Metformin vs. insulin in gestational diabetes. A randomized study characterizing metformin patients needing additional insulin.

AIMS: We compared metformin with insulin as treatment of gestational diabetes mellitus (GDM). Furthermore, we aimed to characterize metformin-treated patients needing additional insulin to achieve prespecified glucose targets. METHODS: We conducted a single centre randomized controlled study with non-inferiority design comparing metformin and insulin in the treatment of 217 GDM patients having birth weight as primary outcome variable. RESULTS: There were no significant differences in mean birth weight expressed in grams [+15 (90% confidence interval (CI): -121 to 89)] or SD units [+0.04 (90%

Diabetes Optimization in Total Joint Arthroplasty: Perioperative Markers, Pharmacologic Strategies, and Wound Care Best Practices.

INTRODUCTION: Diabetes mellitus (DM) accompanies approximately 30% of hip/knee arthroplasty cases and more than doubles the risk of periprosthetic joint infection (PJI), wound failure, thromboembolism, and readmission. Consensus on perioperative glycemic management remains limited. METHODS: The PubMed/MEDLINE database (January 2000 to March 2025) was queried for English language studies of adults with DM undergoing primary or revision hip or knee arthroplasty. Combinations of "arthroplasty," "diabetes," "HbA1c," "fructosamine," "glycated albumin," "GLP1," "SGLT2," and "negative-pressure wound

A Pilot Study Investigating the Clinically Significant Association Between Overweight, Hyperlipidemia, and Insulin Resistance in Dogs With Chronic Liver Disease.

The liver plays a central role in glucose and lipid metabolism. In human medicine, glucose metabolism alterations and insulin resistance are commonly associated with chronic liver disease (CLD), particularly in metabolic dysfunction-associated steatotic liver disease (MASLD). In veterinary medicine, however, the role of glucose metabolism in CLD remains poorly understood. We enrolled 67 client-owned dogs, including healthy controls, hepatopathic dogs with (metabolic hepatopathy [MH]) or without (non-metabolic hepatopathy [non-MH]) metabolic factors such as overweight and hyperlipidemia, chroni

The effect of glycemic control on diabetic cataract progression rate in dogs: A preliminary study.

Diabetes mellitus (DM) is a leading cause of canine cataracts. This study aimed to determine the association between diabetic cataracts progression rate and long-term glycemic control in dogs. Nine dogs recently diagnosed with DM and early incipient cataracts were enrolled. Dogs were fitted with sensors for continuous glucose monitoring and underwent biweekly medical and ophthalmic examinations for 6 months or until phacoemulsification surgery was indicated. Measures of glycemic control were compared between dogs that required phacoemulsification surgery (PS) and those that did not (N), and be

Successful Insulin Glargine Treatment in Two Pet Guinea Pigs with Suspected Type 1 Diabetes Mellitus.

Scientific information on spontaneous type I diabetes mellitus (DM) and treatment modalities in guinea pigs is scarce. As most diabetic guinea pigs are overweight and respond to dietary changes, a disorder resembling type II-DM in humans seems to be most prevalent in this species. In the present report, a nine-month-old female intact guinea pig (GP1) was presented because of a cataract and polyphagia. The physical examinations in GP1 and its littermate, GP2, were unremarkable. Laboratory tests revealed hyperglycemia, hyperlipidemia, elevated fructosamine concentrations, and glucosuria in GP1 a

Recurrence of type 1 diabetes after simultaneous pancreas-kidney transplantation in the absence of GAD and IA-2 autoantibodies.

We report herein the patterns of type 1 diabetes recurrence in a simultaneous pancreas-kidney transplant (SPK) recipient, in the absence of rejection. A 38-year-old female underwent SPK for end-stage nephropathy secondary to type 1 diabetes. Fasting blood glucose, HbA1c, fructosamine, C-peptide and autoantibodies (GAD-65, IA-2) were monitored throughout follow-up. At 3.5 years post-SPK, HbA1c and fructosamine increased sharply, indicating loss of perfect metabolic control, despite C-peptide levels in the normal-high range. Exogenous insulin was restarted 4 months later. C-peptide levels abrupt

Impairment of serum albumin antioxidant properties in obstructive sleep apnoea syndrome.

Antioxidant counteraction of oxidative stress has been poorly explored in obstructive sleep apnoea (OSA). Serum albumin is a major antioxidant agent and structural modifications induced by glucose or free radicals impair its antioxidant properties. The aim of the present study was to compare antioxidant capacities and structural changes of albumin in nonobese OSA patients and healthy volunteers. Albumin structural changes were studied by quenching of fluorescence in the presence of acrylamide. Albumin thiols and fructosamines, reflecting oxidation- and glycation-induced changes in serum albumi

[Assays of HbA1c and Amadori products in human biology].

Different Amadori products, formed during the early steps of the non-enzymatic glycation of proteins, may be assayed in current practice in human biology. The most important marker is HbA1c, resulting from the binding of glucose to the N-terminal extremity of HbA beta chains. HbA1c may be evaluated by various techniques (ion exchange or affinity high performance liquid chromatography, capillary electrophoresis, immunoassay, enzymatic technique) and is considered the best marker of diabetic patient survey. Due to its irreversible and cumulative formation, it provides a retrospective information

[Clinical significance of measuring glycated fibrinogen in plasma in diabetes mellitus].

The clinical significance of measuring glycated fibrinogen (G Fbg) in plasma was studied using a simple, rapid method for determining plasma G Fbg developed by the authors. This method is based on modified fibrin clot formation for purification of plasma Fbg, and G Fbg was estimated by improved colorimetric fructosamine assay. The values of plasma G Fbg were expressed as glycated polylysine equivalents (mumol/l). In this clinical study, 106 examinees who underwent a screening test for diabetes mellitus (DM) because of glycosuria or hyperglycemia were investigated. According to the WHO diagnost

Fructosyl Amino Oxidase as a Therapeutic Enzyme in Age-Related Macular Degeneration.

Age-related macular degeneration (AMD) is an age-related disorder that is a global public health problem. The non-enzymatic Maillard reaction results in the formation of advanced glycation end products (AGEs). Accumulation of AGEs in drusen plays a key role in AMD. AGE-reducing drugs may contribute to the prevention and treatment of AGE-related disease. Fructosamine oxidase (FAOD) acts on fructosyl lysine and fructosyl valine. Based upon the published results of fructosamine 3-kinase (FN3K) and FAOD obtained in cataract and presbyopia, we studied ex vivo FAOD treatment as a non-invasive AMD th
