Inspecting the Ribozyme Region of Hepatitis Delta Virus Genotype 1: Conservation and Variability.

The hepatitis delta virus (HDV) genome has an autocatalytic region called the ribozyme, which is essential for viral replication. The aim of this study was to use next-generation sequencing (NGS) to analyze the ribozyme quasispecies (QS) in order to study its evolution and identify highly conserved regions potentially suitable for a gene-silencing strategy. HDV RNA was extracted from 2 longitudinal samples of chronic HDV patients and the ribozyme (nucleotide, nt 688-771) was analyzed using NGS. QS conservation, variability and genetic distance were analyzed. Mutations were identified by aligni

Clevudine inhibits hepatitis delta virus viremia: a pilot study of chronically infected woodchucks.

In a small controlled study, clevudine, a potent inhibitor of hepadnaviruses, including hepatitis B virus and woodchuck hepatitis virus, suppressed hepatitis delta virus (HDV) viremia in chronically infected woodchucks. Suppression was correlated with the marked reduction of woodchuck hepatitis virus surface antigen in individual animals, consistent with the concept that repression of surface antigen expression may be a useful antiviral strategy for HDV.

An enzyme immunoassay for detection of IgA class antibodies against hepatitis delta virus.

A sensitive and reproducible enzyme-linked immunoassay (ELISA) for IgA class antibodies against the Delta antigen (HDAg) is described. Specificity of the assay was demonstrated by the absence of binding to an unrelated antigen or to uncoated plates and the finding that binding to HDAg was independent of total IgA concentrations in sera. Positive results were obtained with sera from 11 of 14 patients with chronic Delta virus infection (seropositive for HBsAg and IgM anti-HDAg, negative for IgM anti-HBc) at serum dilutions of up to 1:10(6). Sera from four normal healthy individuals and from 25 p

Delta agent superinfection. Rapidly progressive liver disease in a hepatitis B virus carrier.

We report a case of rapidly progressive chronic active hepatitis (CAH) following acute delta infection in a previously asymptomatic hepatitis B virus carrier. Serologic evidence of delta superinfection coincided with an acute icteric hepatitis, and was followed by the development of clinically evident chronic liver disease. The liver biopsy specimens documented the acute hepatitis and subsequent progression of severe CAH. This case illustrates the clinical and pathologic changes that may follow acute delta infection in chronic hepatitis B carriers.

Clinical and histological features of delta infection in chronic hepatitis B virus carriers.

One hundred and six consecutive chronic hepatitis B virus (HBV) carriers were studied for the prevalence of delta markers in serum and tissue, and the clinical and histological features of those with and without delta infection were compared. Twenty (18.9%) patients were positive for anti-delta in serum or delta antigen in the liver or both. They presented at a younger age (30.3 v 38 years). All of them were symptomatic at the time of biopsy, in contrast to 35% of patients without delta infection who were not symptomatic. Those with delta infection had higher serum transaminase values and show

A Rapid Point-of-Care Test for the Serodiagnosis of Hepatitis Delta Virus Infection.

Hepatitis Delta virus (HDV) is a satellite of the Hepatitis B virus (HBV) and causes severe liver disease. The estimated prevalence of 15-20 million infected people worldwide may be underestimated as international diagnostic guidelines are not routinely followed. Possible reasons for this include the limited awareness among healthcare providers, the requirement for costly equipment and specialized training, and a lack of access to reliable tests in regions with poor medical infrastructure. In this study, we developed an HDV rapid test for the detection of antibodies against the hepatitis delta

Influence of hepatitis B virus (HBV) genotype on the clinical course of disease in patients coinfected with HBV and hepatitis delta virus.

OBJECTIVE: We evaluated the influence of hepatitis B virus (HBV) genotype on the course of disease in patients coinfected with HBV and hepatitis delta virus (HDV). METHODS: We evaluated HBV genotypes in 190 patients, 140 of whom had chronic HBV monoinfection and 50 of whom had chronic HBV-HDV coinfection. Real-time polymerase chain reactions for the amplification of HBV DNA and HDV RNA were developed, and we compared the patient groups with respect to HBV genotype, viral load, alanine aminotransferase (ALT) and bilirubin levels, and disease severity. RESULTS: Coinfected patients had higher A

Heterosexual transmission of hepatitis delta virus in the general population of an area endemic for hepatitis B virus infection: a prospective study.

A prospective study was conducted in 268 consecutive patients with overt hepatitis to examine the transmission of hepatitis delta virus (HDV) in the general population of an area hyperendemic for hepatitis B virus (HBV) infection. The patients were interviewed extensively for potential risk factors associated with transmission. Of the 203 patients with type B hepatitis, 34 (30 men, 4 women) showed serologic evidence of acute HDV infection (HDV+ group), while the other 169 did not (HDV- group). Of the 30 HDV+ men, 27 (90%) had had sexual intercourse with prostitutes within 3 months of admission

Fate of hepatitis D virus-specific CD8+ T cells during bulevirtide monotherapy in patients with chronic hepatitis delta.

BACKGROUND & AIMS: Bulevirtide reduces viremia in chronic hepatitis D virus (HDV) infection, but long-term treatment is required in most patients to prevent relapse. Sustained treatment responses may be fostered by therapy-induced amelioration of HDV-specific CD8+ T-cell responses that are exhausted because of high viremia and antigen loads during chronic HDV infection. We therefore studied the effect of bulevirtide monotherapy on HDV-specific CD8+ T-cell repertoire, phenotype, and functionality. METHODS: Twenty-eight patients with HDV infection and cirrhosis starting bulevirtide treatment we

Clinical impact of hepatitis delta virus coinfection on liver fibrosis in hepatitis B patients: a population-based study.

BACKGROUND: Evaluation of liver fibrosis appears to be easily overlooked in the clinic for the chronic Hepatitis D. Herein, we determine the Clinical Impact of liver fibrosis among HBsAg-positive/Anti-HDV positive US general adults. METHODS: Data were obtained from the National Health and Nutrition Examination Survey (NHANES) 1999-2020. Liver fibrosis was assessed by the Fibrosis-4 (FIB-4) and aspartate aminotransferase to platelet ratio index (APRI) score. To minimize confounding, propensity score matching (PSM) was applied to compare HBV/HDV-coinfected and HBV-monoinfected individuals. RES

Prevalence of hepatitis delta virus among chronic hepatitis B carriers in a large tertiary center in the Netherlands.

BACKGROUND & AIMS: Hepatitis D virus infection (HDV) is considered the most severe form of viral hepatitis. In this study, we aimed to evaluate the prevalence of HDV infection in a tertiary center of a large, multi-ethnic city in the Netherlands. Moreover, we validate the reliability of a novel anti-HDV CLIA assay. METHODS: All HBsAg-positive patients visiting the outpatient clinic between 2017 and 2019 were tested for HDV serology. Seropositive serum samples were further assessed by HDV RNA PCR and Sanger sequencing to identify the HDV genotype. RESULTS: The CLIA assay was 100% sensitive an

Statin inhibits large hepatitis delta antigen-Smad3 -twist-mediated epithelial-to-mesenchymal transition and hepatitis D virus secretion.

BACKGROUND: Hepatitis D virus (HDV) infection may induce fulminant hepatitis in chronic hepatitis B patients (CHB) or rapid progression of CHB to cirrhosis or hepatocellular carcinoma. There is no effective treatment for HDV infection. HDV encodes small delta antigens (S-HDAg) and large delta antigens (L-HDAg). S-HDAg is essential for HDV replication. Prenylated L-HDAg plays a key role in HDV assembly. Previous studies indicate that L-HDAg transactivates transforming growth factor beta (TGF-β) and induces epithelial-mesenchymal transition (EMT), possibly leading to liver fibrosis. However, the

Hepatitis delta virus super-infection in a co-infected patient with the human immunodeficiency virus type 1 and a surface antigen-negative hepatitis B virus variant.

Human immunodeficiency virus infection has a major impact on the natural history of chronic hepatitis B and favours the emergence of viral mutants. We describe an acute hepatitis D virus superinfection in an HIV-1-infected patient under HAART treatment who was previously a chronic carrier of a surface negative HBV variant resistant to lamivudine.

Low rate of hepatitis Delta virus co-infection in first-time blood donors diagnosed with chronic hepatitis B virus infection in the Netherlands.

BACKGROUND: Hepatitis Delta virus (HDV) requires co-infection with hepatitis B virus (HBV) and increases the risk of hepatitis-related morbidity and mortality compared to HBV mono-infection. HBV/HDV co-infected patients will likely benefit from new HDV antiviral drugs, but reliable estimates of co-infection rates are lacking due to limited HDV testing of HBV-infected patients. STUDY DESIGN AND METHODS: First-time blood and bone tissue donors in the Netherlands (2006-2023) with a newly diagnosed chronic HBV infection were retrospectively tested for HDV antibodies, and for HDV RNA if HDV antibo

Providers' Perspectives of Hepatitis Delta Virus Screening and Recommendations for Improvement: A Qualitative Investigation.

BACKGROUND AND AIMS: The prevalence of hepatitis delta virus (HDV), which causes more severe liver disease than infection with hepatitis B virus alone, has risen in the United States and globally. However, the true burden of HDV is poorly understood due to low screening rates. Both patient- and provider-level factors may contribute to low HDV screening; however, our understanding of such factors is limited. The purpose of our study was to gain a comprehensive understanding of the perceived barriers and facilitators to HDV screening and treatment from the perspective of healthcare providers. M
