Intrathoracic airstream temperatures during acute expansions of thoracic blood volume.

1. To determine the validity of employing intrathoracic heat flux as a reflection of changes in bronchial blood flow, we used a thermal probe to record airstream temperatures within the tracheobronchial tree in five normal and five asthmatic subjects during isocapnic hyperventilation challenges with and without inflation of the lower limb bladders of a pressure suit. 2. During hyperpnoea, airstream temperatures fell progressively in both subject groups. When blood volume was acutely shifted from the legs into the thorax via anti-shock trousers, airstream temperatures within the tracheobronchia

A safe, pre-fabricated, immediate postoperative prosthetic limb system for rehabilitation of below-knee amputations.

Thirty-eight below-knee amputees were treated with a commercially available pneumatic prosthetic limb system, applied immediately postoperatively, that would allow immediate weight bearing following lower extremity amputation. Thirty-four limbs were amputated for peripheral vascular insufficiency and four were amputated due to trauma. Weight bearing was initiated an average of 4.7 days following surgery and patients were discharged with a non-removable temporary prosthesis at an average of 8.9 days. Thirty-three healed and were uneventfully fit with standard below-knee prosthetic limbs. Three

Immediate hemodynamic consequences of MAST inflation in normo- and hypovolemic anesthetized swine.

Use of the military antishock trouser (MAST) remains controversial in part because its mechanism(s) of action are poorly understood. We studied two aspects of the hemodynamic response to MAST inflation in 14 anesthetized swine. First, in six swine the relation that existed between inferior vena cava flow and aortic pressure/cardiac output was determined before and during inflation of the MAST, and then before and after removal of 30% of the calculated blood volume. Inflation of the MAST before hemorrhage had little effect on cardiac output but increased aortic pressure by 25%. Inflation of the

A cluster randomized controlled trial of the non-pneumatic anti-shock garment for obstetric haemorrhage: sub-analysis of the Zimbabwean Arm.

OBJECTIVES: To determine whether earlier application of the Non-pneumatic Anti-Shock Garment (NASG) at clinic level compared to the referral hospital level reduces maternal morbidity and mortality and recovery time from shock due to severe Obstetric Haemorrhage (OH) and to determine the safety of the NASG when applied at clinic level. DESIGN: A cluster Randomized Controlled Trial (RCT) SETTING: Harare and Parirenyatwa Referral Hospitals (RH) in Harare and the twelve Harare City Council clinics that offer maternity care. SUBJECTS: Women who had suffered severe OH at clinic level and were bei

Acceleration endurance with pressure breathing during G with and without a counterpressure vest.

INTRODUCTION: The purpose of this study was to test whether pressure breathing during G (PBC) without a counterpressure vest negatively influences G endurance or increases breathing fatigue during extended duration high-G exposures. METHODS: While using PBG, 10 subjects underwent 2 trials of +3 Gz exposures: once when wearing a counterpressure vest and once without. The exposures consisted of a relaxed, gradual G onset run until peripheral or central light loss, a straining rapid onset GC run to +6 Gz for 15 s, and a simulated aerial combat maneuver (SACM) G profile consisting of 10-s periods

The use of pressure and silicone in hypertrophic scar management in burns patients: a pilot randomized controlled trial.

This pilot study investigates whether pressure and silicone therapy used simultaneously are more effective in treating multiple characteristics of hypertrophic scars than pressure alone. A pilot randomized controlled trial was conducted. Twenty-two participants with hypertrophic burn scars were randomized to receive Jobskin pressure garments and Mepiform silicone sheeting or Jobskin pressure garments alone. The Vancouver Scar Scale (VSS) was used to measure multiple scar characteristics at baseline, week 12, and week 24. No statistically significant difference was found in the rate of change o

Anti-shock garment in postpartum haemorrhage.

The non-pneumatic anti-shock garment (NASG) is a first-aid device that reverses hypovolaemic shock and decreases obstetric haemorrhage. It consists of articulated neoprene segments that close tightly with Velcro, shunting blood from the lower body to the core organs, elevating blood pressure and increasing preload and cardiac output. This chapter describes the controversial history of the predecessors of NASG, pneumatic anti-shock garments (PASGs), relates case studies of PASG for obstetric haemorrhage, compares pneumatic and non-pneumatic devices and posits why the NASG is more appropriate fo

Alterations in upper airway cross-sectional area in response to lower body positive pressure in healthy subjects.

BACKGROUND: Fluid accumulation in the neck during recumbency might narrow the upper airway (UA) and thereby contribute to its collapse in patients with obstructive sleep apnoea (OSA). It is hypothesised that acute fluid shifts from the legs to the upper body in healthy subjects would increase neck circumference and reduce the cross-sectional area of the UA (UA-XSA). METHODS: In 27 healthy non-obese subjects of mean (SE) age 39 (3) years and body mass index 23.2 (0.6) kg/m2 studied while supine, leg fluid volume was measured using bioelectrical impedance, neck circumference using a mercury str

Compliance with pressure garment use in burn rehabilitation.

Pressure garment use is recommended 23 to 24 hours a day for hypertrophic scar control after a burn injury. Compliance with this treatment has not been documented. A 52-question survey was administered to 101 adult outpatient burn survivors to rate compliance with this program. The average compliance of each patient was assessed. The framework of the Health Belief Model was used to identify the patients' beliefs and factors reported to interfere with compliance. Strategies patients believed to enhance garment use were identified. Forty-one percent of the patients reported total compliance. The

Anesthetic implications of the child with homocystinuria.

We present a 7 1/2-year-old girl with type I homocystinuria who required anesthetic care during repair of bilateral ectopia lentis. Homocystinuria is an autosomal recessive disorder and represents the second most common error of amino acid metabolism. Defective function of the enzyme cystathionine synthetase results in elevated serum levels of methionine and homocysteine. Physiologic disturbances include an increased risk of thromboembolic events and hypoglycemia. Perioperative measures to prevent thromboembolic events include diet therapy to lower serum methionine and homocysteine levels, ade

Hemodynamics of miniature swine during +Gz stress with and without anti-G support.

Nine unanesthetized, chronically instrumented, female miniature swine (MS) (avg wt, 39.7 kg) were exposed to head-to-tail inertial load (+Gz) levels of +3, +5, and +7 Gz for 60 s, with and without anti-G-suit inflation. Venous flow (VF) was measured by an electromagnetic flow sensor around the inferior thoracic vena cava at the diaphragm. Central venous pressure (CVP), abdominal venous pressure (AVP), eye-level blood pressure (ELBP), and esophageal pressure (EP) were also measured before, during, and after +Gz. There was a progressive significant decrease from control of both ELBP (P less than

Pilot Study: Pneumatic Compression Garment Therapy for Postradiotherapy Laryngopharyngeal Edema.

OBJECTIVES: Pneumatic compression garment therapy (PCGT) has been established as treatment for postradiotherapy lymphedema, and its use in head and neck patients is becoming more common. Although effects on interstitial edema of the cervical soft tissues have been studied, effects on internal laryngopharyngeal edema, as well as associated symptoms of dysphagia and dysphonia, have yet to be published. METHODS: We surveyed 7 patients treated with radiation for head and neck cancer (HNC) who had also been prescribed PCGT for cervical lymphedema. Patients were asked about subjective experience wi

Iatrogenic vascular injuries. A review of 125 cases (excluding angiographic injuries).

From the files of an insurance company and from a number of our own files as expert medical witness to the court we have been able to collect 125 cases of malpractice suits following a iatrogenic vascular injury. An analytic study has revealed the following: --12 vascular injuries occurred after stripping of varicose veins of the lower extremities; --4 accidents occurred during arterial surgery; --35 lesions followed orthopedic surgery; and --2 general surgery. Besides these 53 injuries following surgical procedures, 72 injuries are described following non-surgical diagnostic or therapeutic pr

Current status on new anticoagulant and antithrombotic drugs and devices.

Several new drugs for the management of thromboembolic disorders have recently become available. Low-molecular-weight heparins are being evaluated for the prophylaxis of medical and surgical deep venous thrombosis and pulmonary embolism; for the treatment of pre-existing thrombosis; and for cases of coronary syndrome (unstable angina, myocardial infarction), thrombotic and ischemic stroke, interventional cardiology, pregnancy, cancer, and transplantation-associated thrombosis. A chemically synthesized heparin pentasaccharide, which has purely anti-factor Xa activity and does not induce thrombo

Effect of a pneumatic antishock garment on drug delivery via distal venous access.

We examined the effect of an inflated pneumatic antishock garment (PASG) on simulated drug delivery through a distally placed venous catheter, during both normal flow and cardiopulmonary resuscitation (CPR). A PASG device was applied to anesthetized mongrel dogs and was inflated to 60 mm Hg. A small bolus of radionuclide was injected through an intravenous catheter placed distal to the PASG. Emission counts were made over both ventricles during conditions of normal flow and then during CPR following cardiac arrest. Mean times from injection to peak counts were determined. A control group of an

Efficacy of compression of different capacitance beds in the amelioration of orthostatic hypotension.

Orthostatic hypotension (OH) is the most disabling and serious manifestation of adrenergic failure, occurring in the autonomic neuropathies, pure autonomic failure (PAF) and multiple system atrophy (MSA). No specific treatment is currently available for most etiologies of OH. A reduction in venous capacity, secondary to some physical counter maneuvers (e.g., squatting or leg crossing), or the use of compressive garments, can ameliorate OH. However, there is little information on the differential efficacy, or the mechanisms of improvement, engendered by compression of specific capacitance beds.

Venous thromboembolism. Risk factors and prophylaxis.

Most deaths from pulmonary embolism can be prevented by providing adequate prophylaxis for hospital patients who are at high risk for venous thromboembolism. Physicians, however, must be able to identify high-risk patients (including those with major trauma, hip or knee replacement, or pelvic fractures) and prescribe an appropriate prophylactic agent for each patient according to his or her level of risk. A number of prophylactic methods have been proven effective in the prevention of venous thromboembolism, including low-dose heparin, low-molecular-weight heparin, warfarin, graduated compress

[Mechanisms of early changes in water-electrolyte metabolism in man in various ground-based models of the effects of microgravity].

Analysed was an array of data amassed in 4 series of experiments with simulated microgravity effects (HDT (-6 degrees); dry and suited immersions in horizontal and vertical position). Water-electrolyte turnover in blood and urine and hormonal controls have been under study. It was demonstrated that irrespective of the body axis position relative to the gravity vector in the immersion models, reactions of the active regulation of the water--electrolyte metabolism were noted already within the initial hours, i.e. more rapidly than in HDT. During the early phase of adaptation to the immersion mod
