[Diastolic reinforecement by intra-aortic balloon. Critical review of temorary mechanical ventricular assistance technic].

The treatment of the cardiogenic shock complicating acute myocardial infarction has been drastically improved by the progress in temporary mechanical cardiovascular support. The improvement of the synchronous assist device, such as the intra aortic balloon, associated with the progress in the monitoring of the patients, in the surgical treatment of myocardial insufficiency and ventricular abnormalities such as ventricular septal defect and mitral insufficiency, has changed the seroius prognosis in patients. A review of some theretical, physiological and clinical problems are discussed in this

[Hypothermia for cardiac surgery with cessation of circulation--start of open heart surgery in Germany].

The first surface hypothermia with cessation of the circulation in Europe has been performed in Duesseldorf on February 9, 1955 for the direct suture of an atrial septal defect. Risks and insufficient basic knowledge are mentioned and the procedure is described with a protocol of anesthesia, cooling in ice water and rewarming in a water mattress. Eighth weeks later, the first series of 7 patients with closure of an atrial septal defect were presented at the congress of the German Society of Surgery. One patient died on the 13th postoperative day due to a cerebral thrombosis, unrelated to hypot

[Surgical Repair for Blunt Cardiac Rupture].

Blunt cardiac injury is a life-threatening condition. We report 3 successful cases in which we performed surgery for blunt cardiac injury. Three individuals were injured, 2 in traffic accidents and the other being caught between a crane and a steel frame. Echocardiograms and computed tomography scans revealed pooling of bloody pericardial fluid in all 3 patients, who underwent emergency surgery. Two patients needed sutures to control persistent bleeding. Although a heart-lung machine was prepared at the start of surgery in all 3 cases, we did not need to use it for any patient. Thus, prior to

Closed Mitral Valvotomy: Celebrating 100 Years of Surgical History.

The year 2023 marks the 100th anniversary of the first successful valvotomy for mitral valve stenosis by Elliott C. Cutler in 1923. Closed-chest mitral valve commissurotomy developed further before being replaced by an open procedure after the advent of the heart-lung machine. Currently, because of the almost complete disappearance of rheumatic disease in the Western World, mitral commissurotomies are infrequently performed in those countries, although the procedure-either closed or open-is still performed in developing countries and select patients. This review retraces the 100-year journey f

[Cave canulam et dilatationem. Cardiopulmonary resuscitation and surgical repair of a vessel perforation following central venous catheterization].

A 58-year-old female patient was scheduled for minimally invasive cardiac surgery. For drainage of venous blood to the heart-lung machine insertion of a central venous large diameter catheter was a prerequisite. During insertion into the right internal jugular vein by the anesthesiologist a life-threatening perforation of the subclavian artery occurred. The patient suffered hemorrhagic shock and advanced cardiac life support was carried out for approximately 18 min. Immediate surgical repair was successful and the originally planned operation was then performed. Concurrently, mild therapeutic

Thoracic ultrasound: A complementary diagnostic tool in cardiology.

Clinical assessment and workup of patients referred to cardiologists may need an extension to chest disease. This requires more in-depth examination of respiratory co-morbidities due to uncertainty or severity of the clinical presentation. The filter and integration of ecg and echocardiographic information, addressing to the clues of right ventricular impairment, pulmonary embolism and pulmonary hypertension, and other less frequent conditions, such as congenital, inherited and systemic disease, usually allow more timely diagnosis and therapeutic choice. The concurrent use of thoracic ultrasou

Organ donors: heartbeating and non-heartbeating.

The limits of organ donation from heart-beating (HB) donors reached a plateau illustrated by the number of postmortem kidneys for transplantation. Programs such as the European Donor Hospital Education Program (EDHEP) and Donor Action have helped to stop a further decrease in the number instead of an expected increase. For kidneys, heart, liver, and lungs one must also explore the use of marginal donors as a possible additional source. Examples are donors with a horseshoe kidney, those at both ends of the age spectrum, and those with medical contraindication such as diabetes. We have enlarged

When do cerebral emboli appear during open heart operations? A transcranial Doppler study.

The transcranial Doppler technique enabled the detection of cerebral air emboli in 10 of 10 patients during open-heart valve operations despite standard deairing procedures. With this technique, the occurrence of emboli in the right middle cerebral artery was followed continuously in patients undergoing aortic or mitral valve replacement. Membrane oxygenators were used. Scattered emboli were observed during the insertion of the aortic cannula, at the start of cardiopulmonary bypass, and after the declamping of the aorta with the heart beating while empty. During the period of aortic cross-clam

Experimental studies of the influence of priming solutions on the systemic activation of complement during cardiopulmonary bypass: comparison between the use of albumin, hydroxyethyl starch and HWA-138.

Cardiopulmonary bypass surgery may be complicated by a systemic inflammatory reaction, which has been ascribed to activation of complement. For such activation, the choice of priming solution for the heart-lung machine may be of importance. The peripheral blood of three groups of eight donors was exposed to albumin, hydroxyethyl starch (HES) or to HWA-138 (pentoxifylline analogue) in addition to the priming solutions. The study confirmed that activation of complement is a consistent phenomenon during cardiopulmonary bypass surgery. The concentration of the C3 activation product C3a in the plas

Machine Learning Accurately Predicts Short-Term Outcomes Following Open Reduction and Internal Fixation of Ankle Fractures.

Ankle fractures are common orthopedic injuries with favorable outcomes when managed with open reduction and internal fixation (ORIF). Several patient-related risk factors may contribute to poor short-term outcomes, and machine learning may be a valuable tool for predicting outcomes. The objective of this study was to evaluate machine-learning algorithms for accurately predicting short-term outcomes after ORIF for ankle fractures. The Nationwide Inpatient Sample and Nationwide Readmissions Database were queried for adult patients ≥18 years old who underwent ORIF of an ankle fracture during 2013

Chest auscultation - tradition and expectations Concerns and promises in the era of smart stethoscopes.

AI-assisted digital stethoscopes can classify heart and lung sounds with accuracy comparable to or exceeding that of the average clinician and help the physician in the diagnostic process. With this essay we want to discuss benefits and downsides of this new technology. Will proficiency in auscultation, which brings joy and pride to many stethoscope users, be suffering when interpretation of chest sounds is taken over by AI? And how will recommendations from AI in the diagnostic process influence decision making and the patient-doctor relationship? A tendency toward deskilling of auscultation

Heart surgery and transplantation: innovations impacting on concepts of life and death.

The development of heart surgery is briefly reviewed, and the impact it has made on our concepts of life and death are considered. For centuries, death was defined by the cessation of heart beat. In the early days of heart surgery in the 1940s and 1950s, the heart sometimes temporarily stopped beating, but could be resuscitated, and some concluded that the patient had been 'dead' for a period of time. Subsequently, when the patient's brain and other vital organs were protected either by the induction of a state of total body hypothermia or by the support of a heart-lung machine, the heart was

Left ventricular assist devices: evolving devices and indications for use in ischemic heart disease.

PURPOSE OF REVIEW: The mortality with end-stage heart failure is extremely high, especially when patients become refractory to conventional medical therapy and require frequent hospitalization. Ischemic heart disease remains the primary cause of advanced heart failure. Mechanical pumps or devices have been developed called ventricular assist devices and are being used to support an increasing number of patients with refractory heart failure. RECENT FINDINGS: The use of ventricular assist devices has evolved from initially only support of patients unable to be weaned from a heart-lung machine

Clinical Pearls of Venoarterial Extracorporeal Membrane Oxygenation for Cardiogenic Shock.

Extracorporeal membrane oxygenation (ECMO) is a technique that uses a pump to drain blood from a body, circulate blood through a membrane lung, and return the oxygenated blood back into the body. Venoarterial (VA) ECMO is a simplified version of the heart-lung machine that assists native pulmonary and/or cardiac function. VA ECMO is composed of a drainage cannula in the venous system and a return cannula in the arterial system. Because VA ECMO can increase tissue perfusion by increasing the arterial blood flow, it is used to treat medically refractory cardiogenic shock or cardiac arrest. VA EC

May 6, 1953: the untold story.

A landmark case in the history of cardiac surgery was the first successful use of cardiopulmonary bypass (CPB) in May 1953 by Dr. John H. Gibbon, Jr. The operation was closure of an atrial septal defect, but there were complications in the conduct of CPB that nearly led to a catastrophe. Quick action by personnel on the team saved the patient's life.

Primary cardiogenic shock during acute myocardial infarction: results of emergency cardiac transplantation.

Fifteen patients with acute myocardial infarction and cardiogenic shock underwent emergency cardiac transplantation after medical treatment failed to improve their haemodynamic status. Their mean age was 49 +/- 7 years. The infarction was anterior in 12 cases, inferoposterior in two cases, and septal in one. Shock occurred within 3 days after the onset of chest pain in nine patients, and during the first day in six of them. Mechanical circulatory assistance was used in six patients as a bridge to transplantation when their haemodynamic status could not be stabilized pharmacologically. Orthotop

Modifying priming techniques in cardiopulmonary bypass circuit in known case of sickle-cell trait undergoing open heart surgery.

Modifying cardiopulmonary bypass (CPB) circuit's priming technique before the onset of CPB in cardiac patients known with sickle-cell disease or sickle cell trait has been observed to be of substantial significance in dealing with such challenges without having any life-threatening consequences. We modified our routine heparinized crystalloid priming of the CPB circuit with partial exchange transfusion by adding donor blood (packed red blood cells), fresh frozen plasma (FFP), and bicarbonate. This has helped us bring down the overall sickle cell hemoglobin in the blood thereby reducing its ris

[Microclots of perfusion mixtures during open heart surgery with artificial circulation].

During 9 experimental and 93 clinical operations on the open heart under circulatory bypass the authors studied microclots of multicomponent perfusion mixtures. They observed an increased number of microclots after infusion of high-molecular plasma-substituting solutions and long-term intraapparatus recirculation. During the bypass there was an increased number of microclots in the "machine-patient" system. The complex of measures suggested by the authors contributes to a delay in additional formation of microclots during the perfusion and also reduces the number of posttransfusion complicatio
