Evaluating Dynamic Smile Outcomes of Free Gracilis Muscle Transfer in Pediatric Facial Palsy Using Image-Based Analysis.

BACKGROUND: Facial nerve palsy in children leads to significant functional impairment and facial asymmetry. While free gracilis muscle transfer (FGMT) is a cornerstone technique for smile reanimation in both pediatric and adult patients, its evaluation has mainly focused on the single metric of commissure excursion. This study seeks to evaluate the effectiveness of FGMT in restoring dynamic smiles in pediatric patients with facial palsy using image analysis. METHODS: A retrospective review was conducted in children who underwent FGMT for facial palsy at a major children's hospital between 200

Correction of Residual Static and Dynamic Labial Deviations in a Paralyzed Face After Free Gracilis Muscle Transplantation.

Free muscle flap transfer is currently the procedure of choice for longstanding facial paralysis to restore symmetry both at rest and when smiling. However, movements obtained are generally localized, unidirectional, and philtrum centralization and lower lip movement is not proportionally achieved. The stability of free flap insertion at the lips also interferes with the results, as gradual disinsertion and shifting of the nasolabial fold can be caused by repetitive movements. Asymmetry of smile can also be caused by lip depressor inactivity due to marginal mandibular paralysis, and both dynam

One versus two venous anastomoses in microvascular lower extremity reconstruction using gracilis muscle or anterolateral thigh flaps.

INTRODUCTION: Free tissue transfers are a highly reliable procedure routinely performed for reconstruction of a wide range of defects. Main complication in free flap surgery is usually venous thrombosis. Many technical controversies exist regarding the technical details of the microvascular anastomosis in order to prevent occurrence of thrombosis and optimize outcomes. We therefore evaluated our results regarding the execution of one versus two venous anastomoses in a variety of free flaps (fasciocutaneous- or muscle free flap) utilized for lower limb reconstruction. PATIENTS AND METHODS: Bet

Donor nerve sources in free functional gracilis muscle transfer for elbow flexion in adult brachial plexus injury.

BACKGROUND: With complete plexus injuries or late presentation, free functional muscle transfer (FFMT) becomes the primary option of functional restoration. Our purpose is to review cases over a 10-year period of free functioning gracilis muscle transfer after brachial plexus injury to evaluate the effect of different donor nerves used to reinnervate the FFMT on functional outcome. METHODS: A retrospective study from April 2001 to January 2011 of a single surgeon's practice was undertaken. During this time period 22 patients underwent FFMT at Washington University in St Louis, Missouri for el

[Free re-innervated gracilis muscle transfer to restore flexion or extension of the fingers: about three cases].

Free re-innervated gracilis muscle transfer is used to restore fingers or wrist flexion or extension after severe forearm injuries and when none muscle transfer is possible. The long-term function after this procedure is evaluated for three patients treated in our unit. This microsurgical transfer is technically demanding but allows good fingers mobility and good autonomy for daily activities and working.

Effectiveness and outcome of gracilis muscle flap reconstruction following perineal surgery: a retrospective single-center cohort study.

BACKGROUND: Perineal wound healing complications frequently occur following abdominoperineal resection (APR) for low-lying rectal cancers, posing significant reconstructive challenges. Among various reconstructive options, gracilis muscle flaps have gained prominence due to their reliability in addressing sacral dead-space defects. Despite their popularity, decision-making criteria regarding unilateral versus bilateral gracilis flap use, supplementary adipose tissue flaps, and optimal timing for reconstruction remain inadequately defined. MATERIALS AND METHODS: We conducted a retrospective si

Quality-of-life improvement after free gracilis muscle transfer for smile restoration in patients with facial paralysis.

IMPORTANCE: Facial paralysis can contribute to disfigurement, psychological difficulties, and an inability to convey emotion via facial expression. In patients unable to perform a meaningful smile, free gracilis muscle transfer (FGMT) can often restore smile function. However, little is known about the impact on disease-specific quality of life. OBJECTIVE: To determine quantitatively whether FGMT improves quality of life in patients with facial paralysis. DESIGN, SETTING, AND PARTICIPANTS: Prospective evaluation of 154 FGMTs performed at a facial nerve center on 148 patients with facial para

Nonuniform hip adductor muscle deficits in children with cerebral palsy: a comparison between the gracilis and adductor longus.

Cerebral palsy (CP) is the most common cause of childhood physical disability and often results in muscle weakness, spasticity, and contractures. Although cellular and sarcomeric impairments have been documented in muscles from children with CP, systematic comparisons of multiple muscles within the same child remain limited. Therefore, we investigated within-subject differences in single muscle fiber mechanics, sarcomeric protein content, and histological properties between patient-paired adductor longus (AL) and gracilis (GR) muscle biopsies obtained from eleven children with severe spastic C
