This study reports the results of 12 unstable extraarticular fractures of the proximal phalanx treated with transarticular intramedullary Kirschner wires. Early proximal interphalangeal joint motion was allowed and all patients achieved uneventful union, with an average total active motion of 265 degrees. Objective physical assessment revealed one significant flexion contracture, one flexor tendon adhesion and one significant rotational deformity. Excellent results were observed in ten of the 12 patients.

The treatment of segmental multilevel fractures of the femur is problematic. Open methods of fracture fixation which strip the soft tissue attachments from the bone cause devascularisation of the middle segment. Mechanical fixation using 2 separate plates results in a stress riser. Eleven fractures of the base of P2 and 8 fractures of the head of P1 were treated surgically. We review the results after a mean follow-up of 4 years 5 months. Due to their complexity is no real consensus on the treatment of P2 and various treatments were used. This series demonstrated two points: first the was no correlation between the degree of patient satisfaction and the section of digital immobilization; second, residual subluxation and/or arthrosic remodeling was frequent but was not incompatible with a good objective and subjective outcome.

In the Department of Traumatology of the County Hospital "Kaposi Mór" 150 children with supracondylar fracture of humerus were treated during 10 years (1980-1989). 119 cases reduced and pinned with percutaneous Kirschner wire are dealt with. 98 cases could be controlled 2-10 years after the accident. The importance of urgent treatment is stressed. The urgent reduction and the closed pinning with Kirschner wire are thought to be an efficient prophylaxis against Volkmann's ischaemic contracture and varus deformity, caused by secondary movements.

Forty-seven open tibial fractures in 43 patients were internally fixed by delayed closed nailing after reaming the medullary canal. The procedure was usually undertaken after complete healing of the wound. Early mobilization was encouraged in all cases. In 37 patients (86%) the fractures were caused by road traffic accidents; 21 patients (49%) had other skeletal and soft tissue injuries. All fractures united with a mean of 19 weeks, but five had delayed union. Most patients had a full range of knee and ankle movements. No infection was encountered in the series.

Thirty-four patients with severe multiple injuries underwent either open or closed nailing of 35 femoral fractures. Open nailing was performed in 17 femurs and closed nailing in 18 femurs. The average abbreviated injury score was 27 in both the open group (range: 17-45) and closed group (range: 22-36).
