Education and training in psychiatry in the U.K.

BACKGROUND/OBJECTIVE: Recent training and education changes have raised important issues in delivery of psychiatric education at all levels. In this article, the authors describe the current status of mental health education in the training of all doctors and postgraduate training and education in psychiatry in the U.K. METHOD: The authors explore and describe some of the initiatives that are being used in order to increase exposure to mental health placements in the Foundation Program, and they then describe the existing specific mental health opportunities within general practice and other

Forensic Neurology and the Role of Neurologists in Forensic Evaluations.

Forensic psychiatrists may be asked to opine on neurological evidence or neurological diseases outside the scope of their expertise. This article discusses the value of involving experts trained in behavioral neurology in such cases. First, we describe the field of behavioral neurology and neuropsychiatry, the subspecialty available to both neurologists and psychiatrists focused on the behavioral, cognitive, and neuropsychiatric manifestations of neurological diseases. Next, we discuss the added value of behavioral neurologists in forensic cases, including assisting in the diagnostic evaluatio

Forbidden insanity: the systematic denial of mental health issues from a state-run psychiatry.

The authors discuss the issue of psychiatric care and patients during the second half of XX century in Albania, when the country was under an absolute regime of communism. Completely isolated from the rest of the world, the territory remained a prohibited area for scholars, and local authors due to censorship or self-censorship, offered very scarce information. The general feeling of the public was that of a denial of psychic disorders in total, combined with the fear that this kind of disorders has ever since provoked. Nevertheless, insanity defence was a formulation encountered with a certai

The research has been carried out on a sample of ten subjects sent to forensic psychiatric examination through court order, in order to evaluate their accountability and to suggest possible treatment. The sample included the diagnostic category of personality disorders only. Persons with psychotic disturbances, alcohol and drug addicts, mentally retarded and delinquents with organic-cerebral damage were not taken into consideration. Diversion evaluations: a specialized forensic examination.

Diversion programs screen justice-involved individuals for the presence of psychiatric disorders, and after negotiations take place with attorneys and treatment providers, these programs link participants with community-based treatment programs in lieu of incarceration. As the number of diversion programs, including mental health courts, continues to rapidly grow, so does the need for "diversion evaluations". Diversion evaluations are a type of forensic mental health assessment (FMHA) conducted to assist the courts in making decisions regarding diversion eligibility. As a result, they should f

Hospital break. An eight-year review of escapes and absconds from two high security forensic centers.

BACKGROUND: Increasing freedom of movement and community reintegration is a vital part of recovery for forensic service users. Clinical teams realize that risk of unauthorized leave exists when granting leave, tasking them with balancing the recovery-based treatment needs of the patients with the larger obligation to protect the public from undue risk of harm. While considerable literature exists on unauthorized leave from acute psychiatric units, there is still little research specific to unauthorized leave from forensic settings. AIMS: The aim of this study is twofold. First, to examine the

Recording routine forensic mental health evaluations should be a standard of practice in the 21st century.

The standard of practice for forensic interviews in criminal and delinquency cases, other than those conducted as part of brief preliminary screening evaluations or in emergency situations, should include a digital recording requirement. This standard should be adopted because of the greater availability of, and familiarity with, recording technology on the part of mental health professionals, the greater use and proven effectiveness of recording in other contexts of the criminal justice system, and the improvement in court presentation and accuracy of judicial determinations involving forensi

Transsexual genital self-mutilation.

Most genital self-mutilations in nonpsychotic patients are found in transsexuals, and premeditation of sex-conversion surgery is the main objective. In this paper, we will describe the case of a male-to-female transsexual who took out his testes as a way to facilitate the surgery and to circumvent the Brazilian laws.

Rapid Review of Literature Reporting the Experience of Patient and Public Involvement in Prison and Forensic Mental Health Research.

Health research authorities around the world are increasingly committed to embedding public involvement in health and social care research. However, such involvement in internationally published prison and forensic mental health research remains rare. It is therefore particularly important to understand how people are experiencing collaborative research in this field, barriers they may encounter and how these might be overcome. The aim of this review is to explore how academic, practitioner and lived-experience researchers have experienced collaborating on prison and forensic mental health res

Extreme Overvalued Belief and the Legacy of Carl Wernicke.

Individuals with extreme overvalued beliefs often carry out abhorrent and inexplicable acts of violence. They hold odd and bizarre beliefs that are shared by others in their culture or subculture. This creates a dilemma for the forensic psychiatrist because the definition of delusion may not be adequate. We discuss the evolution of the term "overvalued idea" first described in a forensic context by neuropsychiatrist Carl Wernicke in 1892. The overvalued idea is invoked in British scholarship as a definition for beliefs seen in anorexia nervosa, morbid jealousy, paranoid litigious states, and o

[Who is expert on justice--specialists or the local medical and social observers?].

A young Norwegian father was convicted of having battered his 5 month-old daughter to death. The trial relied heavily on a medicolegal autopsy report, the conclusions of which have been later questioned by clinical specialists. These specialists claim that the child died a natural death from hitherto undiagnosed osteogenesis imperfecta. The correspondence raises the question of why the opinions of local medical instances, and of people who had known the family and the circumstances for a long time, were neglected in favour of the opinions of experts during all phases of the trial. These people

Feigned psychiatric symptoms in forensic settings: Diagnostic challenges and lessons from a case series.

BackgroundIn forensic psychiatry, malingering poses unique diagnostic and ethical challenges. Individuals may feign mental illness to avoid criminal liability or gain legal advantage, placing a burden on clinicians to distinguish the genuine psychopathology from simulation. Delusional disorder and criminal responsibility: A systematic review to help improve Iranian judicial perspectives.

Through a review of papers and study reports we researched different nations' approaches and views on patients with delusional disorder and looked at the liability of criminals with this disorder, in hope of finding a more appropriate approach, revision of law and creating fairer law regarding these cases in our country. We used PRISMA standards to select related papers from the following databases: Web of Science, Web of Knowledge, PubMed, Scopus and Google Scholar, as well as SID and Irandoc (national databases). We found 10 cases of delusional disorder and folie à deux, whose process of the

The neurobiology of antisocial behavior in adolescence; current knowledge and relevance for youth forensic clinical practice.

Antisocial behavior in adolescents is generally seen as a neurodevelopmental problem; however, in spite of increasing knowledge on the neurobiology of persistent antisocial behavior, conduct disorders, and psychopathic traits, this knowledge is hardly used in clinical practice. The aim of this review is to give an overview of current research on the neurobiology of antisocial behavior in adolescents and to discuss how this knowledge can be translated to youth forensic clinical practice. First, an overview of recent literature on genetics, neuroimaging, neuropsychology, neurophysiology/neuroend

The impact of legal coercion on the therapeutic relationship in adult schizophrenia patients.

The quality of the therapeutic relationship between psychiatric patients and their attending physicians plays a key role in treatment success. We hypothesize that mandatory treatment is negatively associated with the quality of the therapeutic relationship. In a cross-sectional study design, data on psychopathological symptom load (as captured with the Brief Psychiatric Rating Scale) and on the quality of the therapeutic relationship (as measured with the Scale to Assess the Therapeutic Relationship) were collected from 113 adult male psychiatric patients and 35 attending physicians. Patients
