatment or to transfer of patients to other centres for stent insertion. We assessed safety and effectiveness of expandable esophageal metal stent placement under endoscopic control without fluoroscopy using a thin gastroscope. PATIENTS AND METHODS: From October 2002 to June 2004, thirty-three consecutive patients have been included for esophageal stent placement under endoscopic control alone with a nasogastroscope (5.9 mm). A proximal release covered stent (Ultraflex; Boston Scientific Microvasi

Do radiologists' radiation-related opinions predict their fluoroscopy doses?

The influence of radiologists' radiation-related opinions to fluoroscopy doses was studied. Dose-area products (DAP), screening times and the number of exposures per patient were recorded in 528 barium enema examinations performed by 23 residents (14 female, 9 male). The residents' scored opinions (on the general radiation risk in radiology, the importance of radiation protection in radiology and their estimate of their own dose level on which they operate when compared with their colleagues) were correlated with these radiation variables. Residents' opinion on the importance of radiation prot

Detection of moving objects in pulsed-x-ray fluoroscopy.

We investigated the detectability of moving, low-contrast objects in white-noise image sequences. The computer-generated, cylindrical phantoms mimicked arteries, catheters, and guide wires in medical, x-ray fluoroscopy image sequences at 16 acquisitions/s (pulsed-16) or 32 acquisitions/s (pulsed-32). We measured detectability by using a reference-test, adaptive forced-choice method whereby reference and test presentations were alternated during an experimental session to minimize effects of subject attention and accuracy criteria. In the case of the largest cylinder (diameter 0.48 deg), the hi

Breast cancer following multiple chest fluoroscopy: the Ontario experience.

The hypothesis, derived from experience in Nova Scotia, that artificial pneumothorax, requiring repeated fluoroscopy of the chest, is associated with an increased risk of subsequent breast cancer was tested in Ontario by means of a retrospective cancer patient/control study using record linkage. Sanatorium patients treated by artificial pneumothorax were estimated to have incurred a risk of breast cancer between two and three times as high as that of matched, contemporary sanatorium patients treated by other means. There was a tendency for the malignant disease to develop on the side of collap

Fluoroscopy and Radiographs for Detecting Retained Surgical Needles in the Hand.

When a surgical needle is lost, the protocol is to explore the surgical field and to obtain a plain radiograph if the needle cannot be located. The size of the needle that can be detected with imaging is debated. Plain-film radiographs, C-arm, and mini C-arm fluoroscopy imaging was obtained of a cadaveric hand with retained needle of varying lengths (suture sizes 4-0 - 10-0). The authors performed analyses to determine the sensitivity and specificity of the imaging modalities. There were no differences in diagnostic area under the receiver operating characteristic curve between the three modal

Ablation of an atriofascicular accessory pathway with a zero-fluoroscopy procedure.

A 16-year-old patient with recurrent palpitations and documented left bundle branch block superior axis wide complex tachycardia underwent an electrophysiological study and ablation with a zero-fluoroscopy procedure. The electrophysiological study showed a decremental antegrade conducting atriofascicular pathway. Three-dimensional CARTO-guided mapping of the tricuspid annulus in sinus rhythm was performed, and a distinct signal corresponding to the accessory pathway potential of the atriofascicular pathway was found in the posterolateral region. By using an SR0 sheath and a 4-mm-tip catheter,

The effect of NavX on fluoroscopy times in pediatric catheter ablation.

PURPOSE: Catheter ablation is the established curative therapy for pediatric tachyarrhythmias. However, exposure to ionizing radiation from fluoroscopy during the procedure is of concern to both patients and caregivers. We sought to assess the impact of an impedance-based three-dimensional navigation system (NavX(TM), Endocardial Solutions, Inc., St. Paul, MN) on pediatric catheter ablation procedures. METHODS: We retrospectively analyzed procedural data during a 7-year period (2002-2008), which spanned the transition between standard fluoroscopic mapping and adoption of NavX(TM) mapping for

Catheter ablation of atrial fibrillation without the use of fluoroscopy.

BACKGROUND: In performing catheter ablation of paroxysmal atrial fibrillation (PAF), the advent of electroanatomical mapping (EAM) has significantly reduced fluoroscopy time. Recent advances in the ability of EAM systems to simultaneously visualize multiple catheters have allowed some operators to perform certain procedures, such as catheter ablation of supraventricular tachycardias, with zero fluoroscopy use. OBJECTIVE: The purpose of this study was to evaluate the feasibility and safety of pulmonary vein (PV) isolation with zero fluoroscopy use, using a combination of three-dimensional EAM

Diaphragmatic myoclonus: diagnosis by fluoroscopy and electromyography with response to phenytoin.

A 40-year-old woman presented a 6-week history of "fluttering" in her chest. A diagnosis of bilateral diaphragmatic myoclonus was made by fluoroscopy and electromyography of both hemidiaphragms. No central or peripheral cause was identified, but treatment with intravenous phenytoin had an immediate effect. With therapy, she has been symptom free for 12 months.

Total and subtotal colonoscopy with short instrument and without fluoroscopy.

The technique and results of 111 consecutive colonoscopies with a short colonoscope without the use of X-ray screening are reviewed. The caecum could be reached in 44 percent of the cases. No complications occurred. The great clinical value of the procedure is shown by the following findings:X-ray-negative lesions--including 2 cases of carcinoma--were found in 35 percent of the cases, radiologically demonstrated lesions could be defined more precisely in 18 percent, and the presence of colonic lesions could be ruled out in 11 percent in spite of equivocal X-ray findings. It is concluded that m

Utility of airway fluoroscopy as a diagnostic test in children with stridor.

Stridor is a high-pitched airway sound that can be produced from static or dynamic lesions along the upper airway. Airway fluoroscopy (AF) has been utilized as a diagnostic test to evaluate pediatric stridor, but prior studies have shown limitations. We aim to assess the reliability of airway fluoroscopy as compared to direct laryngoscopy and bronchoscopy (DLB) in evaluating pediatric stridor, with stratification of airway subsite. A retrospective chart review was performed. 184 patients were evaluated who had undergone both AF and DLB within one year of each other. Sensitivity and specificity

Is ultrasound superior to fluoroscopy in distal radius volar fixation?

OBJECTIVES: In volar distal radius fixation, conventional and additional fluoroscopic views could not be sufficient to assess dorsal screw penetration. Ultrasound (US) has been suggested as a technique to improve this assessment. The objective was to determine the agreement between these two explorations in a clinical study. Quantify time-consuming of intraoperative US was the secondary objective. MATERIAL AND METHODS: A prospective descriptive study was performed. Thirty patients with a surgical distal radius fracture were treated with volar fixation by five consultant surgeons in a level I

Sleep fluoroscopy for localization of upper airway obstruction in children.

The management of children with upper airway obstruction (UAO) in whom previous airway surgeries or concomitant craniofacial or neuromuscular abnormalities exist is complicated by potential obstruction at multiple sites. Sleep fluoroscopy (SF) provides a dynamic representation of relative degrees of obstruction at multiple levels of the pediatric airway. Fifty-five SF studies were performed on 50 infants and children to localize obstructive sites. Correlation was assessed with findings on direct laryngoscopy and bronchoscopy under spontaneous ventilation. In 24 (44%), endoscopic and SF finding
