Efficacy and safety of domestic and imported gefitinib in patients with advanced non-small cell lung cancer.

BACKGROUND: Gefitinib is a first-generation epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI). It was approved by the U.S. Food and Drug Administration (FDA) for clinical use in 2003. However, gefitinib has only come to China in recent years. Previous studies have not compared the efficacy and safety of domestic and imported gefitinib. Therefore, we conducted this study. METHODS: This study included 227 patients with advanced non-small cell lung cancer (NSCLC) who received gefitinib treatment in four medical institutions: The First Affiliated Hospital of USTC, Division of

Steatocystoma multiplex as initial impression of non-small cell lung cancer with complete response to gefitinib.

Cutaneous metastases are rare and seldom present at the time of first diagnosis of cancer. Data from various studies show that 1-12% of lung cancer patients experience tumor spread to the skin. The scalp, chest, and abdomen are favored sites of skin metastases from lung cancers, but metastases to multiple skin sites in a single patient are rarely reported. We describe a 56-year-old lung adenocarcinoma patient, initially diagnosed with steatocystoma multiplex who responded well to gefitinib treatment. The efficacy of conventional chemotherapy for cutaneous metastases has been limited because of

Significant cytotoxic activity in vitro of the EGFR tyrosine kinase inhibitor gefitinib in acute myeloblastic leukaemia.

OBJECTIVES: Gefitinib inhibits epidermal growth factor receptor (EGFR) signalling, but may also act by non-EGFR dependent mechanisms. We have investigated the activity of gefitinib in haematological tumour cells, in particular acute myeloblastic leukaemia (AML). METHODS: Cytotoxic activity of gefitinib, alone or in combination with standard anti-leukaemic drugs, was assessed by the short-term fluorometric microculture cytotoxicity assay in tumour cells from 117 patients representing five haematological and five non-haematological malignancies. In AML, the EGFR status was analysed by immunoche

MiR-23c Regulates the Resistance to Gefitinib in EGFR Mutant Non-Small-Cell Lung Cancer Cells.

Mechanisms of primary and acquired resistance are responsible for treatment failure with the Epidermal Growth Factor Receptor-Tyrosine Kinase Inhibitors (EGFR-TKIs) in the majority of patients with advanced Non-Small-Cell Lung Cancer (NSCLC) carrying EGFR-activating mutations. MicroRNAs (miRNAs) are important modulators of EGFR signaling in lung cancer. Recent studies suggested the role of miR-23c as a tumor suppressor or oncogenic miRNA in different tumor types. However, the role of miR-23c in NSCLC carrying EGFR mutations and its involvement in resistance to EGFR-TKIs has not been explored y

Polycationic dendrimers synergizes with gefitinib to overcome EGFREx19Del-driven resistance in non-small-cell lung cancer.

Lung cancer is a heterogeneous disease and the leading cause of cancer-related deaths worldwide. The aggressiveness of non-small-cell lung cancer (NSCLC) is often associated with oncogenic activation of the mitogen-activated protein kinase (MAPK) pathway. Among the most prevalent mutations in NSCLC, epidermal growth factor receptor 1 (EGFR) and Kirsten rat sarcoma (KRAS) alterations play a key role in cancer progression and patient survival. In this study, we investigated the influence of EGFR and KRAS mutational status on the response to polycationic core-shell dendrimers, using NSCLC cell li

BYL719 reverses gefitinib-resistance induced by PI3K/AKT activation in non-small cell lung cancer cells.

Non-small cell lung cancer (NSCLC) patients with epidermal growth factor receptor (EGFR) mutation often obtain de novo resistance or develop secondary resistance to EGFR tyrosine kinase inhibitors (EGFR-TKIs), which restricts the clinical benefit for the patients. The activation of phosphatidylinositol 3-kinase (PI3K)/AKT signal pathway is one of the most important mechanisms for the EGFR-TKIs resistance beyond T790M mutation. There are currently no drugs simultaneously targeting EGFR and PI3K signal pathways, and combination of these two pathway inhibitors may be a possible strategy to revers

p28-functionalized PLGA nanoparticles loaded with gefitinib reduce tumor burden and metastases formation on lung cancer.

Lung cancer is still the main cause of cancer-related deaths worldwide. Its treatment generally includes surgical resection, immunotherapy, radiotherapy, and chemo-targeted therapies such as the application of tyrosine kinase inhibitors. Gefitinib (GEF) is one of them, but its poor solubility in gastric fluids weakens its bioavailability and therapeutic activity. In addition, like all other chemotherapy treatments, GEF administration can cause damage to healthy tissues. Therefore, the development of novel GEF delivery systems to increase its bioavailability and distribution in tumor site is hi

Impact of Gefitinib on Traumatic Fracture Healing in Rats via the OPG/RANKL/RANK Signaling Pathway.

This study investigates how gefitinib (Gef), an epidermal growth factor receptor (EGFR) inhibitor, affects traumatic fracture healing in rats, particularly through the OPG/RANKL/RANK signaling pathway, which regulates osteoclast and osteoblast activity. Sixty Sprague-Dawley rats were divided into different treatment groups, including fracture only, fracture plus gefitinib, fracture plus functional OPG antagonist (i.e., recombinant mouse RANKL protein), and combinations of both. After 28 days of treatment, bone healing was evaluated using techniques such as micro- CT, histological staining, and

Molecular mechanisms of cardiotoxicity of gefitinib in vivo and in vitro rat cardiomyocyte: Role of apoptosis and oxidative stress.

Gefitinib (GEF) is a multi-targeted tyrosine kinase inhibitor with anti-cancer properties, yet few cases of cardiotoxicity has been reported as a significant side effect associated with GEF treatment. The main purpose of this study was to investigate the potential cardiotoxic effect of GEF and the possible mechanisms involved using in vivo and in vitro rat cardiomyocyte model. Treatment of rat cardiomyocyte H9c2 cell line with GEF (0, 1, 5, and 10μM) caused cardiomyocyte death and upregulation of hypertrophic gene markers, such as brain natriuretic peptides (BNP) and Beta-myosin heavy chain (β

Tubulointerstitial nephritis and IgA nephropathy in a patient with advanced lung cancer treated with long-term gefitinib.

A 52-year-old Japanese female was admitted to our hospital for microhematuria, proteinuria and progressive renal dysfunction. Two years prior to admission, she was diagnosed with lung adenocarcinoma and multiple bone and brain metastases, and was treated with gefitinib (250 mg/day). Treatment for 6 months induced partial response with 30% regression of the primary lung tumor, and resolution of metastatic tumors. After confirmation of the partial remission state, we performed percutaneous renal biopsy. Glomeruli showed mild to moderate mesangial proliferation, segmental endocapillary proliferat

Gefitinib-based therapy in refractory hepatocellular carcinoma with EGFR overexpression: a spatial profiling-guided case report.

Current systemic treatments for advanced hepatocellular carcinoma (HCC) such as antiangiogenic targeted therapies, immune checkpoint inhibitors (ICIs), and their combinations have shown benefit, but widespread drug resistance severely limits patient outcomes and underscores an urgent need for effective post-progression strategies. We herein present a case of a patient with advanced HCC who developed cutaneous, pulmonary, and lymphatic metastases following failure of multiple lines of tyrosine kinase inhibitors (TKIs) including donafenib and regorafenib, and lenvatinib plus anti-programmed deat

Lacticaseibacillus rhamnosus GG enhances gefitinib efficacy and reduces treatment-related toxicity via propionyl-L-carnitine regulation.

Gefitinib improves outcomes in patients with EGFR-mutant non-small cell lung cancer; however, treatment-related gastrointestinal and hepatic toxicity can limit its long-term use. Here, we investigate whether probiotic strains can improve gefitinib efficacy while reducing treatment-associated toxicity. Among the tested strains, Lacticaseibacillus rhamnosus GG (LGG) shows the strongest enhancement of gefitinib efficacy while reducing intestinal and hepatic injury without altering systemic gefitinib exposure. Metabolomic analyses identify propionyl-L-carnitine as an LGG-associated metabolite dete

Membrane cholesterol-dependent dual VEGFR2/FGFR1 inhibition by ginsenoside Rg3 to overcome gefitinib resistance in NSCLC.

BACKGROUND: Drug resistance severely hinders the clinical application of epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) in the treatment of non-small cell lung cancer (NSCLC). Notably, resistance caused by rare target mutations (with a mutation incidence rate below 5 %) accounts for approximately 15 % of total resistance cases in NSCLC. Due to the diversity and complexity of these mutations, targeted therapies against them are currently very limited. PURPOSE: To address the challenge of multi-driver resistance in NSCLC, this study aimed to explore a novel therapeutic

SLC16A3 drives lung adenocarcinoma progression and gefitinib resistance through coordinated regulation of ferroptosis and lactate metabolism.

BACKGROUND: Ferroptosis is an iron-dependent form of regulated cell death that plays a critical role in tumor suppression and therapeutic response. However, the metabolic mechanisms that drive ferroptosis resistance in lung adenocarcinoma (LUAD), particularly in the context of EGFR-TKI tolerance, remain unclear. METHODS: We integrated transcriptomic and clinical data from TCGA LUAD cohort and performed survival and enrichment analyses. Functional assays, including proliferation, invasion, ferroptosis indicators, and in vivo xenograft models, were used to evaluate the role of SLC16A3. Lactate

Exosomal proteomics reveals fatty acid metabolism linked to gefitinib resistance in non-small cell lung cancer.

Exosomes play a crucial role in the transmission of drug resistance in tumors. However, the mechanism of exosomes-mediated transmission in non-small cell lung cancer (NSCLC) under gefitinib treatment remains limited. In this work, we demonstrated that exosomes derived from HCC827/GR cells (drug-resistant) enhanced the survivability of HCC827 cells (drug-sensitive) under treatment with gefitinib. A total of 157 shared upregulated proteins between exosomes and their parent cells were identified in the comparison of the gefitinib-resistant groups versus the gefitinib-sensitive groups. Notably, 69

Gefitinib for EGFR mutated lung adenocarcinoma during pregnancy: A case report with 5-Year Follow-Up.

We report the case of a 37-year-old pregnant woman at 19 weeks of gestation who was diagnosed with a stage M1b EFGRmut lung adenocarcinoma, in 2019. After a right upper and middle bilobectomy (T3N1 and R0 pathological status) and a complete cerebellar metastasis resection, she was treated with gefitinib, which offered her a prolonged recurrence-free survival without any negative consequence for the fetal development or the future development of her now 5- year- old child.

IGFBP2/ITGA5 promotes gefitinib resistance via activating STAT3/CXCL1 axis in non-small cell lung cancer.

There is a paucity of comprehensive knowledge pertaining to the underlying mechanisms leading to gefitinib resistance in individuals diagnosed NSCLC harboring EGFR-sensitive mutations who inevitably develop resistance to gefitinib treatment within six months to one year. In our preceding investigations, we have noted a marked upregulation of IGFBP2 in the neoplastic tissues of NSCLC, predominantly in the periphery of the tissue, implying its plausible significance in NSCLC. Consequently, in the current research, we delved into the matter and ascertained the molecular mechanisms that underlie t

FTO promotes gefitinib-resistance by enhancing PELI3 expression and autophagy in non-small cell lung cancer.

The established recognition of N6-methyladenosine (m6A) modification as an indispensable regulatory agent in human cancer is widely accepted. However, the understanding of m6A's role and the mechanisms underlying its contribution to gefitinib resistance is notably limited. Herein, using RT-qPCR, Western blot, Cell proliferation and apoptosis, as well as RNA m6A modification assays, we substantiated that heightened FTO (Fat Mass and Obesity-associated protein) expression substantially underpins the emergence of gefitinib resistance in NSCLC cells. This FTO-driven gefitinib resistance is hinged
