Wedgeless V-Shaped Osteotomy of the Distal Medial Femur with Locking Plate Fixation for Correction of Genu Valgum in Adolescents and Young Adults.

BACKGROUND: Genu valgum is a common disorder affecting adolescents and young adults. Treatment of this disorder requires restoration of normal mechanical axis alignment and joint orientation, for which it is important to assess whether the deformity arises from the distal femur, knee joint, or proximal tibia. Most commonly, the deformity originates from the distal femur, and various osteotomies of the distal femur have been described1-6. The presently described wedgeless V-shaped osteotomy7,8 is a good option among the various alternative procedures listed below. DESCRIPTION: The anesthetized

Unilateral genu valgum associated to vitamin D deficiency: A multidisciplinary approach in a patient followed from 3 to 21 years - A case report.

INTRODUCTION: Lower limb postural deformities are usually bilateral and in the majority of cases tend to spontaneous correction without any treatment. On the contrary, unilateral genu valgum is a rare deformity in children, it is frequently associated with systemic pathological conditions and require a specific treatment. PRESENTATION OF CASE: A three-year-old child affected by severe unilateral left genu valgum was followed by a multidisciplinary team. The initial treatment was medical, based on 10,000 IU of cholecalciferol every 2 months without any improvement after 6 months. Therefore, th

Outcomes of Hemiepiphyseal Stapling for Genu Valgum Deformities in Patients With Multiple Hereditary Exostoses: A Comparative Study of Patients With Deformities of Idiopathic Cause.

BACKGROUND: Patients with multiple hereditary exostoses (MHE) frequently present with a genu valgum deformity. Temporary hemiepiphysiodesis, such as hemiepiphyseal stapling, is a relatively safe surgical method to correct angular deformities in skeletally immature patients, but its outcomes for genu valgum deformity in MHE patients have not been extensively reported. We investigated the outcomes of hemiepiphyseal stapling in MHE patients (MHE group) and compared those with the outcomes in patients with idiopathic deformities (idiopathic group) after adjusting for potential bias. METHODS: Data

Effect of Medial Closing Wedge Distal Femoral Osteotomy on Tibial Tuberosity-Trochlear Groove Distance in Patients With Genu Valgum According to Diagnosis (Lateral Osteoarthritis or Patellar Dislocation).

BACKGROUND: Medial closing wedge distal femoral osteotomy (CWDFO) is used to correct valgus deformities and manage lateral osteoarthritis (OA) and patellar dislocation (PD). However, its effect on the tibial tuberosity-trochlear groove (TT-TG) distance remains debated, with limited studies evaluating contributing factors. PURPOSE: To determine whether TT-TG distance changes after CWDFO differ by the underlying diagnosis prompting the procedure and to identify preoperative factors influencing the extent of TT-TG distance changes. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: This

Mid-term outcomes of temporary medial distal femoral hemiepiphysiodesis with and without medial patellofemoral ligament repair for recurrent patellar instability in skeletally immature patients with genu valgum.

Genu valgum contributes to patellar instability, though outcomes following temporary hemiepiphysiodesis via eight plating are less understood. The purpose of this study was to evaluate the outcomes and need for additional procedures following temporary hemiepiphysiodesis for the treatment of pediatric genu valgum and patellar instability, as well as evaluating the utility of a concurrent medial patellofemoral ligament (MPFL) repair. Patients who underwent medial distal femoral epiphysiodesis for the treatment of genu valgum and recurrent patellar instability were identified. Inclusion criteria

Hemiepiphysiodesis with tension band plate in idiopathic genu valgum: Insights on indications, outcomes, and follow-up from a systematic review and meta-analysis of more than 600 cases.

AIMS: Hemiepiphysiodesis with tension band plates is widely used for the correction of idiopathic genu valgum in skeletally immature patients. Although widely adopted, considerable heterogeneity persists regarding indications, timing of intervention, and radiographic outcome assessment, limiting the development of standardized clinical pathways. METHODS: A systematic review and meta-analysis were conducted in accordance with PRISMA guidelines. Studies reporting radiographic and clinical outcomes of hemiepiphysiodesis with tension band plates in idiopathic genu valgum were included. The study

Temporary hemi-epiphysiodesis with tension band plates in skeletally immature patients with genu valgum: Faster correction in patients with more than 2 years of expected time of growth remaining.

PURPOSE: Hemi-epiphysiodesis procedures in skeletally immature patients are commonly done to correct genu valgum over time. This study seeks to demonstrate the average rate of deformity correction for genu valgum using hinged tension band plates, while examining different age groups and sex-related differences. METHODS: A retrospective chart review of patients who underwent hemi-epiphysiodesis with hinged tension band plates for valgus knee deformity from 2012 to 2022 by one pediatric orthopaedic surgeon was performed. Expected time of growth remaining was calculated as the difference between

Skeletal Deformity in Children with Primary Hyperparathyroidism.

Skeletal deformation like genu valgum is reported to be rare in Primary hyperparathyroidism (PHPT). The solitary adenoma or hyperplasia of the parathyroid glands are the cause in 80-85% of the cases. We report 2 cases of girls on 12 years and 15 years of age, complaining from pain and genu valgum deformation of the lower extremities before the planned orthopaedic surgical correction. The first patient had complaints for 3 years and lost ability to walk independently, the second case lost normal gate for a period of 5 months. The paraclinical screening discovered hypercalcemia, hypophosphatemia

Calf augmentation with supraperiostic solid prosthesis associated with fasciotomies.

This article describes, for the first time, the supraperiostic technique used for aesthetic purposes to remedy hemiatrophy, genu valgum, poliomyelitis sequela, and thin legs. For genu valgum and thin legs, a solid prosthesis was used, whereas for severe hemiatrophy a double light prosthesis was used as a tissue expansor, with excellent reports. The advantages of this technique are delineated further in this article.

Implant-Mediated Guided Growth for Coronal Plane Angular Deformity in the Pediatric Patient with Patellofemoral Instability.

Pediatric patellofemoral instability is a complex problem, for which there are several anatomic risk factors. Coronal plane malalignment (i.e., genu valgum) is one cause of patellofemoral instability, and treatment of genu valgum has been associated with improved patellofemoral stability. Coronal plane angular deformity correction, typically achieved by distal femoral osteotomy in the adult population, can be achieved with less invasive surgical techniques in pediatric patients using implant-mediated guided growth. By temporarily tethering one side of an open physis to generate differential gr

Non-rachitic deformities of the knees in Nigerian children.

Non-rachitic lesions of the knee are a common condition found in Nigerian children, the aetiology of which is unknown. 67 patients were studied, 24 had genu valgum, 28 genu varum and 15 with varo-valgum. Possible aetiology has been discussed and the following hypotheses have been put forward. The early weight bearing of the immature bones of the lower limbs has uneven pressure effects on the growth plate and also the laxity of the knee ligaments produces abnormal posture. The abnormal posture during growth and pressure effect of a tight band around the knee when the child is carried on the bac

A constellation of orthopaedic deformities in connection with cartilage oligomeric matrix protein mutation.

BACKGROUND: Trendelenburg's gait can be observed in Legg-Calvé-Perthes disease, antalgic gait observed in osteoarthropathy and waddling gait is usually seen in genu varum and circumduction gait in patients with genu valgum. Disabling pain was a prime manifestation in slipped capital femoral epiphysis (SCFE). Limited joint range of motion with an inability to bear full weight on an affected extremity with swaying and wide-based gait is seen in patients with malalignment of the lower limbs. All the above-mentioned deformities have been labelled as idiopathic. The main objective of this article i

Postosteotomy healing in Pyle's disease (familial metaphyseal dysplasia). A case report.

Pyle's disease is rare skeletal dysplasia in which a defect in metaphyseal remodeling leads to grossly widened metaphyses of long bones. Genu valgum, one of the few clinical findings in this disease, may warrant surgical intervention. Postoperative healing is documented only in Pyle's original patient. A case is presented in which bilateral osteotomies were performed through the proximal tibia for progressive genu valgum. The osteotomies healed within 12 weeks when treatment was performed with standard techniques. This information will allow osteotomies to be performed with confidence.

Lower limb alignment correction can improve knee stability in congenital absence of anterior cruciate ligament.

We report the 4-year postoperative outcome of a child with Manner Type I aplasia of the anterior cruciate ligament (ACL) in her left knee and severe genu valgum. The child was experiencing pain, reduced mobility, and frequent left knee locking and instability. The patient was successfully treated with hemiepiphysiodesis (guided growth procedure) of the left knee joint to treat genu valgum deformity using 8-plates, resulting in improvement of knee stability. This spared her major ligament reconstruction of the knee to treat the aplastic ACL, and the patient was able to fully participate in phys

Patellofemoral dysbalance and genua valga: outcome after femoral varisation osteotomies.

INTRODUCTION: Patellofemoral dysbalance may be caused by trochlear dysplasia, an elevated TTTG distance, femoral or tibial torsional deformities, patella alta, or a genu valgum. The surgical procedure for the treatment of a genu valgum is varisation osteotomy, usually in the femoral aspect. Several authors believe that a genu valgum is one cause of patellofemoral dysbalance, but studies about the outcome of the treatment with a varisation osteotomy are rare. MATERIALS AND METHODS: Nineteen knees in 18 patients, aged on average 28 (16-52) years were investigated in a retrospective study. The p

[Arthroscopic evaluation and treatment of the disorder resulted from open meniscectomy of the knee].

Between 1989 and 1994, twenty four cases of knee disorders caused by open meniscectomy had undergone the second operation by arthroscopic procedures. The results of retrospective study indicated that main reasons of early disorders following meniscectomy were locking of remanent posterior horn, tears of remanent meniscus and injuries of fat pad and the cause of late disorders were single compartment osteoarthritis and genu varum/valgum caused by meniscectomy. In early cases, the arthroscopic reshape or removal of meniscal remanents and partial resection of fat pads could get satisfactory resul
