Evaluation of abnormal bleeding in women.

The evaluation of the woman with abnormal bleeding, particularly menorrhagia, carries a relatively high yield of discovering an underlying disorder of hemostasis. The most common underlying hemostatic disorder would be von Willebrand disease, with an estimated prevalence of 7% to 20%. In addition, preliminary results suggest that another 20% to 30% of patients may have impaired platelet aggregation as another cause of menorrhagia. Disorders of fibrinolysis may be an additional underlying hemostatic disorder. Initial intake should include documentation of menorrhagia by the pictorial chart asse

The role of viscoelastic hemostatic assays for postpartum hemorrhage management and bedside intrapartum care.

Viscoelastic hemostatic assays are point-of-care devices that assess coagulation and fibrinolysis in whole blood samples. These technologies provide numeric and visual information of clot initiation, clot strength, and clot lysis under low-shear conditions, and have been used in a variety of clinical settings and subpopulations, including trauma, cardiac surgery, and obstetrics. Emerging data indicate that these devices are useful for detecting important coagulation defects during major postpartum hemorrhage (especially low plasma fibrinogen concentration [hypofibrinogenemia]) and informing cl

How to manage bleeding disorders in aging patients needing surgery.

With improvements in medical care, the life expectancy of patients with bleeding disorders is approaching that of the general population. A growing population of older adult patients with bleeding disorders is at risk of age-related comorbidities and in need of various elective and emergent age-related procedures. The increased risk of thrombosis and volume overload in older adults complicates perioperative hemostatic management. Furthermore, antithrombotic treatment such as antiplatelet or anticoagulant therapy, which is frequently required for various cardiovascular interventions, requires a

Chitosan/oxidized cellulose composite nanofiber sponges: a rapid and effective hemostasis strategy for non-compressible hemorrhage.

Non-compressible hemorrhagic wounds, prevalent in combat and mass trauma scenarios, are characterized by arterial injury-induced exsanguination and geometrically complex wound morphology that challenges conventional hemostatic interventions. This paper presents a novel nanofiber hemostatic sponge (CT@GP-HAc), made from 2,2,6,6-tetramethylpiperidine-1-oxyl radical (TEMPO) oxidized cellulose nanofibers (TOCN) and chitosan nanofibers (CSNF), crosslinked with genipin (GP) in an acetic acid atmosphere. The sponge features excellent shape memory and rapid hemostatic capabilities, specifically design

Hemostatic abnormalities and relationships to metabolic and hormonal status in polycystic ovarian syndrome.

Polycystic ovarian syndrome (PCOS), diagnosed based on hyperandrogenism, ovulatory dysfunction, and polycystic ovaries, is one of the most common disorders of reproductive-aged females. Etiology includes both genetic and environmental/lifestyle factors contributing to both insulin resistance and hyperandrogenism. Clinically, PCOS has reproductive, psychological, and metabolic features, the latter predisposing to cardiovascular disease (CVD). Hemostatic abnormalities have an association with and a demonstrated pathophysiological role in CVD in non-PCOS populations but have not been adequately e

Soluble fibrin monomer complexes and other hemostatic parameters in patients with intrauterine fetal death.

Hemostatic parameters, especially soluble fibrin monomer complexes, have been studied in 114 patients with intrauterine fetal death. The patients were classified into three groups according to the duration of fetal retention. A correlation was observed between a longer duration of fetal retention and the hemostatic alteration. Moreover, in the groups of patients in which this hemostatic disorder was more evident, an improvement in these alterations was detected after fetal evacuation. A hemorrhagic picture with consumption of coagulation factors and cross-linked fibrin oligomers was detected a

von Willebrand disease combined with other hemostasis disorders: an overlooked clinical entity.

von Willebrand disease (VWD) is common, affecting ∼1% of the general population. Due to this relatively high prevalence, it can co-occur with other coagulation disorders. However, combined defects are often underrecognized in clinical practice. Diagnostic anchoring, overlapping symptoms, and limited access to comprehensive hemostatic and genetic testing contribute to these missed diagnoses. A growing number of case reports, case series, and cohort studies have documented the coinheritance of VWD with hemophilia A or B, rare coagulation factor deficiencies, and platelet function disorders. In s

ARFI ultrasound monitoring of hemorrhage and hemostasis in vivo in canine von Willebrand disease and hemophilia.

A validated method for assessing hemostasis in vivo is critical for testing the hemostatic efficacy of therapeutic agents designed for patients with bleeding disorders such as von Willebrand disease (VWD) and hemophilia A. We hypothesize that rate of bleeding and time to hemostasis can be monitored in vivo by acoustic radiation force impulse (ARFI) ultrasound. We performed ARFI imaging following 12-gauge needle puncture of hind limb muscle encompassing an ∼2 mm vein in six normal, eight naïve hemophilia A before and after infusing canine factor VIII, three hemophilia A expressing canine factor

Corpus luteum hemorrhage with acquired hemophilia A: a case report and literature review.

BACKGROUND: The rupture of the corpus luteum (CL) may occur at all stages of a woman's reproductive life. Bleeding of the ruptured CL varies from self-limiting hemorrhage to massive hemoperitoneum, causing the shock and subsequent emergency surgery. But hemoperitoneum secondary to ruptured CL is a rare complication and situation for women with bleeding disorders. CASE PRESENTATION: We here describe a case of severe CL hemorrhage with factor VIII deficiency. We chose conservative management instead of surgery for the abnormal hemostatic condition. With blood product and factor concentrate supp

Changing hemostatic management in post-partum hemorrhage.

Early and fast assessment of hemostasis during postpartum hemorrhage (PPH) is essential to allow early characterization of coagulopathy, estimate bleeding severity and improve outcome. During PPH, fibrinogen decrease occurs earlier than other coagulation factors deficiency and hypofibrinogenemia is an early marker of PPH severity of progression. With good evidence in the context of PPH, point-of-care viscoelastic (VET) hemostatic assays have been shown to provide rapid assessment of hemostatic disorders, low fibrinogen levels, and allow VET-guided fibrinogen replacement. Further studies are ne

[Disorders of hemostasis in dysthyroidism].

Many potentially severe hemostatic disorders have been reported in thyroid diseases. Hyperthyroidism has been associated with thrombocytopenia, coagulation factor abnormalities, and decreased fibrinolytic and plasminogen activities. In hypothyroidism, potentially severe hemorrhagic disorders have been reported, including platelet function abnormalities and coagulation factor alterations such as acquired von Willebrand disease.

The relevance of developmental hemostasis to hemorrhagic disorders of newborns.

The hemostatic system is a dynamic evolving process that is age-dependent. Components of the hemostatic system are synthesized in early fetal life and do not cross the placenta from mother to fetus. However, plasma concentrations of proteins involved in hemostasis significantly differ from adults. Physiological reference ranges are available for premature infants, full-term infants and children from ages 1 to 16 years. In the coagulation system, plasma concentrations of the vitamin K-dependent and contact factors are decreased at birth, whereas other factors such as fibrinogen, FV, FVIII, and

Polyendocrine metabolic ovarian syndrome and venous thromboembolism: Hemostatic considerations in diagnosis and management.

Polyendocrine metabolic ovarian syndrome (PMOS), previously known as polycystic ovary syndrome (PCOS), is a complex disorder affecting females of reproductive age. Patients with PMOS can develop several comorbidities, including obesity, hyperlipidemia, arterial hypertension, insulin resistance, infertility, and increased risk for endometrial carcinoma. Venous thromboembolism (VTE) is another complication associated with PMOS, thought in part to be related to chronic inflammation leading to prothrombotic hemostatic changes. Hormonal therapy is the mainstay of management, but the presence of oth

Sickle cell disease: Hemostatic and inflammatory changes, and their interrelation.

Sickle cell disease, the most common genetic blood disorder in the world, has high clinical variability, negatively impacts quality of life and contributes to early mortality. Sickled erythrocytes cause blood flow obstruction, hemolysis, and several hemostatic changes that promote coagulation. These events, in turn, induce chronic inflammation, characterized by elevated plasma levels of pro-inflammatory markers, which aggravates the already unfavorable state of the circulatory system. Empirical evidence indicates that the hemostatic and inflammatory systems continuously interact with each othe

Platelet disorders in adolescents.

Acquired and inherited platelet disorders can present with bleeding symptoms during adolescence. Platelet disorders include disorders of platelet number and disorders of platelet function. In adolescent females with platelet disorders, menorrhagia is a common presenting bleeding symptom. Other associated bleeding symptoms are also primarily mucocutaneous, including epistaxis and bruising. Excessive bleeding may also occur after hemostatic challenges. Diagnosis of disorders of platelet function usually requires light transmission platelet aggregometry. Management of menorrhagia in the adolescen

Calcium loaded biodegradable porous microspheres for the hemostasis of coagulopathy.

The development of efficient hemostatic materials is a challenge for patients with coagulation disorders. In this study, we developed advanced biodegradable porous microspheres for the hemostasis of coagulopathy. The microspheres were fabricated using multiblock amphiphilic poly(ε-caprolactone-r-glycolide) (PCGA)/poly(ethylene glycol) (PEG) polyurethanes (PUE). Calcium ions were loaded in the porous microspheres (CPBMs) via solution immersion. The microspheres possessed high porosity, uniform particle and pore sizes, and excellent biocompatibility. The CPBMs demonstrated an ultra-high water ab

Utility of thromboelastography with platelet mapping for monitoring platelet transfusion in qualitative platelet disorders.

BACKGROUND: Patients with pathogenic variants in RASGRP2 (inherited platelet disorder (IPD)-18) exhibit normal platelet counts but impaired platelet aggregation and αIIbβ3 activation. Moderate-to-severe bleeding episodes require patients to be transfused with platelets and/or pro-hemostatic agents. We recently demonstrated that hemostatic efficacy of transfused platelets is limited by dysfunctional endogenous platelets in a mouse model of IPD-18 (Rasgrp2-/- mice), as dysfunctional platelets were recruited to the forming hemostatic plug but did not participate in clot contraction. Thus, higher
