[Lung function after thoracic trauma. A contribution to the assessment of late functional lesions (author's transl)].

Respiratory function was investigated in 32 patients not later then 5 years after thoracic trauma. Forced expiration curves (volume/time and flow/volume mode) and nitrogen washout methods (single breath, multiple breath) were used including blood gas analyzes and thorax-X-rays. The typical effect of chest injury is mild to moderate restriction, which was found in 72% of the patients. Usually all volumes (FVC, FRC, TC) are reduced. 5 patients had slight to moderate obstruction, which we do not consider to be immediate consequence of trauma. 12 patients had signs of ventilatory maldistribution,

A rationale for epidural analgesia in the treatment of multiple rib fractures.

Thoracic epidural analgesia (EA) is described as an alternative to controlled ventilation in patients presenting with multiple rib fractures. Lung mechanics were especially studied in 6 patients selected from a total of 49. The average ICU stay for this group was 4.5 days (2-11) and the mean age 55.7 years. The EA group was compared with 51 patients primarily ventilated who had an average stay in the ICU of 9.8 days and a mean age of 44.7 years. Mean number of rib fractures of the ventilated group at 6.5 was almost equal to the mean of 6.8 in the EA group. There was a difference in the number

Supine position and sleep loss each reduce prolonged maximal voluntary ventilation.

Because of the prevalence of supine posture and sleep deprivation in both health and disease, we wondered how each of them influences prolonged maximal voluntary ventilation (MVV). Accordingly, we compared 12-second, 1-min, and 10-min isocapnic MVV supine with that measured in the upright posture in 8 healthy subjects. MVV decreased 6-10% supine, independent of test duration (p less than 0.01). Although end-expiratory lung volume was 0.47 liter lower during supine resting breathing (p less than 0.001), end-expiratory lung volumes during short-term MVV maneuvers were identical. To investigate a

[Instrumentation support in respiratory kinesiotherapy].

The points of impact of instrumental support in respiratory physiotherapy are numerous; they concern primarily the pulmonary expansion, bronchial drainage and function of respiratory muscles. The pulmonary expansion may be helped by incitant spirometry and either intermittent or continuous positive pressure respiration, or indirectly by the utilisation of respiration against resistance (expiratory bottles, masks with uni-directional valves and expiratory resistances etc.). These different techniques may be used in the presence of instability of the respiratory units, secondary to an alteration

Prolonged expiratory phase in sleep apnea. A unifying hypothesis.

The mechanisms responsible for the various sleep apnea patterns have not been elucidated. In 5 hypersomnolent patients, we analyzed 613 events that would have been labeled mixed and central apneas had usual recording sensitivity been employed. Recording air flow at high sensitivity revealed that in most of these events expiratory flow was present throughout most of the prolonged period during which there was no inspiratory effort. In 378 events, resumption of inspiratory effort was unaccompanied by air flow (occluded events), whereas in 144 events resumption of inspiratory effort was accompani

[Failure of static pulmonary volume measurements in mucoviscidosis].

With worsening of bronchial obstruction during the course of cystic fibrosis the functional residual capacity (CRF) measured by plethysmography increases progressively. The difference between values of CRF obtained by plethysmography or by Helium dilution increases even more. The difference between the two methods (for CRF) is supposed to show the volume of "trapped"' gas. A similar outcome, although less marked, is observed after physiotherapy. The extent of pulmonary distention and gas trapping is markedly overestimated by plethysmographic measurements, when one considers the anatomical and

Dynamic maintenance of end-expiratory lung volume in full-term infants.

We recorded airflow, tidal volume, respiratory muscle electromyogram (EMG), and chest wall configuration in eight normal newborn infants to investigate the determination of end-expiratory lung volume (EEV). The expiratory flow-volume representation was nearly linear and EMG evidence of respiratory muscle activity was absent during the latter part of expiration in both supine and upright postures, consistent with passive expiration. Occasional breaths were associated with marked retardation of expiratory airflow (braking). During unobstructed apnea, expiration proceeded to the relaxation volume

Effects of continuous positive airway pressure on pulmonary function and blood gases of infants with respiratory distress syndrome.

Nitrogen washout measurements and blood-gas analyses were made on 32 newborn infants with severe RDS at continuous positive airway pressures (CPAP) of 5, 10, and 15 cm H2O. Increases in airway pressure resulted in significant increases in PaO2 and functional residual capacity (FRC). It also produced significant decreases in alveolar turnover rates of the "fast" and "slow" alveolar spaces of a two-space lung model. Changes in CPAP did not significantly affect the distribution of ventilation. The changes in PaO2, due to changes in CPAP, did not correlate well with changes in FRC/wt nor with chan

Respiratory effects of extradural nerve block in the postoperative period.

The effects of continuous extradural block in the postoperative period on arterial hypoxaemia are discussed. Comparing two matched groups of patients following upper abdominal surgery, one receiving continuous extradural analgesia and one narcotic analgesia, there were small but statistically insignificant improvements in PaO2 in the former group. It was concluded that abdominal muscle spasm is not an important cause of postoperative arterial hypoxaemia.

Canine awake head-out plethysmography (HOP): characterization of external resistive loading and spontaneous laryngeal paralysis.

We applied a novel head-out plethysmographic (HOP) method to study awake canine responses to external resistive loading and natural laryngeal paralysis. Measurements of inspiratory and expiratory specific airway resistance (sRaw(insp), sRaw(exp)) were obtained before and after uni- and bidirectional loading (R(add) = 5 cmH(2)O/L/s) in large-breed dogs (n = 9). Mean sRaw(insp) after inspiratory, and sRaw(exp) after expiratory loading were 31.4 and 33.3 cmH(2)Os, respectively. Bidirectional loads induced a significantly greater rise in both sRaw(insp) and sRaw(exp) (55.1 and 61.3 cmH(2)Os) compa

Pulmonary response to nasal-challenge testing of atopic subjects with stable asthma.

Nasal deposition of allergen or histamine could cause bronchoconstriction in subjects with asthma by a reflex mechanism. To investigate this possibility, six atopic subjects with stable asthma and five normal control subjects were studied by nasal-challenge testing with saline, Bermuda grass-pollen allergen, and histamine on paper disks. Challenges were done on 3 separate days by use of a double-blind, randomized trial design. Fivefold serial dilutions were used to determine threshold doses for provocation of at least a twofold increase in the postsaline nasal airway resistance. No patient dev

Individualized positive end-expiratory pressure in laparoscopic surgery: a randomized controlled trial.

BACKGROUND: The reduction in functional residual capacity (FRC) is a significant pathological factor in the development of postoperative pulmonary complications. Appropriate positive end-expiratory pressure (PEEP) is critical to preserve FRC during mechanical ventilation. Our previous study suggests that using driving pressure-guided PEEP can reduce postoperative pulmonary complications. In this study, we hypothesize that individualized PEEP can increase immediate postoperative FRC and improve lung ventilation. METHODS: This single-centered, randomized controlled trial included a total of 91

Effect of inflation on the interaction between the left and right hemidiaphragms.

Loss of the dystrophin glycoprotein complex (DGC) or a subset of its components can lead to muscular dystrophy. However, the patterns of symptoms differ depending on which proteins are affected. Absence of dystrophin leads to loss of the entire DGC and is associated with susceptibility to contractile injury. In contrast, muscles lacking gamma-sarcoglycan (gamma-SG) display little mechanical fragility and still develop severe pathology. Animals lacking dystrophin or gamma-SG were used to identify DGC components critical for sensing dynamic mechanical load. Extensor digitorum longus muscles from

Respiratory mechanics in very low birth weight infants during continuous versus intermittent gavage feeds.

This study was designed to determine whether respiratory mechanics in stable, very low birth weight infants changed after replacing intermittent feeds with continuous feeding. We measured static respiratory system compliance, respiratory system resistance, functional residual capacity (FRC), and tidal volume immediately before feeds and at 20, 60, and 120 min after feeds, and again the next day on continuous feeds. Patients selected for enrollment into the study needed to fulfill the following criteria: 1) birth weight and postnatal weight < 1,500 g, 2) no need for mechanical ventilation, posi

Pulmonary effects after surfactant treatment in premature infants with severe respiratory distress syndrome.

To understand the mechanisms behind the improved oxygenation after intratracheal surfactant instillation, the immediate effects of lung volume and pulmonary mechanics were analyzed. All infants studied were enrolled in multicenter trials in which surfactant therapy was instituted according to a rescue protocol. Infants received either synthetic surfactant (Exosurf) or modified porcine surfactant (Curosurf). Measurements of lung volume and pulmonary mechanics were successfully performed in 12 patients with a birth weight of 1.3 +/- (SD) 1.4 weeks. Functional residual capacity (FRC) and pulmonar

Respiratory function in the prune-belly syndrome.

Respiratory function was evaluated in 11 patients with prune-belly syndrome. Nine had evidence of gas trapping and six of restrictive lung disease. These abnormalities of lung function appear to be secondary to the musculoskeletal disorder associated with prune-belly syndrome rather than parenchymal lung disease.

Beneficial impact of weight loss on respiratory function in interstitial lung disease patients with obesity.

BACKGROUND: Interstitial lung disease (ILD) patients commonly become obese or overweight due to deteriorated daily living activities and the adverse effects of prednisolone. This study aimed to clarify the effect of weight loss on pulmonary function test (PFT) in ILD patients with obesity. METHODS: Among all consecutive ILD patients with a body mass index (BMI) ≥ 27 kg/m2 who received nutrition education for improving obesity between June 2014 and December 2018, we retrospectively included patients who successfully decreased their body weight by over 2 kg and underwent follow-up PFT within 6

Relationship between pulmonary function and unsupported arm exercise in patients with COPD.

UNLABELLED: In patients with chronic obstructive pulmonary disease (COPD) the limitation on unsupported arm exercise (UAE) is predominantly respiratory muscle function-dependent. It is characterized by neuromechanical dysfunction (thoracoabdominal dyssynchrony) of the inspiratory muscles (diaphragm, accessory), superimposed by lung mechanics dysfunction. The undergoing mechanism is probably multifactorial. To study the relationship of resting pulmonary function and UAE performance in patients with COPD.
