Enhancement of drug absorption by antacids. An unrecognised drug interaction.

Antacids are widely used for many disorders. The potential of antacids to interact with other concomitantly ingested drugs is well recognised. These interactions usually result in reduced or delayed absorption of the affected drug. However, this is not always the case. In contrast to aluminium hydroxide, magnesium hydroxide and sodium bicarbonate can enhance the absorption of some drugs. For example, magnesium hydroxide can increase the rate and sometimes even the extent of absorption of certain nonsteroidal anti-inflammatory drugs (e.g. tolfenamic acid, mefenamic acid and ibuprofen), sulphony

The role of activated charcoal and gastric emptying in gastrointestinal decontamination: a state-of-the-art review.

Gastrointestinal decontamination has been practiced for hundreds of years; however, only in the past few years have data emerged that demonstrate a clinical benefit in some patients. Because most potentially toxic ingestions involve agents that are not toxic in the quantity consumed, the exact circumstances in which decontamination is beneficial and which methods are most beneficial in those circumstances remain important topics of research. Maximum benefit from decontamination is expected in patients who present soon after the ingestion. Unfortunately, many overdose patients present at least

A review of methods for assessment of the rate of gastric emptying in the dog and cat: 1898-2002.

Gastric emptying is the process by which food is delivered to the small intestine at a rate and in a form that optimizes intestinal absorption of nutrients. The rate of gastric emptying is subject to alteration by physiological, pharmacological, and pathological conditions. Gastric emptying of solids is of greater clinical significance because disordered gastric emptying rarely is detectable in the liquid phase. Imaging techniques have the disadvantage of requiring restraint of the animal and access to expensive equipment. Radiographic methods require administration of test meals that are not

Population pharmacokinetic of paracetamol and atorvastatin with co-administration of semaglutide.

Semaglutide is a glucagon-like-peptide-1 (GLP-1) analogue marketed for once-weekly subcutaneous administration for type 2 diabetes mellitus. Like other long-acting GLP-1 analogues, semaglutide reduces gastric emptying and, potentially, alters the rate of absorption of orally co-administered drugs. The objective of the current analysis was to evaluate the effects on the gastric emptying rate caused by semaglutide on pharmacokinetic model parameters of paracetamol and atorvastatin in healthy subjects. Non-linear mixed effect modeling was used to estimate population pharmacokinetic model paramete

Influence of the route of administration of prostaglandin E1 on rat gastric secretion.

Changes in gastric secretion induced by the subcutaneous, intraduodenal or intragastric administration of prostaglandin E 1 (PGE1) were evaluated in pylorus-ligated rats. Subcutaneous and intraduodenal injections produced a dose-related inhibition in both total acid and volume of gastric secretion. Dose-response curves for inhibition obtained by these routes were parallel, although PGE1 was more potent when given subcutaneously. Gastric administration produced a dose-related decrease in acid and an increase in volume. The slope of the dose-response curve for acid inhibition with this route was

Impact of gastric mixing and water intake on postprandial pharmacokinetics of orally disintegrating tablets: A PBBM-based study.

OBJECTIVE: Food intake substantially influences the oral absorption of many drugs, and the ability to quantitatively predict food effects in humans prior to clinical studies is critical for optimizing dosing regimens and formulation design. Although various in silico approaches have been proposed to predict food effects on oral solid dosage forms, their predictive performance remains limited. Orally disintegrating tablets (ODTs), which can be administered without water, are known to exhibit characteristic postprandial absorption profiles that are inadequately captured by conventional models. Effects of xylitol on carbohydrate digesting enzymes activity, intestinal glucose absorption and muscle glucose uptake: a multi-mode study.

The present study investigated the possible mechanism(s) behind the effects of xylitol on carbohydrate digesting enzymes activity, muscle glucose uptake and intestinal glucose absorption using in vitro, ex vivo and in vivo experimental models. The effects of increasing concentrations of xylitol (2.5%-40% or 164.31 mM-2628.99 mM) on alpha amylase and alpha glucosidase activity in vitro and intestinal glucose absorption and muscle glucose uptake were investigated under ex vivo conditions. Additionally, the effects of an oral bolus dose of xylitol (1 g per kg BW) on gastric emptying and intestina

Role of gastrointestinal transit in the delay of absorption by viscous fibre (guar).

The influence of guar gum on gastric emptying of liquids in healthy volunteers and on mouth-to-caecum transit time in controls and partially gastrectomized patients has been studied. Together with a flattening effect on blood glucose profiles after a liquid glucose load, guar significantly delays mouth-to-caecum transit time both in healthy controls and partially gastrectomized patients. The retardation of transit is not explicable by a delay in gastric emptying, as can be shown by means of an isotope technique. Only in "quick starters' was a delay in initial gastric emptying observed. The exp

Alteration in digestion and absorption of nutrients during profound acid suppression.

Gastric acid suppression therapy has for many years been the cornerstone of the treatment of peptic disease. The availability of more potent inhibitors of gastric acid secretion and the increasing demand for maintenance therapy has renewed interest in the potential side-effects of profound and/or long-lasting therapy. This chapter focuses on the potential interference of gastric acid suppression therapy with the process of the digestion and absorption of nutrients. The theoretical mechanisms by which hypochlorhydria resulting from gastric acid suppression therapy may hamper digestion and absor

Maltitol inhibits small intestinal glucose absorption and increases insulin mediated muscle glucose uptake ex vivo but not in normal and type 2 diabetic rats.

This study investigated the effects of maltitol on intestinal glucose absorption and muscle glucose uptake using ex vivo and in vivo experimental models. The ex vivo experiment was conducted in isolated jejunum and psoas muscle from normal rats. The in vivo study investigated the effects of a single bolus dose of maltitol on gastric emptying, intestinal glucose absorption and digesta transit in normal and type 2 diabetic rats. Maltitol inhibited glucose absorption in isolated rat jejunum and increased glucose uptake in isolated rat psoas muscle in the presence of insulin but not in the absence

Intestinal absorption of fluoride preparations.

Gastric intestinal absorption of five different preparations of sodium fluoride was measured by means of ten hour urinary excretion. Best absorption was by nonenteric coated sodium fluoride. All three enteric coated preparations showed poorer absorption, with marked individual variations. Florical, the currently available preparation, showed better absorption than any enteric coated tablets and only slightly less than sodium fluoride alone. For active cochlear otospongiosis two capsules three times a day should be prescribed.

THE ROLE OF THE UPPER GASTROINTESTINAL FLORA IN THE MALABSORPTION SYNDROME.

Bacterial contamination of the upper gastrointestinal tract by colonic flora appears important in the production of some forms of secondary steatorrhea. Gastric contents have been studied in patients with normal intestinal absorption and with malabsorption syndrome. In secondary steatorrhea there is a significantly increased incidence of gastric contamination by colon organisms, and this is usually associated with absence of free hydrochloric acid. It seems probable that such contamination interferes with intestinal digestion and absorption. The benefit resulting from the oral administration o

The effect of parietal cell vagotomy and selective vagotomy with pyloroplasty on iron absorption. A prospective randomized study.

Iron absorption from a composite meal was studied in 37 male patients before and 1 year after parietal cell vagotomy (PCV) and selective vagotomy with pyloroplasty (SV + P) in a prospective randomized series. The ability to absorb dietary non-haem iron was studied by relating in each subjects the food iron absorption to the absorption from a small dose of ferrous iron, which has been shown to be unaffected by gastric surgery. After both PCV and SV + P there was a malabsorption of food iron which was statistically significant in patients with increased iron requirements caused by phlebotomy. Ma

Human adult-onset lactase decline: an update.

Human adult-onset lactase decline is a biologic feature characteristic of the maturing intestine in the majority of the world's population. The digestion and absorption of lactose, the major carbohydrate in milk and also the main substrate for lactase, is often variable, a consequence of lactase levels, gastric emptying rate, and colonic salvage. Although commercially available "lactase" products alleviate symptoms in many lactose-intolerant people, a greater understanding of this variability in lactose tolerance could lead to interventions that reduce the rate of gastric emptying and/or incre

The drinking test: a current noninvasive technique to evaluate gastric accommodation and perception.

Impaired gastric accommodation has been proposed as an im- portant mechanism in the generation of functional dyspepsia. There is an interest in methods that allow recording and quantifica- tion of the gastric accommodation reflex. Drinking tests, with water or nutrients, have been developed as a noninvasive, inexpensive method to assess gastric perception and accommodation. These tests are easily performed, do not need any special equipment and are well tolerated by patients. Drink test results are reported as the maximum tolerated volume, individual and cumulative symptom scores. Patients wit

Effect of histamine H2-receptor antagonists on vitamin B12 absorption.

OBJECTIVE: To discuss the potential of histamine H2-receptor antagonists (H2RAs) to cause malabsorption of vitamin B12 (cyanocobalamin). DATA SOURCES: Pertinent literature was identified via a MEDLINE search. Journals and references cited in published articles also were used as data sources. STUDY SELECTION: Studies evaluating the effect of H2RAs on vitamin B12 absorption were reviewed. DATA SYNTHESIS: H2RAs decrease acid secretion by the gastric parietal cells. Gastric acid and pepsin produced by these cells are required for the cleavage of vitamin B12 from dietary sources. Intrinsic facto

Contrasting effects of the stomach and small intestine of rats on copper absorption.

Since the severity of copper deficiency has been shown to be enhanced by feeding diets containing fructose but ameliorated by diets containing starch, we decided to investigate the effect of fructose or starch on copper absorption. As copper transport has been reported to occur also from the stomach, it was possible that copper absorption is inhibited by fructose already from that tissue. Under anesthesia, stomachs of 72 rats fed copper-deficient or supplemented diets containing fructose or starch were ligated prior to the oral administration of 64Cu. Gastric absorption of 64Cu was studied whe

Mechanistic embedding of concise physiological absorption models for enhanced prediction in population pharmacokinetic modeling of oral ibuprofen.

Ibuprofen, a commonly used non-steroidal anti-inflammatory drug (NSAID), exhibits considerable variability in pharmacokinetics following oral administration, particularly during the absorption phase. This study aimed to enhance the mechanistic understanding of ibuprofen absorption by developing and integrating physiologically-informed models of absorption within a population pharmacokinetic (PopPK) framework. Using existing datasets of ibuprofen plasma concentrations, two one-compartment PopPK models with linear elimination were developed: the Sequential Absorption Pharmacokinetic (SAPK) model

Exocrine gastric secretion and gastritis: pathophysiological and clinical relationships.

Gastric exocrine secretion, both acid and non-acid, is required for micronutrients absorption, such as iron, calcium and vitamin B12, drugs absorption, protein digestion. Clinical presentation of a gastric secretion impairment might be then characterized by the presence of both gastrointestinal and non-gastrointestinal specific symptoms (i.e. anemia) or to a non-response to therapies. The main factor that impairs gastric exocrine secretion homeostasis is mucosal chronic inflammation that principally occurs after colonization by Helicobacter pylori (Hp). The extent and distribution of gastritis
