The closure of wide palatal clefts and recurrent oronasal fistulae may be challenging. After repeated failure of conventional techniques, microvascular tissue transfer may be indicated in the closure of such fistulae. Depending on the location and the size of the palatal fistula, different tissues are required to sufficiently close the palatal gaps. A subdivision of common flaps into mucosa, muscular, bony, skin, and fascia flaps was carried out to analyze their suitability for alveolar, hard, and soft palate reconstruction. Furthermore, the bulk of flaps and the length of the vascular pedicle

Free tissue transfer has become the most important means of limb salvage treatment after severe trauma. This one-step procedure shortens healing and hospitalization time and minimizes the danger of infection. However, very few studies have considered the use of free tissue transfer for the reconstruction of traumatic limb injuries in children. This study reports 22 such cases treated in the authors' unit between 1993 and 2000 (17 boys and 5 girls; mean age, 8.9 years; age range, 18 months-15 years; 16 lower and 6 upper limbs). All flaps were indicated for repair of acute traumatic defects (20-

Authors report on substitution of severe skin and extensor tendon defect on the back of the hand, developed after a II. grade open septic metacarpal fracture. For the covering of the defect of the soft tissue the paratenon--tendon--skin free flap unit was used, supplied by the a. dorsalis pedis. In one operation the skin in full thickness and the extensor tendons could be substituted at the same time the suppuration was eliminated and the pseudarthroses stabilized with lamellar osteosynthesis.

OBJECTIVE: Hardware exposure after microvascular free tissue transfer (MFTT) for mandibular reconstruction is a significant complication. While osteocutaneous MFTTs are the preferred option, some patients require soft tissue-only MFTTs due to medical or anatomic limitations. Data comparing hardware exposure risk between these approaches are limited. This study compares the rate and timing of hardware exposure between osteocutaneous and soft tissue MFTTs.
METHODS: We conducted a retrospective review of patients undergoing MFTT for mandibular defects at a tertiary care center (11/2011-6/2023).

Background: Digital pulp reconstruction with toe-based flaps reliably restores sensibility and contour, yet the healing behavior of viable subcutaneous fat remains underexplored. Because adipose tissue exhibits patient-specific regenerative and volumetric responses, its preservation represents a key element of personalized fingertip reconstruction. This study evaluates the outcomes of toe pulp flaps with targeted fat preservation to assess how individual tissue biology influences contour and functional recovery. Methods: We retrospectively reviewed consecutive digital reconstructions performed

BACKGROUND: The primary goal of the study is to assess bleeding complications and evaluate ketorolac for its safety in cancer patients undergoing head and neck free-flap surgery.
METHODS: Two cohorts of head and neck cancer patients undergoing free-flap surgery were identified using ICD-10 and CPT codes. Cohort 1 received ketorolac within 5 days of surgery (n = 2280); Cohort 2 did not receive ketorolac within 5 days of surgery (n = 2280). Postoperative hemorrhage occurrence and secondary outcomes were assessed.
RESULTS: Ketorolac was not associated with increased risk of postoperative hemorr

INTRODUCTION: Complex soft tissue injuries in the facial area can arise from various causes. Surgeons face significant challenges when reconstructing these injuries, as they must select appropriate materials based on texture and color, while also considering their composition and properties. The anterolateral thigh (ALT) flap has emerged as a versatile option in clinical reconstructive surgery, offering many advantages over other free flaps. One of its main advantages is the ability to create a multicomponent chimeric flap, allowing for the reconstruction of various complex facial defects.

A 39-year-old woman who had undergone fibular flap reconstruction and simultaneous implant placement after the surgical removal of a cemento-ossifying fibroma was experiencing mild discomfort 3 years later and requested an implant-supported prosthesis to restore her edentulous left maxillary quadrant. Initially, an interim removable partial denture was provided and later duplicated with clear resin. The interim prosthesis identified the excessive thickness of the grafted cutaneous soft tissue and the inadequate emergence angle of the nonfunctioning implants. By using the interim prosthesis as

Sensate cross-finger flaps and homodigital and heterodigital island flaps can usually be used for sensory reconstruction in the volar aspect of the digit. However, when the donor areas are damaged by concomitant injuries, these flaps are not available. The free dorsal digital flap, including both dorsal branches of the proper digital nerves, can be used as an alternative to resolve this problem. The flap was used in 16 patients with soft tissue defects on the volar aspect of the digits. For comparison, we collected a consecutive series of 31 patients with similar defects treated using a cross-

BACKGROUND: Protected elevation represents a critical component of postoperative care, particularly in posteriorly located flaps, to prevent pressure on the flap's vascular pedicle and ensure a successful skin graft. Although several short case series and technique papers have described kickstand placement to prevent heel ulcers as an adjuvant to fixator placement for fracture management, there remains a paucity of reports describing external fixator placement solely for extremity elevation and pressure alleviation in the postoperative care of flap coverage procedures.

Reconstruction of soft-tissue defects on multiple fingers is challenging because the number of recipient vessels for free flaps is limited. We report the use of a free radial forearm flap with multiple perforator-based skin islands for the reconstruction of complex soft-tissue defects involving multiple fingers. The injuries were caused by a heat press. The flap was transferred, like "bunting," to the injured hand with an exteriorized pedicle. The technique presented here is advantageous because it simultaneously covers multiple defects and allows immediate hand therapy after the operation.

BACKGROUND: Complex defects of the forearm often require microvascular reconstruction with osteocutaneous free flaps to salvage the limb. In this review, we report our experience with the use of the free osteocutaneous lateral arm flap to reconstruct such defects in four patients.
METHODS: Three male patients with osseous defects of the ulna and one defect of the radius with associated soft-tissue defects were treated with a free osteocutaneous lateral arm flap between 2004 and 2007. The indications for the procedure included posttraumatic osteitis (3) and bone with soft-tissue defects after

Free omental tissue transfer is a versatile reconstructive option for trunk, head and neck, and extremity reconstruction. Its utility is due to the length and caliber of the vascular pedicle and the malleability and surface area of the flap. We report our experience with omental free flap coverage of complex upper-extremity defects. A retrospective analysis of eight omental free-tissue transfers in seven patients with complex upper-extremity defects between 1999 and 2008 was performed. Indications, operative technique, and outcome were evaluated. Patient age ranged from 12 to 59 years with fiv

Reconstruction for facial contour deformities is still a challenging process and treatment for most cases is achieved only by soft tissue augmentation. The use of free tissue transfer offers the advantage of one step vascularized soft tissue augmentation. This article summarized the authors' use of de-epithelialized free superficial circumflex iliac artery/superficial inferior epigastric artery flap for facial contour deformities. Of these patients, two had hemifacial microsomia, one depressed scar, and one had hemifacial progressive atrophy. Stable restoration of the facial contour was achiev

Reconstruction of combined loss of the Achilles tendon and overlying soft tissue was performed using an antero-lateral thigh free flap in three patients. The cutaneous portion is used to cover the open wound, and a piece of fascia lata is utilised to replace the missing segment of the Achilles tendon. The skin defect ranged from 5 x 2.5 to 7 x 5 cm, and the tendon loss measured from 3.5 to 5.5 cm in length. All of the patients showed satisfactory functional results with a follow-up period from 3 to 9 months. The advantages of the procedure are that: it is a single-staged operation; it promotes

Stable wound coverage after extensive soft-tissue loss of the upper extremity remains a difficult problem in the management of large defects of the upper limb. To prevent further tissue loss owing to infection or inadequate cover when important structures such as vessels, tendons, nerves, joints, and bones are exposed, various free flaps have been introduced into the therapeutic armamentarium of acute plastic surgical management options. Emergency or delayed early reconstruction has been proposed to prevent chronic infection and further tissue loss. We report a series of 12 emergency and delay

A nine-year-old boy who sustained severe grease-fire burns over his right scalp, face, neck, and trunk 14 months previously, presented with extensive alopecia over the right frontotemporoparietal scalp. Near complete coverage of the scalp defect was obtained with pretransfer expansion of the left temporoparietal scalp, followed by free-flap transfer of the expanded tissue to the defect. Pretransfer tissue expansion has the advantages of increasing flap surface area and minimizing donor site deformity. This case illustrates the applicability of pretransfer tissue expansion of free flaps, and de

In patients with hand tumors, advances in medicine have led to a preference for limb-sparing surgery that offers functional and aesthetic advantages. However, limb-sparing surgery often results in soft tissue defects after tumor resection. We present our clinical results using arterialized venous free flaps to reconstruct defects after soft tissue tumor resection. Four patients who underwent arterialized venous free flap for hand reconstruction after soft tissue tumor resection between June 2016 and July 2020 were retrospectively reviewed. Pathological causes of soft tissue tumors were maligna
