Despite the widespread use of rigid fixation techniques in craniofacial surgery, there is a paucity of studies in the literature that serve to better define the reasons for the subsequent removal of plates and screws. The current study appears to be the first to attempt to assess these issues among a broad range of craniofacial surgery patients. Fifty-five patients who underwent hardware removal following craniofacial surgery at the University of Michigan Medical Center between 1989 and 1995 were retrospectively studied via an in-depth chart review. Common reasons for hardware removal included

BACKGROUND: Fractures of the distal third of the leg are increasingly common and are often handled by open reduction and internal fixation. Exposure and infection of internal hardware could occur, especially after high energy traumas, requiring hardware removal and delayed soft tissue reconstruction. Nevertheless immediate soft tissue reconstruction without internal hardware removal is still possible in selected patients. In this study the effectiveness and the complications of immediate soft tissue reconstruction without internal hardware removal is analyzed.
METHODS: 13 patients, affected b

BACKGROUND: Vertical expandable prosthetic titanium rib (VEPTR) devices were designed to treat childhood scoliosis and thoracic insufficiency syndrome. Although they have drastically reduced patient mortality, they present a profound subcutaneous hardware burden. The authors examined the surgical and nonsurgical factors associated with salvage of VEPTR hardware exposures.
METHODS: Between 2014 and 2020, a prospective institutional database was queried for patients with VEPTR hardware complications who required soft-tissue reconstruction. Hardware salvage was considered successful if reconstru

PURPOSE: While the indications and options expand for treatment of pediatric fractures with metallic implants, routine removal of implants remains controversial. There is a paucity of randomized prospective trials to determine the risk of retention versus removal. Prior studies provided insight into the preferences of POSNA members versus their nonpediatric counterparts; however, they did not capture specifics regarding timing, anatomic location, implant type, or patient age. This study aims to report POSNA members' practice habits regarding the routine removal of orthopaedic implants after fr

In a study of 41 supracondylar-intercondylar femur fractures treated with open reduction and plate fixation, 15 patients requested plate removal due to lateral knee pain over the hardware. A refracture of the distal femur occurred within 10 weeks of hardware removal in 4 (27%) of 15 patients. All refractures occurred during normal functional activities. Patients who request hardware removal following union of a distal femur fracture treated with open reduction and plating should be informed of the possibility of refracture.

INTRODUCTION: Neuromodulation using deep brain stimulation (DBS), spinal cord stimulation (SCS), and peripheral nerve field stimulation (PNFS) to treat neurological, psychiatric, and pain disorders is a rapidly growing field. Infections related to the implanted hardware are among the most common complications and result in health-related and economic burden. Unfortunately, conservative medical therapy is less likely to be successful. In this retrospective study, we aimed to identify characteristics of the infections and investigated surgical and antimicrobial treatments.
METHODS: A retrospect

Background The inclusion of the third carpometacarpal (CMC) joint in the fusion mass in total wrist fusion (TWF) remains controversial. Our goal was to evaluate the clinical outcomes and effects of third CMC joint arthrodesis compared with bridging the CMC joint during TWF. A retrospective chart review was performed. Outcomes assessed included hardware loosening, hardware failure, symptomatic hardware necessitating removal, and need for revision arthrodesis. Case Description/Literature Review We found that concomitant third CMC joint arthrodesis was associated with a significantly reduced ra

Cortical fragments within the intramedullary canal can pose a challenge during intramedullary nail treatment of tibial and femoral shaft fractures. Complications including reamer entrapment, malunion, and inability to pass final implants have all been reported. To limit soft tissue injury and avoid an open approach, laparoscopic instrumentation can be a useful adjunct to manipulate or retrieve cortical fragments at distant sites. Our objective is to outline a method to remove cortical fragments during treatment of tibial and femoral shaft fracture and characterize the standard shaft lengths di

AIMS: Superior plating has long been the routine surgical approach for mid-shaft fractures of the clavicle. However, concerns about the prominence of the plate and neurological risk have led to an increased interest in anteroinferior plating. Whether anteroinferior plating offers comparable or improved outcomes without compromising the stability of fixation has been investigated in several studies. The aim of this meta-analysis was to evaluate the clinical outcomes, complications, and complication-related outcomes following superior compared with anteroinferior plating of mid-shaft fractures o

CASE: A 41-year-old woman with prior T12-L4 instrumentation for adolescent lumbar scoliosis presented with left posteromedial thigh pain initially diagnosed as an adductor strain. Two years later, she re-presented with knee stiffness, lower leg mottling, and paresthesia suspected to be reflex sympathetic dystrophy. Radiographs later revealed a 13.6-cm radiopaque object in the popliteal fossa and rod absence from the lumbar spine. The spinal rod was removed from the popliteal vein by orthopaedic surgery.
CONCLUSION: Radiographs are an easily obtained, low-cost, and high-yield resource that are

BACKGROUND: Orthopaedic surgery frequently requires the use of metal plate and screw constructs for osteosynthesis. These constructs may be subsequently removed for a variety of indications. In the lower extremity in particular, implants (ie, hardware) can lead to prominence and pain secondary to a thin soft-tissue envelope with little subcutaneous tissue. Often, removal is performed without knowledge of the exact type of screw head configuration and/or size, which can make removal technically challenging. The purpose of this study is to consolidate screw head sizes and configurations from com

BACKGROUND: Contemporary maxillofacial osteosynthesis has become the standard of care in Nigeria. Grey literature exists on the fate of implanted hardware in the world's most populous Black nation. Therefore, this study aimed to determine the rate and indications for hardware removal following open reduction and internal fixation (ORIF) of facial fractures.
MATERIALS AND METHODS: A 10-year (2013-2022) retrospective review of patients' charts at a tertiary hospital in Nigeria was undertaken. Demographics, location and duration of implants in situ and indications for removal were retrieved and

BACKGROUND: Refracture after implant removal in clavicle fractures is a relevant but insufficiently studied complication. Reported refracture rates vary due to small cohorts and heterogeneous populations. This study aims to determine the incidence of refracture following elective implant removal after confirmed fracture union and to identify potential demographic, clinical, and fracture-related risk factors.
METHODS: A retrospective cohort of 575 adults who underwent implant removal after radiographically confirmed union between 2011 and 2024 was analyzed. Patients were assigned to a refractu

BACKGROUND: Separated instruments in the apical third of the infected root canals can lead to persistent periapical lesions. Retrieval requires appropriate preoperative assessment and a skilled technique, especially in cases of separated instruments in the apical third of curved root canals. Otherwise, retrieval will lead to removal failure and concomitant complications.
CASE PRESENTATION: Herein, we report two cases of separated instrument retrieval failure with complications due to inadequate preoperative assessment and an incorrect technical approach. In these cases, we re-evaluated the fe

BACKGROUND: Calcaneal osteotomy has been used to successfully treat both valgus and varus hindfoot deformities. Pain associated with implanted hardware may lead to further surgical intervention for hardware removal. Headless screws have been used to reduce postoperative hardware-associated pain and accompanying hardware removal, but data proving their effectiveness in this regard is lacking. The purpose of this study is to compare the rates of removal of headed and headless screws utilized in calcaneal osteotomy.
METHODS: We conducted a retrospective chart review of 74 patients who underwent

BACKGROUND: Persistent pain in the region of implanted hardware following fracture fixation commonly leads to implant removal. This prospective study evaluated patient outcomes and pain reduction following removal of orthopaedic hardware implanted for fracture fixation.
METHODS: Sixty patients who had been treated previously for a fracture and complained of pain in the region of the fracture fixation hardware constituted the study cohort. Patients were carefully examined by the treating physician to rule out other causes of pain such as infection and nonunion. Baseline data were recorded preo

This systematic review and meta-analysis evaluated the effectiveness of resorbable versus titanium plating systems in the management of paediatric maxillofacial fractures. A comprehensive search of PubMed, Embase, Scopus, and specialty journals up to December 2024 identified six retrospective studies published between 2010 and 2024, involving 1,584 patients aged ≤18 years. Both resorbable and titanium systems provided satisfactory fracture stability and healing. Titanium fixation was consistently associated with higher complication rates, ranging from 8%-12% whereas resorbable systems demonstr

BACKGROUND: Most olecranon fractures are intra-articular, affecting the extensor mechanism of the elbow, and are treated surgically with dorsal plate fixation or tension band. Due to shortcomings of dorsal plates related to prominence, insufficient fixation of sagittal fracture lines, and difficulty matching proximal ulna dorsal angulation (PUDA), dual medial and lateral plating (DP) has been developed. We hypothesized that olecranon fractures treated with DP would have low complication rates and low incidence of hardware removal compared with those treated with traditional methods of fixation

BACKGROUND: Early definitive fixation of clavicular fractures is rising in popularity when compared with conservative management. Despite this, the relative risk of subsequent hardware removal or revision surgery is relatively undocumented in the literature. The aim of this study was to review all clavicle fractures treated operatively in a single tertiary referral trauma unit and determine the true incidence of hardware removal and revision rates among this cohort.
METHODS: A retrospective electronic review was performed in a single tertiary trauma unit for all open reduction internal fixati

Fractured instrument retrieval remains one of the most challenging aspects of endodontics, with reported incidence rates ranging from 0.3% to 12%. This complication is particularly common with nickel‒titanium (NiTi) rotary files used in curved canals. Instrument separation hinders effective canal disinfection, compromises the quality of obturation, and may necessitate additional interventions such as bypassing, retrieval, or surgical management. Conventional retrieval techniques often lead to dentin loss, compromising root structure. The use of dental operating microscopes and their integratio
