Gastrostomy in a patient with situs inversus totalis.

Situs inversus totalis (SIT) is a rare condition in which there is complete right to left reversal of the abdominal and thoracic organs. SIT generally does not bear any pathophysiological significance, and the survival rate of patients with SIT does not differ from that of healthy individuals. However, patients with SIT require a thorough radiological examination to identify the presence of associated anatomic variations before undergoing invasive procedures such as surgery or hemostasis of gastrointestinal hemorrhage because they may have accompanying abnormalities in anatomical structures al

Laparoscopic fundoplication in children with a preexisting gastrostomy.

BACKGROUND: Children with a gastrostomy may require laparoscopic fundoplication (LF). METHODS: Children with a gastrostomy who underwent LF between 1998 and 2002 were reviewed. The outcomes of children undergoing LF were compared with a group of children without a gastrostomy who had the same operation during the same period. RESULTS: Ten children with a gastrostomy in place and eight children without a gastrostomy underwent LF. The median operating time was 198 minutes (range, 115-300 minutes) in the gastrostomy group and 110 minutes (range, 80-130 minutes) in the non-gastrostomy group (P =

Percutaneous endoscopic gastrostomy and outcome in dementia.

The use of percutaneous endoscopic gastrostomy for the administration of food and medications in patients with dementia has been on an increase. Many studies have failed to demonstrate the positive outcome expected of this feeding modality for the indications that required tube placement. Hence, the concept of feeding through gastrostomy tubes has become the subject of much discussion and controversy in recent times. We have reviewed the literature with regard to outcome in older patients with dementia and percutaneous endoscopic gastrostomy with respect to nutritional parameters, quality of l

Surgical feeding gastrostomy: are we overdoing it?

Feeding gastrostomy is a commonly performed procedure in North America. Our aim was to study the outcome of patients undergoing feeding gastrostomy to better define patients who will benefit from the procedure as opposed to those in whom it may be futile. A cohort of the most recent 100 consecutive patients undergoing feeding gastrostomy in a community teaching hospital was retrospectively studied. The main indication for gastrostomy was neurologic disorder interfering with eating/swallowing (group A-54 patients), followed by debilitating systemic disease (group B-26 patients) and obstructive

Percutaneous endoscopic gastrostomy in children with cancer.

We reviewed the clinical course of 32 children with cancer who received nutrition through a feeding tube placed percutaneously during gastroscopy (PEG). Their median age was 5.1 y (75%, range: 1.8-13.7 y, min: 3.5 mo) when the PEG was done 0.7-23 mo after diagnosis (median: 1.8 mo, 75%; range: 0.9-8 mo). Five of the children underwent bone marrow transplantation with the gastrostomy in place. There was a significant (p = 0.0001) decrease in the median weight-for-age SDS of 0.55 (75%, range: -1.18-0.28) from the time of diagnosis to placement of the gastrostomy. Twenty-two percent of the childr

Gastrostomy tube infections in a community hospital.

BACKGROUND: Percutaneous gastric feeding tubes are becoming increasingly more common to provide nutrition in debilitated patients, while they decrease the risk of aspiration associated with nasogastric tubes. METHODS: We reviewed infectious complications of 372 feeding gastrostomy tubes placed in a small urban community hospital over a recent period. RESULTS: In our study there was an infection rate of 4.8%. Four serious infections occurred: two cases of peritonitis and two deep abscesses, but there were no infectious deaths. The most common infecting organisms were staphylococci, gram negat

A simplified and safe method of open gastrostomy.

The establishment of a gastrostomy by the percutaneous endoscopic route (PEG) is now accepted as a safe and easy procedure which can be performed under combined sedation and infiltration of local anaesthetic at the chosen site on the anterior abdominal wall. However, surgeons are still not infrequently called upon to perform an open gastrostomy in a selected group of patients, namely: (1) those with extensive laryngeal and pharyngeal tumours in whom it is impossible to pass even a paediatric gastroscope; (2) neonates with oesophageal atresia. Various forms of open gastrostomy have been describ

Gastric volvulus--a late complication of gastrostomy.

Two cases of gastric volvulus are reviewed. Both patients were nonverbal, mentally handicapped children, who were fed through a gastrostomy. They had intermittent intolerance to bolus feeds through the gastrostomy, accompanied by abdominal distension and vomiting of gastric contents. One had a previous partial fundoplication and gastrostomy, and the other had had two pyloroplasties and gastrostomy. The axis of torsion ran from the esophagogastric junction to the gastrostomy site. Both children were treated by detorsion and gastropexy. It is postulated that the gastrostomy served as a fixed poi

Transitioning the child fed by gastrostomy into school.

Young children dependent on gastrostomy feeding are examples of special-needs children now eligible for expanded nutrition services in schools through Public Law 99-457. With the implementation of this law, nutritionists have an opportunity to expand their roles as consultants to special education teachers. A team approach involving nutrition, nursing, and special education was used to prepare a gastrostomy-fed preschool child and his family for school. The family of the multihandicapped child had been isolated by the child's feeding problems and viewed the problems as barriers to his entering

A new complication of percutaneous endoscopic gastrostomy.

Two cases of complications of percutaneous endoscopic gastrostomy are described that have not been reported previously. Each had misplacement of a gastrostomy tube in the intestine, one in the transverse colon resulting in profuse diarrhea and metabolic acidosis, and the other in the jejunum causing adhesions with subsequent small bowel obstruction. An explanation of these complications and suggestions for prevention are presented.

Unusual Complication of Percutaneous Gastrostomy Tube Replacement.

Percutaneous endoscopic gastrostomy tubes are commonly used for long-term nutritional support in patients with impaired swallowing due to neurological conditions, malignancy, or critical illness. Although placement, removal, and replacement are generally safe, complications may occur. We report a case of complete gastric outlet obstruction due to unusually deep percutaneous endoscopic gastrostomy tube insertion during replacement in a 63-year-old man with esophageal cancer. Symptoms resolved after repositioning the tube. This case highlights the need for proper training and postreplacement ver

Pancreatico-Gastrostomy: A Modified Two-Layered Technique.

INTRODUCTION: Pancreato-duodenal resections are commonly done for periampullary carcinomas. The outcome of the procedure is decided by pancreato-enterostomy. Pancreato-jejunostomy (PJ) has been employed largely as pancreato-enterostomy. Recently, there has been a renewed interest in pancreato-gastrostomy (PG). The debate continues on the choice of reconstruction. METHODS: A hundred cases of periampullary carcinoma were subjected to modified pancreatico-gastrostomy. The pancreatico-gastrectomy was evaluated by drain fluid amylase done on days 1, 3, and 5 post-operatively and clinical findings.

Laparoscopic Magnet-Assisted Percutaneous Endoscopic Gastrostomy Placement.

BACKGROUND: There are various methods to place a gastrostomy tube. We report a laparoscopic method to place a standard percutaneous endoscopic gastrostomy (PEG) tube without the use of endoscopy. METHODS: Laparoscopic magnet-assisted PEG placement was carried out using an orogastric tube attached with a magnet that is used to retrieve the PEG wire that has been percutaneously placed into the stomach. RESULTS: Four pediatric patients (mean age 31 months) underwent a PEG tube placement using the laparoscopic magnet-assisted PEG tube insertion technique during 2017. There were no immediate and

Farrell valve relieves bloating in gastrostomy patient.

Dysphagia and weight loss are experienced by up to 80% of patients with motor neuron disease (MND). Enteral tube feeding can benefit these patients but side effects including fullness, bloating, diarrhoea and constipation are frequent and can lead to non-compliance. Changes in feed formulation, regimens and the use of prokinetic medication can help symptoms. We describe a case where the use of gastric pressure relief bags during enteral feeding improved symptoms of bloating in a hospice inpatient with MND. Symptoms recurred when these were not used, which were only partially relieved by manual

Reappraisal of Pneumoperitoneum After Percutaneous Endoscopic Gastrostomy.

BACKGROUND/AIMS: Pneumoperitoneum is recognized as a benign and self-limiting finding after the insertion of a percutaneous endoscopic gastrostomy (PEG) tube, while complicated pneumoperitoneum is rarely reported. The aim of this study was to reappraise pneumoperitoneum following PEG. METHODS: We retrospectively reviewed 193 patients who underwent PEG from May 2008 to May 2014. All patients had a follow-up upright chest or simple abdominal radiograph after PEG. Pneumoperitoneum was quantified by measuring the height of the air column under the diaphragm and graded as small (<2 cm), moderate (

Estimating Adverse Events After Gastrostomy Tube Placement.

OBJECTIVE: Gastrostomy feeding tube placement in children is associated with a high frequency of adverse events. This study sought to preoperatively estimate postoperative adverse events in children undergoing gastrostomy feeding tube placement. METHODS: This was an observational study of children who underwent gastrostomy with or without fundoplication at 1 of 50 participating hospitals, using 2011-2013 data from the American College of Surgeons' National Surgical Quality Improvement Program Pediatric. The outcome was the occurrence of any postoperative complications or mortality at 30 days
