Acute vestibular syndrome: clinical head impulse test versus video head impulse test.

HINTS battery involving head impulse test (HIT), nystagmus, and test of skew is the critical bedside examination to differentiate acute unilateral peripheral vestibulopathy from posterior circulation stroke (PCS) in acute vestibular syndrome (AVS). The highest sensitivity component of the battery has been reported to be the horizontal HIT, whereas skew deviation is defined as the most specific but non-sensitive sign for PCS. Video-oculography-based HIT (vHIT) may have an additional power in making the differentiation. If vHIT is undertaken, then both gain and gain asymmetry should be taken int

Treatment of Radial Head Fractures with a Modular Metallic Radial Head Replacement.

INTRODUCTION: Radial head arthroplasty with a smooth-stemmed metallic modular implant is a reliable treatment option for patients with acute unreconstructible radial head fractures, and good clinical outcomes may be expected beyond 5 years of follow-up (Video 1). STEP 1 PREOPERATIVE PLANNING: Obtain a careful history and perform a physical examination along with appropriate imaging to facilitate appropriate treatment decisions. STEP 2 OPERATING ROOM SETUP AND PATIENT POSITIONING: Perform proper operating room setup and patient positioning, as they are required to gain access to all affected

Radial head button holing: a cause of irreducible anterior radial head dislocation.

"Buttonholing" of the radial head through the anterior joint capsule is a known cause of irreducible anterior radial head dislocation associated with Monteggia injuries in pediatric patients. To the best of our knowledge, no report has described an injury consisting of buttonholing of the radial head through the annular ligament and a simultaneous radial head fracture in an adolescent. In the present case, the radiographic findings were a radial head fracture with anterior dislocation and lack of the anterior fat pad sign. Magnetic resonance imaging (MRI) clearly demonstrated anterior dislocat

Head drop syndrome secondary to multimodality treatments for head and neck cancer.

Five patients under follow-up for head and neck squamous cell carcinoma (SCC) at Southern General Hospital Glasgow presented with a fixed flexion deformity of the neck. These cases are characterized by the onset of severe weakness and atrophy of the neck extensor muscles and anterior fibrosis forcing the mandible to the chest wall. This causes considerable morbidity with communication, feeding, and appearance. This is a rare complication with no curative treatments that has not been reported previously by a UK center and is likely to become important with the increased use of multimodality tre

[Results after Judet radial head prosthesis for non-reconstructable radial head fractures].

BACKGROUND: With regard to the treatment of non-reconstructable radial head fractures, both the resection and the implantation of a prosthesis are considered. Various studies have shown poor results concerning the resection of the radial head with accompanying osteoligamentous injuries. Due to these experiences, different types of prosthesis have been developed. However, the majority of them were not convincing. Judet developed a type of a bipolar prosthesis which had been modeled on the anatomy of the radial head. The aim of our retrospective study consists in examining the results of the Jud

Dizziness from whiplash and head injury: differences between whiplash and head injury.

OBJECTIVE: Large discrepancies exist in the literature regarding incidence and types of symptomatology in whiplash. This is because of the evolution of whiplash injury over the years with the advent of head rests and seat belts. Previous authors have regarded symptoms of dizziness as a result of brainstem or cerebellar injury or both. It has been difficult in those studies to ascribe a mechanism of injury, as patients with whiplash injury only have been grouped with those who have incurred mild traumatic brain injury as a result of a significant blow to the head. The authors saw the need to de

[Femoral head dysplasia and its relationship to femoral head necrosis (Perthes' disease)].

Fifty-five patients totaling 98 hips afflicted by Dysplasia Capitis Femoris (DFC), monitored between 1960 and 1983, were analyzed. Mean follow-up: 8 years and 4 months (minimum 2 years, maximum 22 years and 2 months). Forty-three (78%) patients were males; fourteen (25%) were females. Non displayed any alteration in other osteoarticular segments. In 24 of the 98 affected hips it was possible to determine the exact radiological age appearance of the Epiphyseal Nucleus. Thirty-seven of the 55 patients had moderate signs and symptoms which were of short evolution. According to presence or absence

Horse riding and head injury: admissions to a regional head injury unit.

A retrospective survey has been carried out of 59 patients who suffered head injury related to horse riding out of 7172 cases of head and spinal injury admitted to a regional head injury unit in the 5-year period 1980-85. Most (85%) were female, against the usual 80% male predominance of head injury, 56 were minor injuries and 3 severe, of whom 2 died. Skull fracture was present in 10 patients (17%) of whom at least 5 had been wearing headgear at impact, and scalp trauma was noted in 22 (37%) with a predominance of occipital injuries. All the severely injured cases had an occipital skull fract

Exploring Sex-Based Variations in Head Kinematics During Soccer Heading.

While studies indicate that females experience a higher concussion risk and more severe outcomes in soccer heading compared to males, comprehensive data on the underlying factors contributing to these sex-based differences are lacking. This study investigates the sex differences in the head-to-ball impact kinematics among college-aged soccer headers in a laboratory-controlled setting. Forty subjects (20 females, 20 males) performed ten headers, and impact kinematics, including peak angular acceleration and velocity (PAA, PAV) and peak linear acceleration (PLA), were measured using mouthguards.

Sword-Like Trauma to the Shoulder with Open Head-Splitting Fracture of the Head.

Head-splitting fractures occur as a result of violent compression of the head against the glenoid; the head splits and the tuberosities may remain attached to the fragments or split and separate. Isolated humeral head-splitting fractures are rare injuries. Favorable results with osteosynthesis can be difficult to achieve because of the very proximal location of the head fracture and associated poor vascularity. We present a case of a 67-year-old man who sustained a severe, sword-like trauma to his left shoulder after a road traffic accident with associated isolated open Gustilo-Anderson IIIA h

Neck strength imbalance correlates with increased head acceleration in soccer heading.

BACKGROUND: Soccer heading is using the head to directly contact the ball, often to advance the ball down the field or score. It is a skill fundamental to the game, yet it has come under scrutiny. Repeated subclinical effects of heading may compound over time, resulting in neurologic deficits. Greater head accelerations are linked to brain injury. Developing an understanding of how the neck muscles help stabilize and reduce head acceleration during impact may help prevent brain injury. HYPOTHESIS: Neck strength imbalance correlates to increasing head acceleration during impact while heading a

[A brief history of treatment for radial head fracture with radial head prosthesis.].

Since Speed first treated the radial head fracture with a metal prosthesis in 1941, the purpose of the treatment of radial head fracture with a radial head changed from the prevention of heterotopic ossification to the prevention of proximal migration of the radius and instability of the elbow. The optimal indication is gradually determined as a non-reconstructable radial head fracture with associated injuries that would leave the elbow unstable if the radial head were resected. Types of prosthesis changed from mono-block, bipolar to modular prosthesis. Material of prosthesis changed from acry

[A head-to-head comparative study for evaluation of effectiveness among drugs for osteoporosis].

Recent progress in osteoporosis treatment has realized prevention of osteoporotic fractures. However, there is no guideline regarding which agent should be used in an individual patient. In this report, we investigated the effectiveness of several drugs for osteoporosis in a head-to-head comparative study. A total of 776 osteoporotic patients were treated with alendronate (ALN, n=197), etidronate (EHDP, n=199), alfacalcidol (VD3, n=221) and vitamin K2 (K2, n=159). The control group (No, n=266) was followed without treatment. We followed their lumbar bone mineral density (LBMD), bone turnover m

Where are We Headed? Evidence to Inform Future Football Heading Guidelines.

Given the scientific and public concern regarding the short-, medium- and long-term consequences of heading on brain health, being proactive about developing and implementing guidelines that help reduce the burden (volume, impact magnitude and injury risk) of heading in young and beginner players appears justified. This narrative review explores the evidence underpinning strategies that could be incorporated into future heading guidelines to reduce heading burden in players across all levels of football. A four-step search strategy was utilised to identify all data-based papers related to head

Diagnostic value of amyloid-PET and tau-PET: a head-to-head comparison.

PURPOSE: Assess the individual and combined diagnostic value of amyloid-PET and tau-PET in a memory clinic population. METHODS: Clinical reports of 136 patients were randomly assigned to two diagnostic pathways: AMY-TAU, amyloid-PET is presented before tau-PET; and TAU-AMY, tau-PET is presented before amyloid-PET. Two neurologists independently assessed all reports with a balanced randomized design, and expressed etiological diagnosis and diagnostic confidence (50-100%) three times: (i) at baseline based on the routine diagnostic workup, (ii) after the first exam (amyloid-PET for the AMY-TAU

Addressing head injury risk in youth football: are heading guidelines the answer?

BACKGROUND: Recent findings of neurodegenerative pathology in former professional football players have once again called into question the role that "heading", a fundamental aspect of the game, plays in the onset of neurological disease. By introducing guidelines aimed at limiting heading among youth players, the United Kingdom recently joined the United States as the only two nations yet to implement heading regulation in response to growing concerns surrounding football's head injury burden. PURPOSE: Evaluating the efficacy of risk mitigation strategies requires the continual reviewal of a

Head to head: should the intraductal component of invasive prostate cancer be graded?

The reporting of intraductal carcinoma of the prostate (IDCP) is controversial, with conflicting recommendations having recently been published by the International Society of Urological Pathology (ISUP) and the Genitourinary Pathology Society (GUPS). Both recommend that isolated (pure) IDCP should not be graded. However, the ISUP recommends incorporating the IDCP component of invasive prostate cancer in the Gleason score, whereas the GUPS recommends reporting IDCP as a comment, independently of the Gleason score. The arguments for and against incorporating the IDCP component of invasive prost

[Problems and thoughts of preserving femoral head treatment of osteonecrosis of femoral head].

By literature review and experience summary, some problems are found in conservative treatment of osteonecrosis of femoral head(ONFH): lacking in researches of natural history of ONFH, disunion in classfication and the standard of outcome evaluation, lacking in high-level-evidence study and standard of treatment methods. It is necessary to improve the study on natural history of ONFH, unify the classfications and the standard of outcome evaluations, normalize the treatment methods, and design impeccable multi-centre study for improving the effect of conservative treatment of ONFH.
