BACKGROUND: Transplant patients have an increased incidence of lymphoproliferative disorders. The development of a sensitive immunoassay for serum free light chain (sFLC) determination has improved the diagnosis of monoclonal gammapathy. However, metabolism of FLC largely depends on renal function which could lead to misinterpretation of results. The aim of this study was to assess polyclonal FLC in kidney transplant recipients in relation to markers of renal function.
MATERIALS AND METHODS: 97 renal transplant patients (median transplantation follow-up of 2.2 years) were included in this stu

In addition to the structural changes in the kidney associated with aging, physiological changes in renal function are also found in older adults, such as decreased glomerular filtration rate, vascular dysautonomia, altered tubular handling of creatinine, reduction in sodium reabsorption and potassium secretion, and diminished renal reserve. These alterations make aged individuals susceptible to the development of clinical conditions in response to usual stimuli that would otherwise be compensated for in younger individuals, including acute kidney injury, volume depletion and overload, disorde

Microperfusion experiments have shown that increases in flow rate of tubule fluid through the loop of Henle are followed by reductions in single nephron glomerular filtration rate (SNGFR) and stop-flow pressure (SFP) measured in the proximal tubule of the same nephron. Because changes in luminal sodium concentration are not consistently related to changes in SNGFR and SFP, we explored the possibility that a transport step at a flow-dependent distal-sensing site might be involved in feedback control of SNGFR. Because the macula densa cells of the distal tubule are adjacent to the glomerular ves

INTRODUCTION: A recent chronic kidney disease (CKD) progression hierarchical composite endpoint (HCE) utilizes the glomerular filtration rate (GFR) slope for participants without a dichotomous event. Here, we evaluated clinical interpretations when HCE analyses are driven by GFR slope comparisons.
METHODS: Using CKD trial data, we calculated win odds using only GFR slope; dichotomous kidney events and GFR slope; all-cause mortality, dichotomous kidney events, and GFR slope; and all-cause mortality with dichotomous kidney events.
RESULTS: Win odds (95% confidence interval) calculated from pai

STUDY OBJECTIVES: Sleep apnea syndrome (SAS) is potentially linked to life-threatening conditions. The decline in kidney function is involved in the development of various diseases; however, it remains unclear whether it is implicated in the onset of SAS. Therefore, this study aimed to investigate the relationship between kidney function and the incidence of SAS.
METHODS: The association of estimated glomerular filtration rate (eGFR) with the incidence of SAS was assessed retrospectively using real-world administrative claims and health checkup data collected between April 2014 and November 2

BACKGROUND: Amikacin monotherapy is recommended for urinary tract infection (UTI) treatment with multi-resistant pathogens. Even though amikacin efficacy in the treatment of UTIs is dependent on its urinary concentration, there are no robust data proving that sufficiently high urinary concentration is reached in patients with reduced glomerular filtration rate (GFR).
METHODS: A prospective study to monitor amikacin penetration into urine of 70 patients [40 males, median (interquartile range) age 70 (65-79) years] with different levels of glomerular filtration decline, including patients treat

BACKGROUND: Estimated glomerular filtration rate (eGFR) is one of the most important guidelines in deciding the optimal timing of dialysis initiation. In the present study, we calculated the eGFR at the time of hemodialysis (HD) initiation using 5 commonly used equations to relate them with clinical and laboratory characteristics of the patients and to evaluate which of these equations best define the eGFR at HD initiation.
METHODS: We retrospectively analyzed 409 end-stage renal disease patients who were newly started on HD treatment in our institution. The eGFR was calculated using the Cock

PURPOSE: Albuminuria, a marker of diabetic kidney disease, is closely associated with diabetic retinopathy (DR) and diabetic macular edema (DME). However, the relationship between estimated glomerular filtration rate (eGFR) with DR and DME remains unclear, particularly in type 2 diabetes. We investigated the association of eGFR with DR and DME in a sample of patients with type 2 diabetes.
METHODS: We included 263 Caucasian patients with type 2 diabetes aged ≥ 18 years who participated in a clinic-based cross-sectional study in Melbourne, Australia. Diabetic retinopathy (n = 140) and DME (n =

OBJECTIVE: The aim of this study was to evaluate the oral health status of patients with mildly decreased glomerular filtration rate (GFR).
STUDY DESIGN: The study group comprised 80 adults with GFR 60-89 mL/min. A group of 80 age- and gender-matched control subjects with GFR > or =90 mL/min were selected. Medical history, clinical examination, and biochemistry blood tests were performed in patients and control subjects. Renal function was estimated using the Modification of Diet in Renal Disease formula. A single dentist performed an intraoral examination of each patient and control subject.

Furosemide has been reported to produce disproportional changes in blood flow in cortical zones and to inhibit tubuloglomerular feedback (TGF), suggesting that furosemide might alter the intracortical distribution of glomerular filtrate. We have tested this hypothesis by a new method for measuring local and total glomerular filtration rate (GFR) based on proximal tubular accumulation of the basic polypeptide aprotinin (mol wt 6513). Local GFR was calculated in tissue samples dissected from outer cortex (OC), inner cortex (IC) and the corticomedullary border zone (CM) from the plasma clearances

INTRODUCTION: Intra-arterial thrombectomy (IAT), which is widely used to treat acute ischemic stroke, occasionally results in adverse outcomes. IAT-associated risks in patients with decreased estimated glomerular filtration rate (eGFR) remain undefined. Therefore, this study aimed to investigate the effect of IAT on outcomes in patients with acute ischemic stroke and decreased eGFR and to explore other prognosis-associated factors.
METHODS: This study included 309 patients with acute ischemic stroke and decreased eGFR to compare demographics, risk factors, and clinical outcomes based on IAT s

Estimated glomerular filtration rate (eGFR) based on creatinine (eGFRCreatinine) or cystatin C (eGFRCystatinC) require steady-state conditions and thus have limitations in intensive care unit (ICU) patients. Gentamicin is a potential exogenous marker for eGFR but poorly investigated. This retrospective study included adult ICU patients (≥ 18 years) treated with gentamicin and not on renal replacement therapy (RRT) at admission. eGFRCreatinine and eGFRCystatinC were calculated using the LM-rev and CAPA equations, respectively. Gentamicin clearance was estimated using a population pharmacokineti

Chronic kidney disease (CKD) is a global health concern strongly associated with cardiovascular disease (CVD) and high mortality rates. Glomerular filtration rate (GFR) abnormalities, such as glomerular hyperfiltration (GHF) and selective glomerular hypofiltration syndrome (SHS), although not traditionally included in the CKD framework, share chronic inflammation as a pivotal mechanism driving systemic complications and CVD progression. In recent decades, GHF has gained substantial importance due to the high cardiovascular (CV) risk and mortality observed in populations within the highest tert

Many treatment guidelines do not recommend the use of thiazide diuretics or thiazide-like diuretics in patients with impaired kidney function. The rationale is a presumed lack of efficacy of these diuretics in cases of a reduced glomerular filtration rate (GFR); however, this paradigm could not be verified in recent studies. Thiazide diuretics and thiazide-like diuretics are also effective in patients with substantially reduced GFR, which pertains to natriuresis, correction of volume overload and lowering of blood pressure; however, in patients with chronic kidney disease loop diuretics can co

BACKGROUND: People living with HIV (PLWH) have higher rates of chronic kidney disease (CKD) compared with HIV-uninfected individuals. The pathogenesis of CKD in HIV remains poorly understood but is likely from a combination of various factors, such as traditional comorbidities, prolonged antiretroviral therapy, immune dysregulation, and direct HIV effect on the kidneys. We evaluated plasma galectin-3 (Gal-3), a circulating marker of fibrosis, and its association with renal function.
METHODS: Estimated glomerular filtration rate (eGFR) was assessed by CKD-EPI. Plasma galectin-3 was obtained fr
