Effects of ankle-foot orthosis with dorsiflexion resistance on the quasi-joint stiffness of the ankle joint and spatial asymmetry during gait in patients with hemiparesis.

BACKGROUND: Reduced ankle quasi-joint stiffness affects propulsion in the paretic side of patients with hemiparesis, contributing to gait asymmetry. We investigated whether the use of an ankle-foot orthosis with dorsiflexion resistance to compensate for reduced stiffness would increase quasi-joint stiffness and spatiotemporal symmetry in patients with hemiparesis. METHODS: Seventeen patients walked along a 7-m walkway in both ankle-foot orthosis with dorsiflexion resistance and control (i.e., ankle-foot orthosis) conditions. Dorsiflexion resistance by spring and cam was set to increase linear

Extrinsic foot muscle forces and joint contact forces in flexible flatfoot adult with foot orthosis: A parametric study of tibialis posterior muscle weakness.

BACKGROUND: The posterior tibialis tendon dysfunction (PTTD) is typically associated with progressive flatfoot deformity, which could be alleviated with foot orthosis. However, the evaluation of tibialis posterior (TP) weakness on lower limb mechanics of flatfoot adults with foot orthoses is scarce and requires further investigation. RESEARCH QUESTION: This study aimed to examine the effects of TP weakness on lower limb mechanics in flatfoot adults with foot orthosis through gait analysis and musculoskeletal modelling. METHODS: Fifteen young adults with flatfoot were recruited from Universit

Active foot synovitis in patients with rheumatoid arthritis: applying clinical criteria for disease activity and remission may result in underestimation of foot joint involvement.

OBJECTIVE: To determine whether application of criteria for remission in rheumatoid arthritis (RA) may result in underestimation of foot joint involvement among patients in a clinic setting. METHODS: RA patients (n = 123) were assessed at baseline and 6 months after commencement of a response-driven combination disease-modifying antirheumatic drug (DMARD) protocol. Remission was assessed using disease activity measures (the 28-joint Disease Activity Score using the erythrocyte sedimentation rate [DAS28-ESR], Simplified Disease Activity Index [SDAI], and Clinical Disease Activity Index [CDAI])

Diabetic neuroarthropathy (Charcot joints): the importance of recognizing chronic sensory deficits in the treatment of acute foot and ankle fractures in diabetic patients.

Patients with diabetic neuropathy are at a higher risk of developing complications, especially Charcot arthropathy. Early diagnosis and intervention is the key to optimizing outcome. Therefore, diabetic patients with a lower extremity injury should be screened with sensory testing using a 5.07 monofilament.

The effects of an articulated ankle-foot orthosis with resistance-adjustable joints on lower limb joint kinematics and kinetics during gait in individuals post-stroke.

BACKGROUND: Resistance is a key mechanical property of an ankle-foot orthosis that affects gait in individuals post-stroke. Triple Action® joints allow independent adjustment of plantarflexion resistance and dorsiflexion resistance of an ankle-foot orthosis. Therefore, the aim of this study was to investigate the effects of incremental changes in dorsiflexion and plantarflexion resistance of an articulated ankle-foot orthosis with the Triple Action joints on lower limb joint kinematics and kinetics in individuals post-stroke during gait. METHODS: Gait analysis was performed on 10 individuals

The effects of wrist height at lead foot contact on shoulder and elbow joint moments in baseball pitching.

The effects of arm position at lead foot contact (FC) on shoulder and elbow joint kinetics are not well understood. This study investigated the effects of wrist height relative to the shoulder at FC on the joint moments and pitching kinematics in high school and collegiate pitchers using a motion capture system. Peak elbow varus and shoulder internal rotation moments, along with upper extremity, pelvis, and upper torso kinematics, were compared among pitchers with the wrist positioned below, at, or above the shoulder at FC. Compared with pitchers whose wrist remained below the shoulder, those

Effects of habitual foot strike patterns on patellofemoral joint and Achilles tendon loading in recreational runner.

BACKGROUND: Most running biomechanics studies have focused on either the patellofemoral joint (PFJ) or Achilles tendon (AT) alone, generating fragmented understanding of how these structures interact as components of an integrated kinetic chain during running. This study was to investigate concurrent biomechanical changes in the PFJ and AT in recreational runners. METHODS: The recreational runners who are accustomed to run with rearfoot strike (RFS, n = 15) and forefoot strike (FFS, n = 15) patterns were recruited. They were instructed to run at 10 km/h in cushion shoes with their habitual st

Outcomes Following Surgical Correction of Talocalcalcaneal Joint Dislocation in Diabetes Associated Charcot Foot Arthropathy.

BACKGROUND: Growing clinical interest in the treatment of acquired foot deformity due to diabetes-associated Charcot foot arthropathy has led to multiple reports of favorable clinical outcomes in patients when their acquired deformity is at the midfoot level. Clinical failures and less than optimal clinical outcomes are achieved when the deformity is at the hindfoot or ankle levels. METHODS: A retrospective review was performed of all patients who underwent surgical correction of diabetes-associated Charcot foot arthropathy with talocalcaneal dislocation over an 18-year period. Consensus on Indications for Isolated Subtalar Joint Fusion and Naviculocuneiform Fusions for Progressive Collapsing Foot Deformity.

RECOMMENDATION: Peritalar subluxation represents an important hindfoot component of progressive collapsing foot deformity, which can be associated with a breakdown of the medial longitudinal arch. It results in a complex 3-dimensional deformity with varying degrees of hindfoot valgus, forefoot abduction, and pronation. Loss of peritalar stability allows the talus to rotate and translate on the calcaneal and navicular bone surfaces, typically moving medially and anteriorly, which may result in sinus tarsi and subfibular impingement. The onset of degenerative disease can manifest with stiffening

Foot orthoses for first metatarsophalangeal joint osteoarthritis: study protocol for the FORT randomised controlled trial.

BACKGROUND: First metatarsophalangeal (MTP) joint osteoarthritis (OA) is a painful and debilitating condition affecting nearly one in 10 people aged over 50 years. Non-drug, non-surgical treatments are recommended by OA clinical guidelines, yet there have only ever been two randomised controlled trials (RCTs) evaluating such strategies in people with first MTP joint OA. Foot orthoses are a common non-drug, non-surgical strategy used by allied health professionals for people with first MTP joint OA, however, it is unknown whether these devices are effective in improving the symptoms associated

A clinical evaluation of the total first metatarsophalangeal joint prosthesis: the use of footprints in assessing foot contact.

The use of footprints in evaluating surgical results is most significant to the physician. This clinical investigation consisted of patients whose surgical correction of severe hallux valgus included the use of the Swanson-total great toe hinged prosthesis. The resulting failure of weightbearing by the great toe pulp postoperatively were found to be associated with excessive weightbearing by the lesser metatarsal heads, evidenced by discrete keratomas, a relative lesser metatarsalgia, and footprinting analysis. In conclusion, we believe that there is a definite relationship between the lack of

Opposing effects of foot-loading strategies on patellofemoral and tibiofemoral joint compression forces during walking.

BACKGROUND: Patellofemoral (PF) and tibiofemoral (TF) joint loading during gait are strongly influenced by sagittal-plane mechanics and muscle force distribution. Individuals with knee disorders often modify foot-loading patterns, yet the effects of such strategies on PF and TF joint compression forces (PFCF and TFCF) during walking remain unclear. RESEARCH QUESTION: How do forefoot, preferred, and rearfoot weight-bearing walking strategies differentially affect PFCF, TFCF, and knee muscle forces during the stance phase of gait? METHODS: Twenty-eight healthy adults performed forefoot, prefer

Biomechanical effects of adding an articulating toe joint to a passive foot prosthesis for incline and decline walking.

Walking on sloped surfaces is challenging for many lower limb prosthesis users, in part due to the limited ankle range of motion provided by typical prosthetic ankle-foot devices. Adding a toe joint could potentially benefit users by providing an additional degree of flexibility to adapt to sloped surfaces, but this remains untested. The objective of this study was to characterize the effect of a prosthesis with an articulating toe joint on the preferences and gait biomechanics of individuals with unilateral below-knee limb loss walking on slopes. Nine active prosthesis users walked on an inst
