Electronic charts do not facilitate the recognition of patient hazards by advanced medical students: A randomized controlled study.

Chart review is an important tool to identify patient hazards. Most advanced medical students perform poorly during chart review but can learn how to identify patient hazards context-independently. Many hospitals have implemented electronic health records, which enhance patient safety but also pose challenges. We investigated whether electronic charts impair advanced medical students' recognition of patient hazards compared with traditional paper charts. Fifth-year medical students were randomized into two equal groups. Both groups attended a lecture on patient hazards and a training session o

[Encryption technique for linkable anonymizing].

Linkage of different records such as health insurance claims or medical records for the purpose of cohort studies or cancer registration usually requires matching with personal names and other personally identifiable data. The present study was conducted to examine the possibility of performing such privacy-sensitive procedures in a "linkable anonymizing" manner using encryption. While bidirectional communication entails encryption and deciphering, necessitating both senders and receivers sharing a common secret "key", record linkage entails only encryption and not deciphering because research

[Inadequacy of legal norms of evaluation the percentage of detriment to health--a problem of appropriate assessment].

Analyses of expert opinions issued in case of long-term and permanent detriment to health indicate an anachronistic nature of solutions offered by legal assessment systems. The fundamental problem the authors observed is the assessment of detriment percentage that is divorced from reality and oversimplifies this problem, especially in relation to severe traumas, where multiple injures cause health damage exceeding 100%. Our observations indicate that such a situation is largely determined by progress in medicine. Nowadays, many individuals, who would have died immediately after the accident tw

Mining 100 million notes to find homelessness and adverse childhood experiences: 2 case studies of rare and severe social determinants of health in electronic health records.

OBJECTIVE: Understanding how to identify the social determinants of health from electronic health records (EHRs) could provide important insights to understand health or disease outcomes. We developed a methodology to capture 2 rare and severe social determinants of health, homelessness and adverse childhood experiences (ACEs), from a large EHR repository. MATERIALS AND METHODS: We first constructed lexicons to capture homelessness and ACE phenotypic profiles. We employed word2vec and lexical associations to mine homelessness-related words. Next, using relevance feedback, we refined the 2 pro

The predictive value of ICD-10 diagnostic coding used to assess Charlson comorbidity index conditions in the population-based Danish National Registry of Patients.

BACKGROUND: The Charlson comorbidity index is often used to control for confounding in research based on medical databases. There are few studies of the accuracy of the codes obtained from these databases. We examined the positive predictive value (PPV) of the ICD-10 diagnostic coding in the Danish National Registry of Patients (NRP) for the 19 Charlson conditions. METHODS: Among all hospitalizations in Northern Denmark between 1 January 1998 and 31 December 2007 with a first-listed diagnosis of a Charlson condition in the NRP, we selected 50 hospital contacts for each condition. We reviewed

Associations between childhood maltreatment and psychiatric disorders: analysis from electronic health records in Hong Kong.

There has been a lack of high-quality evidence concerning the association between childhood maltreatment and psychiatric diagnoses particularly for Axis II disorders. This study aimed to examine the association between childhood maltreatment exposure and Axis I and Axis II psychiatry disorders using electronic health records. In this study, the exposed group (n = 7473) comprised patients aged 0 to 19 years with a first-time record of maltreatment episode between January 1, 2001 and December 31, 2010, whereas the unexposed group (n = 26,834) comprised individuals of the same gender and age who

Extracting Concepts Related to Homelessness from the Free Text of VA Electronic Medical Records.

Mining the free text of electronic medical records (EMR) using natural language processing (NLP) is an effective method of extracting information not always captured in administrative data. We sought to determine if concepts related to homelessness, a non-medical condition, were amenable to extraction from the EMR of Veterans Affairs (VA) medical records. As there were no off-the-shelf products, a lexicon of terms related to homelessness was created. A corpus of free text documents from outpatient encounters was reviewed to create the reference standard for NLP training and testing. V3NLP Fram

Assessing methods to identify Veterans Health Administration populations with Veterans Justice Programs contact to improve health research.

Military veterans with criminal legal involvement have a need for healthcare and housing to attenuate their risks of suicide, overdose, and homelessness. Links between criminal legal involvement and Veterans Health Administration (VHA) care are poorly understood, partly because it is challenging to correctly identify veterans with criminal legal involvement. This study's primary objective was to assess VHA data sources and codes from the electronic health record that indicate contact with Veterans Justice Programs (VJP) staff - a broad proxy for criminal legal involvement. We examined three da

Self-report versus medical records for assessing cancer-preventive services delivery.

Accurate measurement of cancer-preventive behaviors is important for quality improvement, research studies, and public health surveillance. Findings differ, however, depending on whether patient self-report or medical records are used as the data source. We evaluated concordance between patient self-report and medical records on risk factors, cancer screening, and behavioral counseling among primary care patients. Data from patient surveys and medical records were compared from 742 patients in 25 New Jersey primary care practices participating at baseline in SCOPE (supporting colorectal cancer

Patient at Risk: Emergency medical service providers' opinions on improving an electronic emergency information form database for the medical care of children with special health care needs.

BACKGROUND: Patient at Risk (PAR) is an online database using an emergency information form (EIF) that parents of children with special health care needs (CSHCN) complete to store their child's medical information. Emergency medical service (EMS) providers can then access PAR during medical emergencies or for pre-planning. PAR was active for over 10 years but use by patients and EMS was limited. OBJECTIVE: To determine EMS perspectives on why PAR was not used to its full potential and how to improve PAR for EMS. METHODS: Seven questions regarding PAR were developed and presented in focus gro

Exploring reasons and motivations for suicide attempts in prison before and during the SARS-CoV-2 pandemic.

BACKGROUND: Suicide attempts represent a critical public health concern in prison settings, where rates are substantially higher than in the general population. The COVID-19 pandemic introduced additional stressors, yet little is known about its impact on suicide attempts among detained persons. This study aimed to identify the underlying reasons and motivations for suicide attempts in a Swiss pre-trial prison and to examine changes before and during the pandemic. We analyzed 205 suicide attempts by 125 detained persons between 2016 and 2021. Data were collected from clinical and prison record

Feasibility study of multi-site split learning for privacy-preserving medical systems under data imbalance constraints in COVID-19, X-ray, and cholesterol dataset.

It seems as though progressively more people are in the race to upload content, data, and information online; and hospitals haven't neglected this trend either. Hospitals are now at the forefront for multi-site medical data sharing to provide ground-breaking advancements in the way health records are shared and patients are diagnosed. Sharing of medical data is essential in modern medical research. Yet, as with all data sharing technology, the challenge is to balance improved treatment with protecting patient's personal information. This paper provides a novel split learning algorithm coined t

Methods and rationale for keeping records of hepatitis virus testing in Japanese workplaces.

In Japan, the Ministry of Health, Labour and Welfare issued corporate guidance that employers should recommend their employees to have tests for hepatitis B virus and hepatitis C virus (HV) at least once in their lifetime. However, employers should treat this information as confidential, even though the testing is carried out along with the health examination designated by the Industrial Safety and Health Law. Therefore, the records of HV tests should not be kept by employers, even though records of medical examinations designated by the law must be. This study aimed to clarify the present met

Medical photography with a mobile phone: useful techniques, and what neurosurgeons need to know about HIPAA compliance.

Medical photographs are commonly employed to enhance education, research, and patient care throughout the neurosurgical discipline. Current mobile phone camera technology enables surgeons to quickly capture, document, and share a patient scenario with colleagues. Research demonstrates that patients generally view clinical photography favorably, and the practice has become an integral part of healthcare. Neurosurgeons in satellite locations often rely on residents to send photographs of diagnostic imaging studies, neurological examination findings, and postoperative wounds. Images are also freq

Using Health Information Technology to Foster Engagement: Patients' Experiences with an Active Patient Health Record.

Personal health records (PHRs) typically employ "passive" communication strategies, such as non-personalized medical text, rather than direct patient engagement in care. Currently there is a call for more active PHRs that directly engage patients in an effort to improve their health by offering elements such as personalized medical information, health coaches, and secure messaging with primary care providers. As part of a randomized clinical trial comparing "passive" with "active" PHRs, we explore patients' experiences with using an "active" PHR known as HealthTrak. The "passive" elements of t

Possibilities and Implications of Using the ICF and Other Vocabulary Standards in Electronic Health Records.

There is now widespread recognition of the powerful potential of electronic health record (EHR) systems to improve the health-care delivery system. The benefits of EHRs grow even larger when the health data within their purview are seamlessly shared, aggregated and processed across different providers, settings and institutions. Yet, the plethora of idiosyncratic conventions for identifying the same clinical content in different information systems is a fundamental barrier to fully leveraging the potential of EHRs. Only by adopting vocabulary standards that provide the lingua franca across the
