Endoscopic therapy for main pancreatic-duct rupture after Silastic-ring vertical gastroplasty.

BACKGROUND: Acute pancreatitis with pancreatic-duct rupture and fluid collections is a rare complication after Silastic ring vertical gastroplasty. It can be attributed to pancreatic trauma occurring during surgery. METHODS: Endoscopic therapy with transmural drainage of collections was performed in 4 patients who had undergone Silastic ring vertical gastroplasty and who had presented with acute pancreatitis with main pancreatic-duct rupture at the body of the pancreas. OBSERVATIONS: All patients had successful transmural drainage with cystogastrostomy, followed by stent insertion. Only one

Some hormonal changes before and after vertical banded gastroplasty for severe obesity.

BACKGROUND: Hormonal disturbances play a role in the development of obesity, but may be a consequence of obesity itself. In this study we assessed the influence of the surgically-induced weight loss on some important hormonal abnormalities in the morbidly obese patients. MATERIAL AND METHODS: Fasting serum prolactin, insulin, cortisol and thyroid hormones: free thyroxin (FT4), free triiodothyronine (FT3) and thyrotropin (TTH), have been studied by radioimmune methods before vertical banded gastroplasty (VBG) and after operation in the early (10-14 days) and late period when excess weight loss

42-month preliminary follow-up of the silastic ring vertical banded gastroplasty.

BACKGROUND: The authors review preliminary experience with silastic ring vertical gastroplasty (SRVG). METHODS: Of 202 patients who underwent SRVG, 191 are more than 3 months postoperation and of these 165 were accessible for review. RESULTS: Pouch volume could not be readily measured. The TA90BN stapler was occasionally difficult to apply exactly at the angle of His. There was one subphrenic abscess, one gastric bleed, and one dehiscence. Vomiting occurred in eight patients who required reoperation: ring removal, three; cholecystectomy, one; conversion to vertical banded gastroplasty, one;

Fertility, miscarriage and pregnancy after vertical banded gastroplasty operation for morbid obesity.

Vertical banded gastroplasty (VBG) is today the most widely performed bariatric operation. The procedures used previously for the same purpose resulted in metabolic derangements and other pathologies during pregnancy. In a retrospective study we compared the reproductive performance of nine women prior to VBG and after it. Of eighteen pregnancies prior to the operation, seven ended with miscarriage, while only one of fourteen pregnancies conceived after the operation was aborted. The rate of spontaneous conception was greatly increased after the operation and there were fewer complications in

Results of revisional operation following vertical banded gastroplasty performed due to morbid obesity--comparison between restoration of vertical banded gastroplasty and conversion of gastric bypass up to three years.

The aim of this study was to analyse incidence and efficacy of revisional surgery for failed vertical banded gastroplasty among 458 patients who underwent primary surgery between 1993 and 2003. Staple line disruption was diagnosed in 29 patients and was an indication for restoration of gastroplasty in 10 cases and a conversion to Roux-en-Y gastric bypass in 19 patients. In two cases of outlet stenosis the band was exchanged to enlarge the collar. In two cases of psychological intolerance of restriction the band was removed because of refusion by patients the conversion to Roux-en-Y gastric byp

Endoscopic Sleeve Gastroplasty versus Laparoscopic Sleeve Gastrectomy: A Systematic Review and Meta-Analysis.

BACKGROUND: Laparoscopic sleeve gastrectomy (LSG) is the most common bariatric procedure worldwide, while endoscopic sleeve gastroplasty (ESG) has emerged as a less invasive alternative. This study compared their effectiveness and postoperative outcomes. METHODS: A systematic search of PubMed, Scopus, and Cochrane Central was conducted up to June 2025. Non-randomized studies comparing ESG with LSG were included. Pooled odds ratios (ORs) and mean differences (MDs) with 95% confidence intervals (CIs) were pooled using random-effects models. Heterogeneity was assessed with I². Analyses were perfo

Financial buy-in does not affect outcomes of endoscopic sleeve gastroplasty: Retrospective cohort.

BACKGROUND AND STUDY AIMS: Endoscopic sleeve gastroplasty (ESG) is an effective treatment for obesity but typically is not covered by insurance. It is not known whether patients with financial investment in their endoscopic procedure are more likely to achieve and/or maintain weight loss as compared with those who have no financial buy-in. We aimed to compare treatment adherence and outcomes between patients paying out-of-pocket (OOP) and those who underwent ESG as part of any clinical trial where costs were covered by a study protocol (no payment; NP). PATIENTS AND METHODS: Data were collect

Two cases of conversion of vertical ring gastroplasty to adjustable silicone gastric banding.

BACKGROUND: Since May 1995, we have used laparoscopic adjustable silicone gastric banding (ASGB) as an alternative to silastic ring vertical gastroplasty (SRVG) to treat morbid obesity. Moreover, it seemed that ASGB was an appropriate procedure to use when SRVG had failed and no alternative procedure could be attempted again, which occurred in two patients. Because of adhesions, the laparoscopic approach was inappropriate in both cases. The size of the pouch and the staple-line were not obstacles to ASGB. METHODS: Case 1 was a 53-year-old woman of 111 kg (BMI = 46) who had SRVG in July 1994.

Necrotizing Sialometaplasia after Silastic Ring Vertical Gastroplasty: Case Report and Review of Literature.

Necrotizing sialometaplasia (NS) is a self-limiting benign condition of unknown origin that affects mainly the minor salivary glands on the hard palate. In its clinical and histological appearance, it mimics malignancy. This report presents a patient with NS who suffered from frequent bouts of vomiting subsequent to silastic ring vertical gastroplasty which resulted in remarkably low pH of the oral cavity. The literature pertaining to NS appearing in conjunction with gastrointestinal disorders was reviewed, and the possible contribution of recurrent emesis to the pathogenesis of NS is discusse

Weight loss after gastroplasty: psychological sequelae in relation to clinical and metabolic observations.

Twenty-one grossly obese patients were studied before and repeatedly after gastroplasty. Eighteen months after surgery, three groups of patients were identified which had similar preoperative weights but showed significantly different patterns of weight loss. About one third of the patients were considered unsuccessful (less than 20% weight loss). Psychological assessment, using the meta-contrast technique, showed that signs of regressive defense and of immature identity decreased postoperatively. Mild to moderate depressive reactions were found in two thirds of the patients postoperatively an

Dynamic magnetic resonance imaging evaluation of stomach geometry modifications after endoscopic sleeve gastroplasty.

INTRODUCTION: Sleeve gastrectomy (SG) induces weight loss through tubulization of the stomach. Up to 35% of SG patients develop Gastroesophageal Reflux Disease (GERD). The pathophysiological mechanism for GERD development has been associated to changes in the geometry and mechanics of the Esophago-Gastric junction (EGJ). Endoscopic sleeve gastroplasty (ESG) is an endoscopic bariatric procedure leading to weight loss and obesity-related comorbidities resolution through gastric restriction and shortening. Unlike SG, ESG preserves the gastric fundus, potentially resulting in lesser impact on EGJ

Endoscopic sleeve gastroplasty versus single anastomosis sleeve ileal bypass: a single-center experience.

BACKGROUND: Endoscopic sleeve gastroplasty (ESG) is a minimally invasive procedure that is less effective for weight loss. Single anastomosis sleeve ileal bypass (SASI) combines sleeve gastrectomy with an ileal bypass to maximize weight loss. We compared the postoperative results of ESG vs. SASI at 6 months to evaluate weight reduction and nutritional outcomes. METHODS: This retrospective study included 21 patients. Body weight and levels of serum iron, calcium, zinc, vitamin B12, and folate were measured preoperatively and at 1, 3, and 6 months postoperatively. The SASI group received postope

Four-year outcomes for endoscopic sleeve gastroplasty from a single centre in India.

BACKGROUND: Bariatric endoscopy has emerged for non-surgical treatment of obesity, providing a treatment option for weight loss and associated comorbidities. Outcomes of endoscopic sleeve gastroplasty (ESG) of 12 months have been published by our team and there is a need for longer follow-up period understanding the effects of ESG techniques. AIM: This report emphasises on weight loss pattern in follow-up time points and monitors the post-procedure improvement in comorbidities with minimum 4-year follow-up of patients undergoing ESG at a single academic centre in India. SUBJECTS AND METHODS:

Endoscopic assessment of morphological and histopathological upper gastrointestinal changes after endoscopic sleeve gastroplasty.

BACKGROUND: Endoscopic sleeve gastroplasty (ESG) is a promising bariatric endoluminal procedure. Restriction and shortening of the stomach are obtained by means of non-resorbable full-thickness sutures, thus inducing the formation of several endoluminal pouches in which food can stagnate. The effect of ESG on the upper gastrointestinal tract has never been investigated. OBJECTIVES: This study objectively evaluates endoscopic macroscopic and histopathologic changes within 12-month follow-up (FU) in patients who underwent ESG. SETTING: Retrospective study on a prospective database of patients
