Ruptured Sinus of Valsalva Aneurysm Causing Acute Right Heart and Hepatorenal Failure Requiring Urgent Surgery.

A sinus of Valsalva aneurysm is a rare cardiac anomaly that may be congenital or acquired. Very rarely, they present with acute symptoms as a result of rupture into the cardiac chamber. If left untreated, they almost always result in deteriorating cardiac function. We report a patient presenting with acute right heart failure and hepatorenal syndrome requiring urgent surgery and double patch repair.

Cardiac rupture caused by penetration of the heart by fractured costal cartilage following blunt chest trauma: A case report.

BACKGROUND: Cardiac rupture is rare but has a high mortality rate. Cardiac rupture due to blunt trauma is generally caused by the heart being pinched into the bony thorax. CASE PRESENTATION: A 90-year-old man in shock was transported to the hospital after his precordium was stepped on by a cow. He developed right atrial rupture from penetration of the heart by fractured and calcified costal cartilage, with massive hemothorax, hemorrhagic shock, and respiratory failure. We immediately performed cardiac injury repair. The patient was discharged without sequelae. CONCLUSION: Emergency physician

Loss of Protease-Activated Receptor 4 Prevents Inflammation Resolution and Predisposes the Heart to Cardiac Rupture After Myocardial Infarction.

BACKGROUND: Cardiac rupture is a major lethal complication of acute myocardial infarction (MI). Despite significant advances in reperfusion strategies, mortality from cardiac rupture remains high. Studies suggest that cardiac rupture can be accelerated by thrombolytic therapy, but the relevance of this risk factor remains controversial. METHODS: We analyzed protease-activated receptor 4 (Par4) expression in mouse hearts with MI and investigated the effects of Par4 deletion on cardiac remodeling and function after MI by echocardiography, quantitative immunohistochemistry, and flow cytometry.

Biomechanical engineering analysis of an acute papillary muscle rupture disease model using an innovative 3D-printed left heart simulator.

OBJECTIVES: The severity of acute papillary muscle (PM) rupture varies according to the extent and site of the rupture. However, the haemodynamic effects of different rupture variations are still poorly understood. Using a novel ex vivo model, we sought to study acute PM rupture to improve clinical management. METHODS: Using porcine mitral valves (n = 32) mounted within an ex vivo left heart simulator, PM rupture was simulated. The mitral valve was divided into quadrants for analysis according to the PM heads. Acute PM rupture was simulated by incrementally cutting from 1/3 to the total numbe

Spontaneous rupture of right aortic sinus of Valsalva leading to massive cystic dissection of interventricular septum and complete heart block.

The interventricular septal dissection is an uncommon, and is occasionally associated with sinus of Valsalva aneurysm. A spontaneous dissection with a normal sinus of Valsalva is extremely rare and is mostly a result of a congenital defect. We report the case of a 23-year-old male admitted for complete heart block and presenting as an incidental finding during the TTE multiple and huge cystic-like mass in the interventricular septum (IVS) which is the TTE characteristic of IVS dissection. Most patients remain asymptomatic until the anatomical and hemodynamic changes lead to complications like:

Staged endovascular management of a ruptured common iliac artery aneurysm with iliocaval fistula and high-output heart failure.

Rupture of a common iliac artery aneurysm into the inferior vena cava is rare and can cause high-output heart failure. We report staged endovascular management in a 59-year-old man with a 70-mm right common iliac artery aneurysm and iliocaval fistula. Emergency embolization of the right internal iliac artery and endovascular repair rapidly improved heart failure (brain natriuretic peptide decreased from 1300 to 60 pg/mL at 1 week). Persistent iliolumbar artery-related type II endoleak with residual venous enhancement was treated by secondary transarterial coil embolization on postoperative day

Ventricular septal rupture complicating acute myocardial infarction: identification of simple and complex types in 53 autopsied hearts.

Fifty-three hearts with rupture of the ventricular septum complicating acute myocardial infarction (AMI) were studied. Thirty-three of the hearts were from men (average age 76 years) and 20 were from women (average age 73 years). The study showed 2 types of rupture of the ventricular septum: simple (28 patients) and complex (25 patients). Simple ruptures were direct through-and-through defects. Complex ruptures were associated with serpiginous dissection tracts remote from the primary site of tear of the ventricular septum. Specimens were classified as to the location of the underlying AMI and

When the Heart Holds Its Own Rupture: A Case Report of a Clinically Silent Left Ventricular Pseudoaneurysm Following Myocardial Infarction.

Left ventricular pseudoaneurysm is a rare but potentially fatal mechanical complication of acute myocardial infarction, often associated with a non-specific clinical presentation and a high risk of rupture. We report a case of a patient who developed a left ventricular pseudoaneurysm following an acute myocardial infarction that initially remained undiagnosed. The lesion was incidentally identified during follow-up imaging. Multimodal imaging confirmed the diagnosis and guided urgent surgical management, resulting in a favorable outcome. Left ventricular pseudoaneurysm remains a diagnostic cha

Combined Heart-Liver Transplant in a Patient With End-Stage Liver Disease and Contained Rupture of an Aortic Root Pseudoaneurysm.

A patient with end-stage liver disease, aortic stenosis, and rapid progression of an aortic root pseudoaneurysm underwent successful combined heart-liver transplantation. This patient was a 56-year-old woman with decompensated cirrhosis, severe aortic stenosis, and an aortic root pseudoaneurysm with streptococcal bacteremia and aortic valve endocarditis. The pseudoaneurysm rapidly progressed, with evidence of contained rupture. Given the operative risk posed by liver disease, the patient was evaluated and ultimately underwent heart-liver transplantation. This case highlights the novel use of c

Hypertrophic obstructive cardiomyopathy complicated by ruptured chordae tendinea to the posterior mitral leaflet and severe congestive heart failure.

A 63-year-old woman with hypertrophic obstructive cardiomyopathy developed rapidly progressive fatigue, shortness of breath and congestive heart failure. A transesophageal echocardiogram demonstrated ruptured chordae to the posterior mitral valve leaflet with severe mitral regurgitation. Mitral valve replacement eliminated the outflow gradient. Acute or subacute hemodynamic deterioration in a patient with hypertrophic obstructive cardiomyopathy should lead to a search for associated lesions.

[Associated septal and intrapericardial rupture of the heart in the acute phase of myocardial infarction. Apropos of a case surgically treated with success].

The authors report the case of a double rupture of the heart (septal and parietal intrapericardial) on the 5th day of primary transmural anterior myocardial infarction in a 73 year old women. The diagnosis of septal rupture was made after cardiac arrest and the findings of a systolic murmur, and by echocardiography. Intrapericardial free wall rupture was diagnosed at surgery. The incidence, predisposing factors, gravity, prognostic factors and different medico-surgical therapeutic approaches to septoparietal complications of acute myocardial infarction are reviewed.

Sternum wound contraction and distension during negative pressure wound therapy when using a rigid disc to prevent heart and lung rupture.

BACKGROUND: There are increasing reports of deaths and serious complications associated with the use of negative pressure wound therapy (NPWT), of which right ventricular heart rupture is the most devastating. The use of a rigid barrier has been suggested to offer protection against this lethal complication by preventing the heart from being drawn up against the sharp edges of the sternum. The aim of the present study was to determine whether a rigid barrier can be safely inserted over the heart with regard to the sternum wound edge movement. METHODS: Sternotomy wounds were created in eight p

Transcatheter edge-to-edge mitral valve repair for post-myocardial infarction papillary muscle rupture and acute heart failure: A systematic review.

Papillary muscle rupture (PMR) is a rare complication of acute myocardial infarction (AMI) associated with high mortality and morbidity. Surgery is the gold-standard treatment for these patients, but it is burdened by a high perioperative risk due to hemodynamic instability. Mitral transcatheter edge-to-edge repair (M-TEER) was reported to be safe and effective in unstable patients with significant mitral regurgitation. However, data in patients with post-AMI PMR are limited to a few case reports. In this review, we summarized all data available regarding percutaneous treatment of post-AMI PMR

Fetal heart rate abnormalities associated with uterine rupture: a case-control study: A new time-lapse approach using a standardized classification.

OBJECTIVE: The aim of this study was to identify fetal heart rate abnormalities (FHRA) in the two hours preceding uterine rupture during trial of labor after a previous C-section compared with successful vaginal birth after cesarean controls. STUDY DESIGN: A multicenter case-control study was conducted from 2006 to 2012. Fetal heart rate tracings of the two-hour period preceding delivery were segmented, anonymized and independently classified by two obstetricians according to a standardized grid based on FIGO guidelines (4 grades: 1 - normal, 2 - intermediate, 3 - abnormal, 4 - preterminal).

Rupture of heart with all layers after a massive blunt thoracic trauma without any lesion on the bones: a case report.

Our case is a male student which was dead because of traffic accident as passenger in October, 01, 2007. His heart was found to be lacerated and ruptured horizontally from atrio-ventricular region through all layer of the wall at autopsy. There were not any changes of skeletal system. Our case is considered as a rare and interesting case because there was no lesion on the bones, though the case was exposed to so massive trauma that cause rupture of heart from all layers.

A case report: change in fetal heart rate pattern on spontaneous uterine rupture at 35 weeks gestation after laparoscopically assisted myomectomy.

A 31-year-old nulligravid woman who underwent laparoscopically assisted myomectomy 5 months before becoming pregnant suffered uterine rupture at 35 weeks gestation. A 50 g intramuscular myomatous node had been removed laparoscopically. Early signs of rupture included sudden onset of severe abdominal tenderness and frequent uterine contractions despite reassuring FHR tracing. Variable deceleration was observed as late as 7.5 hours after onset. Emergency cesarean section was performed due to increasing severity of tenderness, revealing complete uterine rupture at the fundus site without extrusio
