The article analyses the results of pylorus-preserving resections of the stomach in 116 patients among whom 80 had gastric ulcer, 29 ad duodenal ulcer, and 7 had gastroduodenal ulcers. Changes of acidity and motor-evacuation function of the gastric stump were studied according to the method of pylorus-preserving resection. The late-term results of treatment in follow-up periods of 2 to 5 years are shown, they were found to be good in 96.6% of patients treated by operation and poor in 1.7%. It is noted that the function of the gastric stump is better when the vagus innervation of the pylorus is

Pancreatic fistula after pancreaticoduodenectomy represents a critical trigger of potentially life-threatening complications and is also associated with markedly prolonged hospitalization. Many arguments have been proposed for the method to anastomosis the pancreatic stump with the gastrointestinal tract, such as invagination vs. duct-to-mucosa, Billroth I (Imanaga) vs. Billroth II (Whipple and/or Child) or pancreaticogastrostomy vs. pancreaticojejunostomy. Although the best method for dealing with the pancreatic stump after pancreaticoduodenectomy remains in question, recent reports described

BACKGROUND: Risk factors for anastomotic leakage include local factors such as excessive tension across anastomosis and increased intraluminal pressure on the gastric conduit; therefore, we consider the placement of a nasogastric tube to be essential in reducing anastomotic leakage. In this study, we devised a safe and simple technique to place an NGT during an end-to-side, automatic circular-stapled esophagogastrostomy.
METHODS: First, a 4-0 nylon thread is fixed in the narrow groove between the plastic and metal parts of the tip of the anvil head. After dissecting the esophagus, the tip of

The development of a peptic jejunal ulcer after partial gastrectomy may result either from a technical deficiency of the anastomotic region, a seldom-occurring gastrin producing tumor, a displacement of the pyloric antrum into the alkaline milieu of the duodenal stump, an abnormally high vagus tone, or from too low a resection. In 17 out of 32 patients with a peptic jejunal ulcer preoperative gastric secretion analysis were performed during consecutive stimulation of the gastric secretion by gastrin-pentapeptide, histalog, and insulin-induced hypoglycemia. A high basal secretion, which could o

Buried bumper syndrome (BBS) was described as a complication of percutaneous endoscopic gastrostomy (PEG) that occurs when the internal stump of the probe migrates and is located between the gastric wall and the skin. The increase of compression between the internal stump and the external stump of the gastrostomy tube causes pain and the inability to feed. We present the cases of three patients with BBS managed by the metabolic and nutritional support department. These cases intend to illustrate one of the less frequent complications of PEG, clinical presentation, risk factors, diagnosis, and

Because of the higher risk of cancer in the gastric stump, an increased incidence of pre-cancerous conditions should be exspected also in the resected stomach. Therefore, a combined endoscopic and bioptic study was performed in order to investigate the incidence of dysplasias in the gastric stump after resection for benign conditions. Among 101 patients with gastric resection, 2 cases were excluded from this study because of preceeding gastric cancer and one because of cancer of the gastric stump. In 43 of the remaining 98 patients, a Billroth-I-resection (gastroduodenostomy) had been carried

Inflammatory bowel disease, vascular disease, volvulus, adhesions, or abdominal trauma may necessitate extensive small-bowel resection resulting in an ultra-short distal duodenal or jejunal stump. If this distal duodenal or short jejunal stump is too short for stoma creation and bowel continuity restoration is hazardous or not possible at all, a distal duodenogastrostomy or proximal jejunogastrostomy in combination with drainage of the stomach is an option to prevent stump leakage. Although successful, this distal duodenogastrostomy has been described only in very few patients and in older rec

The development of simultaneous primary gastric lymphoma and carcinoma is a rare event for which a possible etiopathogenetic role for Helicobacter pylori (HP) recently has been postulated. We report a series of eight such cases diagnosed from 1980 to 1995. In two cases, both tumors arose in a gastric stump, at 26 and 34 years, respectively, after gastric resection for a duodenal ulcer. Grossly, the lymphoma and carcinoma formed a single lesion in four cases (collision tumor); they were separated in the other four cases. Histologically, all the lymphomas fit into the category of B-cell mucosa-a

INTRODUCTION: Achalasia is a rare esophageal smooth muscle disorder. There is a global increase in the prevalence of obesity, so the coincidence of obesity and rare conditions like achalasia will more occur.
MATERIALS AND SURGICAL TECHNIQUE: Here we present the Heller myotomy and Dor fundoplication using gastric remnant in patient with history of one anastomosis gastric bypass (OAGB).
DISCUSSION: Heller myotomy and Dor fundoplication using gastric remnant is feasible with good results in patient with achalasia and history of OAGB.

Two unusual cases of anastomotic ulcer complicated by bleeding after gastric resection are reported. There were retained antrum in one patient and long 'dry' intestinal loop after laparoscopic Roux resection in another one. Emergency gastric stump resection with Haberer procedure were successfully performed in both cases.

Based on the data obtained in fibrogastrochromoscopy and histological studies of gastric biopsy specimens from patients with ulcerous gastroduodenal disease, prior to surgical intervention and after gastric resection for this affection, it was found that in "standard" gastric resection with 2/3 of the organ the possibility of remaining a portion of the antral mucous membrane in the gastric stump is practically excluded and, consequently, the role of this factor in the development of peptic ulcer of the anastomosis seems to unliely after the so-called inadquate in extent gastric resections.

Gastric bypass is the most frequently performed bariatric technique, with a notable increase in this procedure in recent decades. Its effectiveness in weight loss and resolution of comorbidities is well established, but it is not free from complications. Gastric remnant neoplasms after bypass develop in the excluded stomach portion, usually adenocarcinomas. Though infrequent, their risk increases due to anatomical and physiological changes induced by surgery, such as altered gastric secretion and microbiota. Early diagnosis is challenging due to endoscopic inaccessibility and nonspecific sympt

Background: Duodenal stump fistula represents an infrequent but serious complication after laparoscopic radical gastrectomy with Billroth II or Roux-en-Y reconstruction for gastric cancer. The present study was designed to evaluate the effectiveness of laparoscopic double half purse-string sutures plus "8" pattern of stitching for reinforcement of duodenal stump. Methods: The data of patients undergoing laparoscopic radical gastrectomy with Billroth II or Roux-en-Y reconstruction were retrospectively analyzed between August 2022 and June 2023. According to the different reinforcement methods o

Gastric ulcer is situated at a locus minoris resistentiae of the gastric body, an area of which operative removal is technically simple and without major risks. A gastric ulcer which has not completely healed after 3 months of medical therapy is potentially malignant and requires surgical therapy. For type I gastric ulcer we perform partial gastrectomy with Billroth I reconstruction. Type II gastric ulcers call for partial gastrectomy with Billroth I reconstruction and vagotomy of the gastric stump. For type III gastric ulcers, 2 equivalent surgical procedures - proximal gastric vagotomy and p

We report a rare case of resected endocrine cell carcinoma in the remnant stomach. A 68-year-old man had undergone laparoscopy-assisted distal gastrectomy for gastric cancer. Eleven months later, postoperative endoscopy survey showed a flat, depressed lesion in the posterior wall of the remnant gastric cardia. After 3 months, endoscopic re-examination indicated that the tumor had grown rapidly and had changed in shape. Endocrine cell carcinoma was diagnosed by biopsy. He then underwent total resection of the remnant stomach with lymph node dissection. Although adjuvant chemotherapy was given,

The analysis of Helicobacter pylori (HP) and secretory IgA (SIgA) effects on the course and recurrence of gastroduodenal ulcer provides grounds to suggest that a pathogenic role of HP in secretory hypofunction of the gastric stump, unchanged mucosa and normal SIgA production is not leading. Elimination of the pathological process may bring functional restoration of the stump mucosa which adapts to HP invasion. Therefore, expansion of indications to eradication therapy in late postoperative period needs further study.

INTRODUCTION: The aim of this study was to determine the contribution of preoperative gastric secretory and hormonal response, to the appearance of Barrett's esophagus in the esophageal stump following subtotal esophagectomy.
METHODS: Thirty-eight end-stage chagasic achalasia patients submitted to esophagectomy and cervical gastric pull-up were followed prospectively for a mean of 13.6 +/- 9.2 years. Gastric acid secretion, pepsinogen, and gastrin were measured preoperatively in 14 patients who have developed Barrett's esophagus (Group I), and the results were compared to 24 patients who did

AIM: We aimed to establish the independent predictive factors (from Helicobacter pylori infection, biliary reflux, histologic features of the gastric mucosa, drugs, comorbidities, and social habits) for gastric stump ulcer occurrence more than 15 years after surgery.
METHODS: 76 patients with previous gastric surgery were included: 21 patients with gastric ulcer (marginal ulcer or ulcer of the rest of the gastric remnant-study group) and 55 controls (nonulcer group).
RESULTS: Helicobacter pylori infection tended to be higher in the control group than in the ulcer group (14.5% vs. 4.8%, p = 0

There is not general agreement concerning the ideal way of preventing pancreatic leakage after duodenopancreatectomy. The aim of the present study is to present the experience of a three years period of performing a pancreatogastroanastomosis (PGA) after duodenopancreatectomy (DPC) in 12 consecutive patients. In 7 cases PGA was performed after closure of the gastric stump using a transverse incision of the posterior wall of the stomach. A total layer of nylon 9 points was doubled by an interrupted serocapsular suture. In one case only the seromuscular layers of the stomach were cut, adjusted t
