OBJECTIVES: To find out if the ratio of facial nerve to facial canal diameter plays any role in the etiopathogenesis and grade of Bell's palsy.
PATIENTS AND METHODS: Twenty-nine consecutive patients (16 females, 13 males) diagnosed with unilateral Bell's palsy were enrolled into the study. At admission, 5 patients were grade V, 11 were grade IV, 11 were grade III and 2 were grade II. The grade of Bell's palsy was documented by House-Brackmann facial nerve grading system at admission, 15th day, 1st month and 3th month. Temporal MRI and CT scans were obtained at the time of diagnosis. Diameter

OBJECTIVE: To evaluate the risk of the facial nerve injury during operations for chronic otitis media without cholesteatoma by analysis of the intraoperative findings of the facial canal dehiscence.
METHODS: We retrospectively reviewed operative findings of 152 patients who underwent tympanoplasty with mastoidectomy for chronic otitis media. We examined every segment of the facial canal from the geniculate ganglion to the mastoid segment. Facial canal dehiscence was confirmed by palpation with a Rosen pick after inspection with a surgical microscope.
RESULTS: The rate of facial canal dehisce

In outer retinal degenerative diseases such as retinitis pigmentosa, choroideremia, and geographic atrophy, 30% of the ganglion cell layer in the macula remains intact. With subretinal and epiretinal prostheses, these inner retinal cells are stimulated with controlled electrical current by either a microphotodiode placed in the subretinal area or a microelectrode array tacked to the epiretinal region. As the patient learns to interpret the resulting phosphene patterns created in the brain through special rehabilitation exercises, their orientation, mobility, and quality of life increase. Impla

BACKGROUND: Ramsay Hunt Syndrome (RHS) is a rare, severe complication of varicella zoster virus (VZV) reactivation in the geniculate ganglion. Facial paralysis is one of the features and without treatment, it fully recovers in as little as 20% of cases; this is much improved if treatment is started within 72 hours. This case is noteworthy in that coexistence of facial palsy with cervical dermatome involvement by VZV is not typical of RHS. Yet, it has been reported before.
CASE PRESENTATION: A 54 year old Caucasian woman presented with right ear discomfort, difficulty hearing and a vesicular r

The efficient organization of the human afferent visual system meets enormous computational challenges. Once visual information is received by the eye, the signal is relayed by the retina, optic nerve, chiasm, tracts, lateral geniculate nucleus, and optic radiations to the striate cortex and extrastriate association cortices for final visual processing. At each stage, the functional organization of these circuits is derived from their anatomical and structural relationships. In the retina, photoreceptors convert photons of light to an electrochemical signal that is relayed to retinal ganglion

Gd-DTPA-enhanced MRI provided accurate demonstration of the site and extension of a haemangioma of the petrous bone. Enhanced-MRI could allow early detection and differentiation from the other more common tumours of the geniculate ganglion area, thereby permitting the surgeon to remove the tumour, while preserving the facial nerve. CT is, however, required to display the characteristic ossification in these tumours.

Bell's palsy has no specific treatment because its cause is still debated. The most widely accepted theory postulates an immunovirological origin consisting of the reactivation of a silent herpesvirus on the geniculate ganglion. We studied three groups of patients with Bell's palsy who received different treatments in an attempt to discern differences in evolution between treatment groups.

A case of congenital bilateral conductive deafness is described. Some years ago absence of the oval window was found in the left ear. Exploratory tympanotomy of the right ear revealed a large persistent stapedial artery while the facial nerve took its course from the geniculate ganglion straight down across the promontory anteriorly of the stapedial artery.

The etiology of Bell's palsy remains unknown but clinical serological investigations have suggested herpes simplex virus (HSV) induced facial neuritis to be a potential cause. In order to verify the viral etiology of Bell's palsy it must be proved by animal experimentation. The author first succeeded in producing a transient facial paralysis of mice, with a herpes simplex viral neuritis simulating human Bell's palsy. Type 1 HSV (strain KOS, 4.5 X 10(6) pfu/ml) was inoculated into the posterior aspect of the auricle. Fifty-nine out of 104 mice (56.7%) developed facial paralysis, on the experime

The thickness of two layers of the retina (the inner plexiform layer and the inner nuclear layer), together with the density of cells in the ganglion cell layer, were measured in a sample of mice from a population segregating for albinism and retinal degeneration. The expression of retinal degeneration was found to involve not only the disappearance of retinal receptors but also a reduction in thickness of the inner plexiform layer and a reduction of cell density in the ganglion cell layer: these changes were found to be associated with a reduction of neuronal density in the lateral geniculate

Surgical decompression of the intratemporal facial nerve from the geniculate ganglion to the stylomastoid foramen was carried out in 91 patients with Bell's palsy. All of the patients had denervation exceeding 95%, and a suprastapedial lesion. Edematous swelling of the nerve was assessed using the following three grades: + +, nerve swells beyond the bony facial canal; +, nerve swells beyond the nerve sheath, but within the bony canal, and -, no notable swelling observed. Varying degrees of swelling of the nerve were noted in all of the patients from onset to the end of the ninth week. The inci

We present a case of a 62-year-old Caucasian male with laryngeal cancer and Ramsay Hunt Syndrome otherwise known as herpes zoster oticus due to reactivation of the varicella zoster virus. Classic findings include the triad of ipsilateral facial paralysis, otic pain, and herpetic lesions in the sensory supply of the facial nerve. The common pathogenesis is associated with anterograde axonal reactivation of the varicella zoster virus in the geniculate ganglion. Unique features of our case include retrograde transaxonal spread of the varicella-zoster virus from the geniculate ganglion into the br

In albinos, the retinofugal projections to the ipsilateral side of the brain are reduced (e.g., see Guillery, 1969; La Vail et al., 1978; Lund, 1965). Although all ganglion cell types are affected, in mice the displaced ganglion cell population is the main target of the albino mutation (Dräger & Olsen, 1980). Here we tested whether this preferential effect on displaced ganglion cells is a general consequence of the melanin reduction or a pleiotropic effect unique to the albino locus, by retrogradely tracing retinal ganglion cells in normal C57BL/6J mice and in several non-allelic hypopigmentat

Reactivation of the varicella-zoster virus (VZV) along the sensory nerves innervating the ear, including the geniculate ganglion, is responsible for herpes zoster oticus (HZO). In some cases, HZO is associated with polyneuropathy of the cranial nerves, although the mechanism of this involvement is not known. To explain this phenomenon and based on some clinical considerations, the present authors hypothesize an intersynaptic spread of VZV along the reflex pathways of the brainstem.

The neurones of the afferent and efferent auditory pathways were labelled by axonally transported horseradish peroxidase. The first neurone of the ascending system is the acoustic nerve with the spiral ganglion. These fibres terminate mainly in the ventral cochlear nucleus (VCN). The neurones of the latter chiefly run to the contralateral superior olivary complex (SOC), whereas the neurones of the dorsal cochlear nucleus (DCN) terminate mainly in the central nucleus of the contralateral inferior colliculus (IC). The nerve cells of the superior olivary complex and nuclei of the lateral lemniscu

The facial nerve schwannoma is a rare tumor and it seldom extends into the middle cranial fossa. The typical clinical presentations are progressive facial paralysis and hearing disturbance. We report here a case of huge facial nere facial nerve schwannoma extending into the middle cranial fossa without facial palsy. A 50-year-old man presented with left hearing disturbance. Neurological examination on admission revealed no deficits except for sensorineural hearing loss. MRI demonstrated a cystic tumor extending into the ddle cral fossa from the petrous bone CT the middle cranial fossa from the

The diagnosis and management of facial nerve disorders are challenging aspects of neurotological practice. In Bell's palsy and Herpes Zoster oticus facial nerve decompression is indicated in approximately 10 per cent of cases within the first three weeks if 90-95 per cent degeneration has occurred. The presence of a dry eye determines whether the lesion is proximal to the geniculate ganglion and a middle fossa approach is necessary. Injury to the facial nerve occurs most commonly as a result of temporal bone fractures, from either blunt trauma or penetrating wounds. Occasionally iatrogenic inj

We present a reproducible, stepwise middle fossa approach for facial nerve decompression focused on the labyrinthine segment, geniculate ganglion, and meatal foramen, with consistent anatomical landmarks to preserve hearing. The article and video detail patient setup, safe corridor creation, and retrograde drilling with practical tips to avoid cochlear or semicircular canal injury, aiming to lower the learning curve for facial nerve decompression.
