Epidemiology of Surgically Treated Foot and Ankle Bone Tumors: A Systematic Review.

BACKGROUND: Bone tumors of the foot and ankle account for 1% to 2% of all skeletal tumors. Our review aimed to determine their distribution according to histologic type, anatomical site, affected bone, patient sex, and age group. METHODS: We reviewed institutional epidemiologic studies focusing on surgically treated bone tumors of the foot and ankle, following the latest classification of the World Health Organization (2020). Our selection process adhered to the PRISMA guidelines. We provide a descriptive synthesis of the results from the included studies, along with subgroup analyses. RESUL

[Nonoperative treatment for the management of osteonecrosis and bone marrow oedema of the foot and ankle].

BACKGROUND: Today, there are several nonoperative treatment measures available for the management of osteonecrosis and bone marrow oedema of the foot and ankle. TREATMENT: These include immobilisation, the use of anti-inflammatory drugs (NSAIDs), physical therapy, orthotics and extracorporeal shockwave therapy (ESWT). Furthermore, off-label use of either bisphosphonates, denosumab or prostacyclins have proven to be effective treatment options. Notably, in many cases, symptoms resolve after several months without any specific treatment. As patients often present with insufficient vitamin D lev

A Case of Rheumatoid Nodulosis with Extensive Bone Erosion by a Rheumatoid Nodule in the Foot.

Rheumatoid nodulosis is a rare variant of rheumatoid arthritis, characterized by similar but milder joint and systemic symptoms compared to typical rheumatoid arthritis. We report a case of rheumatoid nodulosis in a 61-year-old woman with a 6-month history of swelling and pain in her right foot, with subsequent swelling on the back of her right hand, in her left wrist, and over the right tibial tubercle. Radiography showed well-defined radiolucent lesions in several bones of the right foot and the left hand, without narrowing of joint spaces. The 18F-fluorodeoxyglucose positron emission tomogr

Bone and soft tissue reconstruction with tendon balance in severe foot trauma.

In severe foot trauma, it is difficult to determine the level of amputation when the crush injury is severe. We report a case of amputation near Lisfranc that achieved forefoot amputation-like results by using bone and soft tissue reconstruction while considering tendon balance. The patient was a 40-year-old male. The patient's left leg was caught in a garbage truck and sustained a crush injury. The second to fifth metatarsals were amputated at the diaphysis, and a high degree of instability of the Lisfranc joint was observed. A high degree of contamination was detected in the patient's wound,

Relationship Between Type of Accessory Navicular Bone and Radiographic Parameters of the Foot.

BACKGROUND: We evaluated the relationship between the type of accessory navicular bone (ANB) and radiographic parameters of the foot in patients with bilateral ANBs of different types. METHODS: Patients with bilateral ANBs of different types participated in this study between May 2019 and April 2020. Patient data, including age, sex, body mass index (BMI), and presence of symptoms, were obtained. We aimed to compare the radiographic parameters of both feet to evaluate the differences from one another in patients with bilateral ANBs of different types (one side type 1 and one side type 2) beca

Diagnostic Accuracy of Bone Culture Versus Biopsy in Diabetic Foot Osteomyelitis.

OBJECTIVE: To compare the diagnostic accuracy of bone culture (microbiology) and biopsy (histology) in patients with acute or chronic diabetic foot osteomyelitis (DFO). METHODS: This cross-sectional study involved patients for whom providers had a clinical suspicion of DFO. Two bone samples were taken: one for microbiologic testing and another for histologic testing. The sensitivity, specificity, positive predictive value, negative predictive value, and likelihood ratio were calculated for bone culture results in relation to the probability of DFO diagnosis. RESULTS: Fifty-two patients were

Clinical features and outcomes in children with bone and joint infections of the ankle or foot.

BACKGROUND: The foot and ankle are uncommon sites of bone and joint infections (BJIs) in children. The objectives of the present study were to determine the clinical and bacteriologic features of BJIs and to assess any associated complications and orthopedic sequelae. METHODS: We performed a retrospective, single-center study of children treated for foot or ankle BJIs between 2008 and 2018 in a French university medical center. A total of 23 children were included. The median age at diagnosis was 9.1 years. Osteomyelitis was noted in 14 cases; it involved the calcaneus in seven cases, the dis

Intraosseous leiomyoma of the calcaneum: An unusual bone tumor of foot and review of literature.

Leiomyoma is a benign tumor of smooth muscle origin and commonly diagnosed in the uterus, gastrointestinal tract, skin, and mucous membranes. To the best of our knowledge, the only reported intraosseous leiomyomas in extremities occurred in the proximal aspect and distal aspect of the femur, in the tibia, and in the ulna. We are not aware of any previous reports of intraosseous leiomyomas in the foot. The radiograph of the intraosseous leiomyoma shows unilocular or multilocular lytic lesion with sclerotic rim. Due to lack of definitive radiological features on magnetic resonance imaging and co

Talonavicular joint coverage and bone morphology between different foot types.

This study explored three dimensional (3D) talonavicular joint (TNJ) coverage/orientation and bone morphology to reveal parameters that could classify and identify predispositions to cavus and planus feet. 3D models of 65 feet from 40 subjects were generated from computed tomography images classified as pes cavus, neutrally aligned, or asymptomatic/symptomatic pes planus. We calculated the talar and navicular overlap (TNJ coverage). We also measured orientation of the navicular, morphological parameters of the talus and navicular, and angular position of the talar head to body. Pes cavus showe

Inter-observer reproducibility of probing to bone in the diagnosis of diabetic foot osteomyelitis.

AIMS: The objectives of our study were (i) to analyse the inter-observer reproducibility or diagnostic variability of the probing-to-bone test, depending on the training of the professional involved, and (ii) to assess whether the probing-to-bone test can be extrapolated to any professional specialty that deals with these patients. METHODS: This was a cross-sectional study, involving 75 patients with diabetic foot ulcer and clinical suspicion of osteomyelitis. A registration sheet was completed for all patients involved in the research study, gathering data relative to the results of the prob

The use of proximal and distal tibial bone graft in foot and ankle procedures.

The techniques of proximal and distal tibial bone grafting have been well described in the literature. With the growth of a variety of new bone grafting techniques, the proximal and distal tibial bone sites remain reliable and safe for the retrieval of cancellous graft. These sites, particularly the upper tibia, provide large amounts of cancellous graft with little donor site morbidity. Proximal and distal tibial bone grafting remains a technique against which other grafting techniques should be measured.

The indications and use of bone morphogenetic proteins in foot, ankle, and tibia surgery.

Tissue engineering is an area of rapid growth. Tissue engineering in orthopedic surgery involves the use of growth factors, mesenchymal stem cells, and scaffolds, individually or in combination, toward the growth and restoration of various musculoskeletal tissues, such as ligaments, tendons, muscles, nerves, and bone. These advances are constantly evolving in foot and ankle surgery as well. Bone morphogenetic proteins (BMPs) have played an integral role in the advancement of tissue engineering strategies across multiple orthopedic subspecialities and have proved to play a role in the developme

Use of structural allograft to compensate for bone loss in arthrodesis of the foot and ankle.

Allografts have several important advantages over other forms of bone graft augmentation. In addition to preventing the morbidity of autogenous bone graft harvesting, the quantity of allograft bone is essentially unlimited and is therefore valuable for use in treating very large defects that exceed the yield of the iliac crest. Allografts are particularly helpful in obese patients or patients with systemic disease because the they has the potential to reduce blood loss and anesthesia time. Regional anesthesia with either an ankle or a spinal block may be safer for the rheumatoid patient with c

MR imaging of the foot and ankle: patterns of bone marrow signal abnormalities.

Diagnosis of marrow disorders of the foot and ankle is among the more challenging aspects of MR interpretation. Evaluation of normal and abnormal bone marrow with regard to pattern, distribution, and signal characteristics on different sequences often allows a specific diagnosis. This pictorial review illustrates MR imaging findings of normal variants of bone marrow of the foot and ankle, and the varied responses of bone marrow to trauma, stress, or disease.

Chondroblastoma of foot bones; a clinicopathological study of 29 cases confirming the diagnostic utility of H3K36M and H3G34W antibodies at an uncommon site.

OBJECTIVE: Chondroblastoma (CB) is a benign cartilaginous bone neoplasm which commonly occurs in long bones of adolescents. CB can uncommonly involve foot. Its mimics include both benign and malignant lesions. H3K36M immunohistochemical (IHC) stain is a helpful tool for establishing the diagnosis of CB in such challenging situations. In addition, H3G34W IHC stain helps to rule out giant cell tumor which is the closest differential of CB. Our objective was to describe the clinicopathological features and frequencies of H3K36M, H3G34W and SATB2 IHC stains in CB of foot. MATERIALS AND METHODS: W

Midterm and Long-term Follow-up After Surgical Reconstruction for Lateral Polydactyly of the Foot With Mosaic-like Alignment of the Phalanx Bones.

BACKGROUND: The phalanx bones in several cases of lateral polydactyly of foot revealed complicated alignment with radiographic findings revealing medial-protrusion and lateral deviation of the middle phalanx. We previously defined such cases as showing "mosaic-like alignment" and demonstrated favorable postoperative outcomes using our surgical procedure. The aim of this study was to evaluate the midterm and long-term postoperative outcomes in such cases. METHODS: The study included 17 feet from 16 patients; 5 male and 11 female, with one bilateral case. The radiographic findings in all cases
