Hand and Wrist Injuries: Common Problems and Solutions.

Injuries to the hands and wrist are common in athletes. Injuries include acute fractures, dislocations, ligamentous, and tendon injuries, as well as more chronic injuries such as sprains and strains. Complications in the treatment of sports injuries of the hand and wrist may be divided into 2 categories: incorrect or delayed diagnosis and iatrogenic injury related to treatment. This article highlights common sports injuries of the hand and wrist and their complications, and includes tips for successful management.

Tendon Injuries of the Hand in Kirikkale, Turkey.

BACKGROUND: Tendon injuries are one of the most common injuries of the hand and challenging problems in trauma surgery. They usually require surgical repair and unlike the single tendon injuries, flexor tendon injuries have higher morbidities when accompanied with nerve injuries. In the present study, I aimed to report the clinical experiences and outcomes about tendon injuries. METHODS: A total of 180 patients (149 males, 31 females) between 17 and 56 years old were operated in the clinic due to tendon injury. Seventy isolated extensor tendon injuries, 60 isolated flexor tendon injuries, 30

Agricultural injuries to the hand and upper extremity.

Agricultural injuries involving the hand and upper extremity are common, debilitating injuries that reflect the significant occupational hazards associated with the agricultural industry. Farm injuries occur in all age groups and are associated with significant resource utilization and treatment costs. Most of these injuries are associated with machinery, including tractors, power take-off devices, grain augers, hay balers, and combine harvesters. Each piece of machinery produces specific injury patterns and a spectrum of bone and soft-tissue injuries that are frequently characterized by the l

Sports-specific injuries of the hand and wrist.

This article provides a philosophic overview of the management of athletes at all levels from high school to professional. It further reviews sports-specific injury patterns and position-specific injury patterns. This aspect is crucial, as many injuries may be relatively common for specific sports but rare in the general population, so recognition of these injuries requires a high degree of suspicion. The guiding principles of management are also discussed.

US findings in traumatic wrist and hand injuries.

Traumatic injuries of the wrist and hand are common. Ultrasonography (US) is a cost-effective, noninvasive, and effective imaging method for diagnosing traumatic lesions affecting the tendons, annular pulleys, ligaments, vessels, and nerves, and for detecting foreign bodies in the wrist and hand. The objective of this article was to review the main US findings of traumatic lesions in the wrist and hand.

[Costs in hand amputations derived from labor injuries].

BACKGROUND: Hand injuries by labor accidents are first rank. It is necessary to have a multidisciplinary medical approach to frequently generated temporary and permanent disabilities that affect costs to an institution and to enterprise. OBJECTIVE: To determine the direct cost (DC) and the indirect cost (IC) of complete and partial amputations in hand caused by labor injuries. METHODS: An observational study was performed. The data was obtained from labor injuries with amputation of a finger or hand that received multidisciplinary management. The costs were calculated according to the list o

[Hand infections resulting from underestimation of minimal injuries].

Frequently, the timely onset of adequate treatment is delayed when a hand infection is the result of minor trauma. The patients as well as the physicians are tempted to underestimate this complication. To prove that certain forms of trauma correlate with the degree of underestimation, we have prospectively studied the cases of 105 patients who required hospitalisation and surgical interventions between 2001 and 2003. We defined a cut-off point at 48 hours following trauma and differentiated between patients who appeared within this period from those who came later. Lesions at the fingertips, s

Hand and wrist injuries: Part II. Emergent evaluation.

Primary care physicians must be able to recognize wrist and hand injuries that require immediate attention. A complete history and physical examination, including assessment of distal limb function, are essential. Hemorrhage control is necessary in patients with vessel lacerations and amputations. Amputations require an understanding of the indications and contraindications in the management of the amputated limb. High-pressure injection injuries and compartment syndromes require a high index of suspicion for early recognition. Infectious entities include "fight bite," open fractures, purulent

Tendinous cutaneous dorsal hand injuries. One-stage reconstruction.

The reconstruction of complex cutaneous tendinous dorsal hand injuries represents a problem that is not easy to solve. The transferral in a single surgical stage, with a single flap, of skin, tendons, and nerves, all completely vascularized, is probably the ideal solution. Between 1988 and 1999 the one-stage reconstruction method was used in 13 patients. A cutaneous tendinous dorsalis pedis free flap was used in 7 cases, and a cutaneous tendinous radial forearm island flap with an inverted flow was used in 6. The dorsalis pedis flap allows for the inclusion of 4 tendons that are completely vas

[Flexor and extensor tendon injuries of the hand].

Injuries of the flexor tendon are usually open injuries. In most cases primary treatment is therefore seldom performed by orthopaedic surgeons. This is different for injuries of the extensor tendon. Three quarters of injuries of the extensor tendon are closed injuries. Therefore we do go into primary and secondary reconstruction of the flexor tendon, but focus on primary and secondary reconstruction of the extensor tendon in the most common zones (1, Th I, 3, Th III).

Primary care of hand and wrist athletic injuries.

Athletic injures to the hand and wrist can range from simple sprains to severe fractures or soft-tissue disruptions that can permanently threaten the normal function of the extremity. This article deals with some of the more commonly noted sports-related injures to the hand and wrist to help the team physician make the correct diagnosis and establish the most effective treatment plan, so that the athlete may achieve maximum results and ultimately return to full participation in their sport.

Psychologic assessment of patients with industrial hand injuries.

The benefits and rationale for including a psychologic assessment of a patient with an industrial hand injury are discussed. Issues of compliance and malingering in patients are addressed. The role of pre-existing conditions in understanding an injured patient's current emotional state is explored and contrasted with posttraumatic stress disorders. Recent trends in psychologic assessment techniques are highlighted.

Epidemiology of hand and wrist injuries in sports.

Most sports are associated with specific patterns of hand injury. In some cases, these are unavoidable due to the nature of hand involvement dictated by the rules or environment of the sport. In other cases, such injuries point out the need for better protective gear or training.

Injuries to the hand inflicted by rotary snowcutters.

In the period January 1, 1975, through December 31, 1986, 28 patients were treated for hand injuries caused by small home rotary snowcutters. These injuries were characterized by extensive lacerations and contusions, particularly on the dorsal side of the hand and the fingers, with concomitant extensor tendon ruptures, comminuted and complicated fractures, and traumatic amputations of fingers of the dominant hand. The index and middle finger were nearly always injured; the thumb was usually totally spared. The frequency of postoperative complications was low after delayed primary surgical trea

Paediatric hand injuries and the galvanized-iron fence.

A review of all cases of operative hand surgery at The Adelaide Children's Hospital during the period January 1-June 30, 1988 yielded 38 cases that resulted from trauma (burns and fractures excluded). Lacerations from uncapped galvanized-iron fences occurred in 10 patients. Of these patients, two patients required skin grafts, seven patients had flexor-tendon injuries and three patients suffered digital-nerve lacerations. Twenty-five fingers were damaged. The pattern of injury involves multiple digits at different levels, with distally-based skin flaps, frequent divisions of flexor tendons and

Injuries to the hand and wrist in athletes.

Injuries involving tendons, ligaments, and bones are commonplace with many physical activities. Although the majority of these injuries are minor and cause only temporary discomfort, their potential for producing serious and occasionally permanent disabilities must not be ignored. The initial physical and roentgenographic examination of the injured part must be thorough. Only then can the severity of the injury be recognized and appropriate treatment undertaken. Promptness will limit the period of disability and minimize the chances of development of a chronic condition.

Repair of extensor tendon injuries of the hand.

We have presented methods for treating extensor tendon injuries from the interphalangeal joint to the wrist and the musculotendinous junction in the forearm. Early and proper splinting in the treatment of extensor tendon injuries is more important than a specific method of surgical repair. We emphasize the need for prolonged splinting, up to eight weeks in distal injuries. Immobilizing the finger in full extension or hyperextension is necessary at the distal and proximal interphalangeal joints. Correct splinting is mandatory in any method of treatment. Reconstruction of the extensor mechanism
