Heart-Brain Axis: Effects of Neurologic Injury on Cardiovascular Function.

A complex interaction exists between the nervous and cardiovascular systems. A large network of cortical and subcortical brain regions control cardiovascular function via the sympathetic and parasympathetic outflow. A dysfunction in one system may lead to changes in the function of the other. The effects of cardiovascular disease on the nervous system have been widely studied; however, our understanding of the effects of neurological disorders on the cardiovascular system has only expanded in the past 2 decades. Various pathologies of the nervous system can lead to a wide range of alterations

A Retained Bullet in Pericardial Sac: Penetrating Gunshot Injury of the Heart.

Penetrating cardiac trauma is rarely seen but when present there is a short time lag to keep the patients alive. Cardiac gunshot injuries are exceptional and it occurs mostly during interpersonal disagreements casualties or a mistakenly fired gun nowadays. Here we present a case of cardiac gunshot injury from the war of Kobani, Syria. The patient was mistakenly diagnosed to have a sole bullet in the left shoulder while he had a penetrating cardiac trauma with a bullet in the heart and pericardial effusion possibly giving rise to pericardial tamponade. Luckily the cardiac gunshot injury was not

Fluid overload as a biomarker of heart failure and acute kidney injury.

BACKGROUND/AIMS: Acute heart failure (HF) and acute kidney injury (AKI) are common. These syndromes are each associated with considerable morbidity, mortality, and health resource utilization and are increasingly encountered. Fluid accumulation and overload are common themes in the pathophysiology and clinical course of both HF and AKI. METHODS: This narrative literature review provides an overview of the pathophysiology of fluid accumulation with a focus on HF and AKI, along with a discussion of the importance of assessment of fluid balance in these syndromes and how it correlates with clini

Tolerance of the developing cyanotic heart to ischemia-reperfusion injury in the rat.

OBJECTIVE: Whether chronic hypoxia attenuates myocardial ischemia-reperfusion injury remains controversial because conflicting data have been reported probably due to the existence of many factors influencing the functional recovery of hearts. These factors include the differences of species, the time at which hypoxia begins, the degree of hypoxia, and so on. Regarding chronic hypoxia from birth, so far the only available data are based on findings in rabbit hearts. The purpose of this study was to describe the effect of chronic hypoxia from birth on myocardial reperfusion injury in the rat he

Is oxidative stress primarily involved in reperfusion injury of the ischemic heart?

Reperfusion injury of ischemic organs is suggested to result from metabolic derangements initiating an imbalanced formation of free oxygen radicals. Most investigators in this field have used the spin-trap 5,5'-dimethyl-N-pyrroline-N-oxide (DMPO) to stabilize these short-lived radicals and make them visible by means of the electron spin resonance (ESR) technique. ESR signals obtained from intravascular DMPO were reported to indicate the formation of free OH. radicals and, in some cases, also carbon-centered radicals. We were unable to confirm these findings. Carbon-centered radicals were not o

Role of granulocytes in endothelial injury in coronary heart disease in humans.

Recent studies suggest that granulocytes (PMNs) play a role in the pathogenesis of acute and chronic myocardial ischemia and extension of myocardial injury. A positive correlation was also found between leukocyte count and severity of coronary artery disease. Rabbit derived antiserum dependent-reduction of circulating PMNs in the dog or using monoclonal antibody anti-CD11b/CD18 of PMNs resulted in smaller myocardial infarcts. Granulocytes can release a variety of mediators tissue injury and synergize with these different mediators and other cells resulting in amplification of neutrophil stimul

Remote skin injury: a protean complication of open heart surgery.

In the 12-month period ending August, 1984, 14 adult patients (2.9%) developed within 24 hr following open heart surgery skin defects of varying magnitude remote from the incision site. Electrical injury secondary to a grounding defect was documented in two patients and suspected in one. Four patients' injuries were caused by the roller device when transfering from operating room table to intensive care unit bed. The remaining eight patients all had skin loss at sites of pressure on either the head, back, buttocks, or arm. Retrospective analysis showed no correlation with mean pressure during

Contribution of tissue acidosis to ischemic injury in the perfused rat heart.

The isolated perfused working rat heart preparation has been used to study the effects of respiratory acidosis on myocardial metabolism and contractilly. Hearts were perfused with 5 mM glucose and 10(-2) U/ml of insulin in order to enhance metabolsim of glucose relative to that of fatty acids. After perfusion with Krebs bicarbonate medium at pH 6.6, hearts rapidly ceased performing external work and peak left ventricular pressure fell by 75% after 5 minutes. Oxygen consumption, rate of ATP generation and overall glycolytic flux also declined rapidly. After about 2 minutes of perfusion, the fal

Traumatic brain injury and risk of heart failure and coronary heart disease: A nationwide population-based cohort study.

BACKGROUND: This study examined the long-term risks of heart failure (HF) and coronary heart disease (CHD) following traumatic brain injury (TBI), focusing on gender differences. METHODS: Data from Taiwan's National Health Insurance Research Database included 29,570 TBI patients and 118,280 matched controls based on propensity scores. RESULTS: The TBI cohort had higher incidences of CHD and HF (9.76 vs. 9.07 per 1000 person-years; 4.40 vs. 3.88 per 1000 person-years). Adjusted analyses showed a significantly higher risk of HF in the TBI group (adjusted hazard ratio = 1.08, 95% CI = 1.01-1.17

Decreased recipient survival following orthotopic heart transplantation with use of hearts from donors with projectile brain injury.

BACKGROUND: Fatal gunshot injury to the brain can cause significant alterations in the neuroendocrine state and myocardial dysfunction. Therefore heart allografts from these donors may result in graft failure following orthotopic heart transplantation (OHTx). We evaluated whether receiving a heart from a donor who died from fatal gunshot wound to the brain independently affected the outcome of transplantation. METHODS: A retrospective review of 113 consecutive patients undergoing OHTx at a university hospital from 1996 to 2002 was performed. Group 1 received hearts from donors with fatal gun

Shot through the heart: A 17-year analysis of pre-hospital and hospital deaths from penetrating cardiac injuries.

BACKGROUND: Penetrating cardiac injuries (PCI) are often fatal despite rapid transport and treatment in the prehospital setting. Although many studies have identified risk factors for mortality, few studies have included non-transported field mortalities. This study analyzes penetrating cardiac injuries including hospital and coroner reports in the current era. METHODS: Seventeen years of data were reviewed, including the trauma center (TC) registry, medical records, and coroner reports from 2000-2016. PCI were graded using American Association for the Surgery of Trauma (AAST) cardiac organ i

Endothelin System Blockade Extenuates Sepsis-Induced Acute Heart and Kidney Injuries via Modulating ET-1/Klotho/p38-MAPK.

Sepsis-induced organ failure is a major health problem, characterised by massive inflammatory and oxidative stress responses. Endothelin-1 (ET-1) is one of the peptides expressed during septicemia with proapoptotic, proinflammatory, and oxidant effects. ET-1 plays a role in heart and kidney injuries in sepsis. Accordingly, the current study was conducted to investigate, on a mechanistic basis, whether inhibition of ET-1 signalling either by blocking its receptors or inhibiting its formation attenuates sepsis-induced acute cardiorenal injuries. To analyse the role of ET-1 in sepsis, we used a c

Heart Rate, Electrocardiographic Subclinical Myocardial Injury, and Long-Term Mortality.

Objective: Although an elevated resting heart rate is linked to higher mortality, the pathways underlying this relationship are not fully defined. One candidate mechanism is subclinical myocardial injury (SCMI), an asymptomatic form of cardiac damage indexed by a Cardiac Infarction/Injury Score (CIIS) of at least 10, which has prognostic value for adverse cardiovascular (CV) outcomes. Methods: We analyzed 7152 participants from NHANES III who underwent electrocardiogram (ECG) recording and were free of CV disease. Heart rate was categorized as bradycardia (≤50 bpm), normal (>50-<100 bpm), or t

Immune checkpoint inhibition and heart injury: molecular mechanisms and clinical challenges.

Immune checkpoint inhibitors (ICIs), such as PD-1, PD-L1, and CTLA-4 inhibitors, enhance immune function by targeting T-cell surface receptors, overcoming natural immune inhibition. These drugs activate the immune system to detect and target cancer cells, providing an alternative therapeutic approach, particularly in cases where conventional treatments fail. However, the growing use of ICIs has highlighted several defence-related adverse events, particularly cardiac toxicity, including acute myocarditis, atherosclerosis, and heart failure unrelated to myocarditis. These complications present s

Post-traumatic pericarditis following penetrating injury to the heart: a case report.

Post-traumatic pericarditis is a subtype of post-cardiac injury syndrome (PCIS) that occurs after non-iatrogenic cardiac trauma. Post-traumatic pericarditis is rare and difficult to diagnose. We present a case of post-traumatic pericarditis progressing to transient constrictive pericarditis following a penetrating stab wound to both ventricles, illustrating the role of multimodal imaging in diagnosis and follow-up. A 22-year-old male presented with a two-day history of severe pleuritic chest pain, six weeks after surviving a penetrating stab injury to the left and right ventricles. Initial eva

Cannabidiol Protects the Neonatal Mouse Heart from Hyperoxia-Induced Injury.

Neonatal hyperoxia induces oxidative and inflammatory stress that disrupts cardiac maturation and contributes to long-term cardiovascular morbidity in individuals born preterm. Cannabidiol (CBD), a non-psychoactive phytocannabinoid with antioxidant and anti-inflammatory properties, has demonstrated protective effects in neonatal hyperoxic injury in other organs; however, its impact on the developing heart remains unclear. This study investigated whether CBD mitigates hyperoxia-induced cardiac injury in a neonatal mouse model. Newborn mice were exposed to 80% O2 for 48 h from postnatal day (P)5
