Harnessing the Forgotten Triangles of the Neck for Lingual Artery Ligation in Glossectomies: A Technical Report.

Various geometric structures, such as numerous triangles, are prevalent in the neck. Despite the fact that many of the neck's anatomical triangles have been well documented, others have been almost forgotten about. The repertoire of head and neck cancer surgeons should include an in-depth understanding of these forgotten triangles. This article seeks to contribute valuable information to the existing scientific literature by shedding light on these neglected triangles, which have significant surgical relevance. In this technical report, we provide a detailed description of a technique that emp

Efficacy of High-Intensity Training in Patients with Moderate to Severe Dysphagia after Glossectomy.

Dysphagia is the main impairment arising from glossectomy for tongue cancer treatment. The study aimed to determine if an eight-week training protocol paired with accuracy tasks and swallowing exercises is effective and can improve tongue strength and swallowing in patients after tongue resection. Maximum isometric pressures, tongue endurance, swallowing pressures, mealtime duration, and oropharyngeal swallow function were studied in patients with moderate to severe dysphagia after glossectomy. Twenty-five (25) patients and thirty-one (31) healthy participants were enrolled in the study. The t

Volume and location of the defect as predictors of speech outcome after glossectomy: correlation with a classification.

The evaluation of speech outcomes after resection and reconstruction of the oral tongue remains largely unsystematic. A cross-sectional study was performed to analyse the speech outcomes of patients who underwent curative treatment with appropriate reconstruction. Sixty-nine patients were assessed for speech intelligibility and phonetics using a validated speech intelligibility assessment tool in the local language. Volume defects were classified as class I (less than one third), II (one third to half), III (half to two-thirds), or IV (two-thirds to total glossectomy). Defect location was defi

Oncological outcomes in patients undergoing major glossectomy for advanced carcinoma of the oral tongue.

INTRODUCTION: Major glossectomy is the treatment of choice in locally advanced tongue cancer. It remains the only option in the presence of recurrent or residual disease. The long-term outcomes for patients undergoing major glossectomy have traditionally been poor, with significant morbidity and poor oncological outcomes. The aim of this study was to report on oncological outcomes in patients undergoing major glossectomy. METHODS: All patients undergoing major glossectomy between 2014 and 2018 were included in the study. The data of 85 patients with advanced carcinoma of the oral tongue were

Functional outcomes and survival of patients with oral and oropharyngeal cancer after total glossectomy.

INTRODUCTION: Cancer of the oral cavity and oropharynx presents aggressive behavior and its diagnosis is, in most cases, performed in advanced stages. Total glossectomy is a therapeutic option in locally advanced cancer, and the only one in the recurrent or residual disease, after chemoradiotherapy. OBJECTIVE: To evaluate the clinical-epidemiological profile, postoperative complications, survival rates and functional aspects of patients with oral cavity and oropharynx cancer after total glossectomy. METHODS: It was a retrospective study where 22 patients were included with oral cavity and or

Compensatory Mechanisms in Patients After a Partial or Total Glossectomy due to Oral Cancer.

Excision of a part or the whole of tongue due to oral cancer disturbs swallowing and speech. Lower airways aspiration of the swallowed bolus in patients after such oral structures excision is a symptom of major swallowing disorder and may be the cause of aspiration pneumonia. Restoration of oral nutrition is possible after exclusion or reduction of aspiration threat in the patients. Video fluoroscopic evaluation of the swallowing performed at the beginning of the swallowing rehabilitation in 95 patients after a total or partial glossectomy due to oral cancer, who assessed their saliva swallowi

A systematic approach to functional reconstruction of the oral cavity following partial and total glossectomy.

Mobility, sensation, volume, and to a lesser extent, the shape of the tongue following partial glossectomy are critical elements to the successful rehabilitation of the patient with oral cancer. Our approach to tongue reconstruction is based on the extent and functional status of the residual tongue and whether there is an associated mandibulectomy. Despite the devastating effects of ablative surgery and radiation, the application of available reconstructive techniques can help to improve the quality of life of these patients. Herein, we present a revised classification scheme for tongue defec

Utility of sublingual gland flap for mucosal defects after partial glossectomy and marginal mandibulectomy.

This study aimed to evaluate the utility of reconstruction with the sublingual gland flap for mucosal defects after partial glossectomy and marginal mandibulectomy. Between 2019 and 2024, reconstruction using sublingual gland flaps was performed after tumor resection in 27 patients with oral cancer (21 partial glossectomies and six marginal mandibulectomies). Flap effectiveness was assessed based on defect size, flap success, and postoperative oral functions (tongue pressure and eating). All patients achieved complete flap mucosalization. The largest soft-tissue defect measured 50 × 40 mm. Ton

Comparison of Quality of Life in Tongue Cancer Patients Following Various Glossectomy Reconstructive Techniques.

Tongue cancer treatment often involves glossectomy and flap reconstruction. Since the tongue plays a vital role in swallowing, chewing, speaking, airway protection, and taste, it also plays a major role in the patient's quality of life (QOL) after reconstruction. Therefore, the flap that best preserves QOL should be determined. A literature review was conducted using PubMed, Scopus, and Ovid MEDLINE, with a total of 446 articles retrieved. Four studies were included in this integrative review, which all utilized the University of Washington Quality of Life questionnaire to assess QOL in tongue

[Tongue reconstruction with a bilateral infrahyoid flap innervated by Ansa Cervicalis after total glossectomy].

INTRODUCTION: Reconstruction after total glossectomy remains a functional challenge. It must provide a large volume to ensure adequate phonation and swallowing. We present the larynx sparing bilateral infrahyoid flap reconstruction procedure after total glossectomy. PATIENTS AND METHOD: Three patients managed for an epidermoid carcinoma of the tongue, classified T4N0, underwent total glossectomy. The tongue was reconstructed with a bilateral infrahyoid flap pedicled on two superior thyroid arteries and innervated by Ansa Cervicalis. RESULTS: Oral food intake was resumed after 8 to 20 days. N

Craniofacial and Dental Complications Following Total Glossectomy Without Multidisciplinary Follow-Up: A Case Report.

Background: Lymphangiomas are rare benign malformations of the lymphatic system, most frequently affecting the head and neck region. Oral involvement is uncommon, and when the tongue is affected, particularly its anterior two-thirds, it can cause major functional and esthetic disturbances. Case Presentation: We report the case of a 13-year-old boy who had undergone total glossectomy at the age of 4 for cystic lymphangioma. During follow-up, the patient presented marked dental and craniofacial alterations, including severe maxillary and mandibular crowding and a skeletal Class III pattern. This

3D modeling of anterior 2/3rds glossectomy reconstruction: A volume based donor site evaluation.

OBJECTIVE: Anterior 2/3rds glossectomy results in significant patient morbidity due to speech and swallowing impairment. Microvascular free flap reconstruction compensates for large volume defects. Flap volume is based on the adipose content of the donor site and varies by patient body mass index (BMI) and donor site location. We sought to correlate flap thickness at different donor sites with patient BMI to determine optimal donor site selection. METHODS: Patients with CT scans of the oral cavity, thorax and lower extremity were identified and included. The volumes of the anterior 2/3rds of

Five-year long-term functional and quality of life outcomes in total glossectomy survivors.

BACKGROUND: Total glossectomy is proven to be a superior treatment option for advanced tongue cancer. Despite its efficacy, the procedure is associated with profound morbidity, notably impacting Speech, swallowing, and overall quality of life. Limited therapeutic benefits and the potential for considerable morbidity render total glossectomy a subject of controversy. METHODS: We performed a multidimensional quality of life evaluation of long-term (more than 5 yrs.) total glossectomy survivors. In this study, we evaluated 25 total glossectomy survivors with comprehensive functional and quality

Oncological outcomes of total glossectomy procedure for advanced tongue cancer: a single-centre experience.

Surgeons treating advanced carcinoma of the tongue with total glossectomy face many conflicts in view of the morbidity and poor functional and survival outcomes following surgery. It is pertinent to study the patients undergoing total tongue compartment resection as a separate cohort to analyse their outcomes. This study investigated the oncological outcomes of 150 patients with advanced tongue squamous cell carcinoma who underwent total glossectomy. The results suggest that compartment resection significantly improved local control, irrespective of margin status. The presence of multiple posi
