Comparison of Local Injection of Fresh Frozen Plasma to Traditional Methods of Hemostasis in Minimally Invasive Procedures.

RATIONALE AND OBJECTIVES: To evaluate different techniques for reducing hemorrhagic complications in coagulopathic patients with elevated international normalized ratio having an image-guided percutaneous invasive procedure; techniques included systemic transfusion of fresh frozen plasma (FFP), local injection of FFP, percutaneous injection of gelatin sponge, and percutaneous placements of angiographic coils. MATERIALS AND METHODS: Retrospective review of 232 consecutive patients with known coagulopathy undergoing image-guided minimally invasive procedures were selected. Ninety-one patients h

Ligasure versus Ultracision in thyroid surgery: a prospective randomized study.

PURPOSE: New hemostatic technologies (NT) are often employed in thyroid surgery in the effort to reduce operating time and complications. The aim of this study is to compare three different hemostatic techniques. METHODS: This is a prospective randomized study. There were 150 patients, aged 56 +/- 14 years, randomized for total thyroidectomy with conventional technique (CT), Ligasure vessel sealing system (LI) or Harmonic Scalpel (HS) at the university surgical department. One hundred thirty-five patients had benign diseases; 15 had malignancies. RESULTS: Mean postoperative hospital stay was…

Control of epistaxis in the multiple trauma patient.

Severe epistaxis in the multiple trauma victim can be rapidly controlled with immediate insertion of the intranasal anteroposterior balloon. The patient's other life threatening injuries can then be assessed in an orderly fashion. Once the patient has been stabilized, the balloon can be deflated and removed and the patient's maxillofacial injuries assessed and treated in a controlled setting.

Evaluation of hydrogel tissue sealant in porcine laparoscopic partial-nephrectomy model.

BACKGROUND AND PURPOSE: Laparoscopic partial nephrectomy (LPN) is technically challenging with a steep learning curve, primarily because techniques used to control bleeding on the cut surface of the kidney can be ineffective, inconsistent, or challenging. Hemostatic techniques can include intracorporeal suturing, vascular coagulation (argon-beam coagulator, bipolar cautery, laser), and application of various tissue sealants. There is no uniformity of opinion regarding which hemostatic technique is optimal for this application. CoSeal, a hydrogel (Baxter Healthcare Corp, Deerfield, IL), has bee…

Prehospital emergency response in a partial hand amputation with ulnar artery transection: a case report.

INTRODUCTION: Approximately 50% of preventable trauma fatalities are associated with a lack of hemorrhage control and occur in the prehospital environment. The application and efficacy of traditional hemostatic techniques, such as tourniquets, gauze packing, and impregnated dressings, are frequently limited by factors including pain, risk of ischemia, wound configuration, and challenges associated with removal at definitive care. CASE REPORT: We report the prehospital application of a hemostatic flowable biopolymer, Traumagel, in a 46-year-old male presenting with a partial hand amputation an…

A Simple Technique of Cerebrospinal Fluid Leak Prevention Following Endoscopic Third Ventriculostomy: A Technical Note.

BACKGROUND: There are a variety of complications of endoscopic third ventriculostomy (ETV) that have been described in the literature. Cerebrospinal fluid leak is one of the common complications that need to be addressed properly and timely management plays a crucial role in prevention of meningitis/encephalitis. OBJECTIVE: To share our experience of using autologous bone (from burr hole) debris and "BloodSTOP" absorbable hemostatic agent as an effective tool in cerebrospinal fluid leak prevention. METHODS: We have observed 14 individuals who have undergone an ETV procedure using the aboveme

Surgical management options for control of presacral venous plexus hemorrhage - a review.

Introduction: Presacral venous plexus (PSVP) is the lowest portion of the anterior external vertebral venous system, and injury to it with subsequent hemorrhage is a recognised and potentially lethal complication that may be encountered during pelvic surgical operations. Conventional hemostatic treatments may not be able to control this hemorrhage, and innovative techniques are required to address this issue.Aim: By examining the current techniques for managing PSVP hemorrhage, this study aims to remind the surgeons about possible management approaches tha

A study comparing rates of deflation of nasal balloons used in epistaxis.

A study was set up to compare the rates of deflation between three commonly used nasal balloons for securing haemostasis in epistaxis, namely Brighton balloons, Simpson Balloons, and Foley catheters. Brighton balloons were found to have the slowest rate of deflation, followed by Simpson balloons. Foley catheter balloons have the fastest rate of deflation, having deflated to less than half their original diameter by day five. However, these differences are relatively small.

Practical management of epistaxis.

Epistaxis is a very common presenting symptom of patients seen in the emergency room or the physician's office. An understanding of the nasal anatomy and physiology is important for proper treatment of these patients. New methods of treatment are discussed briefly.

[Advances and prospects of hemostatic strategies in hepatic surgery].

The liver is characterized by abundant blood supply and intricate anatomical architecture, rendering intraoperative and postoperative hemorrhage a primary clinical challenge. Conventional compression and suture ligation fail to enable precise perioperative hemorrhage prediction. Driven by the integration of precision medicine, artificial intelligence, and biomaterials, hemostatic strategies are undergoing a paradigm shift from passive compression to precise targeted intervention. This paper systematically reviews advances and future perspectives in hemostatic strategies for hepatic surgery. Preope

Total and Hidden Blood Loss in Biportal Endoscopic Versus Tubular-Based Minimally Invasive Transforaminal Lumbar Interbody Fusion.

BACKGROUND: The purpose of this study was to compare total blood loss (TBL) and hidden blood loss (HBL) between biportal endoscopic transforaminal lumbar interbody fusion (BE-TLIF) and tubular-based minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) using hematocrit-based calculations. The study aimed to compare blood loss outcomes between the 2 techniques during their respective initial adoption phases to ensure a fair comparison of hemostatic control. METHODS: This retrospective study included 72 patients who underwent single-level lumbar fusion. Thirty-six consecutive BE-

Viscoelastic Hemostatic Assays for Postpartum Hemorrhage.

This article discusses the importance and effectiveness of viscoelastic hemostatic assays (VHAs) in assessing hemostatic competence and guiding blood component therapy (BCT) in patients with postpartum hemorrhage (PPH). In recent years, VHAs such as thromboelastography and rotational thromboelastometry have increasingly been used to guide BCT, hemostatic adjunctive therapy and prohemostatic agents in PPH. The three pillars of identifying hemostatic competence include clinical observation, common coagulation tests, and VHAs. VHAs are advantageous because they assess the cumulative contribution
