Effect of glycemic control on outcomes after aneurysmal subarachnoid hemorrhage: A comprehensive analysis of stress-induced hyperglycemia and chronic glycemic status.

BACKGROUND: Aneurysmal subarachnoid hemorrhage (aSAH) frequently precipitates stress-induced hyperglycemia, yet the relationship between baseline glycemic control and clinical outcomes remains poorly understood. This study investigates the association between glycosylated hemoglobin (HbA1c) levels and clinical outcomes in aSAH patients, with a particular focus on stress-induced hyperglycemia(SIH) patterns. METHODS: This prospective observational study enrolled 155 consecutive aSAH patients aged ≥ 18 years. Patients were stratified based on admission HbA1c levels (<6.0 % vs ≥ 6.0 %). Among pat

Perioperative Glycemic Control and Outcomes in Cardiac Surgery: A Retrospective Cohort Study.

BACKGROUND Perioperative hyperglycemia is a common metabolic response to surgical stress and has been shown to be associated with adverse outcomes after cardiac surgery. However, the optimal glycemic target in this setting remains uncertain, since both hyperglycemia and hypoglycemia can be harmful. This study aimed to evaluate the association between perioperative glucose levels and postoperative infection and in-hospital mortality in patients undergoing cardiac surgery at a tertiary cardiovascular center. MATERIAL AND METHODS This study included consecutive adult patients who underwent electi

Social and rural determinants of glycemic control in type 2 diabetes mellitus.

Background and objectives The global epidemic of type 2 diabetes mellitus (T2DM) is disproportionately severe in rural and agricultural populations, driven by the complex interplay of socioeconomic, behavioural, and genetic risk factors. While these determinants are acknowledged individually, few studies have validated a parsimonious, integrated measurement model to effectively structure and assess their collective influence on glycaemic control within a rural primary care context. We aimed to address this critical gap. Methods We conducted a cross-sectional, observational study utilising a pu

Beyond Glycemic Control: Ocular Effects of Glucagon-like Peptide-1 Receptor Agonists.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) and newer dual-incretin therapies have become central to the treatment of diabetes mellitus and obesity, with benefits extending beyond glycemic control. Their expanding use has prompted growing interest in their potential ocular effects. Experimental data support plausible protective mechanisms, including reduction in oxidative stress and neuroprotective effects on retinal and optic nerve tissues. Clinical evidence, however, remains heterogeneous. In diabetic retinopathy, the main concern appears to be transient early worsening associated w

Unmasking the Emotional and Behavioral Determinants of Glycemic Control in Diabetes Management.

Background In type 2 diabetes mellitus (T2DM), glycemic regulation is influenced not only by physiological parameters but also by behavioral patterns, emotional states, and socioeconomic variables. These nonclinical factors are increasingly recognized for their potential role in personalizing diabetes care. Objectives This study examined the influence of behavioral factors (such as medication adherence and diabetes self-care), diabetes-related emotional distress, and socioeconomic status, in addition to clinical determinants, on glycemic control among urban Indian individuals with T2DM. Electroacupuncture in glycemic control: Transitioning from clinical controversies to potential basic research.

Diabetes is a major global metabolic disorder, with the type 2 diabetes mellitus (T2DM) population in China expected to reach 168 million by 2050. Electroacupuncture (EA) and auricular acupuncture represent safe, accessible, and multi-targeted strategies for glycemic control with strong translational potential. Clinical studies indicate that EA, targeting trunk and limb acupoints, can significantly reduce fasting plasma glucose and glycated hemoglobin. Mechanistically, EA modulates the neuro-endocrine-immune axis, regulates gut microbiota, and activates insulin signaling pathways. Auricular ac

Effect of dolutegravir-based antiretroviral therapy on glycemic control in female mice.

Dolutegravir (DTG), is recommended for all people with HIV, including pregnant women. Weight gain and hyperglycemia have been reported with DTG use, as well as a signal for neural tube defects (NTDs) that has waned over time. Obesity and hyperglycemia are risk factors for NTDs. We explored the impact of DTG-based antiretroviral therapy (ART) on weight gain and glucose homeostasis in female mice. C57BL/6 mice were treated daily for 9 weeks with water (control), 1xDTG (2.5 mg/kg DTG + 33.3/50 mg/kg emtricitabine/tenofovir-yielding therapeutic levels), or 5xDTG (12.5 mg/kg + 33.3/50 mg/kg emtrici

Role of endoscopic duodenojejunal bypass liner in obesity management and glycemic control.

The treatment of obesity and its comorbidities ranges from clinical management involving lifestyle changes and medications to bariat-ric and metabolic surgery. Various endoscopic bariatric and metabolic therapies recently emerged to address an important therapeutic gap by offering a less invasive alternative to surgery that is more effective than conservative therapies. This article compre-hensively reviews the technical aspects, mechanism of action, outcomes, and future perspectives of one of the most promising endoscopic bariatric and metabolic therapies, named duodenojejunal bypass liner. Using insulin pump for glycemic control in patients with severe insulin resistance.

BACKGROUND: Patients with type 2 diabetes using a continuous subcutaneous insulin infusion (CSII) often require large doses of insulin and need to change their insulin administration sets frequently. A proposed solution to this problem is to use concentrated insulin in their insulin pump; however, insulin pumps are programmed to administer U-100 insulin. Therefore, these patients are at greater risk of hypoglycemia and are responsible for adjusting daily doses. CASE SUMMARY: The solution for our patient encountering this problem was to administer half of his daily basal insulin via subcutaneo

Unlocking the Full Potential of SGLT2 Inhibitors: Expanding Applications beyond Glycemic Control.

The number of diabetic patients has risen dramatically in recent decades, owing mostly to the rising incidence of type 2 diabetes mellitus (T2DM). Several oral antidiabetic medications are used for the treatment of T2DM including, α-glucosidases inhibitors, biguanides, sulfonylureas, meglitinides, GLP-1 receptor agonists, PPAR-γ agonists, DDP4 inhibitors, and SGLT2 inhibitors. In this review we focus on the possible effects of SGLT2 inhibitors on different body systems. Beyond the diabetic state, SGLT2 inhibitors have revealed a demonstrable ability to ameliorate cardiac remodeling, enhance my

Treatment of Hepatitis C Virus and Long-Term Effect on Glycemic Control.

BACKGROUND: Chronic hepatitis C virus (HCV) infection has been linked to worsening glycemic control in patients with diabetes due to insulin resistance. Studies have shown that treating HCV improves glycemic control in this patient population. Most studies assess glycemic control until the patient's sustained viral response at 12 weeks (SVR12). OBJECTIVES: To (a) assess the sustainability of glycemic control after the SVR12 date and (b) determine the change in diabetic medication use over time. METHODS: This was a retrospective chart review of patients treated at an academic medical center's

Steroid-Free Immune Suppression Impairs Glycemic Control in a Healthy Cynomolgus Monkey.

The need for chronic immune suppression (IS) is one of the hurdles precluding widespread use of islet cell transplantation to restore glycemic control in patients with type 1 diabetes. We report the case of a healthy nonhuman primate (NHP) treated on and off for over 2.5 years with steroid-free IS, consisting of daclizumab induction and maintenance therapy with rapamycin and low dose tacrolimus. Treatment for 1 year resulted in a striking destabilization of glycemic control, with concomitant decreases in fasting c-peptide and insulin levels. Although these changes gradually reversed during a w
