Development of a new outcomes instrument for conductive hearing loss.

OBJECTIVE: The objective was to design and validate a disease-specific outcomes instrument for use in conductive hearing loss (CHL). STUDY DESIGN: The study was a retrospective survey of 47 patients recently treated for CHL with either a hearing aid or surgery. Patients were tested with the newly designed instrument (the Hearing Satisfaction Scale [HSS]), previously validated hearing-specific instruments, and a generic quality-of-life instrument. SETTING: The study was performed in an academic tertiary referral center. MAIN OUTCOME MEASURES: These included test-retest reliability, internal

[Some remarks concerning the treatment of the conductive hearing loss].

The author points out a decrease tendency in using middle ear reconstruction surgery due to the development and the accessibility of hearing aids. Such an attitude inevitably leads to ordering hearing aids especially air conductive ones in cases which ought to be operated. The author thus wants to underline the fact that bone conductive hearing aids are very rarely recommended, even though the should be used more often. He also recommends a precise analysis of one's unsuccessful surgical results as a method of gaining skills and proficiency. The author also points out the fact that one should

Conductive hearing loss in autistic, learning-disabled, and normal children.

Katz (1978) has suggested that mild, fluctuating conductive hearing loss due to middle-ear anomalies may account for the language and attention problems of learning-disabled children. His position was extended here to include autism. Normal, learning-disabled, and autistic children received repeated impedance measures over 5 weeks. A repeated-measures ANOVA of central tendency and variability values led to the conclusions that (1) fluctuating, negative middle-ear pressure greater than normal characterizes both autistic and learning-disabled children, (2) the negative pressure is greater in aut

[The diagnosis of conductive hearing loss by tympanometry (author's transl)].

The use of electronic impedance measuring instruments to determine the acoustic impedance of the ear drum is very helpful in the diagnosis of middle ear diseases. Fluid filled middle ear spaces, otosclerosis and tympanosclerosis can be differentiated easily. In addition tube and facial nerve testing is possible. All tests can be done in less than 10 minutes for both ears.

Middle ear choristoma presenting as cholesteatoma with conductive hearing loss.

A 6-year-old male was seen for evaluation of middle ear polyp with associated hearing loss and otorrhea. CT scan revealed canal polyp with a stalk extending to the middle ear with malformation of the malleus and incus. The patient underwent exploratory tympanotomy which revealed a fistula from the bony-cartilaginous junction connecting to a middle ear mass which had eroded the incus. Intraoperative pathology suggested columnar epithelium without cholesteatoma or muscle. Final pathologic diagnosis reported dense fibrous tissue as well as ectopic salivary gland tissue, consistent with salivary g

SIX2 haploinsufficiency causes conductive hearing loss with ptosis in humans.

The ossicles represent one of the most fundamental morphological features in evolutionary biology of the mammalians. The mobile ossicular morphology abnormalities result in the severe conductive hearing loss. Development and patterning of the middle ear malformation depend on genetic and environmental causes. However, the genetic basis for the risk of congenital ossicle malformation is poorly understood. We show here nine affected individuals in a Chinese pedigree who had bilateral conductive hearing loss with ptosis. We performed whole-genome sequencing and array comparative genomic hybridiza

Bilateral middle cranial fossa encephaloceles presenting as conductive hearing loss.

We report a case involving a patient with bilateral middle cranial fossa encephaloceles extending into the middle ear and causing conductive hearing loss. An obese, 47-year-old woman with a history of a seizure disorder presented with a slow-onset subjective hearing loss. Examination revealed opaque tympanic membranes, and audiometry showed a mixed hearing loss bilaterally. Myringotomy demonstrated soft tissue behind each tympanic membrane. Biopsy, computed tomography, magnetic resonance imaging, and mastoidectomy confirmed the diagnosis of bilateral middle cranial fossa encephaloceles. Bilate

Another cause for conductive hearing loss with present acoustic reflexes.

There are numerous potential causes of conductive hearing loss (HL). It is important to obtain a thorough history and perform a complete examination, including audiometric testing and radiographic evaluation when necessary. In this report, we present a patient with an intact tympanic membrane, no history of ear disease or trauma who as an adult developed progressive, conductive HL because of an anomalous course of a dehiscent facial nerve. In the patient with a conductive HL and at least partially intact reflexes, superior semicircular canal dehiscence, fracture of the stapes superstructure pr

Histopathology of residual and recurrent conductive hearing loss after stapedectomy.

HYPOTHESIS: Histopathologic examination of temporal bones from patients who had undergone stapedectomy may provide information concerning the causes of both residual and recurrent conductive hearing loss (CHL). BACKGROUND: Although closure of the air-bone gap to within 10 dB occurs in approximately 90% of primary stapedectomies, a residual CHL occurs in approximately 10% and recurrent CHL may occur in up to 35% of cases. Putative causes of failure of surgery as determined during revision include erosion of the incus, bony regrowth at the oval window, and displacement of the prosthesis. Most r

Effect of conductive hearing loss on the vestibulo-collic reflex.

The vestibulo-collic reflex represents a promising test for evaluating the integrity of otolith function. We have investigated the threshold of this response in a group of normal subjects, and the effect of a conductive hearing loss. A positive response was recorded in 31 of 32 normal subjects. The threshold of the vestibulo-collic reflex varied from 80 to 97 dBHL in these subjects with a 95% response rate at a threshold at 96 dBHL. A total of 23 ears with a conductive hearing loss in 17 patients were also investigated. The average conductive hearing loss (at 0.5, 1, 2 and 4 kHz) ranged from 8

Treatment of conductive hearing loss with ossicular chain reconstruction procedures.

Middle ear infections and ossicular disorders can cause conductive hearing loss. Ossicular disorders include fibrous unions in which scar tissue replaces the long process of the incus; disarticulations caused by previous surgical procedures or cholesteatomas; fixation from otosclerosis, cholesteatomas, or congenital malformations; and dislocations caused by trauma. Surgical reconstruction procedures, with the insertion of partial or total ossicular replacement prostheses, help restore conductive hearing. Patients who undergo these delicate procedures have significant hearing deficits and requi

The effect of rapid maxillary expansion on conductive hearing loss.

The effects of rapid maxillary expansion (RME) on conductive hearing loss were investigated in 14 subjects (11 females and 3 males). The subjects ranged in age from 10 years 4 months to 16 years 9 months (mean age 12 years 11 months +/- 1 year 9 months) and had narrow maxillary arches and conductive hearing loss. Hearing levels were determined by means of pure-tone audiometric records. Three records were taken for each subject. The first was taken before RME, the second after sufficient midpalatal suture opening was achieved (mean = 15 days), and the third after the retention period (mean = 4.

Mixed conductive and sensorineural hearing loss in LP/J mice.

Air and bone conduction thresholds for the detection of a compound action potential response were measured in mice of the LP/J inbred strain, which has been proposed as a possible model for human otosclerosis. Thresholds were compared with control data from CBA/Ca mice. Evidence of a mixed sensorineural and conductive hearing loss was obtained in LP/J mice. Few signs of hair cell degeneration or middle-ear bony lesions were found in 20-day old mice, although they generally showed raised thresholds to both air and bone conducted stimuli. The reason for the observed sensorineural component of th

The possible effects of conductive hearing loss on language development.

Recently some attention has been paid to the possibility that chronic serous otitis could adversely affect language development. Up to ten or fifteen years ago the hearing loss resulting from the disease was considered neglectable. In this contribution some arguments will be given as to prove that the latter might not always be true. Follows a review of some recent studies suggesting a causal relationship between secretory otitis and language development and scholastic learning. The need for further (longitudinal) research is emphasised.

A case of Beckwith-Wiedemann syndrome with conductive hearing loss.

A case of Beckwith-Wiedemann syndrome with conductive hearing loss due to fixation of the stapes is described. This is the second case described in the literature. However, some patients may not have been diagnosed. We suggest that hearing of patients with BWS should be tested at intervals from early childhood.
