BACKGROUND: In lateral cervical disc herniations, posterior foraminotomy (PF) provides direct nerve root decompression and maintains segmental mobility. However, partial facetectomy can cause instability. This study evaluated long-term clinical outcomes related to cervical sagittal alignment after PF.
METHODS: The study included 48 consecutive patients with lateral cervical disc herniations who underwent PF. Pain and mobility were evaluated using the Numeric Rating Scale and Neck Disability Index (NDI), respectively. Sagittal alignment was evaluated using the modified Toyama method.
RESULTS:

Anterior cervical foraminotomy (ACF) is a surgical procedure for cervical radiculopathy to avoid fusion and adjacent segment disease (ASD), but its long-term outcome has yet to be investigated. It is also unclear whether ACF enables preservation of range of motion (ROM) and decreases ASD compared with anterior cervical discectomy and fusion (ACDF). This study included nine patients who underwent ACF, and 12 who underwent ACDF and with follow-up period of at least 5 years (average follow-up: 8.7 years). Preoperative and postoperative radiological findings were investigated, comparing the change

PURPOSE: To form an osteotomy in endoscopic dacryocystorhinostomy (endo-DCR); the thickness and angulation of the frontal process of the maxilla is a limiting factor often necessitating the use of powered instrumentation. Curved foraminotomy rongeurs might enhance surgical access and improve ostium formation.
METHODS: Endo-DCRs were performed by ophthalmic surgeons on 24 sides of 12 fresh frozen cadavers in an anatomy laboratory. Once the biggest possible ostium had been created with standard rongeurs, surgeons switched to curved foraminotomy rongeurs to further enlarge the ostium. At each ch

OBJECTIVE: Posterior cervical foraminotomy (PCF) is an established motion-preserving alternative to anterior cervical discectomy and fusion (ACDF) for cervical radiculopathy. However, concerns remain regarding the efficacy of PCF in patients with vertically narrow foramina, where decompression in the craniocaudal direction is limited. This study was conducted to evaluate the impact of narrow foraminal height (FH; < 6 mm) on clinical and radiographic outcomes following PCF.
METHODS: This retrospective cohort study reviewed the records of 82 patients who underwent PCF with ≥ 2 years of follow-u

BackgroundThe development of full endoscopic procedures enables surgeons to visualize the operative field very clearly. Posterior foraminotomy using endoscopy was developed as a minimally invasive procedure to reduce the complication of the anterior cervical approach and to preserve the segmental motion without decreasing the effectiveness of nerve decompression. Our aim is to evaluate the result of full endoscopic posterior cervical foraminotomy in our center.MethodsThis is a prospective single-arm study of 65 foraminal disc herniation and foraminal stenosis patients that underwent full endos

Full-endoscopic lumbar foraminotomy (FELF) under local anesthesia has been developed as a minimally invasive alternative for lumbar foraminal stenosis. Some authors have described this technique for treating various lumbar spondylolisthesis. However, few studies have reported the outcomes of FELF for foraminal stenosis in patients with stable spondylolisthesis. This study aimed to demonstrate the specific technique and clinical outcomes of FELF for foraminal stenosis in patients with spondylolisthesis. Twenty-three consecutive patients with foraminal stenosis and stable spondylolisthesis were

BACKGROUND: Intractable hiccups can last for more than 1-2 months and can occur, as shown in this case study, due to cervical nerve root compression.
CASE DESCRIPTION: A 76-year-old male presented with intractable hiccups and neck pain of 7 months' duration. The patient underwent magnetic resonance imaging studies of the entire neuraxis. The only abnormality found was on the cervical magnetic resonance images that demonstrated left C4 nerve root compression due to the C3- C4 lateral/foraminal osteophyte. Following a left-sided C3-C4 laminoforaminotomy, the hiccups and the neck pain improved.

Minimally invasive posterior cervical foraminotomy (MPCF) has shown comparable outcomes to those of an open approach, with shorter operation times and length of hospital stays, as well as decreased blood loss and inpatient analgesic use. This surgical technique is mainly used to treat unilateral radiculopathy due to foraminal soft disc fragments or bone spurs. Three-dimensional (3D) navigation-guidance facilitates the surgical workflow, and it is utilized in planning the incision, determining the extent of the medial facetectomy, and confirming sufficient decompression, especially in the lower

BACKGROUND CONTEXT: Posterior cervical foraminotomy (PCF) with or without microdiscectomy (posterior cervical discectomy [PCD]) is a frequently used surgical technique for cervical radiculopathy secondary to foraminal stenosis or a laterally located herniated disc. Currently, these procedures are being performed with increasing frequency using advanced minimally invasive techniques. Although the safety and efficacy of minimally invasive PCF/PCD (MI-PCF/PCD) have been established, reports on long-term outcome and need for secondary surgical intervention at the index or adjacent level are lackin

OBJECTIVE: Posterior cervical foraminotomy and discectomy (PCFD) is regarded as an effective treatment option for cervical radiculopathy. However, limited exposure of the disc space is one of its major disadvantages. To address this problem, we used a CO(2) laser for sophisticated decompression. The purpose of this study was to demonstrate the clinical outcomes of laser-assisted PCFD and to discuss the benefits of laser use.
METHODS: A total of 47 consecutive patients with cervical radiculopathy were treated with PCFD. Among them, 24 patients were treated with laser-assisted PCFD, and the rem

STUDY DESIGN: A retrospective review of a case series.
OBJECTIVES: Giant thoracic disc herniations remain a surgical challenge and historically have been associated with significant complications. While neurological outcomes have improved with the abandonment of decompressive laminectomy, the attempt to minimize surgical complications and associated morbidities continues through less-invasive approaches. With the current study, we describe a surgical technique to treat giant thoracic disc herniations while minimizing approach-related morbidity.
METHODS: Demographic and radiographic data; cli

OBJECTIVE: Postoperative dysesthesia (POD) is a common complication in surgery involving foraminal diseases, including lumbar foraminal or extraforaminal herniated nucleus pulposus (HNP). Minimal dorsal root ganglion (DRG) retraction is key to preventing POD. We compared the clinical results, safety, and efficacy between the paraspinal transforaminal approach requiring DRG retraction and the interlaminar contralateral approach without DRG retraction for foraminal and extraforaminal diseases.
METHODS: A retrospective cohort study was performed of 50 patients who underwent uniportal transforami

BackgroundPosterior cervical foraminotomy (PCF) and anterior cervical discectomy and fusion (ACDF) are 2 commonly used surgical approaches to address cervical radiculopathy. Demonstrating superiority in clinical outcomes and durability of one of the approaches could change clinical practice on a large scale. This is the largest reported single-institutional retrospective cohort of single-level PCFs compared with single-level ACDFs for cervical radiculopathy.MethodsPatients undergoing either ACDF or PCF between 2014 and 2021 were identified using Current Procedural Terminology codes. Lower cervical and cervicothoracic radiculopathies can be challenging to treat through an anterior approach in patients with short-statured necks. With unilateral pathology, a posterior foraminotomy affords preservation of motion and avoids risks to anterior structures; yet, traditional open or even tubular retractor-based open systems are associated with postoperative muscle pain. Endoscopic approaches reduce muscle retraction and resection and are associated with shorter recovery time. This video demonstrates the endoscopic technique for performing cervicothoracic and thoracic foraminotomies

OBJECTIVE: Although concomitant foraminotomy has been reported to increase the risk of postoperative upper limb palsy (ULP) in cervical laminoplasty, the specific effects of concomitant foraminotomy on ULP remain uncertain. This study aimed to clarify the effect of concomitant foraminotomy on ULP in cervical laminoplasty.
METHODS: We identified 19 patients who developed ULP after laminoplasty with concomitant foraminotomy for radiculomyelopathy with nerve root impingement (laminoplasty with concomitant foraminotomy group [F-group]) from 4080 patients who underwent primary cervical laminoplast

BACKGROUND: Lumbar spinal stenosis is a common cause of radicular and generalized back pain among older adults. Endoscopic minimally invasive surgery, in contrast to open decompression, may provide the opportunity for a less invasive surgical intervention. Thus, the purpose of this study is to evaluate the safety (operative complications, estimated blood loss, operative room time) and effectiveness (pre- versus postoperative level of disability and pain severity) of minimally invasive surgery using endoscopic laminotomy and foraminotomy among a large sample of patients with lumbar spinal steno

BACKGROUND CONTEXT: Posterior cervical foraminotomy (PCF) is a motion-preserving procedure for cervical radiculopathy. Traditional guidelines recommend limiting facet resection to less than 50% to preserve spinal stability, a threshold derived from cadaveric biomechanical studies. However, the clinical relevance of this limit has not been fully validated.
PURPOSE: To evaluate whether facet joint resection exceeding 50% during PCF adversely affects clinical outcomes or radiographic stability.
STUDY DESIGN/SETTING: Retrospective cohort study conducted at a single tertiary referral center.

BACKGROUND: Hydrocephalus is a brain abnormality frequently encountered in neonates. There are several known etiologies of hydrocephalus, which can be classified as either congenital or acquired. Occlusion of the foramen of Monro (FOM) is a cause of hydrocephalus and can be either congenital or secondary to various disorders. Idiopathic obstruction of the FOM presenting as prenatal ventriculomegaly is extremely rare but has been reported.
CASE PRESENTATION: We present the case of a preterm newborn with severe bilateral hydrocephalus due to FOM occlusion in the prenatal period. Although the ne

STUDY DESIGN: Human cadaveric biomechanical analysis.
OBJECTIVE: To investigate the effect on cervical spine segmental stability that results from a posterior foraminotomy after cervical disc arthroplasty (CDA).
SUMMARY OF BACKGROUND DATA: Posterior foraminotomy offers the ability to decompress cervical nerves roots while avoiding the need to extend a previous fusion or revise an arthroplasty to a fusion. However, the safety of a foraminotomy in the setting of CDA is unknown.
METHODS: Segmental nondestructive range of motion (ROM) was analyzed in 9 human cadaveric cervical spine specimens.
