Early Cold-stored Platelet Transfusion following Traumatic Brain Injury: A Randomized Clinical Trial.

OBJECTIVE: To determine the feasibility, efficacy, and safety of cold storage compared with room temperature (RT) platelet transfusion in patients with traumatic brain injury (TBI). BACKGROUND: Data demonstrating the safety and efficacy of cold-stored platelet (CSP) transfusion are lacking after TBI. METHODS: A phase 2, randomized, open-label clinical trial was performed at a single U.S. trauma center. Traumatic brain-injured patients with positive brain imaging and a need for platelet transfusion received up to 2 apheresis units of CSPs stored out to 14 days versus standard care RT platelet

Effects of Brain Temperature on the Outcome of Patients with Traumatic Brain Injury: A Prospective Observational Study.

A prospective observational study collected temperature data from 51 patients in 11 neurosurgical centers and follow-up outcome information at 6 months in 49 patients. Brain temperature (Tbr) was measured directly by an intraventricular temperature sensor. Axillary temperature (Tax) and rectal temperature (Tre) were measured by electric thermometers. Tbr was 0.4 to 1.5°C higher than body temperature. Tre correlated well with the Tbr (coefficient: 0.7378; p < 0.05). Among all patients, Glasgow Coma Scale (GCS) scores on admission were significantly lower in the patients with post-operatively ex

Minimum serum cholinesterase levels as a prognostic marker for 6-month outcomes in pediatric traumatic brain injury: a retrospective cohort study.

OBJECTIVE: Although cholinesterase levels have been linked to disease severity in critically ill patients, their prognostic significance in pediatric traumatic brain injury (TBI) remains uncertain. In this study, the authors investigate the association between minimum cholinesterase levels during hospitalization and 6-month outcomes in children with TBI, as well as the potential dose-response relationship between these factors. METHODS: This retrospective cohort study included children with TBI who were admitted to Jiangsu Children's Medical Center between June 2017 and June 2024. Inverse pro

New psychometric data from the Spanish versions of the Glasgow Dyspepsia Severity Score and the Dyspepsia-Related Health Scale measures.

BACKGROUND AND OBJECTIVE: There are no structural abnormalities in functional dyspepsia, therefore it is essential to have a viable questionnaire to measure treatment outcome according to patient perception. The aim of the study was to extensively document psychometric characteristics of the Glasgow Dyspepsia Severity Score and the Dyspepsia-Related Health Scale that are currently available in Spanish. METHODS: Patients with functional dyspepsia (n = 158) were recruited from a randomized trial that assessed standard vs. standard and psychological treatment. Participants had completed the vali

Intracranial compliance monitoring using pulse shape index in traumatic brain injury: relation to cerebral physiology and clinical outcome.

BACKGROUND: The intracranial pressure (ICP) pulse waveform reflects intracranial compliance. The pulse shape index (PSI), an artificial intelligence (AI)-based metric ranging from 1 (normal) to 4 (disturbed), quantifies morphological waveform pathologies. Early findings in smaller cohorts indicate that elevated PSI is associated with mass lesions, aging, higher ICP, and worse outcome. This study examined how PSI relates to other markers of intracranial compliance, the risk of ICP crisis, and clinical outcome in a large traumatic brain injury (TBI) cohort. METHODS: This retrospective study inc

How Early Can We Perform Cranioplasty for Traumatic Brain injury After Decompressive Craniectomy? A Retrospective Multicenter Study.

OBJECTIVE: Decompressive craniectomy (DC) is used to treat intractable intracranial hypertension after severe traumatic brain injury (TBI). Cranioplasty (CP) is typically performed weeks or months later. However, the optimal timing for CP is unknown. We aimed to determine the earliest possible time point for CP. METHODS: We retrospectively reviewed brain computed tomography images from 159 patients who underwent CP after DC for TBI at 3 hospitals. We determined the earliest possible day for CP by reviewing the resolution of intracranial pressure in serial brain computed tomography images betw

Effect of cisternostomy in managing cerebral swelling: A clinical study.

Traumatic brain injury (TBI) often leads to cerebral edema, resulting in elevated intracranial pressure (ICP) and poor neurological outcomes. Therefore, it is of interest to evaluate the efficacy of cisternostomy in managing cerebral edema, showing a significant reduction in ICP and improved neurological outcomes, including Glasgow Coma Scale (GCS) and Glasgow Outcome Scale (GOS) scores. The procedure offers a less invasive alternative to decompressive craniectomy with fewer complications and favorable recovery. Data from 200 patients suggest its potential for improving long-term functional re

Intracranial Pressure Assessment in Traumatic Head Injury with Hemorrhage Via Optic Nerve Sheath Diameter.

Our purpose was to improve the technique of measuring optic nerve sheath diameter (ONSD) for intracranial pressure (ICP) monitoring in cases of traumatic head injury with hemorrhage. In a retrospective study, computed tomography (CT) data of 312 adult patients were collected and analyzed. ONSDs were measured at 3 mm and 8-10 mm distance from the globe together with the eyeball transverse diameter (ETD). The ONSD/ETD ratio was calculated. The correlation analysis was performed with gender, age, Glasgow Coma Scale score, and Glasgow Outcome Score. ONSD was enlarged in all cases when CT scans ind

Hyperbaric Oxygen Therapy in Children with Brain Injury: A Retrospective Case Series.

Introduction and Importance: Some experimental studies on brain injury associated with traumatic brain injury (TBI) and hypoxic-ischaemic encephalopathy (HIE) reveal a positive effect of hyperbaric oxygen therapy (HBOT). However, in clinical medicine, most of the scientific evidence available in the current literature relates only to TBI. Methods: The primary objective is to empirically assess the efficacy of HBOT in mitigating the symptoms of disability associated with brain injury in children, with a view to elucidating its therapeutic potential and clinical benefits. Outcomes: A total of 21

Evaluation and Application of Ultra-Low-Resolution Pressure Reactivity Index in Moderate or Severe Traumatic Brain Injury.

BACKGROUND: The pressure reactivity index (PRx) has emerged as a surrogate method for the continuous bedside estimation of cerebral autoregulation and a predictor of unfavorable outcome after traumatic brain injury (TBI). However, calculation of PRx require continuous high-resolution monitoring currently limited to specialized intensive care units. The aim of this study was to evaluate a new index, the ultra-low-frequency PRx (UL-PRx) sampled at ∼0.0033 Hz at ∼5 minutes periods, and to investigate its association with outcome. METHODS: Demographic data, admission Glasgow coma scale, in-hospit

[Prognostic factors of post-traumatic vegetative states: 522 cases].

Long-term prognosis of post-traumatic vegetative state (VS) remains poorly defined. Three kinds of data have been collected for 522 patients in VS 1 month after head injury: pre-traumatic data; early data, collected during the first two weeks after trauma; late data, collected one month or more after trauma. Statistical relationships have been searched for, between these data and outcome one year after trauma, according to the Glasgow Outcome Scale. Some late data appear as essential prognostic factors of VS: threat blink; complications; ventricular dilatation; motor score (Glasgow Coma Score)
